Patients and/or attendants were asked questions regarding PPD ingestion reason, evidence of poison ingestion, symptoms, color, and route and time interval to reach hospital. All of the collected were entered in SPSS and analyzed using SPSS version 21. Descriptive statistics in terms of mean ± standard deviation or frequencies with percentages were computed to summarize the collected data as appropriate. Result: Total 44 patients came to emergency department with diagnosis of black stone (kalapathar poisoning) as evident by patient or by attendant. Out of them 29 patients were female (65.9%). Majority of patients 35 (79.54%) were age between 15-30 years Most of the patients 41 (93.18%) took more than 2 hours to reach hospital patients were of age between 41-50 years. Forty-two (95.5%) out of 44 ingested PPD with an intention to suicide while only 2 (4.5%) patients ingested PPD accidentally. Forty patients (90%) had severe tongue and neck swelling and underwent emergency tracheostomy while on the other hand, only 4 patients (9.09%) presented with minor tongue swelling and cervicofacial edema impending stridor clinically. Out of these 4 patients, 2 further underwent emergency tracheostomy. Conclusion: Paraphenylenediamine poisoning is commonly used in Pakistan for suicidal purpose especially in females as its easily available and cheap. All of the patients need Emergency tracheostomy due to angioneurotic edema leads to asphyxia. Therefore, we need to spread awareness so that its availability should be banned and Psychiatrists should do counseling of such patients for their mental health.

This prospective consecutive case series was done to evaluate indications, technical pitfalls and... more This prospective consecutive case series was done to evaluate indications, technical pitfalls and functional outcome of elastic stable intramedullary nailing of displaced midclavicular fractures in 34 high demand individuals. Constant score and radiographs were evaluated after 1 and 6 weeks, 3 months and 6 months in 34 patients. Mean age was 28 years. The average follow-up was 12 months. Mean operation time was 42 minutes. Open reduction through an additional small incision was necessary for 7 fractures. Mean hospital stay was 1.2 days. Secondary fracture displacement was observed in 1 patient. The Constant score averaged 81 after 1 week, 96 after 6 weeks and 98 after 6 months. Compared to the contralateral side, average shortening of the clavicle was 1.7 mm. Overall, elastic stable intramedullary nailing provided good restoration of the length of the clavicle and allowed immediate active mobilisation with early return to normal activity. Functional results were excellent.

Lipid levels and diabetic indices were measured, and the correlations among them were analyzed. Results: Positive correlations were found between triglycerides and fasting blood sugar, low density lipoprotein and Insulin when compared among myocardial infarction patients. The current study explores psychophysiological modulation in diabetic distress in response to Surah Al-Rehman recitation. A single-group experimental study with before and after intervention assessments was conducted during April 2018 to February 2019 in which participants (n = 10) listened to Surah Al-Rehman recitation. Blood pressure, blood glucose, cortisol, ECG and EEG were recorded before and after recitation. Significant reduction in systolic blood pressure, increase in low frequency and absolute alpha power at Fp2 were observed. Heartbeat evoked potentials (HEP) were also significantly modulated at Fp1, Fp2 and F4. These findings suggest that Surah Al-Rehman modulated baroreflex activity thereby reducing sympathetic activity and improved heart brain coherence as reflected by HEP.

MI patients between the age of 40 and 60 years. Patients came to NICVD with complaints of chest pain, positive Troponin T test and ECG was the confirmatory test for MI. They were divided into 2 groups 100 patients each. First group comprised of MI patients without DMT-II and second group comprised of MI patients with DMT-II. Serum triglycerides, cholesterol, LDL and HDL, FBS, by enzymatic kits, Insulin by RIA. HbA1C, Interleukin-1 β and CRP by ELIZA. Interleukin1β and CRP were significantly higher (P<0.001) in patients at the time of the infarction, prior to angioplasty as compared post angioplasty levels in both groups, which indicate their importance in development of ischemia and MI. Head and neck Merkel cell carcinoma (MCC) is commonly treated with surgery and adjuvant radiother... more Head and neck Merkel cell carcinoma (MCC) is commonly treated with surgery and adjuvant radiotherapy (RT) for high-risk features. The optimal radiation dose is unknown. One thousand six hundred twenty-five eligible patients with head and neck MCC were identified in the National Cancer Data Base (NCDB). Radiation dose was divided into 3 groups: 30 to <50 Gray (Gy), 50-55 Gy, and >55-70 Gy. Cox regression was used to compare overall survival (OS) between groups, accounting for age, sex, stage, surgery type, margin status, comorbidities, and use of chemotherapy. With a median follow-up of 33.5 months, 3-year OS was 48.9%, 70.3%, and 58.7% for 30 to <50 Gy, 50-55 Gy, and >55-70 Gy, respectively (P < .001). Outbreaks of Middle East respiratory syndrome (MERS) raise questions about the prevalence and evo... more Outbreaks of Middle East respiratory syndrome (MERS) raise questions about the prevalence and evolution of the MERS coronavirus (CoV) in its animal reservoir. Our surveillance in Saudi Arabia in 2014 and 2015 showed that viruses of the MERS-CoV species and a human CoV 229E-related lineage co-circulated at high prevalence, with frequent co-infections in the upper respiratory tract of dromedary camels. Including a betacoronavirus 1 species, we found that dromedary camels share three CoV species with humans. Several MERS-CoV lineages were present in camels, including a recombinant lineage that has been dominant since December 2014 and that subsequently led to the human outbreaks in 2015. Camels therefore serve as an important reservoir for the maintenance and diversification of the MERS-CoVs and are the source of human infections with this virus.

Spinal ependymomas are most frequently found in intramedullary region but few cases of intradural extramedullary ependymoma have also been reported. We report a 24-year-old male patient with a suspected case of intradural extramedullary ependymoma. Magnetic resonance images of the lumbar spine depicted an intradural mass from L1-L4 level. The spinal lesion was isointense on T1-weighted images and hyperintense on T2-weighted images, relative to the spinal cord. Laminectomy L1-L4 with gross-total excision was performed. Histopathological examination was inconclusive but suggested the possibility of ependymoma. Neurological recovery was initially observed but after few months symptoms worsened.

Exposure to biomass fuel during pregnancy lead to LBW, the objective of this study was to determine the association of exposure to biomass fuel and LBW. Methods: In total, 10,023 ever married women were interviewed reported 41,094 births for last 5 years from Pakistan Demographic Health Survey 2006–07. Logistic regression models were used to assess the association between biomass exposure and birth weight after adjusting for demographic, maternal, and child characteristics. Odds ratios (ORs), 95% confidence interval (CI) and P value were calculated. Results: The proportion of LBW was 35.4% ( n = 1777) in common high polluted biomass fuel (wood), 36.3% ( n = 282) in less common high polluted biomass fuel (electricity, cylinder gas, and biogas) and 29.5% ( n = 805) in common low polluted biomass fuel (natural gas). Infants born to common high polluted biomass fuel (wood) users were 28% more likely to have LBW (OR: 1.28, 95% CI: 1.02–1.61, P = 0.03) compared with infants born to common low polluted biomass fuel (natural gas) users while significant positive association of less common high polluted biomass fuel with LBW (OR: 1.41, 95% CI: 1.07–1.84, P value 0.01) after adjusting for potential confounders. Conclusion: Biomass fuel exposure during pregnancy was significantly associated with LBW of child. There is need for reduce the exposure of polluted biomass fuel and replace with low biomass fuel to reduced burden of LBW.

Objective of my study is an assessment of factors leading to the development of CSOM complications in different age groups and which age group is more prone to the development of complications. And to find out most frequent complication occurring in the target population. Introduction: CSOM is a long standing infection of a part or whole of the middle ear cleft characterized by ear discharge and a permanent perforation. It affects both sexes and all age groups. Pathways of spread of infection: Direct bone erosion, venous thrombophlebitis, preformed pathways. Classification of complications: Intracranial - Mastoiditis, facial paralysis, labrynthitis, petrositis. Extracranial - Meningitis, extradural abscess, subdural abscess, brain abscess, lateral sinus thrombophlebitis, otitic hydrocephalus. Development of complications depends upon; organism′s virulence, poor resistance of the patient, systemic disease, resistance to antibiotics, poor socioeconomic status, lack of awareness about healthcare, lack of availability of trained specialist. Materials and Methods: A descriptive cross-sectional study was carried out from 2008 to 2013 in ENT Head and Neck Surgery Department, Jinnah Postgraduate Medical Centre, Karachi. Record of 65 patients was selected through nonprobability convenient sampling; sample was divided into two groups according to age Group A 10-18 years and Group B 18-45 years. Data were analyzed on SPSS software; Chi-square test was used and P < 0.05 was considered significant with confidence interval taken as 95%. Results: Population means age was found to be 18.28. Population sex dominant was male 63.1%. Mean duration of complaints is 1.69. There are strong associations between duration of complaints and family income (P = 0.05), and also between age of patient and diagnosed complication (P = 0.00). Conclusions: Most of the patients were of poor socioeconomic background with disease negligence, delayed treatment, poor access to health facilities and improper use of antibiotics. Mostly low age group patients develop complications.

Methodology: All breast biopsies received at the Dow Diagnostic Research and Reference laboratory (DDRRL) during 2008-2012 were included in the analyze the spectrum with implementation of latest intrinsic molecular subtyping. Results: Out of 2740 cases, 1493 (54.50%) were diagnosed as benign and 1247 (45.40%) as malignant lesions. Most frequent lesion in male was gynecomastia. However, in females, most common benign lesion was fibroadenoma with a frequency of 745 (27.91%), while invasive ductal carcinoma II was the most common malignant lesions observed in 584 (21.88%) cases. Mean age of patients with benign lesion was 30 years and malignant lesion was 45 years. Selected cases (n=460) were stained for ER, PgR and Her-2 expression. Mean operation time was 42 minutes. Open reduction through an additional small incision was necessary for 7 fractures. Mean hospital stay was 1.2 days. Secondary fracture displacement was observed in 1 patient. The Constant score averaged 81 after 1 week, 96 after 6 weeks and 98 after 6 months. Compared to the contralateral side, average shortening of the clavicle was 1.7 mm. Overall, elastic stable intramedullary nailing provided good restoration of the length of the clavicle and allowed immediate active mobilisation with early return to normal activity. Functional results were excellent.

The current study explores psychophysiological modulation in diabetic distress in response to Surah Al-Rehman recitation. A single-group experimental study with before and after intervention assessments was conducted during April 2018 to February 2019 in which participants (n = 10) listened to Surah Al-Rehman recitation. Blood pressure, blood glucose, cortisol, ECG and EEG were recorded before and after recitation. Significant reduction in systolic blood pressure, increase in low frequency and absolute alpha power at Fp2 were observed. Heartbeat evoked potentials (HEP) were also significantly modulated at Fp1, Fp2 and F4. These findings suggest that Surah Al-Rehman modulated baroreflex activity thereby reducing sympathetic activity and improved heart brain coherence as reflected by HEP.

MI patients between the age of 40 and 60 years. Patients came to NICVD with complaints of chest pain, positive Troponin T test and ECG was the confirmatory test for MI. They were divided into 2 groups 100 patients each. First group comprised of MI patients without DMT-II and second group comprised of MI patients with DMT-II. Serum triglycerides, cholesterol, LDL and HDL, FBS, by enzymatic kits, Insulin by RIA. HbA1C, Interleukin-1 β and CRP by ELIZA. The optimal radiation dose is unknown. One thousand six hundred twenty-five eligible patients with head and neck MCC were identified in the National Cancer Data Base (NCDB). Radiation dose was divided into 3 groups: 30 to <50 Gray (Gy), 50-55 Gy, and >55-70 Gy. Cox regression was used to compare overall survival (OS) between groups, accounting for age, sex, stage, surgery type, margin status, comorbidities, and use of chemotherapy. With a median follow-up of 33.5 months, 3-year OS was 48.9%, 70.3%, and 58.7% for 30 to <50 Gy, 50-55 Gy, and >55-70 Gy, respectively (P < .001). Outbreaks of Middle East respiratory syndrome (MERS) raise questions about the prevalence and evo... more Outbreaks of Middle East respiratory syndrome (MERS) raise questions about the prevalence and evolution of the MERS coronavirus (CoV) in its animal reservoir. Our surveillance in Saudi Arabia in 2014 and 2015 showed that viruses of the MERS-CoV species and a human CoV 229E-related lineage co-circulated at high prevalence, with frequent co-infections in the upper respiratory tract of dromedary camels. Including a betacoronavirus 1 species, we found that dromedary camels share three CoV species with humans. Several MERS-CoV lineages were present in camels, including a recombinant lineage that has been dominant since December 2014 and that subsequently led to the human outbreaks in 2015. Camels therefore serve as an important reservoir for the maintenance and diversification of the MERS-CoVs and are the source of human infections with this virus.

Spinal ependymomas are most frequently found in intramedullary region but few cases of intradural extramedullary ependymoma have also been reported. We report a 24-year-old male patient with a suspected case of intradural extramedullary ependymoma. Magnetic resonance images of the lumbar spine depicted an intradural mass from L1-L4 level. The spinal lesion was isointense on T1-weighted images and hyperintense on T2-weighted images, relative to the spinal cord. Laminectomy L1-L4 with gross-total excision was performed. Histopathological examination was inconclusive but suggested the possibility of ependymoma. Neurological recovery was initially observed but after few months symptoms worsened.

Exposure to biomass fuel during pregnancy lead to LBW, the objective of this study was to determine the association of exposure to biomass fuel and LBW. Methods: In total, 10,023 ever married women were interviewed reported 41,094 births for last 5 years from Pakistan Demographic Health Survey 2006–07. Logistic regression models were used to assess the association between biomass exposure and birth weight after adjusting for demographic, maternal, and child characteristics. Odds ratios (ORs), 95% confidence interval (CI) and P value were calculated. Results: The proportion of LBW was 35.4% ( n = 1777) in common high polluted biomass fuel (wood), 36.3% ( n = 282) in less common high polluted biomass fuel (electricity, cylinder gas, and biogas) and 29.5% ( n = 805) in common low polluted biomass fuel (natural gas). Infants born to common high polluted biomass fuel (wood) users were 28% more likely to have LBW (OR: 1.28, 95% CI: 1.02–1.61, P = 0.03) compared with infants born to common low polluted biomass fuel (natural gas) users while significant positive association of less common high polluted biomass fuel with LBW (OR: 1.41, 95% CI: 1.07–1.84, P value 0.01) after adjusting for potential confounders. Conclusion: Biomass fuel exposure during pregnancy was significantly associated with LBW of child. There is need for reduce the exposure of polluted biomass fuel and replace with low biomass fuel to reduced burden of LBW.

Introduction: CSOM is a long standing infection of a part or whole of the middle ear cleft characterized by ear discharge and a permanent perforation. It affects both sexes and all age groups. Pathways of spread of infection: Direct bone erosion, venous thrombophlebitis, preformed pathways. Classification of complications: Intracranial - Mastoiditis, facial paralysis, labrynthitis, petrositis. Extracranial - Meningitis, extradural abscess, subdural abscess, brain abscess, lateral sinus thrombophlebitis, otitic hydrocephalus. Development of complications depends upon; organism′s virulence, poor resistance of the patient, systemic disease, resistance to antibiotics, poor socioeconomic status, lack of awareness about healthcare, lack of availability of trained specialist. Materials and Methods: A descriptive cross-sectional study was carried out from 2008 to 2013 in ENT Head and Neck Surgery Department, Jinnah Postgraduate Medical Centre, Karachi. Record of 65 patients was selected through nonprobability convenient sampling; sample was divided into two groups according to age Group A 10-18 years and Group B 18-45 years. Data were analyzed on SPSS software; Chi-square test was used and P < 0.05 was considered significant with confidence interval taken as 95%. Results: Population means age was found to be 18.28. Population sex dominant was male 63.1%. Mean duration of complaints is 1.69. There are strong associations between duration of complaints and family income (P = 0.05), and also between age of patient and diagnosed complication (P = 0.00). Conclusions: Most of the patients were of poor socioeconomic background with disease negligence, delayed treatment, poor access to health facilities and improper use of antibiotics. Mostly low age group patients develop complications.

Methodology: All breast biopsies received at the Dow Diagnostic Research and Reference laboratory (DDRRL) during 2008-2012 were included in the analyze the spectrum with implementation of latest intrinsic molecular subtyping. Results: Out of 2740 cases, 1493 (54.50%) were diagnosed as benign and 1247 (45.40%) as malignant lesions. Most frequent lesion in male was gynecomastia. However, in females, most common benign lesion was fibroadenoma with a frequency of 745 (27.91%), while invasive ductal carcinoma II was the most common malignant lesions observed in 584 (21.88%) cases. Mean age of patients with benign lesion was 30 years and malignant lesion was 45 years. Selected cases (n=460) were stained for ER, PgR and Her-2 expression.
