Background: Synovial chondromatosis is a benign, relatively rare condition arising from synovial ... more Background: Synovial chondromatosis is a benign, relatively rare condition arising from synovial membrane of joints. It usually affects middle aged individuals arising in large joints. The treatment is surgical by removal of the loose bodies and hyperactive synovium. This case report documents this rare synovial pathology in the hip joint and its management by open synovectomy and loose body excision. Case Report: A thirty one year old female presented with the complaint of insidiously increasing swelling in the right groin with pain and progressively increasing restriction of movements at right hip. After clinical and radiological examination the patient was diagnosed as stage three synovial chondromatosis of hip and was managed by open synovectomy with loose body excision. The histopathology of the excised specimens confirmed the diagnosis. Conclusion: Surgical management is most often required in patients with symptomatic and progressive disease as in this case. Loose body excision and complete synovectomy offers reliable cure in young patients where replacement arthroplasty can be considered later on. Variations in the arterial pattern of the upper limb are very common as observed in many cadaveri... more Variations in the arterial pattern of the upper limb are very common as observed in many cadaveric and angiographic studies. Knowledge of variations in the origin and course of the radial artery is important because they are used for many diagnostic procedures as well as vascular and reconstructive surgeries like coronary angiography, percutaneous coronary intervention and coronary artery bypass surgery. During routine dissection in our institute, we observed a case of high origin of the radial artery in a 33 year old male cadaver. It was found to be unilateral; on left side, radial artery was taking origin from 3 rd part of the axillary artery at the lower border of pectoralis minor before the origin of subscapular artery and anterior circumflex humeral artery. It had a superficial course in the arm crossing the median nerve from medial to lateral side. The further course of this superficial radial artery in the forearm was normal and it terminated by forming a deep Palmar arch in hand. These variations may be of great clinical implications for vascular and plastic surgeons and radiologists. Superficial course of radial artery makes it vulnerable to accidental injuries and elevates the risk of bleeding.

Osteochondromas are common benign tumors affecting the long bones of the body. Often, they are as... more Osteochondromas are common benign tumors affecting the long bones of the body. Often, they are asymptomatic, associated with growing bone and their discovery is incidental. Osteochondromas in the talus are quite rare and their place as a cause for tarsal tunnel syndrome is rarer still. Here we report a case of a 10-year-old female patient having an osteochondroma of the talus that was presenting with the symptoms of tarsal tunnel syndrome along with tibio-talar arthritic changes. Excision of the bony mass was done which relieved the compressive symptoms and improved the range of motion at the joint and the postop follow-up has been uneventful at one year now.

With articular cartilage degeneration, chondrocytes undergo mitosis and clustering. Degeneration of cartilage changes its biomechanical, biochemical, and structural properties. Numerous, non-invasive treatment options are available with emphasis on pain management, improvement in function and with a potential to modify the disease process and progress of cartilage degeneration. Platelets contain many growth factors, cytokines, and bioactive proteins which help regulate tissue healing and restoration. Autologous Platelet rich plasma contains a very high concentration of platelets above baseline levels and hence help in the improvement of disease. Methods: We conducted a study on 70 knees diagnosed as having Kellgren-Lawrence (KL) Grade 2-4 Osteoarthritis knee. Autologous Platelet Rich plasma was prepared and was injected into the affected knee joint. The study group was followed up at 1 week, 4 weeks, 12 weeks. Clinical outcome analysis was done for the efficacy on the basis of WOMAC score and NRS Score. Statistical analysis of data was performed and P-value was calculated. Results: Results obtained after statistical analysis suggested an overall improvement in WOMAC score in all grades of Osteoarthritis knee. Improvement in the patients having early stages was more in comparison to that seen in the patients in late stages. Conclusion: Platelet Rich Plasma therapy in an effective treatment option to improve the clinical outcomes in patients with Osteoarthritis knee. It has an advantage of giving satisfactory outcomes. Also, it is low cost, minimally invasive and safe in application.

Fixation of proximal femoral fractures by cephalomedullary nailing has become very popular due to... more Fixation of proximal femoral fractures by cephalomedullary nailing has become very popular due to the superior biomechanics and prevention of various complications associated with sliding hip screw. Material and Methods: In this study we followed 94 patients admitted and managed in the Department of orthopaedics at SGRRIMHS Dehradun. the followup period was of one year duration during which the functional status of patients was assessed by Oxford hip score at regular intervals. Results: All the patients had uneventful post-op recovery. The pain swas quite severe in 70% of the patients at 1 month but by end of 12 months 76% of patients had very mild or no hip pain. No significant difference in hip score was seen in patients managed by long nails and short nails. for the first 3 followups at 1, 3 and 6 months no significant difference between scores of hips managed by open and closed reduction was seen however at 12 months the score of hips managed by open reduction was lower. All the fractures were united at the end of 1 year. Conclusion: Although cephalomedullary nailingg in proximal femoral fractures is a bit technically demanding it has the advantage of providing rotational as well as axial stability and excellent clinical results allowing a faster post-operative restoration of walking ability, with overall better outcome.

With this study we have tried to study the incidence, mode, pattern and complications during management of fractures in paediatric population. We came across 510 patients of paediatric fractures and due various causes followed a total number of 134 cases and present our findings with this study.

Giant cell tumours are rare entity and the one's involving the small bones of the hand are rarer ... more Giant cell tumours are rare entity and the one's involving the small bones of the hand are rarer still having an incidence rate of just 2% of all giant cell tumours. These lesions are often locally aggressive and have a tendency to recur. In this short case series we present five cases of giant cell tumours of the phalanx that initially presented as slow growing swelling of small bones of the hand and later on showed signs of increased growth and mild to moderate pain. All the patients were initially investigated by fine needle aspiration cytology which gave us the diagnosis of GCT and they were then investigated by an MRI to determine the soft tissue involvement. All the cases were managed by extended curettage and bone grafting and at one year of follow up have shown more or less full functional recovery without any signs of recurrence like pain or recurrence of swelling.

Osteochondromas are a relatively common entity when we talki about benign bone tumours and they c... more Osteochondromas are a relatively common entity when we talki about benign bone tumours and they can occur in almost any bone although they have a predilection for certain sites. Flat bones like scapula are a rare location for occurrence of such a lesion. Ventral osteochondromas of scapula are rarer than dorsal ones and can cause a host of symptoms ranging from just scapular pain to pseudo winging of scapula and snapping syndromes. The treatment of such lesions is by excision of the tumour and here we present a case report of a young male with a ventral scapular osteochondroma with static wingingg of scapula and snapping. The patient was adequately managed by en bloc excision and has been relievd of all complaints without any signs of recurrence at one year of followup.

With articular cartilage degeneration, chondrocytes undergo mitosis and clustering. Degeneration of cartilage changes its biomechanical, biochemical, and structural properties. Numerous, non-invasive treatment options are available with emphasis on pain management, improvement in function and with a potential to modify the disease process and progress of cartilage degeneration. Platelets contain many growth factors, cytokines, and bioactive proteins which help regulate tissue healing and restoration. Autologous Platelet rich plasma contains a very high concentration of platelets above baseline levels and hence help in the improvement of disease. Methods: We conducted a study on 70 knees diagnosed as having Kellgren-Lawrence (KL) Grade 2-4 Osteoarthritis knee. Autologous Platelet Rich plasma was prepared and was injected into the affected knee joint. The study group was followed up at 1 week, 4 weeks, 12 weeks. Clinical outcome analysis was done for the efficacy on the basis of WOMAC score and NRS Score. Statistical analysis of data was performed and P-value was calculated. Results: Results obtained after statistical analysis suggested an overall improvement in WOMAC score in all grades of Osteoarthritis knee. Improvement in the patients having early stages was more in comparison to that seen in the patients in late stages. Conclusion: Platelet Rich Plasma therapy in an effective treatment option to improve the clinical outcomes in patients with Osteoarthritis knee. It has an advantage of giving satisfactory outcomes. Also, it is low cost, minimally invasive and safe in application.

After clinical and radiological examination the patient was diagnosed as stage three synovial chondromatosis of hip and was managed by open synovectomy with loose body excision.
