BACKGROUND Nutritional anaemia especially iron deficiency anaemia is one of the important medical... more BACKGROUND Nutritional anaemia especially iron deficiency anaemia is one of the important medical problems in tropical areas and India. Role of oxidative stress in the pathogenesis of Iron Deficiency Anaemia (IDA) has been established. Oxidative stress is one of the accompaniments in the pathology of IDA which also leads to defective production of other iron containing proteins especially cytochrome, peroxidase, catalase etc. However, different studies have assessed different variables. Especially cutoffs for diagnosis, qualitative properties of erythrocytes, prevalence and complication rates in IDA. Research into gender differences in oxidative stress in IDA is particularly lacking. METHODS The present study aimed to elucidate oxidative stress in IDA by measuring the levels of malondialdehyde (MDA), superoxide dismutase (SOD) and total antioxidant capacity (TAC). We compared these levels among men and women with anaemia and without anaemia (i.e., healthy controls). 36 IDA patients (18 men and 18 women) and 48 healthy controls were recruited. 5 mL blood was drawn and assessed for MDA, SOD and TAC levels. One-way ANOVA with post-test Bonferroni was used for statistical analysis. RESULTS Anaemic women had significantly lower iron than anaemic men (p < 0.01). MDA levels were significantly higher in subjects with IDA (p < 0.01), with levels being higher in women. SOD levels were higher in anaemic men as compared to anaemic women (p < 0.01). TAC levels did not vary significantly (p = 0.09). CONCLUSIONS Oxidative stress is present in IDA, which is more severe and less compensated in women as compared to men.

It involves multiple systems particularly lungs, respiratory tract and gastro intestinal tract. It is mainly considered as a disease of pediatric population. However it is being reported from adult population, more frequently from the western countries due to better survival.CF is reported very rarely from India and report of adult CF is much more rarer. Conformation of CF is by HRCT (High resolution computerized tomography), gene mutation analysis and sweat chloride estimation. The present study was aimed to assess the awareness, knowledge, and attitude towards basic life s... more The present study was aimed to assess the awareness, knowledge, and attitude towards basic life support (BLS) among the interns completing their internship from both medical and dental streams of the Nitte university (a deemed university at Mangalore, Karnataka) and also to identify the areas to be addressed for improving the standards of BLS among the interns at their crucial juncture of moving out to the community, society as health care providers.A descriptive study was conducted by using a Questionnaire comprising of 19 questions to collect the data pertaining to demographic details, awareness and knowledge of BLS, attitude towards BLS among all the medical and dental interns completing their internship during March 2012. The study was conducted in the last week of their one year internship programme.After excluding the incomplete response sheets which were none in the present study, the data from 162 interns were subjected to the analysis. We report a 40 year old male who presented with fever, dyspnoea, right sided pleuritic chest pain... more We report a 40 year old male who presented with fever, dyspnoea, right sided pleuritic chest pain and hemoptysis. His physical examination and chest X-ray revealed features of right sided consolidation with pleural effusion. Worsening of his condition despite adequate antibiotic therapy prompted us to consider an alternative cause. Repeat ECG after 3 days showed S1q3T3 pattern. CT angiogram evidenced thrombus in right pulmonary artery and doppler study of both the lowerlimbs showed normal superficial and deep venous system. Hypercoagulation workup revealed high levels of homocysteine. This is a rare case report of hyperhomocysteinemia presenting as pulmonary embolism without documented deep vein thrombosis, and the patient presented with classical clinical and radiological findings of pneumonia.

There are about 120 million smokers in India in the age group of 30-69 years and have an increased cardiovascular and renal risks in addition to other risks attributable to smoking..The present study was aimed to assess the cardiovascular and renal risks especially among the young smokers in the age group of 20-40 years. After all necessary administrative and ethical clearances a cross sectional study was conducted at the tertiary care medical college hospital located at Mangalore. 75 young male smokers in the age group of 20-40 years along with equal number of age, sex matched non smokers as controls were recruited and each one were assessed for Blood pressure(BP), Electro cardiographic changes(ECG changes), and Glomerular filtration rate(GFR).In the present study Smoking resulted in high systolic blood pressure and prolonged QT interval (QTc) as compared to the non smokers, thus increasing the risk of Sudden cardiac death among smokers. Background and objectives: "Sepsis is a major cause for mortality in critically ill patients all ... more Background and objectives: "Sepsis is a major cause for mortality in critically ill patients all over the world. The number of patients presenting with sepsis, septic shock is gradually increasing in daily clinical practice. Mortality in sepsis is mainly due to a delay in diagnosis and initiation of specific therapy(antibiotics).This is in turn mainly attributed to the difficulty in differentiating infectious trigger(sepsis) from non infectious triggers as both present with similar clinical features. Lack of specific marker adds to this dilemma of differentiating infectious and non infectious factors in critically ill patients. Recently there are some reports from European countries on role of Procalcitonin (PCT) in critically ill patients. Draw backs of these studies are galore mainly due to the difficulties in interpretation of results, as varying definitions for sepsis are used. But also there is paucity of data on Procalcitonin from Indian sub continent. Hence in the present single centre prospective observational study conducted at tertiary care medical college hospital , A total of 50 adult patients with sepsis fulfilling ACCP/SCCM guidelines were included, out of which 23 were in SIRS/Sepsis, 14 in severe sepsis and 13 in septic shock. Procalcitonin was evaluated in the first 24 hours after admission and before initiation of any antibiotic therapy. The role of procalcitonin was analyzed in relation to confirming sepsis, assessing the severity of sepsis and assessing the prognosis(possible out come) of sepsis. Combined role of procalcitonin with other indicators especially ESR, SOFA Score, Blood/relevant material culture was explored. Results: Our study confirmed the importance of procalcitonin in critically ill patients particularly in improving the predictive power while solving the sepsis dilemma. Conclusions: From our study, we conclude that Procalcitonin is not a myth nor a hype but it is a hard reality and is an answer to sepsis dilemma. It is therefore preferable to add Procalcitonin into the standard workup of critically ill patients with suspected sepsis in every day clinical practice.

A middle aged man with prolonged pyrexia was referred to us with a diagnosis of FUO (Fever of unk... more A middle aged man with prolonged pyrexia was referred to us with a diagnosis of FUO (Fever of unknown origin). He was evaluated by various investigations and a diagnosis of Tuberculosis was established. On anti tubercular treatment (ATT) he developed complication of acute renal injury-non oliguric renal failure, from which he recovered fully after the puzzle was successfully solved & managed accordingly. Even though he was proved to have rifampicin induced acute intersticial nephritis(AIN) by biopsy , he had varied & unconventional presentation like without oliguria, without peripheral blood eosinophilia, and more so particularly on the maiden administration of rifampicin. Thus our case highlights the importance of quickly establishing the cause for FUO, and also need for greater vigilance on part of physicians to solve unconventional presentations of complications arising out of treatment.

Pulmonary agenesis and UPAA (unilateral pulmonary artery agenesis) is a rare developmental lung d... more Pulmonary agenesis and UPAA (unilateral pulmonary artery agenesis) is a rare developmental lung defect in which there is complete absence or hypoplasia of one or both lungs. It can be either isolated or associated with other congenital anomalies. The percentage of diagnosis of this condition during life has risen considerably, however it is true that more than 50% of the patients with this condition die in the first year of life. The symptoms and severity of this condition is often variable. Here with we present a male patient with left upper lobe pulmonary agenesis with left unilateral pulmonary artery agenesis (UPAA) which was detected and diagnosed for the first time at the ripe age of 70 years. Here we also review the relevant literature regarding the condition and the radiology including CT scan is described.

Sadly, the relationship between the American people and our government is currently in a dismal state. This relationship between the people and the government has become unbalanced and unfair, reducing the likelihood of change and deterring individuals from believing in their ability to influence such reform. The need to understand our capacity to effect change, though, is absolutely necessary. The issues facing the American government at this time are as numerous as they are serious, but ideas and proposals are coming forward with the potential to rebalance this relationship. We studied bias dependence of spin transfer torque in MgO-based magnetic tunnel junction using a ... more We studied bias dependence of spin transfer torque in MgO-based magnetic tunnel junction using a fieldmodulated spin torque ferromagnetic resonance measurement technique for three devices with tunneling magnetoresistance (MR) of 60%, 67%, and 73%. The devices with lower MR ratio showed the presence of multiple modes, while the device with higher MR device (73%), showed a single resonance mode. We found a lower out-of-plane torkance in our devices compared to the in-plane torkance. The out-of-plane torque is linear with applied bias while the bias dependence of in-plane torque shows a strong dependence on the MR ratio and hence the barrier quality.

Contemporary discourses on Afghanistan at the academic, policy and journalistic levels are heavil... more Contemporary discourses on Afghanistan at the academic, policy and journalistic levels are heavily suffused by attempts to map the future course of events as the shadow of the US-led military-intervention recedes. The editors of this volume aptly remind us of how the state of contemporary narratives on Afghanistan grates sharply against the relative amnesia of the international community towards the country, following the collapse of the USSR. The events of 9/11 have redirected international spotlight onto the country. The impact of 15 long years of the US-led international military intervention in the country has left a strong imprint in shaping the contours of issues and actors at stake. There has been a perceptible shift in the discourse which, in the initial years following the intervention, was impregnated by issues of post-conflict reconstruction , creating inclusive structures of governance, reconciliation, institutionally entrenching protection of women and human rights to a near overarching securiti-sation of structures, issues and actors involved in Afghanistan. This work is reflective of this shift of ground realities in Afghanistan. A discernable sense of pervasive anxiety and pessimism colours discourses on potential course of future events. A steady intensification of the scale and scope of violence coupled with a troubled political transition has served to reinforce these perceptions. What are the likely trajectories of events in a post-American Afghanistan? How would internal sociopolitical and external geopolitical dynamics shape the contours of the future trajectory of events? What would be its impacts on the region and its security and political implications for the seemingly distant Western world? What mechanisms could be deployed to negotiate some of the key challenges that lie ahead? This book attempts to grapple with some of these questions by stringing together a diverse collection of essays, originally presented at seminar organised by the South Asian University, funded by the Konrad Adenauer Stiftung in 2013. The book has been organised under four sections. The first section analyses Afghanistan's engagement in the international arena by bringing the Afghan setting in conversation with theoretical frames of reference. The second section

An important property of spin-torque nano-oscillators (STNOs) is their ability to produce a frequ... more An important property of spin-torque nano-oscillators (STNOs) is their ability to produce a frequency modulated (FM) signal, which is very critical for communication applications. We here demonstrate a novel single sideband (SSB) modulation phenomenon using a magnetic tunnel junction (MTJ)-based STNO, which saves transmission bandwidth and in principle should minimize attenuation for wireless communication. Experimentally, lower single sidebands (LSSBs) have been successfully demonstrated over a wide range of modulation frequency, f m = 150 MHz-1 GHz. The observed LSSBs are determined by the intrinsic properties of the device, which can be modeled well by a nonlinear frequency and amplitude modulation formulation and reproduced in macrospin simulations. Moreover, our macrospin simulation results show that the range of modulation current and modulation frequency for generating SSBs can be controlled by the field-like torque and biasing conditions.

We here demonstrate a novel single sideband (SSB) modulation phenomenon using a magnetic tunnel junction (MTJ)-based STNO, which saves transmission bandwidth and in principle should minimize attenuation for wireless communication. Experimentally, lower single sidebands (LSSBs) have been successfully demonstrated over a wide range of modulation frequency, f m = 150 MHz-1 GHz. The observed LSSBs are determined by the intrinsic properties of the device, which can be modeled well by a nonlinear frequency and amplitude modulation formulation and reproduced in macrospin simulations. Moreover, our macrospin simulation results show that the range of modulation current and modulation frequency for generating SSBs can be controlled by the field-like torque and biasing conditions.
