Background and aim of the study Several environmental factors contribute to making asthma more se... more Background and aim of the study Several environmental factors contribute to making asthma more severe and could accelerate the deterioration of airway calibre in asthmatic subjects. Among these factors, sensitization to moulds, particularly Aspergillus, deserves to be explored. We sought to determine the effect of sensitization to Aspergillus fumigatus on changes in airway calibre in asthmatic patients over a period of at least five years. Material and methods In a retrospective case-control study, 133 asthmatic subjects between the ages of 18 and 40 at first evaluation were divided into two groups: sensitized and not sensitized to Aspergillus. They were paired according to age, sex and FEYI at the first consultation (in % pred). Clinical, functional, allergic and therapeutic parameters were examined. The effect of smoking was taken into account by exc1uding smokers (more than 10 packsyears), as weIl as subjects affected by a concomitant respiratory disease.

This study aims at investigating the role of the presence or the absence of Aspergillus sensitization in the deterioration of airway caliber in asthmatics over a five-year follow-up. One hundred and thirty-three asthmatic subjects aged 18 to 40 years were separated into two groups: those sensitized (cases) and those not sensitized (controls) to Aspergillus were matched according to the following criteria: age, gender and FEV1 (% pred). Clinical, functional, allergic and therapeutic parameters were studied. The FEV1 slope was less pronounced in the group sensitized to Aspergillus (-17 mL/year) in comparison to the controls (27 mL/year). There was also a less noticeable slope of forced vital capacity in the sensitized subjects (-25 mL/year) as compared to the controls (28 mL/year). All of these differences were not statistically significant. We did not find any increase in the decline in FEV1 or FVC associated with sensitization to Aspergillus.
