Telephone survey among a random sample of 61 subjects enrolled at the Smoking Cessation Clinic in the San Juan Veterans Affairs Hospital. The questionnaire was validated and standardized for Hispanic subjects. Twenty eight percent of the subjects interviewed referred they had quit smoking. Quitters used to smoke more cigarettes than current smokers do. Quitters perceived their general health to be worse than current smoker's perception. Even though Quitters were concerned about weight gain after quitting, it was not a negative outcome for smoking cessation. No association was found between active smoking and use of ethanol, neither socializing with friends nor feeling anxious. We conducted a multicenter, double-blind, placebo-controlled, randomized trial of a humanized mon... more We conducted a multicenter, double-blind, placebo-controlled, randomized trial of a humanized monoclonal antibody against a respiratory syncytial virus (RSV) fusion protein (SB 209763) to evaluate its safety, pharmacokinetics, and fusion inhibition and neutralization titers. Forty-three infants who were either delivered prematurely (<35 weeks' gestation) or exhibited bronchopulmonary dysplasia were administered either single or repeat (two doses, 8 weeks apart) intramuscular injections of SB 209763 at a concentration of 0.25, 1.25, 5.0, or 10.0 mg/kg or of a placebo. Four of 229 adverse events were considered related to the study drug, including purpura (n ‫؍‬ 3) and thrombocytosis (n ‫؍‬ 1). No subject developed a detectable level of anti-SB 209763 antibody. Approximately 1 week after administration of the second dose of SB 209763 at 10 mg/kg, the mean plasma concentration (n ‫؍‬ 9) was 68.5 g/ml. The terminal half-life (T 1/2 ) determined by noncompartmental analysis ranged from 22 to 50 days. The population pharmacokinetics for SB 209763 following intramuscular administration was appropriately described by a one-compartment model with first-order input and elimination. Higher values for clearance and volume of distribution at steady state were observed for younger patients, with values decreasing to 0.143 (ml/h)/kg and 161 mL/kg, respectively, by a mean age of 298 days (ϳ10 months). The mean T 1/2 of SB 209763 for the study population was 32.5 days. No other factor (dose, weight, gender, race, premature birth, or bronchopulmonary dysplasia) was observed to alter the population pharmacokinetics of SB 209763 in this study of infants and young children. The mean neutralization titer on day 6 was 286, and the mean fusion inhibition titer was 36. At least 57% of subjects dosed at 1.25 to 10.0 mg of SB 209763 per kg of body weight who were seronegative at baseline experienced a fourfold or greater increase in fusion inhibition titer. Nine RSV infections were documented during the 16-week course of the study; the numbers of RSV infections were similar for the different regimens, including the placebo. The doses of SB 209763 studied may have been insufficient to confer protection against RSV lower respiratory tract disease; these results suggest that additional trials using higher doses of monoclonal antibody for immunoprophylaxis should be considered.

Objectives: We sought to explore the relationship between the diagnosis of hypertension and obstr... more Objectives: We sought to explore the relationship between the diagnosis of hypertension and obstructive sleep apnea (OSA) in a Hispanic population, describe the effect of continuous positive airway pressure (CPAP) on blood pressure regulation, and assess the effect of CPAP on quality of life. Design: A retrospective, recall interview study. Participants: Patients enrolled at the Home Oxygen Program of the San Juan V.A. Medical Center with the diagnosis of OSA and treatment with CPAP. Measurements: The Calgary Sleep Apnea Quality of Life Index was administered to all patients after informed consent. Information regarding co-morbid conditions and fluctuations in blood pressure and anthropometric variables were obtained on a follow-up evaluation. Results: After excluding for the use of antihypertensive medications, weight, and age, a 10% decrease in mean arterial pressure (MAP) from 100 mm Hg to 92 mm Hg was observed in an average of 40 months of therapy (p < 0.05). With the Calgary Quality of Life Index, 67% of the patients reported an extreme improvement in their quality of life. Compliance with CPAP therapy correlated with improved quality of life (r = 0.33, p < 0.015). Conclusions: In this pilot study, there appears to be a correlation between our intervention and decline of blood pressure, independent of body weight, age, or medication usage. CPAP treatment is an effective modality in improving symptoms and quality of life. Prospective, randomized, observer-blind, multicenter study. Four hundred eighty-four children (age range, 2 to 17 years) with signs and symptoms of acute tonsillopharyngitis were enrolled; 377 had a positive throat culture for group A beta-hemolytic streptococci and were fully evaluable. The SNR consists of a cantilever that is 50 μm long, 10 μm wide, and 1.3 μm thick, with an embedded nanochannel that is 2 μm wide and 700 nm tall. The SNR has a resonance frequency near 640 kHz and exhibits the maximum quality factor of approximately 16,000 when dry and when filled with water. Using the SNRs, we demonstrated mass measurements of various nanoparticles down to 20 nm in diameter and viruses in solution with a resolution of 27 ag in a 1 kHz bandwidth, which represents a 100-fold improvement over existing SMRs and, to our knowledge, is the most precise mass measurement in liquid today. To compare bacteriologic and clinical efficacy and safety of 10 vs 5 days of cefpodoxime proxetil... more To compare bacteriologic and clinical efficacy and safety of 10 vs 5 days of cefpodoxime proxetil vs 10 days of penicillin V potassium for the treatment of acute group A beta-hemolytic streptococcal tonsillopharyngitis in children. Prospective, randomized, observer-blind, multicenter study. Four hundred eighty-four children (age range, 2 to 17 years) with signs and symptoms of acute tonsillopharyngitis were enrolled; 377 had a positive throat culture for group A beta-hemolytic streptococci and were fully evaluable. One hundred twenty-one patients received cefpodoxime once a day for 10 days, 126 received cefpodoxime twice a day for 5 days, and 130 received penicillin V three times a day for 10 days. Cefpodoxime for 10 days vs cefpodoxime for 5 days vs penicillin V for 10 days produced bacteriologic eradication at the end of therapy in 95%, 90%, and 78% of the patients, respectively. The SNR consists of a cantilever that is 50 μm long, 10 μm wide, and 1.3 μm thick, with an embedded nanochannel that is 2 μm wide and 700 nm tall. The SNR has a resonance frequency near 640 kHz and exhibits the maximum quality factor of approximately 16,000 when dry and when filled with water. Using the SNRs, we demonstrated mass measurements of various nanoparticles down to 20 nm in diameter and viruses in solution with a resolution of 27 ag in a 1 kHz bandwidth, which represents a 100-fold improvement over existing SMRs and, to our knowledge, is the most precise mass measurement in liquid today.
