Plasma visfatin concentrations were measured using an enzyme-linked immunoabsorbent assay (EIA) kit (Phoenix Pharmaceuticals, Belmont, California, USA). Results: All groups experienced a significant weight loss, as well as a significant improvement in lipid profile and parameters of carbohydrate metabolism. Plasma visfatin levels were not changed significantly in the 3 groups [from 29.1 (16.5-46.0) to 28.1 (18.9-47.3) ng/mL in O group, from 29.0 (15.4-46.0) to 26.9 (17.0-51.0) ng/mL in F group and from 28.7 (13.4-43.0) to 27.6 (15.5-48.0) in OF group, all p=NS]. Conclusions: Fenofibrate and orlistat administration, alone or in combination, does not significantly alter plasma visfatin levels in overweight and obese subjects with MetS.

The main objective of the present study was to determine whether soluble and leukocitary cell adhesion molecules were altered in patients with metabolic syndrome in comparison with control subjects. Moreover, other circulating markers of different candidate atherogenic risk factors were also studied. Methods: Thirty one male patients with metabolic syndrome (ATPIII definition) and 56 male controls were studied. We evaluated different markers of insulin resistance, inflammation and atherosclerosis, as well as protective factors: insulin, HOMA, QUICKI, lipoprotein profile, HDL composition, CETP, LpPLA2, VCAM-1, ICAM-1, E-Selectin (ELISA), and leukocitary CD18, CD49d and CD54 (flow cytometry). Results: Patients with metabolic syndrome showed a) hypoadiponectinemia (4551±2302 vs. 5865±2548 ng/ml, respectively; p<0.05), b) an atherogenic lipoprotein profile, c) altered HDL chemical composition, d) higher CETP activity, e) diminished Lp-PLA2 activity (6.5±1.9 vs. 7.3±2.2, p<0.05, respectively), antioxidant enzyme related with LDL oxidation, which was positively associated with QUICKI and negatively with VCAM-1 and lymphocyte CD18, and f) high soluble (VCAM-1: 17±5 vs. 13±4 ng/ml, respectively; p<0.0005) and leukocyte adhesion molecule expression (monocyte CD54: 52±15 vs. 45±12 arbitrary units, respectively; p<0.0005; and lymphocyte CD49d: 312±56 vs. 284±64 arbitrary units, respectively; p<0.05). Conclusions: The increment in soluble and leukocitary cell adhesion molecules, crucial for leukocyte interaction with the endothelium and migration into the artery wall, in combination with the other disorders described above reinforce the presence of a clinical status with high propensity to atherosclerotic cardiovascular disease.

T... more Renin-angiotensin system activation is recognized to play an important role in atherosclerosis. This study aimed to verify the antiatherosclerotic effects of ACE inhibition on an experimental model of diabetes and hypercholesterolemia. Diabetes was induced in New Zealand male rabbits with a single dose of alloxan (100 mg/kg, IV), and, according to plasma glucose levels obtained after 1 week, the animals were divided into 2 groups ($250 mg/dL or ,250 mg/dL). Each group was randomly assigned to receive or not quinapril (30 mg/d) added to a 0.5% cholesterol-enriched diet. Animals with high glucose levels at 1 week and that remained high after 12 weeks presented higher triglyceride levels (P , 0.02 versus basal). Those initially hyperglycemic but presenting ,250 mg/dL glucose at the end of study formed an additional group. Plasma ACE activity was lower in quinapril-treated animals (P , 0.01 versus untreated groups). However, aorta intima/media ratio and intima area were lower only in the subgroups of quinapril-treated animals with low glucose levels (P , 0.05). Our results support the hypothesis that high plasma glucose may abolish the antiatherosclerotic effect of ACE inhibitors.

However, family physicians in Israel may not routinely diagnosis the MS. Purpose: To measure the prevalence of the MS among hypertensive patients in a community clinic,to identify the characteristics of this population. Method: A sample of hypertensive patients were invited by mail to an appointment in which they filled out a questionnaire, underwent a physical exam, and appropriate lab tests. Additional information was obtained from the patient files. Results: 200 patients were invited to the clinic, of which 82% participated. The age range of the participants was 28-95 years,48% were men,10% were smokers. 47% reported that they followed a diet and 48% reported doing physical activity Only 4 patients had been diagnosed with metabolic syndrome before the study. 110 additional hypertensives were found to meet the criteria for the syndrome. Significant correlation was found between metabolic syndrome and life style. Patients on a diet had a lower prevalence of MS (58 Patients-p<0.008). Hypertensives without MS smoked less (35 Patients-p=0.024) Conclusions: In this sample,69% of hypertensives suffer from MS but only 2.4% had this diagnosis recorded in their medical file before this intervention. Keeping to good diet is associated with less MS Interventions should be planned to increase the awareness of family physicians in Israel to diagnose and treat the MS
