The present study was designed to determine the usefulness of IR thermal camera in scrotal temperature measurement before color Doppler ultrasonography (CDUS) in patients admitted to the emergency department with acute scrotal pain. Method: This study was prospectively conducted on 49 patients with acute scrotal pain and 30 control participants. The findings of CDUS and scrotal temperature measurements by an IR camera were separately evaluated by different physicians. In all patients, temperature measurements with IR camera were made under the same environmental conditions. Results: Of the 49 patients included in the study, four were diagnosed with torsion, 12 with epididymitis, 4 with orchitis, 3 with epididymo-orchitis, and 2 with varicocele. A significant difference was observed between the scrotal temperature of the patients with scrotal pain and the mean testicular temperature of the control group based on the IR camera measurement (p<0.05). IR camera did not detect any difference between the two testicles of the same person in the patient group (p=0.615). Although the lowest temperature was in testicular torsion, the patients' scrotal temperature did not significantly differ according to their diagnoses (p=0.087). Conclusion: Testicular temperature measured by IR device was lower in patients presenting with scrotal pain compared to normal individuals. Although not statistically significant, the lowest temperature was found in cases of testicular torsion. IR camera may be useful in triage when used in conjunction with physical examination in patients presenting with acute scrotal pain.

More than 100,000 of these cases, primarily in the tropics, result in death. This study includes an analysis of a university hospital's three-year experiences of distal extremity snakebites. Methods: This study includes 68 patients presented at the Emergency Department (ED) of Gaziantep University Hospital with snakebites on distal extremities (hands, fingers, and feet) between 01/03/2014 and 01/03/2017. Results: A total of 68 patients had wet snakebites on their distal extremities. Forty-seven patients (69%) were male, 21 (30.9%) were female, and the mean age was 43.03 ± 18.13 years. Snakebites most commonly occurred in September (26.5%, n = 18). Twenty-five (36.76%) patients had systemic symptoms (grades 2, 3, and 4). Hastaların rutin biyokimyasal tetkikleri yapıldı, akciğer grafisi ve elektrokardiyografileri çekildi. Ayrıca hastaların kardiyak fonksiyonlarına ekokardiyografi ile bakıldı. İntertisyel akciğer ödemi tespit edilen hastalara ait dosya bilgileri kaydedildi ve tüm bu veriler incelendi. İki aylık dönemde (2011 ve 2012 Ramazan dönemi) akciğer ödemi tanısı konulan hasta sayısı 82 idi. Bu hastaların 32 sinde (%39) meyan şerbeti tüketimi hikâyesi vardı.

Late diagnosis causes cost increase, long waiting time, and improper treatment. Today, with the rapidly developing technology, the cost of thermal cameras is gradually decreasing day by day. Studies have shown that many diseases are associated with heat. As a result, infrared images are thought to be a tool for diagnosing various diseases. In this study, it has been shown that infrared thermal imaging can be used as a pre-screening test in the diagnosis of Deep Vein Thrombosis with the developed computer aided software. In addition, a sample combination is shown for applications that utilize emergency services to perform diagnosis and treatment of Deep Vein Thrombosis as soon as possible.

Aim: In this study, all interventions made by lay rescuers and health professionals and the short... more Aim: In this study, all interventions made by lay rescuers and health professionals and the shortcomings for cardiopulmonary arrest management outside the hospital were examined. Materials and Methods: The study was conducted between December 2012 and May 2014 in the Emergency Department of Gaziantep University. To ensure orderly and standardized records, a study form was prepared that consisted of 31 questions. The time and location of the cardiac arrest, information regarding the lay rescuers and professional health workers, and the practices followed during transport and at the emergency service were examined. Statistical analyses were performed using the Statistical Package for the Social Sciences (SPSS) for Windows software version 22.0, and p<0.05 was accepted as statistically significant. Results: Of the 205 cardiac arrests, 69.8% were male and 30.2% were female. The mean age in all of the cases was 58.34±19.1 years. The cardiac arrests mostly occurred in the home environment (62.4%) and happened between 13.00 and 20.00 hours (43.4%). The most frequent conditions that caused a worsening of the cases were syncope (unresponsiveness) (35.6%). The people who identified the cardiac arrest case and called for help by informing emergency health personnel (EHP) were mostly family members (50.7%). The activation time was 14.27±20.30 min. The time to arrive at the scene was 8.4±6.4 (1-35) min. Lay rescuers performed resuscitation in 19.5% of cases. The most frequent rhythms on the arrival of the EHP were asystole (74.1%). EHP evaluated the glaskow coma scale (GCS) of 88.3% of the cases as ≤7 at the scene. EHP performed basic life support (BLS) on all cases (100%) and endo-tracheal intubation (ETI) on 29.3% of the cases at the scene. The on-scene time and transport time to ED were 8.09±8.82 and 9.02±7.92 (1-50) min, respectively. The average duration of CPR at ED was 35.15±16.9 min. Of all the cases, 7.8% were discharged from intensive care unit (ICU) to homes. Of all the cases, 77.6% died at the ED, and 14.6% died in ICU. Conclusion: The intervention rate by lay rescuers was far less than the international rates. The survival rates were generally below the internationally reported rates. There is no adequate public awareness in our area for identifying cardiac arrest in patients and for initiating early chest compressions.

Objective: This study aimed to determine the levels of depression, job satisfaction and burnout a... more Objective: This study aimed to determine the levels of depression, job satisfaction and burnout and the variables that could be related to them in the healthcare personnel who were working in the emergency department in the sample from the city of Gaziantep. Methods: Study sample included a total of 347 participants, including doctors, nurses, healthcare officers, paramedics and emergency medical technicians (EMTs), who have been working in the emergency department of Gaziantep University Medical Faculty Hospital, in the emergency departments of the state hospitals and 112 emergency services located in the city center of Gaziantep. In the study, we used socio-demographic data form, Maslach Burnout Inventory (MBI), Minnesota Satisfaction Questionnaire (MSQ) and Beck Depression Inventory (BDI). Results: Of the workers of emergency department, 18.5% (n=50) were working in the university hospitals, 40% (n=108) in the state hospitals and 41.5% (n=112) in 112 emergency services. Of the workers, 23.3% (n=63) were doctors, 31.5% (n=85) were nurses and 45.2% (n=122) were paramedics. Healthcare personnel who were working in the state hospital had significantly higher emotional exhaustion (EE) and BDI scores and significantly lower personal accomplishment (PA) scores compared to the workers of 112 emergency department. Paramedics had significantly lower EE scores compared to both doctors and nurses. In terms of age, it was found that 18-24 age group had significantly lower MBI-EE subscores compared to 25-29 and 30-34 age groups and that 40 and over age group had significantly higher BDI scores compared to 18-24 age group. All scales, except MBI-PA, were significantly different between the subjects who willingly chose the job and those who unwillingly chose the job. Again, all scales, except MBI-PA, were significantly different between the subjects who gave the answer of "I am satisfied with my work environment" and those who gave the answer of "I am not satisfied with my work environment". Discussion: Evaluating the mental health and working conditions of the people who work in a unit under an intense work pressure, which requires efficient, proper and rapid intervention to the patients, would help to improve the quality of the services given in this field.

METHODS The study was conducted at the Faculty of Medicine, Gaziantep University, Turkey, between May 2009 and October 2010. It comprised 44 patients admitted to the emergency department for scorpion sting, and a control group of matched age and gender of 20 persons. Electrocardiograms were taken promptly in the most painful phases of the patients. Cardiac parametres were measured. Erythrocyte packages were prepared to detect toxin/antioxidant levels. SPSS 18 was used for statistical analysis. RESULTS Of the 44 patients, 22 (50%) were male. Overall average age of the patients was 45.22 +/- 17.99 years. In this study, we aimed to compare the analgesic efficacy of intravenous dexketoprofen trometamol... more In this study, we aimed to compare the analgesic efficacy of intravenous dexketoprofen trometamol, fentanyl, and paracetamol in patients presenting to the emergency department with renal colic. Data obtained from the emergency departments of Gaziantep University&#39;s Hospital for Research and Practice along with two other state hospitals in Gaziantep, Turkey between January 2016 and January 2017 was used for this study. A total of three hundred patients (n=300), who presented to the ER with complaints most common to renal colic whose diagnoses were subsequently confirmed with Computerized Tomography were included in the study. Patients&#39; pain scores were recorded using the Visual Analogue Scale, at admission (immediately before drug administration), then at the 15th, and 30th minutes. SPSS 22.0 software package was used for analysis. p&lt;0.05 was considered significant. At the 15th minute comparison, the efficacies of the three groups of drugs were not superior to one other, but at the 30th minute, dexketoprofen trometamol was statistically more effective than paracetamol and fentanyl. There was no statistically significant difference between fentanyl and paracetamol. The need for additional analgesia in the group receiving dexketoprofen trometamol was found to be lower. Dexketoprofen trometamol was statistically superior to the other two agents in achieving full analgesia at the end of the thirty-minute period. Fentanyl was found to be statistically significant in achieving moderate analgesia. As a Non-steroidal antiinflammatory drug dexketoprofen trometamol is superior to paracetamol and fentanyl in achieving analgesia and reducing the need for additional drugs for the treatment of renal colic.

Crimean-Congo hemorrhagic fever (CCHF) is a life-threatening disease that develops as a result of... more Crimean-Congo hemorrhagic fever (CCHF) is a life-threatening disease that develops as a result of infection by a member of the Nairovirus genus of the Bunyaviridae family, and its initial symptoms are not specific. In patients with severe clinical progression, in particular, the neutrophil rate is high, whereas lymphocyte and monocyte levels are low. A total of 149 patients, in whom the diagnosis was confirmed with reverse transcriptase PCR, were included in the study. In order to compare patient clinical progression severity, we divided the patients into two groups. For group 1, Çevik's severity score was used. The patients who had a platelet/ lymphocyte ratio (PLR) <41 constituted group 2. Of 149 patients, 20 (13.4 %) were determined as group 1 (Çevik's classification) and 38 (25.5 %) were determined as group 2 (PLR <41). Of 11 deaths, 4 (36.4 %) patients were from group 1 and 7 (63.6 %) were from group 2. This is the first study to our knowledge to analyse the relationship between severity and PLR in patients with CCHF. PLR is a simple laboratory test that can aid in determining the prognosis of individuals with this disease.

The purpose of this study was to evaluate a rapid cardiac ultrasound assessment performed by trai... more The purpose of this study was to evaluate a rapid cardiac ultrasound assessment performed by trained non-expert sonographers integrated into the advanced cardiac life support (ACLS). This study was prospectively performed in 179 patients (104 males and 75 females) who underwent cardiopulmonary resuscitation (CPR) in an emergency department (ED) during two calendar years (2013 and 2014). Two senior doctors, who had received emergency cardiac ultrasonography training, performed cardiac ultrasound through the apical, subxiphoid, or parasternal windows. Ultrasound evaluation and pulse controls were performed simultaneously. SPSS 18.0 was used for statistical analysis. A total of 63.7% (114) of the cardiopulmonary arrest incidents occurred out of the hospital. Only 13 patients had a femoral pulse during the initial evaluation, while 166 showed no femoral pulse. Initial monitoring showed a regular rhythm in 53 patients, ventricular fibrillation in 18 patients, and no rhythms in 108 patients. The first evaluation with ultrasound detected an effective heart rate in 26 patients and ventricular fibrillation in 14 patients, while no effective heart rate was observed in 139 patients. In addition, ultrasound revealed pericardial tamponade in seven patients and right ventricular enlargement in four cases. Global hypokinesia was detected in four patients and hypovolemia was observed in another four patients. The use of real-time ultrasonography during resuscitation with real-time femoral pulse check can help facilitate the distinguishing of pea-type arrest, ascertain the cause of the arrest, infer a suitable treatment, and optimize medical management decisions regarding CPR termination.

Results: Of the 205 cardiac arrests, 69.8% were male and 30.2% were female. The mean age in all of the cases was 58.34±19.1 years. The cardiac arrests mostly occurred in the home environment (62.4%) and happened between 13.00 and 20.00 hours (43.4%). The most frequent conditions that caused a worsening of the cases were syncope (unresponsiveness) (35.6%). The people who identified the cardiac arrest case and called for help by informing emergency health personnel (EHP) were mostly family members (50.7%). The activation time was 14.27±20.30 min. The time to arrive at the scene was 8.4±6.4 (1-35) min. Lay rescuers performed resuscitation in 19.5% of cases. The most frequent rhythms on the arrival of the EHP were asystole (74.1%). EHP evaluated the glaskow coma scale (GCS) of 88.3% of the cases as ≤7 at the scene. EHP performed basic life support (BLS) on all cases (100%) and endo-tracheal intubation (ETI) on 29.3% of the cases at the scene. The on-scene time and transport time to ED were 8.09±8.82 and 9.02±7.92 (1-50) min, respectively. The average duration of CPR at ED was 35.15±16.9 min. Of all the cases, 7.8% were discharged from intensive care unit (ICU) to homes. Of all the cases, 77.6% died at the ED, and 14.6% died in ICU. Conclusion: The intervention rate by lay rescuers was far less than the international rates. The survival rates were generally below the internationally reported rates. There is no adequate public awareness in our area for identifying cardiac arrest in patients and for initiating early chest compressions.

Objective: This study aimed to determine the levels of depression, job satisfaction and burnout a... more Objective: This study aimed to determine the levels of depression, job satisfaction and burnout and the variables that could be related to them in the healthcare personnel who were working in the emergency department in the sample from the city of Gaziantep. Methods: Study sample included a total of 347 participants, including doctors, nurses, healthcare officers, paramedics and emergency medical technicians (EMTs), who have been working in the emergency department of Gaziantep University Medical Faculty Hospital, in the emergency departments of the state hospitals and 112 emergency services located in the city center of Gaziantep. In the study, we used socio-demographic data form, Maslach Burnout Inventory (MBI), Minnesota Satisfaction Questionnaire (MSQ) and Beck Depression Inventory (BDI). Results: Of the workers of emergency department, 18.5% (n=50) were working in the university hospitals, 40% (n=108) in the state hospitals and 41.5% (n=112) in 112 emergency services. Of the workers, 23.3% (n=63) were doctors, 31.5% (n=85) were nurses and 45.2% (n=122) were paramedics. Healthcare personnel who were working in the state hospital had significantly higher emotional exhaustion (EE) and BDI scores and significantly lower personal accomplishment (PA) scores compared to the workers of 112 emergency department. Paramedics had significantly lower EE scores compared to both doctors and nurses. In terms of age, it was found that 18-24 age group had significantly lower MBI-EE subscores compared to 25-29 and 30-34 age groups and that 40 and over age group had significantly higher BDI scores compared to 18-24 age group. All scales, except MBI-PA, were significantly different between the subjects who willingly chose the job and those who unwillingly chose the job. Again, all scales, except MBI-PA, were significantly different between the subjects who gave the answer of "I am satisfied with my work environment" and those who gave the answer of "I am not satisfied with my work environment". Discussion: Evaluating the mental health and working conditions of the people who work in a unit under an intense work pressure, which requires efficient, proper and rapid intervention to the patients, would help to improve the quality of the services given in this field.

Data obtained from the emergency departments of Gaziantep University&#39;s Hospital for Research and Practice along with two other state hospitals in Gaziantep, Turkey between January 2016 and January 2017 was used for this study. A total of three hundred patients (n=300), who presented to the ER with complaints most common to renal colic whose diagnoses were subsequently confirmed with Computerized Tomography were included in the study. Patients&#39; pain scores were recorded using the Visual Analogue Scale, at admission (immediately before drug administration), then at the 15th, and 30th minutes. SPSS 22.0 software package was used for analysis. p&lt;0.05 was considered significant. At the 15th minute comparison, the efficacies of the three groups of drugs were not superior to one other, but at the 30th minute, dexketoprofen trometamol was statistically more effective than paracetamol and fentanyl. There was no statistically significant difference between fentanyl and paracetamol. The need for additional analgesia in the group receiving dexketoprofen trometamol was found to be lower. Dexketoprofen trometamol was statistically superior to the other two agents in achieving full analgesia at the end of the thirty-minute period. Fentanyl was found to be statistically significant in achieving moderate analgesia. As a Non-steroidal antiinflammatory drug dexketoprofen trometamol is superior to paracetamol and fentanyl in achieving analgesia and reducing the need for additional drugs for the treatment of renal colic.

In patients with severe clinical progression, in particular, the neutrophil rate is high, whereas lymphocyte and monocyte levels are low. A total of 149 patients, in whom the diagnosis was confirmed with reverse transcriptase PCR, were included in the study. In order to compare patient clinical progression severity, we divided the patients into two groups. For group 1, Çevik's severity score was used. The patients who had a platelet/ lymphocyte ratio (PLR) <41 constituted group 2. Of 149 patients, 20 (13.4 %) were determined as group 1 (Çevik's classification) and 38 (25.5 %) were determined as group 2 (PLR <41). Of 11 deaths, 4 (36.4 %) patients were from group 1 and 7 (63.6 %) were from group 2. This is the first study to our knowledge to analyse the relationship between severity and PLR in patients with CCHF. PLR is a simple laboratory test that can aid in determining the prognosis of individuals with this disease.

The purpose of this study was to evaluate a rapid cardiac ultrasound assessment performed by trai... more The purpose of this study was to evaluate a rapid cardiac ultrasound assessment performed by trained non-expert sonographers integrated into the advanced cardiac life support (ACLS). This study was prospectively performed in 179 patients (104 males and 75 females) who underwent cardiopulmonary resuscitation (CPR) in an emergency department (ED) during two calendar years (2013 and 2014). Two senior doctors, who had received emergency cardiac ultrasonography training, performed cardiac ultrasound through the apical, subxiphoid, or parasternal windows. Ultrasound evaluation and pulse controls were performed simultaneously. SPSS 18.0 was used for statistical analysis. A total of 63.7% (114) of the cardiopulmonary arrest incidents occurred out of the hospital. Only 13 patients had a femoral pulse during the initial evaluation, while 166 showed no femoral pulse. Initial monitoring showed a regular rhythm in 53 patients, ventricular fibrillation in 18 patients, and no rhythms in 108 patients. The first evaluation with ultrasound detected an effective heart rate in 26 patients and ventricular fibrillation in 14 patients, while no effective heart rate was observed in 139 patients. In addition, ultrasound revealed pericardial tamponade in seven patients and right ventricular enlargement in four cases. Global hypokinesia was detected in four patients and hypovolemia was observed in another four patients. The use of real-time ultrasonography during resuscitation with real-time femoral pulse check can help facilitate the distinguishing of pea-type arrest, ascertain the cause of the arrest, infer a suitable treatment, and optimize medical management decisions regarding CPR termination.
