To determine cerebrospinal fluid levels of interleukin-6 and tumor necrosis factor-alpha in full-... more To determine cerebrospinal fluid levels of interleukin-6 and tumor necrosis factor-alpha in full-term infants with hypoxic-ischemic encephalopathy, comparing with control infants. Controlled, prospective study, performed between July 1999 and October 2001 with two groups of full-term newborns: 20 controls with no sepsis and/or meningitis and Apgar score > or =9 at first and fifth minutes; and cases, 15 asphyxiated full-term newborns with Apgar < or =4 and < or =6 at first and fifth minutes, umbilical blood cord pH<7.20 and/or umbilical arterial blood lactate>3.0 mmol/L, and requiring positive pressure ventilation for at least 2 minutes after birth. Background Angiogenic and antiangiogenic factors are altered in pregnant women with preeclampsia ... more Background Angiogenic and antiangiogenic factors are altered in pregnant women with preeclampsia (PE), but the pattern of expression of these factors in their newborns remains unknown. Objective This study aims to measure vascular endothelial growth factor (VEGF) and soluble fms-like tyrosine kinase 1 (sFlt-1) levels in preterm neonates born to mothers with PE. Methods Neonates with birth weight < 2,000 g and gestational age ≤ 34 weeks were included and divided into the following two groups: born to mothers with PE and without PE. Blood was collected from neonates within the first 72 hours of life. VEGF and sFlt-1 levels were measured using the enzyme-linked immunosorbent assay method. Results A total of 88 neonates were included (37 born to mothers with PE and 51 born to mothers without PE), with a mean gestational age of 29.12 ± 2.96 weeks and birth weight of 1,223.80 ± 417.48 g. Early nCPAP seems to prevent ventilator-induced lung injury in humans, although the pathophysiolo... more Early nCPAP seems to prevent ventilator-induced lung injury in humans, although the pathophysiological mechanisms underlying this beneficial effect have not been clarified yet. To evaluate plasma levels IL-1β, IL-6, IL-8, IL-10, and TNF-α immediately before the start of nCPAP and 2 hours later in preterm infants. Prospective cohort including preterm infants with 28 to 35 weeks gestational age with moderate respiratory distress requiring nCPAP. Extreme preemies, newborns with malformations, congenital infections, sepsis, surfactant treatment, and receiving ventilatory support in the delivery room were excluded. Blood samples were collected right before and 2 hours after the start of nCPAP. 23 preterm infants (birth weight 1851±403 grams; GA 32.3±1.7 weeks) were treated with nCPAP. IL-1β, IL-10, TNF-α levels were similar, IL-8 levels were reduced in 18/23 preterm infants and a significant decrease in IL-6 levels was observed after 2 hours of nCPAP. All newborns whose mothers received antenatal steroids had lower cytokine levels at the onset of nCPAP than those whose mothers didn&amp;amp;amp;amp;amp;amp;amp;#39;t receive it; this effect was not sustained after 2 hours of nCPAP. Early use nCPAP is not associated with rising of plasma pro-inflammatory cytokines and it seems to be a less harmful respiratory strategy for preterm with moderate respiratory distress.

The study aimed to determine the incidence of congenital infection by Toxoplasma gondii and to de... more The study aimed to determine the incidence of congenital infection by Toxoplasma gondii and to describe neonatal and maternal characteristics regarding newborn infants treated at a teaching hospital in the town of Passo Fundo, State of Rio Grande do Sul, Brazil. Cord blood samples collected from 1,250 live newborns were analyzed. The laboratory diagnosis was established by the detection of Toxoplasma gondii IgM using an enzyme linked fluorescent assay. Gestational age, intrauterine growth, anthropometric measures, and prenatal characteristics were assessed. The incidence of congenital toxoplasmosis at birth was 8/10,000 (95%CI 0.2-44.5). Mean birthweight was 3,080 +/- 215.56 grams and mean gestational age was 38.43 +/- 1.88 weeks. With regard to prenatal care, 58% of the pregnant patients visited their doctors five times or more and 38.9% were serologically tested for toxoplasmosis in the first trimester of pregnancy. The incidence of congenital toxoplasmosis was similar to that found in most studies conducted in our country and abroad. Our study sample is representative of the town of Passo Fundo and therefore it is possible to consider the frequency observed as the prevalence of the disease in this town during the study period.

To evaluate the prevalence of hearing alterations in very low birth weight patients in the Neonat... more To evaluate the prevalence of hearing alterations in very low birth weight patients in the Neonatal Intensive Care Unit of the Hospital de Clínicas de Porto Alegre and to study the variables that can be related to alterations of the hearing acuity. A transversal study was carried out. The study included all very low birth weight neonates admitted to the Neonatal Intensive Care Unit of the Hospital de Clínicas de Porto Alegre from September 1, 2000 to January 31, 2002. All patients were submitted to distortion product otoacoustic emission tests at hospital discharge. When the test showed alterations, it was repeated 30 days later. However, when the same patient presented otoacoustic emission alterations in both tests, he/she was submitted to the auditory brain response testing. The result of this test was considered abnormal from 35 dB NA. We studied 96 neonates. Six children presented alteration in the distortion product otoacoustic emission test as well as in the auditory brain response test. Children&amp;amp;amp;#39;s mean age was 31.5 +/- 2.6 weeks, their birthweight was between 640 g and 1,500 g, and 57.3% of the patients were female. The gestational age and the Apgar score of 5 minutes were inferior in the group presenting abnormal results of otoacoustic emission and auditory brain response tests compared to the other groups, reaching bordering significance. All the remaining aspects investigated did not present any statistically significant difference. The prevalence of hearing loss in the very low birth weight neonates cared for at the Neonatal Intensive Care Unit of the HCPA was 6.3%, and bordering significance associations related to gestational age and 5 minutes Apgar score were observed.

Late-onset sepsis (LOS) is an important cause of morbidity and mortality in very low birth weight... more Late-onset sepsis (LOS) is an important cause of morbidity and mortality in very low birth weight (VLBW) infants. To determine the incidence, risk factors and etiology of LOS. LOS was investigated in a multicenter prospective cohort of infants at eight public university neonatal intensive care units (NICUs). Inclusion criteria included inborn, 23-33 weeks of gestational age, 400-1499 g birth weight, who survived >3 days. Of 1507 infants, 357 (24%) had proven LOS and 345 (23%) had clinical LOS. Infants with LOS were more likely to die. The majority of infections (76%) were caused by Gram-positive organisms. Independent risk factors for proven LOS were use of central venous catheter and mechanical ventilation, age at the first feeding and number of days on parenteral nutrition and on mechanical ventilation. LOS incidence and mortality are high in Brazilian VLBW infants. Most risk factors are associated with routine practices at NICU.
