Cureus

Retinitis pigmentosa is one of the risk factors for intraocular lens dislocation post cataract su... more Retinitis pigmentosa is one of the risk factors for intraocular lens dislocation post cataract surgery which can lead to many complications. A 64-year-old Chinese female with bilateral pseudophakia and retinitis pigmentosa was referred for the continuation of care in 2009 with baseline visual acuity of hand movement bilaterally due to the retinitis pigmentosa. The cataract surgeries with posterior chamber intraocular lens (PCIOL) implantation in her early 50s were uneventful. In 2011, her right eye PCIOL dislocated anteriorly into the anterior chamber spontaneously and touched the cornea. It was complicated with bullous keratopathy and corneal decompensation. Intraocular pressure (IOP) was normal. PCIOL explantation, anterior vitrectomy and surgical peripheral iridotomy were performed. However, the cornea remained decompensated postoperatively. Her left eye was stable until 2019 when she developed acute angle closure secondary to complete anterior dislocation of PCIOL with pupillary block glaucoma. She underwent left eye PCIOL explantation, anterior vitrectomy and surgical peripheral iridotomy when IOP was optimised medically. Finally, both eyes were left aphakic due to poor prognosis with light perception (PL) vision, IOP was stable on single topical antiglaucoma and bilateral decompensated corneas were maintained with topical hypertonic saline. This case highlights the different serious sequelae of bilateral eyes in an unfortunate retinitis pigmentosa patient.

The discussion centers around the case of the Rumah Panjang KTM, one of the most badly affected areas during the ethnic clash in Kampung Medan. Adopting the PINTAS model, it is imperative that concerned social scientists follow through a four-step model to achieve an integrated nation and to reduce conflict between its members. Furthermore, these two elements were intricately linked to the survival and displacement of elites within the ruling system. However, there has recently been a transformation of the government and its policies. The Malaysia's 2008 12 th general election witnessed a major political transformation and the rise of political blogging and other manifestations of the non-mainstream media in Malaysia. The Malay, Chinese and Indian tended to stick within their beneath the tranquility surface of Malaysian society, tensions of the potential for violence still lurks. The data regarding the socio-political tension plays significant impact and tremendous changes in Malaysia reflect relative political hegemony of the Malaysian democracy.

Cataracts may progress to become intumescent (phacomorphic) or hypermature (phacolytic). This can lead to lens-induced glaucoma, which may subsequently cause permanent optic nerve damage. Objective: To report on lens-induced glaucoma (LIG) of cataract cases treated in Hospital Sultanah Nur Zahirah, Kuala Terengganu (HSNZKT), Malaysia. Method: National Eye Database (Malaysia) data from 2011 till 2017 were collected and analysed. Results: A total of 81 (1.08%) cases of lens-induced glaucoma were calculated from 7468 cataract cases treated during the seven years of the study period. The number of cases showed an increasing trend with 0.78% in 2011 to 1.26% in 2017. Most of the patients were in the age group of 70-79 years (44.30%), followed by 60-69 years (34.18%), 50-59 years (11.39%), 80-89 years (7.59%), 40-49 years, and 90-99 years (1.27%). Most of the cases are female (57%). The majority of them (79.75%) underwent cataract operation for the first eye. Conclusion: Overall, this study was able to highlight the significant association between the incidence of LIG and increasing age as well as surgery series (either first or second eye). Therefore, more outreach programs should be conducted in the future to enable younger elderly patients from all areas to receive treatment. More online education and talk series can be organized to increase the community's awareness and acceptance of cataract operations.

Research paper thumbnail of Retinal Width Estimation of High-Resolution Fundus Images For Diabetic Retinopathy Detection

2020 IEEE-EMBS Conference on Biomedical Engineering and Sciences (IECBES), 2021

While this could pave the way to timely diagnosis of such diseases by examining vessels from fundus images, practical application of measuring and quantifying changes in vessel width over time remains a challenge. In this study, we propose a semi-automated estimation method to efficiently summarize vessel width characteristics from fundus images. The method consists of retinal vessel segmentation, optic disc (OD) localization and vessel width parameters estimation. The proposed method is validated using a public database of high-resolution fundus images called HRF, where the significance of obtained vessel width parameters in differentiating the three image groups in the database are analysed. Results indicate that the obtained parameter using the method that summarises the ratio between width of veins to arteries, AVR (Artery-Vein Ratio) can be used to differentiate images from patients with Diabetic Retinopathy against healthy and glaucomatous patients.

This can be attributed to the steady increase in the number of diabetic cases worldwide. DR can be treated if diagnosed early enough to prevent further damages to retina which could lead to total blindness if left untreated. Automatic segmentation of retinal blood vessels from fundus image is useful towards development of an efficient and accurate computer-assisted retinal diagnosis system. In this study, we propose a combination of B-COSFIRE filter and background normalization methods to segment the retinal blood vessels (RBVs) from fundus image. By performing background normalization on B-COSFIRE filter output and combining it with the original B-COSFIRE output, the segmentation performance can be improved in terms of Sensitivity. Validation of the proposed method on two public databases, namely DRIVE and STARE shows comparable segmentation performance to published methods with improved Sensitivity. The method achieves Sensitivity values of 78.33% and 81.04% and Specificity values of 96.51% and 95.69% for DRIVE and STARE database, respectively.

Although benign, intervention is sometimes necessary. Previous authors have reported pyogenic granuloma formation following oculoplastic surgeries. We report a pyogenic granuloma after an Ahmed glaucoma valve implantation. A 65-year-old gentleman presented with right eye redness associated with pain and swelling~2 months after Ahmed glaucoma valve implantation. Examination found a sessile growth on the tube extruding puss with signs of endophthalmitis. The glaucoma drainage device was explanted and culture results grew Staphylococcus aureus. This article discusses the formation of pyogenic granuloma on a glaucoma drainage device and its management.

We aim to evaluate the postural effect of upright and prolong supine positions on IOP in these patients. Methods This is a cross-sectional study involving 27 patients with symptoms of OSAS seen at a tertiary institutional center and 25 normal controls performed between June 2015 and June 2016. All patients and controls underwent a polysomnography (PSG) test and were diagnosed with OSAS based on the apnea-hypopnea index (AHI). Patients are those with OSAS symptoms and had AHI > 5, whereas controls are staffs from the ophthalmology clinic without clinical criteria for OSAS and had PSG result of AHI < 5. IOP was measured 10 min after sitting, immediately and at 30 min on supine position and immediately after resuming sitting position. The main outcome measures were the changes in mean IOP in different positions. Results The increase in mean IOP when changing from sitting to supine position was significantly higher in OSAS patients in both the right eye (RE) (1.01 ± 1.14 vs 0.44 ± 0.41 mmHg, p = 0.033) and left eye (LE) (1.20 ± 1.26 vs 0.48 ± 0.62 mmHg, p = 0.044). After 30 min supine, the IOP in OSAS patients showed a further increase in IOP than controls (RE 1.20 ± 1.79 vs 0.12 ± 0.66 mmHg, p = 0.001 and LE 0.59 ± 2.00 vs 0.15 ± 0.78 mmHg, p = 0.246). Higher IOP when sitting post supine is associated with higher IOP on supine position. Conclusion IOP increase is significantly more in OSAS patients on prolong supine position.

The concept of value and resource management strategies is derived from the self-regulated learning framework. Value consists of three main components, namely intrinsic motivation, extrinsic motivation and task value. Meanwhile, resource management strategies consist of four components, namely time and study management, effort regulation, peer learning and help seeking. The purposes of this study are to investigate (1) the value level among overall students, (2) the correlation between students' value and their use of resource management strategies, and (3) the differences in the use of resource management strategies between students with different level of value. The sample of this study comprised of 455 students and the data were collected through a questionnaire adapted from The Motivated Strategies for Learning Questionnaire (MSLQ). The study reveals that (1) the value level of students is high; (2) there are significant correlations between value and the use of resource management strategies among students; and (3) the high value group of student uses significantly higher in all components of resource management strategies than the moderate group. This study has some classroom implications. It suggests that value components should be nurtured among students by selecting appropriate learning activities. Collaborative learning activities should also involve students of mixed value level which can be assessed through their prior learning experience and gender.
