The data consists of 5 stock character macros. The data analysis is based on three theoretical bases: Berger's Code Violation, McCloud's taxonomy of image and text relations in comics and Fauconnier and Turners' Conceptual Integration Theory. The study aims to explore and investigate the humorous nature of stock character macros; how humour is created semiotically; how different signs interact, either equally or subordinately, to produce laughter; and how texts and images are compressed, projected, and blended to create new humorous meanings. The study also aims to define the unique textual and visual characteristics of this type of memes in Egyptian Arabic, a different culture that was not investigated before in such a context. One dimension of the current study is to present a linguistic analysis of a widely recognized but academically underrepresented digital artefact such as image-macro memes. The current study also focuses on the role of images in such a multi-modal construction. The results reflect causes of humour, types of text and image relations, and the unique textual and visual features of the data in question. It also supports individuals to realize their potential goals for self and societal development. Nevertheless, it is also preconditioned for momentous and continuous nationwide economy-an important key for national advancement in all sectors (Adepoju, 2007; Alabi, Emmanuel & Falode, 2015). Moreover, in order to achieve the stated goals of qualitative education and to move the country forward, there is a need for a concept shift from teachercentred oriented mode of instructional strategy to student-centred learning schematic mode of instruction which is in agreement with the space age of Information Communication and Technology. This will promote easy teaching and learning, thereby fostering proper mastery of what is taught on the part of both the learners and the instructor (Merrill, 2001). The consequence of Information Communication and Technology provides easier knowledge in terms of effective educational change and developing of socioeconomic. Nevertheless, the integration of ICT in instruction also facilitates and promotes new curriculum of instruction, thus improving and stabilising the quality of learning content, methodology, and efficiencies of the system of education (Adomi & Kpangban, 2010; Nsofor, Obunadike, Bello, & Abubakar, 2015). Researchers have confirmed ICT as a helpful hand for supporting classroom instruction in all ramifications as well as educational administration. For instance, the study of Wagner, Bob, Tina, Robert, Jonathan and Tim (2005) confirmed that using Information Communication and Technology in education brings about great increase in utilising technology tools for supporting, disseminating and evaluating educational goals. In other words, SuLuan and Sing (2008) argued that the value of educational growth with technological invention is more felt in administration than in implementing the curriculum of instruction. The study of Adomi and Kpangban (2010) identified some basic tools as prerequisite for the attainment and advancement of quality instruction in the educational system. Although broadcasting technologies, telephony, web, networking on the Internet, are of paramount significance to learners' proficiency for the global workplace. Nyenweand Ishiakaku (2013) submitted that it is imperative to adapt computer education for learners' development right from the inception to the school setting, as we are in the world of technological innovations and development. In the same vein, Nsofor, Obunadike, Bello and Abubakar (2015) submitted that the involvement of digital technology devices rendered learning to be positive; encouraging skills acquisition to transform the globe into a habitable arena. The current study carries out a socio-cognitive analysis of animal metaphors in colloquial Egyptian Arabic. It examines sociolinguistic factors of gender and frequency of use through a four-point Likert questionnaire.

La NAFLD (non-alcoholic fatty liver disease) represente une des complications majeures associees a l'obesite et est consideree comme la manifestation hepatique du syndrome metabolique. Le terme NAFLD regroupe l'ensemble des lesions hepatiques evoluant de la steatose isolee (reversible) vers l'hepatite steatosique (NASH) ayant un potentiel de progression vers la fibrose, la cirrhose et eventuellement le carcinome hepatocellulaire. A l'heure actuelle, les mecanismes responsables de l'instauration chez le patient obese d'une NAFLD, et surtout d'une NASH, ne sont pas elucides. L'objectif de ma these etait de determiner chez des patients obeses si des alterations hepatiques du metabolisme lipidique et/ou de la fonction mitochondriale, dans un contexte de surcharge lipidique du foie, peuvent participer a l'evolution de la NAFLD. Des biopsies hepatiques chirurgicales peroperatoires ont ete obtenues de patients obeses morbides operes d'une chirurgie bariatrique, ainsi que de temoins non obeses. Les flux metaboliques d'acide gras a chaine longue (AGCL) realises a l'aide de l'oleate [1-¹⁴C] et l'exploration de la respiration mitochondriale ont ete realises sur du tissu frais. Les resultats montrent une alteration de la β-oxydation mitochondriale complete des AGCL chez les patients obeses ayant une NASH (reduction de ¹⁴c—CO₂, augmentation des ¹⁴ccorps acido-solubles, et baisse de la cetogenese hepatique). La production de CO₂ (oxydation complete des AGCL) est correlee negativement avec les contenus hepatiques en triglycerides et diacylglycerols, qui sont augmentes chez les patients obeses NASH par rapport aux autres groupes. La respiration mitochondriale a ete mesuree par oxygraphie en presence de differents substrats et inhibiteurs mitochondriaux sur l'homogenat de foie ainsi que sur des mitochondries isolees. Une reduction generale de la respiration a ete observee entre les sujets temoins et tous les groupes de patients obeses. Cependant, la NASH est associee a une diminution significative de la respiration mitochondriale par rapport aux autres patients obeses. Ces alterations ne sont pas associees a une modification de la masse mitochondriale, ni a une alteration de l'expression proteique des complexes OXPHOS de la chaine respiratoire.

We analyzed the impact of time interval to surgery on short-terme outcome parameters in patients undergoing elective laparoscopic left colonic resection for diverticulitis. Materials and methods: Retrospective analysis of two series of case-matched patients according to the timing of operation after the last episode of diverticulitis: group A (within 90 days) and group B (beyond 90 days). Results: 332 patients had left colonic resection for diverticulitis. 117 patients were included in group A vs 114 patients in group B. Overall abdominal morbidity in Group A was 21% vs 5% in group B (p ¼ 0.02). Mean hospital stay was 7.7 days in group A vs 5 days in group B (p ¼ 0.08). Residual inflammation was significantly increased in group A (31%) as compared to group B (11%) (p ¼ 0.01). Conclusions: Laparoscopic left colonic resection for acute diverticulitis is best performed beyond the third month after the last acute episode.

ABSTRACT Objective The objective of this review is to evaluate the results of laparoscopic treatm... more ABSTRACT Objective The objective of this review is to evaluate the results of laparoscopic treatment of ventral hernias using intraperitoneal prosthetic mesh through a review of published surgical series. Methods All series with more than 100 patients published between 2002 and 2011 were identified. The results of controlled randomized series comparing this technique with open surgical repair were analyzed. Results Twenty-two series with a total of 7057 patients were analyzed (range 51–1242); the mean Body Mass Index (BMI) was 32 (range: 28.5–46), and the mean size of the hernia defect was 10.6 cm (range: 1.5–14.9 cm). The mean rate of conversion to open surgical repair was 2.7%. Mean operative duration was 90 ± 33 minutes. Mortality was 0.8%, usually due to an unrecognized intestinal injury (1.6%), which occurred more frequently with second or iterative hernia repairs. Hospital stay averaged 2.5 days. The infection rate was 0.3% and the rate of hernia recurrence was 4.6% (with a mean follow-up of 36 months). No statistically significant differences were found in terms of postoperative complications or hernia recurrence in the randomized studies comparing laparoscopic to open anterior abdominal surgical repair. However, duration of hospital stay was shorter after laparoscopy in some studies. Conclusion Laparoscopic repair of ventral incisional hernia is technically feasible and reliable for large hernia defects, even in obese patients. This approach should be reserved for patients with no history of previous hernia repair in order to avoid the risk of intestinal injury. It appears to allow for a shortened duration of hospitalization.

Elle peut etre consideree comme un composant supplementaire du syndrome metabolique (SM). L'insulinoresistance (IR) serait le substratum physiopathologique commun au SM et a la NAFLD. L'hyperactivite sympathique affecte souvent les sujets obeses ou dysmetaboliques. L'inflammation ainsi que l'hyperactivite sympathique pourraient rendre compte de l'evolution de NAFLD vers la steatohepatite (NASH). Notre but etait de determiner dans une population de patients obeses morbides operes d'une chirurgie bariatrique la part des marqueurs de l'inflammation de bas grade, de l'IR, du cortisol plasmatique et de l'activite sympathique dans la maladie hepatique steatosique confirmee par biopsie chirurgicale. Patients et methodes Dans cette etude prospective approuvee par le comite regional d'ethique, des biopsies peroperatoires du lobe gauche du foie ont ete realisees. Chez 36 patients (6 hommes/30 femmes ; âge: 36,4±9,6 ans, IMC 44,8±5,3 kg/m²), les lipides circulants, la glycemie a jeun et apres charge orale en glucose, le cortisol plas-matique (8 h, 16 h), CRP, fibrinogene ont ete doses. HOMA-IR calcule ; l'activite sympathique a ete exploree par dosage de l'adrenaline et la noradrenaline plasmatiques a jeun et 2 heures apres glucose. Le SM etait defini selon les criteres IDF. Resultats Les patients ont ete classes par la biopsie en: 8 foie normal, 17 NAFLD et 11 NASH. Les patients avec NAFLD ou NASH avaient HOMAIR, noradrenaline post-charge et cortisol a 16 heures plus eleves ( p =0,03 pour tous ces parametres). Plus d'entre eux avaient un SM compares aux patients a foie sain ( p =0,04) avec une CRP un peu plus forte ( p =0,08). Conclusion Les patients obeses morbides avec preuve histologique de NAFLD/NASH ont une inflammation et une insulinoresistance plus marquees et une hyperactivite sympathique, des desordres dont le role dans la progression de la maladie hepatique reste a determiner. This study was conducted to evaluate the results of LG for gastric adenocarcinoma in two French surgical departments. Between 2001 and 2007, 51 patients underwent LG for gastric cancer. The results were compared to those of 79 patients who had open gastrectomy (OG) during the same study period. Mean age was 61 years (31-81) and 66 years (27-88) in the LG group and in the OG group, respectively. The sex ratio was 21 women to 30 men and 25 women for 54 men in the LG group and the OG group, respectively. The mean operative duration was 260 minutes (90-420) and 200 (120-360) the LG group and the OG group, respectively (P=0.11). Estimated operative blood loss was 150 ml (50-870) and 240 (120-955) in the LG group and the OG group, respectively (P=0.07). To reduce the risk of postoperative complications, some surgeons routinely adopt a two-stage strategy. The purpose of this study was to analyze our institution's experience with the two-stage procedure for LAGB conversion to LRYGB MATERIALS AND METHODS: The bariatric database of our institution was reviewed to identify patients who had undergone conversion of LAGB to LRYGB from November 2007 to June 2012. One hundred patients were included. Of these, 62 (62%) required conversion to LRYGB for inadequate weight loss or weight regain and 38 for band-related complications. All the procedures were performed in two stages and laparoscopically. The average time between band removal and LRYGB was 17.3 months. The mean follow-up after LRYGB was 31 ± 18.7 months. We choose to treat these patients with initial antibiotic therapy followed by elective appendectomy. We reported the results of this strategy and compared it with urgent appendectomy. A retrospective analysis of patients diagnosed with complicated acute appendicitis (CAA) between 1998 and 2007 treated either by urgent appendectomy or with antibiotic therapy and elective appendectomy was performed. Sulfide shares with the two others (NO and CO) the same inhibiting properties towards mitochondrial respiration. However, in contrast with NO or CO, sulfide at concentrations lower than the toxic (lM) level is an hydrogen donor and a substrate for mitochondrial respiration. This is due to the activity of a sulfide quinone reductase found in a large majority of mitochondria. An ongoing study of the metabolic state of liver in obese patients allowed us to evaluate the sulfide oxidation capacity with twelve preparations of human liver mitochondria. The results indicate relatively high rates of sulfide oxidation with a large variability between individuals. These observations made with isolated mitochondria appear in agreement with the main characteristics of sulfide oxidation as established before with the help of cellular models.

ABSTRACT Objectif Évaluation des résultats de la sleeve gastrectomie (SG) laparoscopique chez 30 ... more ABSTRACT Objectif Évaluation des résultats de la sleeve gastrectomie (SG) laparoscopique chez 30 patients supersuperobèses (IMC > 60 kg/m2). Méthodes Étude prospective étudiant la durée opératoire, la durée de séjour, les complications postopératoires immédiates et à distance, le pourcentage de perte d’excès de poids (% PEP) et l’éventuelle nécessité d’un second temps opératoire (re-SG ou by-pass gastrique) si amaigrissement insuffisant. Ont été inclus : 23 femmes et sept hommes, d’âge moyen de 35 ans (18–59), IMC préopératoire moyen de 66 kg/m2 (60–85). Le poids préopératoire moyen était de 168 kg (140–258). Un patient était porteur d’un situs inversus complet et quatre autres (14 %) avaient un antécédent de gastroplastie par anneau gastrique. Résultats Le temps opératoire moyen a été de 120 minutes (80–220). Il n’y a pas eu de conversion en laparotomie ni de mortalité postopératoire. La durée moyenne de séjour a été de 7,5 jours (4–28). Quatre patients (14 %) ont eu une complication postopératoire : deux hématomes sous-phréniques, une fistule gastrique et une embolie pulmonaire. Parmi les 23 patients avec suivi supérieur à 18 mois, l’amaigrissement a été jugé satisfaisant chez 17 (77 %) et insuffisant chez six (23 %), pour lesquels nous avons réalisé une re-SG (trois) et un by-pass gastrique (deux). Un patient en échec a refusé un second temps opératoire. Trois ans après la SG initiale, la perte de poids moyenne était de 56 kg (28–144), le %PEP moyen de 51 % (21–82) et la perte moyenne d’IMC de 20 kg/m2 (10–39). Lors du suivi, deux patients ont eu une éventration sur site de trocart. Conclusion La SG est associée à peu de complications postopératoires et à distance, et constitue un traitement efficace de la super-superobésité. Il peut s’agir d’une procédure définitive si le résultat à long terme est bon, ou d’une première étape avant une re-SG ou un by-pass gastrique.

Multiple procedures and types of mesh to treat incisional hernia are available. We evaluated outcomes of MIH treated by retromuscular mesh repair (RMR) using a polyester standard prosthesis. Patients A total of 262 patients were treated for MIH by RMR between June 2000 and November 2007 in an academic tertiary referral center using the same standardized surgical technique and one type of mesh. The early complications and recurrence rate were evaluated. Results The average patient age was 57 years; 51% were women. The mean width was 7.8 cm and defect size was 61 cm 2. Previous MIH repair had been performed in 23% of the patients. Average hospital stay was 7 days. Of the 262 patients studied, 34 patients (13%) developed early complications, and 16 required reoperation for various indications. Early mesh infection occurred in 2 patients (0.8%) requiring mesh removal. The mean follow-up was 58 months. Recurrence was observed in 8 patients (3%) with an average delay of 19 months. There was a significant difference in terms of recurrence in patients with mesh infection versus the group who did not develop infection (2/2 patients versus 6/259; P \ 0.001). Conclusions Our results suggest that RMR with a polyester standard prosthesis for MIH remains a safe ''classic'' treatment with a moderate complication rate and a low infection and recurrence rate, even in large incisional hernia. Objective: To evaluate the single incision laparoscopic appendectomy (SILA) using existing instru... more Objective: To evaluate the single incision laparoscopic appendectomy (SILA) using existing instruments, the 10-mm laparoscope, and glove port technique. Methods: SILA was performed on 16 patients (8 male cases, 8 female cases) between June 2012 and September 2015. A 20-mm incision was made in the umbilicus and a wound retractor was placed. A 10-mm trocar for the laparoscope and two 5-mm trocars were fixed to the three fingers of the latex gloves and it was attached to the wound retractor. Another thin forceps were inserted from right low abdomen. Results: Average age of patients was 32.6 ± 17.7 years. Preoperative average white blood cell was 13,325 ± 4,584 /mm 3 , and average CRP was 1.81 ± 3.70 mg/dL. Preoperative body temperature was 36.8 ± 0.5°C. The mean appendix size was 9.6 ± 2.3 mm and none of the patients had an abscess on preoperative CT. The CT also revealed a fecal pellet in 5/16 (31%) of patients. Mean operation time was 66.4 ± 25.4 minutes, and minimal intraoperative bleeding was observed in all patients. Average hospital stay was 5.3 ± 1.9 days and none of the patients had complications. Conclusion: SILA using the 10-mm laparoscope and glove port technique may be a safe and feasible operation for mild to moderate appendicitis.

ABSTRACT Hydrogen sulfide is considered as the third gasotransmitter, comparison with the two oth... more ABSTRACT Hydrogen sulfide is considered as the third gasotransmitter, comparison with the two others (NO and CO) revealing similarities and differences. All three are in fact poisons as potent as cyanide because of the shared property to inhibit cellular respiration. However, in sharp contrast with the others H2S is also a mitochondrial substrate oxidized by mammalian cells if present below its toxic level (#10–50 μM) in the extracellular medium. The enzymatic machinery involves several enzymes and notably a sulfide quinone reductase as well as a dioxygenase. This mitochondrial sulfide oxidation constitutes a sink able to neutralize continuous release of sulfide as long as its concentration remains lower than the low micromolar range. Thus mitochondria are likely to contribute to the control of sulfide signaling. The disturbance of cellular bioenergetics by the combined action of gasotransmitters might underline some of their pharmacological (if not physiological) effects. In this respect the microbiota hosted in the large intestine of mammals constitutes a sulfide source leading to toxic concentrations challenging the epithelium and surrounding tissues. Accordingly, a large capacity for sulfide oxidation is found in the colonic epithelium. Our studies aim to describe sulfide oxidation in mammals and to provide information with regards to its relevance to signaling, and reveal notably that human liver mitochondria have the capacity to neutralize a significant flux of incoming sulfide.

The data consists of 5 stock character macros. The data analysis is based on three theoretical bases: Berger's Code Violation, McCloud's taxonomy of image and text relations in comics and Fauconnier and Turners' Conceptual Integration Theory. The study aims to explore and investigate the humorous nature of stock character macros; how humour is created semiotically; how different signs interact, either equally or subordinately, to produce laughter; and how texts and images are compressed, projected, and blended to create new humorous meanings. The study also aims to define the unique textual and visual characteristics of this type of memes in Egyptian Arabic, a different culture that was not investigated before in such a context. One dimension of the current study is to present a linguistic analysis of a widely recognized but academically underrepresented digital artefact such as image-macro memes. La NAFLD (non-alcoholic fatty liver disease) represente une des complications majeures associees a l'obesite et est consideree comme la manifestation hepatique du syndrome metabolique. Le terme NAFLD regroupe l'ensemble des lesions hepatiques evoluant de la steatose isolee (reversible) vers l'hepatite steatosique (NASH) ayant un potentiel de progression vers la fibrose, la cirrhose et eventuellement le carcinome hepatocellulaire. A l'heure actuelle, les mecanismes responsables de l'instauration chez le patient obese d'une NAFLD, et surtout d'une NASH, ne sont pas elucides. L'objectif de ma these etait de determiner chez des patients obeses si des alterations hepatiques du metabolisme lipidique et/ou de la fonction mitochondriale, dans un contexte de surcharge lipidique du foie, peuvent participer a l'evolution de la NAFLD. Des biopsies hepatiques chirurgicales peroperatoires ont ete obtenues de patients obeses morbides operes d'une chirurgie bariatrique, ainsi que de temoins non obeses. Les flux metaboliques d'acide gras a chaine longue (AGCL) realises a l'aide de l'oleate [1-¹⁴C] et l'exploration de la respiration mitochondriale ont ete realises sur du tissu frais. Les resultats montrent une alteration de la β-oxydation mitochondriale complete des AGCL chez les patients obeses ayant une NASH (reduction de ¹⁴c—CO₂, augmentation des ¹⁴ccorps acido-solubles, et baisse de la cetogenese hepatique). La production de CO₂ (oxydation complete des AGCL) est correlee negativement avec les contenus hepatiques en triglycerides et diacylglycerols, qui sont augmentes chez les patients obeses NASH par rapport aux autres groupes. La respiration mitochondriale a ete mesuree par oxygraphie en presence de differents substrats et inhibiteurs mitochondriaux sur l'homogenat de foie ainsi que sur des mitochondries isolees. Une reduction generale de la respiration a ete observee entre les sujets temoins et tous les groupes de patients obeses. Cependant, la NASH est associee a une diminution significative de la respiration mitochondriale par rapport aux autres patients obeses. Ces alterations ne sont pas associees a une modification de la masse mitochondriale, ni a une alteration de l'expression proteique des complexes OXPHOS de la chaine respiratoire.

We analyzed the impact of time interval to surgery on short-terme outcome parameters in patients undergoing elective laparoscopic left colonic resection for diverticulitis. Materials and methods: Retrospective analysis of two series of case-matched patients according to the timing of operation after the last episode of diverticulitis: group A (within 90 days) and group B (beyond 90 days). Results: 332 patients had left colonic resection for diverticulitis. 117 patients were included in group A vs 114 patients in group B. Overall abdominal morbidity in Group A was 21% vs 5% in group B (p ¼ 0.02). Mean hospital stay was 7.7 days in group A vs 5 days in group B (p ¼ 0.08). Residual inflammation was significantly increased in group A (31%) as compared to group B (11%) (p ¼ 0.01). Conclusions: Laparoscopic left colonic resection for acute diverticulitis is best performed beyond the third month after the last acute episode.

Methods All series with more than 100 patients published between 2002 and 2011 were identified. The results of controlled randomized series comparing this technique with open surgical repair were analyzed. Results Twenty-two series with a total of 7057 patients were analyzed (range 51–1242); the mean Body Mass Index (BMI) was 32 (range: 28.5–46), and the mean size of the hernia defect was 10.6 cm (range: 1.5–14.9 cm). The mean rate of conversion to open surgical repair was 2.7%. Mean operative duration was 90 ± 33 minutes. Mortality was 0.8%, usually due to an unrecognized intestinal injury (1.6%), which occurred more frequently with second or iterative hernia repairs. Hospital stay averaged 2.5 days. The infection rate was 0.3% and the rate of hernia recurrence was 4.6% (with a mean follow-up of 36 months). No statistically significant differences were found in terms of postoperative complications or hernia recurrence in the randomized studies comparing laparoscopic to open anterior abdominal surgical repair. However, duration of hospital stay was shorter after laparoscopy in some studies. Conclusion Laparoscopic repair of ventral incisional hernia is technically feasible and reliable for large hernia defects, even in obese patients. This approach should be reserved for patients with no history of previous hernia repair in order to avoid the risk of intestinal injury. It appears to allow for a shortened duration of hospitalization.

Elle peut etre consideree comme un composant supplementaire du syndrome metabolique (SM). L'insulinoresistance (IR) serait le substratum physiopathologique commun au SM et a la NAFLD. L'hyperactivite sympathique affecte souvent les sujets obeses ou dysmetaboliques. L'inflammation ainsi que l'hyperactivite sympathique pourraient rendre compte de l'evolution de NAFLD vers la steatohepatite (NASH). Notre but etait de determiner dans une population de patients obeses morbides operes d'une chirurgie bariatrique la part des marqueurs de l'inflammation de bas grade, de l'IR, du cortisol plasmatique et de l'activite sympathique dans la maladie hepatique steatosique confirmee par biopsie chirurgicale. Patients et methodes Dans cette etude prospective approuvee par le comite regional d'ethique, des biopsies peroperatoires du lobe gauche du foie ont ete realisees. Chez 36 patients (6 hommes/30 femmes ; âge: 36,4±9,6 ans, IMC 44,8±5,3 kg/m²), les lipides circulants, la glycemie a jeun et apres charge orale en glucose, le cortisol plas-matique (8 h, 16 h), CRP, fibrinogene ont ete doses. HOMA-IR calcule ; l'activite sympathique a ete exploree par dosage de l'adrenaline et la noradrenaline plasmatiques a jeun et 2 heures apres glucose. Le SM etait defini selon les criteres IDF. Resultats Les patients ont ete classes par la biopsie en: 8 foie normal, 17 NAFLD et 11 NASH. Les patients avec NAFLD ou NASH avaient HOMAIR, noradrenaline post-charge et cortisol a 16 heures plus eleves ( p =0,03 pour tous ces parametres). Plus d'entre eux avaient un SM compares aux patients a foie sain ( p =0,04) avec une CRP un peu plus forte ( p =0,08). Conclusion Les patients obeses morbides avec preuve histologique de NAFLD/NASH ont une inflammation et une insulinoresistance plus marquees et une hyperactivite sympathique, des desordres dont le role dans la progression de la maladie hepatique reste a determiner. This study was conducted to evaluate the results of LG for gastric adenocarcinoma in two French surgical departments. Between 2001 and 2007, 51 patients underwent LG for gastric cancer. The results were compared to those of 79 patients who had open gastrectomy (OG) during the same study period. Mean age was 61 years (31-81) and 66 years (27-88) in the LG group and in the OG group, respectively. The sex ratio was 21 women to 30 men and 25 women for 54 men in the LG group and the OG group, respectively. Sulfide shares with the two others (NO and CO) the same inhibiting properties towards mitochondrial respiration. However, in contrast with NO or CO, sulfide at concentrations lower than the toxic (lM) level is an hydrogen donor and a substrate for mitochondrial respiration. This is due to the activity of a sulfide quinone reductase found in a large majority of mitochondria. An ongoing study of the metabolic state of liver in obese patients allowed us to evaluate the sulfide oxidation capacity with twelve preparations of human liver mitochondria. The results indicate relatively high rates of sulfide oxidation with a large variability between individuals. These observations made with isolated mitochondria appear in agreement with the main characteristics of sulfide oxidation as established before with the help of cellular models.

ABSTRACT Objectif Évaluation des résultats de la sleeve gastrectomie (SG) laparoscopique chez 30 ... more ABSTRACT Objectif Évaluation des résultats de la sleeve gastrectomie (SG) laparoscopique chez 30 patients supersuperobèses (IMC > 60 kg/m2). Méthodes Étude prospective étudiant la durée opératoire, la durée de séjour, les complications postopératoires immédiates et à distance, le pourcentage de perte d’excès de poids (% PEP) et l’éventuelle nécessité d’un second temps opératoire (re-SG ou by-pass gastrique) si amaigrissement insuffisant. Ont été inclus : 23 femmes et sept hommes, d’âge moyen de 35 ans (18–59), IMC préopératoire moyen de 66 kg/m2 (60–85). Le poids préopératoire moyen était de 168 kg (140–258). Un patient était porteur d’un situs inversus complet et quatre autres (14 %) avaient un antécédent de gastroplastie par anneau gastrique. Résultats Le temps opératoire moyen a été de 120 minutes (80–220). Il n’y a pas eu de conversion en laparotomie ni de mortalité postopératoire. La durée moyenne de séjour a été de 7,5 jours (4–28). Quatre patients (14 %) ont eu une complication postopératoire : deux hématomes sous-phréniques, une fistule gastrique et une embolie pulmonaire. Parmi les 23 patients avec suivi supérieur à 18 mois, l’amaigrissement a été jugé satisfaisant chez 17 (77 %) et insuffisant chez six (23 %), pour lesquels nous avons réalisé une re-SG (trois) et un by-pass gastrique (deux). Un patient en échec a refusé un second temps opératoire. Trois ans après la SG initiale, la perte de poids moyenne était de 56 kg (28–144), le %PEP moyen de 51 % (21–82) et la perte moyenne d’IMC de 20 kg/m2 (10–39). Lors du suivi, deux patients ont eu une éventration sur site de trocart. Conclusion La SG est associée à peu de complications postopératoires et à distance, et constitue un traitement efficace de la super-superobésité. Il peut s’agir d’une procédure définitive si le résultat à long terme est bon, ou d’une première étape avant une re-SG ou un by-pass gastrique.

Multiple procedures and types of mesh to treat incisional hernia are available. We evaluated outcomes of MIH treated by retromuscular mesh repair (RMR) using a polyester standard prosthesis. Patients A total of 262 patients were treated for MIH by RMR between June 2000 and November 2007 in an academic tertiary referral center using the same standardized surgical technique and one type of mesh. The early complications and recurrence rate were evaluated. Results The average patient age was 57 years; 51% were women. The mean width was 7.8 cm and defect size was 61 cm 2. Previous MIH repair had been performed in 23% of the patients. Average hospital stay was 7 days. Of the 262 patients studied, 34 patients (13%) developed early complications, and 16 required reoperation for various indications. Early mesh infection occurred in 2 patients (0.8%) requiring mesh removal. The mean follow-up was 58 months. Recurrence was observed in 8 patients (3%) with an average delay of 19 months. There was a significant difference in terms of recurrence in patients with mesh infection versus the group who did not develop infection (2/2 patients versus 6/259; P \ 0.001). Conclusions Our results suggest that RMR with a polyester standard prosthesis for MIH remains a safe ''classic'' treatment with a moderate complication rate and a low infection and recurrence rate, even in large incisional hernia. Objective: To evaluate the single incision laparoscopic appendectomy (SILA) using existing instru... more Objective: To evaluate the single incision laparoscopic appendectomy (SILA) using existing instruments, the 10-mm laparoscope, and glove port technique. Methods: SILA was performed on 16 patients (8 male cases, 8 female cases) between June 2012 and September 2015. A 20-mm incision was made in the umbilicus and a wound retractor was placed. A 10-mm trocar for the laparoscope and two 5-mm trocars were fixed to the three fingers of the latex gloves and it was attached to the wound retractor. Another thin forceps were inserted from right low abdomen. Results: Average age of patients was 32.6 ± 17.7 years. Preoperative average white blood cell was 13,325 ± 4,584 /mm 3 , and average CRP was 1.81 ± 3.70 mg/dL. Preoperative body temperature was 36.8 ± 0.5°C. The mean appendix size was 9.6 ± 2.3 mm and none of the patients had an abscess on preoperative CT. The CT also revealed a fecal pellet in 5/16 (31%) of patients. Mean operation time was 66.4 ± 25.4 minutes, and minimal intraoperative bleeding was observed in all patients. Average hospital stay was 5.3 ± 1.9 days and none of the patients had complications. Conclusion: SILA using the 10-mm laparoscope and glove port technique may be a safe and feasible operation for mild to moderate appendicitis.

However, in sharp contrast with the others H2S is also a mitochondrial substrate oxidized by mammalian cells if present below its toxic level (#10–50 μM) in the extracellular medium. The enzymatic machinery involves several enzymes and notably a sulfide quinone reductase as well as a dioxygenase. This mitochondrial sulfide oxidation constitutes a sink able to neutralize continuous release of sulfide as long as its concentration remains lower than the low micromolar range. Thus mitochondria are likely to contribute to the control of sulfide signaling. The disturbance of cellular bioenergetics by the combined action of gasotransmitters might underline some of their pharmacological (if not physiological) effects. In this respect the microbiota hosted in the large intestine of mammals constitutes a sulfide source leading to toxic concentrations challenging the epithelium and surrounding tissues. Accordingly, a large capacity for sulfide oxidation is found in the colonic epithelium. Our studies aim to describe sulfide oxidation in mammals and to provide information with regards to its relevance to signaling, and reveal notably that human liver mitochondria have the capacity to neutralize a significant flux of incoming sulfide.
