So far the pathophysiology of infection due to coronavirus 2019 (covid 19) is not well elucidated... more So far the pathophysiology of infection due to coronavirus 2019 (covid 19) is not well elucidated, however, several studies suggest that there is a link between the coagulation system and Sars-coV2. The objective of this work is to study the variability of coagulation parameters in patients infected with covid-19. Patients and methods: We have collected 102 confirmed Covid 19 infected patients (group 1), hospitalized at the Covid 19 center of the CHU Mohamed VI of Marrakesh, we have studied the coagulation parameters in these patients and comparing them with a group of control patients (group 2) who did not present any coronavirus infection. Results: The mean age was 46 years in group 1 and 40 years in group 2, with male predominance in both groups (55.4% in G1 and 52.8% in G2). D-dimer (1.35±2.1 vs 0.18±0.05, p<0.001) and fibrinogen (4.04 (±1.85) vs 2.13 (±0.308), p <0.001) levels in G1 are elevated relative to G2, while prothrombin levels are decreased (84.7 (±16.9) vs 93.1 (±6.60), p<0.01). We did not observe a significant difference between the two groups with respect to platelet count, International Normalized Ratio (INR) and APTT (p >0.05). D-dimer and fibrinogen values in severe Covid 19 patients were higher than in non-severe Covid 19 patients (p<0.001). Conclusion: Clotting factors in Sars cov 2 patients are significantly impaired compared to those in the control group. The high level of fibrinogen and D-dimer may be a marker for the early identification of severe cases.

Le pneumothorax et le pyopneumothorax tuberculeux sont des formes rares mais graves de la tuberculose, ils viennent compliquer le plus souvent une tuberculose pulmonaire cavitaire. Methodes Nous avons realise une etude retrospective descriptive et analytique des cas de pneumothorax et d’hydropneumothorax tuberculeux hospitalises en service de pneumologie du CHU Mohammed VI de Marrakech durant la periode allant de janvier 2015 au juillet 2019. Resultats Nous avons collige 37 cas de pneumothorax et d’hydropneumothorax d’origine tuberculeux. L’âge moyen etait de 39,45 ans (21–93 ans), avec une predominance masculine nette (sex-ratio = 2,32). Il s’agissait de 5 cas de tuberculose pulmonaire a microscopie positive, 2 cas de rechute tuberculeuse et un cas de miliaire tuberculeuse. Un contage tuberculeux recent etait present (16,2 %) et le tabagisme actif (54 %). Parmi les tares associees : un diabete et une cardiopathie dans 13,5 % des cas chacun, une psychose (8 %) et un cas d’adenocarcinome pulmonaire. La douleur thoracique, la dyspnee et la fievre etaient les symptomes majeurs. La radiographie thoracique avait objective un epanchement aerique (38 %) et mixte (62 %). Il etait a droite (57 %). Une atteinte parenchymateuse controlaterale etait presente (70 %). La recherche de BK a l’examen direct des expectorations et par geneXpert etait positive (38 %), par geneXpert dans le liquide d’aspiration bronchique (19 %) et dans le liquide pleural (21 %), la confirmation bacteriologique par culture de BK dans les expectorations etait positive chez 11 % des cas, et dans le liquide pleural chez un cas, le diagnostic etait retenu par presence d’un granulome tuberculoide a l’etude anatomopathologique des biopsies pleurales chez 3 cas. La prise en charge therapeutique associait un traitement anti-bacillaire et une kinesitherapie respiratoire dans tous les cas. Tous nos patients ont ete draines d’emblee avec un taux de succes de drainage de 49 %, le recours a la chirurgie etait necessaire chez 19 cas (51 %) avec un taux de reussite de 53 %. Des lesions sequellaires etaient notees chez 16,5 % des cas. Conclusion A travers notre etude, le pneumothorax tuberculeux est une forme rare qui peut reveler ou compliquer une tuberculose evolutive. L’evolution est le plus souvent favorable sous traitement antituberculeux et drainage thoracique mais des sequelles pleurales a type de pachypleurite ayant necessite le recours a la thoracoscopie.

Until now, there is no specific vaccine and no effective treatment, so it is very important from a clinical point of view to predict the evolution of the disease. Thus, in order to further analyze the value of the clinical application of blood count parameters in the diagnosis or management of COVID-19 (Coronavirus disease 2019) patients. Peripheral blood abnormalities of 102 COVID-19 patients hospitalized in the pneumology department of CHU Mohamed VI in Marrakesh were analyzed retrospectively and compared to SARS CoV 2 negative patients. Swallowing disorders are common manifestations in patients with advanced lung cancer, this paper ... more Swallowing disorders are common manifestations in patients with advanced lung cancer, this paper describes a rare clinical case of a foreign body (chicken bone) accidently found in the lumen of the left upper lobe bronchus during bronchoscopy. A 41 year-old man, with smoking history of 20 PA presented to our department with paraplegia and left arm paresis, he complained over 6 months from dyspnea, increasing posterior chest pain and persistent dry coughing. He had a history of swallowing disorders and difficulties with shocking episode passed unnoticed. The patient was apyretic but presented weight loss and weakness. On physical examination, he had a digital hippocratism, a WHO performance status was at 4 and there were diffuse snoring rales on pulmonary auscultation. The body CT scan showed a voluminous left upper mediastinal tumor involving the left upper lobe and the left apical pyramidal lobe, this tumor was extended to vascular elements and to the dorsal spine. The cerebral CT scan showed discreetly a compressive right parietal process measuring 3cm*3cm. The flexible bronchoscopy has been performed showing an extrinsic compression of the posterior wall of the trachea, a blood clot removed showing a piece of chicken bone presented in the lumen of the anterior segmental bronchus. The patient was referred to the oncology department for palliative treatment, after being diagnosed as lung adenocarcinoma by a CT guided lung biopsy. This is the first paper reported an accidently discovery of bronchial foreign body during bronchoscopy in a patient with stage IVB lung cancer. Dysphagia has been reported as common manifestation in patients with advanced lung cancer due to underlying mediastinal disease and brainstem lesions metastasis, it is considered as a disorder which may increase the risk of morbi-mortality and has a potential of life-threatening.

We report a case of an intra-cardiac tumor revealing a bronchial carcinoma. We report the case of a 64-year-old man who was a smoker at 40PA. He presented with a 3 months evolving symptomatology made of dyspnea associated to a productive cough with whitish sputum sometimes streaked with blood. The patient also complained of a retrosternal pain irradiating to the interscapular region. The patient was apyretic but presented anorexia and weight loss. On physical examination, the patient had a tachycardia at 100 beats per minute, a right basal fluid effusion syndrome, with a WHO performance status at 1.The electrocardiogram showed sinus tachycardia at 100 bpm with the presence of some ventricular extrasystoles. The cardiac echocardiography showed a hypoechoic mass in the left atrium attached to the upper wall, The thoracic CT scan showed a locally invasive heterogeneous tumor, localized in the right lower pulmonary lobe and the mediastinum with intracardiac extension to the left atrium. the bronchoscopy showed a friable whitish tumor bud obstructing the right stump bronchus bleeding on contact, with another bud obstructing the segmental bronchus of the left upper lobar bronchus bleeding easily on contact, biopsies of the bud and aspirations for cytodiagnosis were performed.

Presentation of case: We describe a case of a twenty years old woman, who complained from intermittent dry cough with bloody sputum for six months, clinical examination showed left basithoracic condensation syndrome, chest Xray revealed homogeneous dense opacity of the left paracardiac fold.
