The nasopharyngeal angiobroma is a very aggressive, vascular tumor that may represent a challeng... more The nasopharyngeal angiobroma is a very aggressive, vascular tumor that may represent a challenge for the anesthesiologist due to the risk of profuse bleeding. We report the case of a 20 years old, 50 kg, ASA1 patient, who was scheduled for bleeding during nasal packing. Mallampati 2, previous successful intubation reported. Induction was done with propofol, fentanyl, lidocaine and atracurium, followed by intubation by direct laryngoscopy. Sevourane and nitrous were used for maintenance. Central venous access and arterial catheterization were provided. Massive bleeding required intensive volemic resuscitation and hemotransfusion. The maxillary artery was ligated by surgeon. During the whole procedure, the patient received 2000 ml of ringer lactate, 1000ml of hydroxyethyl starch, 4 units PCV, 4 units FFP and 4 units platelet units. Introduction: Isolated tricuspid regurgitation (TR) is very rare and is associated with many cond... more Introduction: Isolated tricuspid regurgitation (TR) is very rare and is associated with many conditions, frequently is seen in association with drug abuse, endocarditis or chest trauma. Case report: More commonly TR is associated with other cardiac abnormalities such as end stage aortic or mitral valve disease most often Mitral stenosis, elevated pulmonary artery pressure leads to right ventricular (RV) strain and eventually RV failure with TR. RV can compensate for volume overload. On the other hand pressure overload is not well tolerated by RV. Conclusion: Most symptoms associated with TR are directly related to increased RV afterload.

When left untreated, survival to adulthood is rare. There are few case reports of uncorrected TOF surviving to adulthood and then presenting for noncardiac surgeries. We present a case of debridement of a necrotizing fasciitis under low dose spinal anaesthesia, in a patient of uncorrected TOF with scoliosis and cushingoid features.

The ensuing nerve block is sufficient to ensure patient’s well being, while motor block facilitates the surgeon’s work. In patients receiving spinal anaesthesia, with local anesthetic agents like bupivacaine, the addition of another drug as adjuvant prolongs the analgesia. The present study is designed to study the effect of dexmedetomidine (D) and Clonidine (C) on the duration of motor and sensory block as well as postoperative analgesia by intrathecal bupivacaine in patients undergoing lower limb orthopedic surgery. Subjects and Methods: This study was carried out on 90 patients in the age group of 18 to 50 years, belonging to the American Society of Anesthesiologists(ASA) physical status I and II presenting for lower limb orthopedic surgery were included in the study.
