Objective Intraoperative ultrasound (IOUS) is an imaging method that can be used in various neuro... more Objective Intraoperative ultrasound (IOUS) is an imaging method that can be used in various neurosurgical procedures. It assists in the removal of brain tumors, hematomas, in the insertion of ventricular catheters, and in spinal and peripheral nerve surgeries. The original studies using IOUS were performed with devices that produced lower-quality images, generating little interest in their use. The introduction of ultrasound devices that yielded higher-quality images associated with a relative low cost and ease of use rendered IOUS attractive. Thus, we started using it in multiple neurosurgical procedures, studying its practicality, efficacy and limitations. Materials and methods A retrospective case study on the use of IOUS in neurosurgery from August 2014 to December 2020 at a single institution, evaluating its practicality, efficacy and limitations. Introdução Lipomas são tumores benignos do tecido adiposo, sendo o mais comum dentre os tumores d... more Introdução Lipomas são tumores benignos do tecido adiposo, sendo o mais comum dentre os tumores de partes moles na vida adulta. O subtipo mais frequente é o lipoma convencional, que se caracteriza por uma massa bem encapsulada de adipócitos de tamanhos variados, localizando-se principalmente nas extremidades proximais e tronco. Os Palavras-Chave ► angiolipoma intracraniano ► aneurisma sacular ► cefaleia ► neoplasia cerebral Resumo Angiolipomas são neoplasias benignas compostas por adipócitos maduros e proliferação vascular angiomatosa de rara ocorrência no Sistema Nervoso Central. É relatado o caso de angiolipoma em cisterna silviana adjacente a aneurisma sacular de artéria cerebral média. São discutidas as características imagenológicas do tumor e a possibilidade de uma origem comum com lesões vasculares intracranianas. Este é o primeiro caso relatado de angiolipoma intracraniano associado a aneurisma de artéria cerebral média.

Vein of Galen aneurysm is a rare pathology, representing less than 1% of intracranial vascular ma... more Vein of Galen aneurysm is a rare pathology, representing less than 1% of intracranial vascular malformations. We report on a 65 years-old man who experienced a generalized tonic-clonic seizure. Brain imaging showed a large calcified expanding mass in the pineal region, confirming the diagnosis of a vein of Galen aneurysm. Because of the spontaneous thrombosis of the malformation, there was no need for microsurgical or endovascular treatment and he is been regularly followed since that.
The thoracic outlet syndrome is a complex clinical entity that encompasses several situations whe... more The thoracic outlet syndrome is a complex clinical entity that encompasses several situations where there is compression of the neurovascular bundle that supplies the upper limbs in the cervicothoracic channel. It can be divided in neurogenic and vascular according to the structure committed, the first being the most frequently found. Among the causes described there are cervical ribs, anomalies of the first rib, anomalous or hypertrophied muscles, trauma and tumors of the region. We report the case of a 47 year old patient who presents with a clinical picture of cervicobrachialgia associated with paresthesia and paresis on the left hand compatible with thoracic outlet syndrome in which the cause of compression was the descending scapular artery.We conducted a literature review and found only one similar case.

Brachial plexus neuropathies are uncommon and are rarely caused by a tumor. The clinical presenta... more Brachial plexus neuropathies are uncommon and are rarely caused by a tumor. The clinical presentation of a brachial plexus neuropathy caused by a tumor depends on the degree of malignancy of the tumor and its localization. We report an illustrative case of a 27-year old female subject with a progressively increasing mass lesion causing brachial plexus compression, ipsilateral shoulder pain, C8 dermatomal paresthesia, and impairment of motor power. The patient underwent surgical resection of the mass and neurolysis of the nerves in the vicinity. Following the surgical procedure, the patient had improvement in the pain and paresthesia that he was suffering from. The histopathological diagnosis revealed a hibernoma, an extremely rare tumor described only once previously in this location. A systematic review of the literature was performed utilizing the PubMed database to access articles published before March 2018, using: A – the term 'hibernoma' in the title/abstract associated with the following MeSH terms: brachial plexus neuropathies OR brachial plexus neuropathy OR nerve compression syndrome, OR brachial plexus; B – the MeSH term 'brachial plexus' associated with the term 'non neural sheath nerve tumor' or 'peripheral non-neural sheath nerve tumor'. The origin of the hibernoma, as well as its metabolic influence, pathology, and treatment have been discussed.

Peripheral nerve sheath tumors (PNST) develop sporadically or are associated with genetic disorde... more Peripheral nerve sheath tumors (PNST) develop sporadically or are associated with genetic disorders and affect individuals mainly between the third and sixth decades of life. PNST affect one or multiple fascicles. The benign ones have slow and painless growth, preceding in years the diagnosis. Diagnosis is often confused with other soft tissue tumors, such as fibroids and lipomas. We present a rare case of giant schwannoma of the median nerve. The patient had a progressive local growth with minimal neurological impairment.

A lesão foi completamente ressecada utilizando-se um procedimento guiado por imagem, sem que houvesse alguma deterioração neurológica. A literatura mostra uma morbi-mortalidade de cerca de 20% para tais lesões. A utilização da nova tecnologia de assimilação de imagem neurodiagnóstica no procedimento cirúrgico é documentada.
