Background: Pulmonary embolism (PE) is the most serious clinical presentation of venous thromboem... more Background: Pulmonary embolism (PE) is the most serious clinical presentation of venous thromboembolism (VTE), a major cause of sudden death. In the current Coronavirus disease 2019 (COVID-19) pandemic, when a patient presents with respiratory symptoms, many serious possible non-Covid causes including PE may be considered the next priority after COVID-19 on the differential diagnosis list. Thus, the correct diagnosis may be delayed or missed, with grave consequences to the patient's outcome and safety. Case Presentation: We herein present a case of a 63-year-old Iranian female who was admitted to our hospital showing symptoms of delirium superimposed on dementia. Soon after admission, she developed fever and respiratory symptoms. However, overestimating the likelihood of COVID-19 pneumonia and attributing the patient's symptoms to this disease led to delayed diagnosis and treatment of pulmonary embolism, resulting in the patient's death. Conclusions: During the COVID-19 pandemic, a high index of suspicion is required for the timely diagnosis of PE, especially in patients with identi able risk factors. This is speci cally true for patients who are unable to express their symptoms due to neurocognitive disorders.

The overlapping clinical symptoms of Coronavirus disease 2019 (COVID-19) and PE may make distinguishing between the two difficult. Thus, the diagnosis of PE may be delayed or missed, with grave consequences for the patient's outcome and safety. We herein present the case of a 63-year-old Iranian female admitted to our hospital showing symptoms of delirium superimposed on dementia. Soon after her admission, she developed a fever and respiratory symptoms. However, overestimating the likelihood of COVID-19 pneumonia and attributing the patient's symptoms to this disease led to a delayed diagnosis and treatment of pulmonary embolism, resulting in the patient's death. Background: Fibromyalgia includes a fatigue syndrome with chronic pain that declines the patients... more Background: Fibromyalgia includes a fatigue syndrome with chronic pain that declines the patients' quality of life. Previous studies confirm these observations, but there is no study on aged-population regarding this issue. Thus we investigated the impacts of anodal stimulation on the life quality in aged-population suffering from fibromyalgia. Methods: Fifty aged females with fibromyalgia were randomized to receive sham stimulation or real tDCS with the anode centered over the primary motor cortex (M1) and a cathode over the contralateral supraorbital area (2 mA for 20 minutes for 10 sessions). The overall effect of fibromyalgia on the quality of life was assessed using the Fibromyalgia Impact Questionnaire (FIQ) and SF-36 quality of life questionnaire. Also, the mood and anxiety levels were evaluated with Beck depression inventory and Hamilton Anxiety Rating Scale. All assessments were done before, after the last session, and 30 days after the last session of the stimulation. ObjectivesTo evaluate the relationship between a number of prevalent foot problems in older peopl... more ObjectivesTo evaluate the relationship between a number of prevalent foot problems in older people and their history of falls and quality of life. To examine the relationship between timed up and go (TUG) test time and ankle muscles’ strength.MethodIn this cross‐sectional observational study on a convenient sample of 350 people 65 years and older, direct logistic and hierarchical regressions were used to assess the relation of self‐reported falls and quality of life with hallux valgus, oedema, sensory loss and callus. The correlation coefficients were calculated to measure the relationship between TUG test time and ankle muscle strength.ResultsModerate negative correlations were measured between ankle muscles’ strength and TUG time (rDorsiflexors = −0.42 p = 0.000, rPlantar flexors = −0.45 p = 0.000). The odds ratio extracted from logistic regression for foot pain was 3.05 (p = 0.000, 95% CI: 1.8–5.1). The results of the hierarchical regression showed that oedema had the highest contribution to depicting the quality of life (standardised B = −0.22, p = 0.001), followed by foot pain (beta = −0.178, p = 0.004) and plantar sensory loss (beta = −0.143, p = 0.019).ConclusionThe results of this study highlight the importance of foot problems in older people. Foot pain showed relationship with self‐reported fall experience. Oedema and foot pain had negative impact on quality of life. Ankle muscles’ strength may affect balance in older people. However, because of the small effects sizes and wide confidence intervals, the results should be interpreted cautiously.

This literature review describes the impact of health professional schools with a social accounta... more This literature review describes the impact of health professional schools with a social accountability mandate by identifying characteristics of medical education found to impact positively on medical students, health workforce, and health outcomes of disadvantaged communities. A critical appraisal tool was used to identify the strengths and weaknesses of the published articles. Data are presented as a narrative synthesis due to the variety of methodologies in the studies, and characterized using a logic model. Health professional schools aiming to improve health outcomes for their disadvantaged local communities described collaborative partnerships with communities, equitable selection criteria, and community-engaged placements in underserved areas as positively impacting the learning and attitudes of students. Students of socially accountable schools were more likely to stay in rural areas and serve disadvantaged communities, and were often more skilled than students from more traditional schools to meet the needs of underserved communities. However, published literature on the impact of socially accountable health professional education on communities and health outcomes is limited, with only one study investigating health outcomes. The findings of this literature review guide schools on the inputs likely to maximize their socially accountability outputs and increase their impact on students, local health workforce and local communities.

This meta-analysis aimed to summarize the evidence regarding attitudes of Iranian nurses and rela... more This meta-analysis aimed to summarize the evidence regarding attitudes of Iranian nurses and related factors towards end-of-life (EOL) care. PubMed, Web of Science, Scopus, Magiran, Iranmedex, Scientific Information Database, and Google Scholar search engine were searched using Persian and English appropriate keywords from the earliest records up to September 11, 2020. A total of 849 nurses were included in six studies. After a meta-analysis of the mean score of nurses' attitudes, the pooled mean was 80.07 out of 120 (Q(5)=4.32, I-squared=0.00%; 95%CI: 73.53–86.60; p < 0.001). Marital status, ward type, education level, a history of participating in EOL care workshops, personal study of EOL care, experience of caring for a dying family member or close people, natural and approach acceptance, fear of death, and professional autonomy had a significant positive relationship with nurses' attitudes towards EOL care. The present study aimed to compare GSs among young and elderly patients with rheumatoid arthritis (RA).METHODS: In a cross-sectional study a total of 98 participants, including 65 elderly (≥60 years) and 33 young adult patients (<60 years) with RA who referred to the geriatric and rheumatologic clinic were enrolled. Patients were categorized into three groups (healthy elderly, n=27; elderly with RA, n=38; and young people with RA, n=33). GSs were assessed using mini-mental state exam (MMSE), five-item geriatric depression scale-15 (GDS-15), mini nutritional assessment (MNA), and asking patients about history of falls in the past year.
