Aim: The management of a missed diagnosis of heterotopic pregnancy complicated by uterine perfora... more Aim: The management of a missed diagnosis of heterotopic pregnancy complicated by uterine perforation. Background: Heterotopic pregnancy (HP) is an exceptional condition where at least two pregnancies are present simultaneously at different implantation sites and one of them is located in the uterine cavity. Case description: We report an interesting near miss case of a 23-year-old P3L3A1 with a heterotopic pregnancy who was misdiagnosed as a case of acute abdomen with hemoperitoneum due to uterine perforation, postsurgical evacuation for an intrauterine pregnancy. Exploratory laparotomy was performed which confirmed the diagnosis of heterotopic pregnancy where the ectopic pregnancy was located in the right fallopian tube which was managed by right-sided salpingectomy and uterine rent repair was performed. Conclusion: A high index of suspicion followed by an early surgical laparoscopic/laparotomy intervention can minimize maternal morbidity and mortality in a heterotopic pregnancy. Clinical significance: How a benign and assumably routine case of missed abortion escalated to an eventual diagnosis of a heterotopic pregnancy.

Background: The hitherto rising rate of cesarean sections is affecting women's obstetric careers ... more Background: The hitherto rising rate of cesarean sections is affecting women's obstetric careers by its inherent operative complications and on the other hand, the fetal complications arising from morbid trials of labor has its own medicolegal repercussions. In this scenario, Robson's Ten Group Classification System (TGCS) provides a comprehensive and conclusive system for classifying indications of cesarean sections and helps to curb down rates of cesarean sections. Methodology: The present study was conducted by retrospective data collection in a tertiary care center from January 2019 to December 2019. The study center is a major teaching hospital in Mumbai, which is also a referral center for all peripheral hospitals as well. Result: The rate of cesarean sections in our institute is 44.24%. The rate of cesarean sections (CS) is undoubtedly high. This is attributable to high-risk women coming into labor, and the condition in which these women come warrants the doctor to proceed for CS directly for maternal and fetal salvage. Conclusion: The major focus to bring down CS rates should be to reduce primary CS rates by encouraging good and ethical trials of labor. Encouraging instrumental vaginal delivery, whenever not contraindicated, also will bring down CS rates. Clinical significance: This study helps us understand the lost art of vaginal delivery and rising rates of poorly indicated cesarean sections and the type of indications for which cesarean sections are performed. It helps us introspect and improvise our clinical judgment on this topic.

Background: Wilson's disease results in increased body retention of copper and its eventual toxicity and related effects on several organ systems, especially neurological and hepatic. Case description: Here we study a case of a female with neuropsychiatric and hepatic manifestations of Wilson disease, conceiving spontaneously eventuating with a successful maternal and fetal outcome. Conclusion: With the accurate application of a multidisciplinary team approach with timely investigations, imaging studies, and counseling, successful maternal and fetal outcome in Wilson disease is well within our reach. Clinical significance: Women of reproductive age-groups with Wilson disease, face recurrent miscarriages and poor obstetric outcomes due to copper deposition in the body with the concomitant chronic liver disease and its sequelae making management challenging.

A... more Endometriosis is a benign condition wherein endometrial tissue lies outside the uterine cavity. Abdominal scar endometriosis is a rare entity following pelvic surgeries caused as a result of deposition of endometrial tissue at scar site. Patient usually presents with triad of presence of mass over scar site, cyclical pain during menses and bleeding. Histopathological examination remains gold standard for diagnosis in which the presence of endometrial glands, stroma and hemosiderin laden macrophages can be seen. We presented a case of 35 years old lady with history of caesarean section done 3 years back who came with complaints of swelling over the scar site and cyclical pain with tenderness present over lump. Ultrasonography revealed presence of firm mass which was removed by wide local excision and histopathology confirmed the diagnosis of scar endometriosis.

Background: To study the outcome of fluticasone nasal sprays in smell disorders and triamcinolone... more Background: To study the outcome of fluticasone nasal sprays in smell disorders and triamcinolone paste in taste dysfunction in a population of laboratory-confirmed SARS-CoV-2 patients as the test group. The control group will not be given any intervention and only monitoring of these symptoms will be done to compare the recovery time. Methods: This prospective interventional study was conducted from June to Nov 2020 at, Datta Meghe University during the COVID-19 outbreak. The 120 enrolled patients were tested at days 1 and 5 after proven infection by RT-PCR test. Result: The mean age for all cases is 50.88 ± 15.93 years, whereas for the controls mean age is 51.2 ± 14.89. 2. Among cases 45 (75%) were males and 15 (25%) were females, among controls 43 (71.66%) were males and 17 (28.33%) were females. Among the case group, after the use of fluticasone spray in the nose and triamcinolone paste in the mouth there was a statistically significant improvement in recognizing all the odours and taste on day 5 compared to day 1. On comparing the smell and taste of cases and control group, either there is no improvement or worsening in smell or taste on day 5 in the control group. Conclusion: The use of fluticasone nasal spray and triamcinolone paste had immensely influenced the basic senses such as smell and taste. Our study showed that olfactory and taste function significantly improved in patients with COVID-19. For all anosmia and dysgeusia cases who received fluticasone nasal spray and triamcinolone medications the recovery of smell senses and the taste was within a week. ☆ Dr C. Aim and Objectives: The present study was undertaken to find out the role and effectiveness of DI... more Aim and Objectives: The present study was undertaken to find out the role and effectiveness of DIPSI guidelines as a replacement for other more time consuming and cost effective methods for detecting gestational diabetes mellitus (GDM) in the Indian population and to compare outcome between GDM and non GDM population.Methods: A total of 345 women were selected of low risk category between the gestational ages of 24 to 28 weeks and were subjected to screening for gestational diabetes by DIPSI guidelines. A 75mg oral glucose load was given irrespective of their last Meal timing followed by blood glucose estimation by glucose oxidase – peroxidase method. A report of ≥140mg/dl were labeled as GDM as per DIPSI guidelines.Results: Out of 345 subjects screened, 30 (8.7%) were positive for GDM. The false positives encountered with DIPSI were 0.57% (2/345) and another 2 cases (0.57%) were false negative by DIPSI criteria. We present this case of ependymoma to highlight the fact that aggressive management might be keystone in saving mothers life and even after surgical intervention, it is possible that the tumour recurs and may prove fatal. Maternal outcome in primary brain tumours in pregnancy largely depend on histologic grade of primary tumour and gestational age at which they present.

Heterotopic salivary glands are rare lesions in the neck and when present appear very similar to branchial cleft sinus or fistula. This congenital lesion is rare. This is probably the first report from India. It is important to report this case to raise the awareness of this condition.

Despite their common origins, the coexistence of these anatomical abnormalities is quite unusual. This cholesteatoma may go unnoticed till adulthood and present with complications. Moreover, first branchial cleft malformations are often unrecognized or misdiagnosed for other inflammatory lesions in the periauricular region. We report a case of middle-ear cholesteatoma in a 68 year old male patient with grade II microtia with canal stenosis and first branchial cleft fistula in supraauricular region presenting with recurrent infranuclear facial nerve palsy. The patient was managed surgically by excision of fistulous tract, canalplasty, auricular reconstruction and middle ear and mastoid exploration for cholesteatoma. This may probably be the oldest person to be managed surgically for cholesteatoma due to congenital ear anomalies and first such reported case.

There are historic descriptions of its association with systemic illnesses like typhoid fever or as a result of spread of infection following upper respiratory tract catarrh. The etiology of laryngeal abscess in recent times has changed to those with underlying malignancy, trauma due to airway instrumentation, nasogastric intubation, and external beam radiotherapy. Here we report a case of acute laryngeal abscess following catarrh, identical to the historic descriptions, probably the first such report in the modern era, highlighting the importance of its early diagnosis and high index of suspicion, which led to a very good recovery in our patient.

Objective: The aim of the present study was to evaluate the structural variations of nasal cavity... more Objective: The aim of the present study was to evaluate the structural variations of nasal cavity in reference to frequency and types at the key area i.e. the ostiomeatal complex. Materials and Methods: Computed tomography of Paranasal sinuses of 50 patients was studied for clinical suspicion of various sinonasal pathologies. Results: The most commonly encountered anatomical variations in this study were Deviated Nasal Septum in 78% (39 patients), followed by Concha Bullosa in 36% (18 patients), Agger Nasi cell in 18% (nine patients), Pneumatised septum in 12% (six patients), Paradoxical Middle Turbinate and Septated Maxillary Sinus in 10% (five patients each) and Pneumatised Uncinate Process 6% (three patients). In quite a few patients we witnessed more than one variation. Conclusion: The anatomical variations in the nose and ostiomeatal complex are not uncommon, with the most frequent ones involving the nasal septum and the middle turbinate.

Background: To study the outcome of fluticasone nasal sprays in smell disorders and triamcinolone... more Background: To study the outcome of fluticasone nasal sprays in smell disorders and triamcinolone paste in taste dysfunction in a population of laboratory-confirmed SARS-CoV-2 patients as the test group. The control group will not be given any intervention and only monitoring of these symptoms will be done to compare the recovery time. Methods: This prospective interventional study was conducted from June to Nov 2020 at, Datta Meghe University during the COVID-19 outbreak. The 120 enrolled patients were tested at days 1 and 5 after proven infection by RT-PCR test. Result: The mean age for all cases is 50.88 ± 15.93 years, whereas for the controls mean age is 51.2 ± 14.89. 2. Among cases 45 (75%) were males and 15 (25%) were females, among controls 43 (71.66%) were males and 17 (28.33%) were females. Among the case group, after the use of fluticasone spray in the nose and triamcinolone paste in the mouth there was a statistically significant improvement in recognizing all the odours and taste on day 5 compared to day 1. On comparing the smell and taste of cases and control group, either there is no improvement or worsening in smell or taste on day 5 in the control group. Conclusion: The use of fluticasone nasal spray and triamcinolone paste had immensely influenced the basic senses such as smell and taste. Our study showed that olfactory and taste function significantly improved in patients with COVID-19. For all anosmia and dysgeusia cases who received fluticasone nasal spray and triamcinolone medications the recovery of smell senses and the taste was within a week. ☆ Dr C. A 75mg oral glucose load was given irrespective of their last Meal timing followed by blood glucose estimation by glucose oxidase – peroxidase method. A report of ≥140mg/dl were labeled as GDM as per DIPSI guidelines.Results: Out of 345 subjects screened, 30 (8.7%) were positive for GDM. This is probably the first report from India. It is important to report this case to raise the awareness of this condition.

Despite their common origins, the coexistence of these anatomical abnormalities is quite unusual. This cholesteatoma may go unnoticed till adulthood and present with complications. Moreover, first branchial cleft malformations are often unrecognized or misdiagnosed for other inflammatory lesions in the periauricular region. We report a case of middle-ear cholesteatoma in a 68 year old male patient with grade II microtia with canal stenosis and first branchial cleft fistula in supraauricular region presenting with recurrent infranuclear facial nerve palsy. The patient was managed surgically by excision of fistulous tract, canalplasty, auricular reconstruction and middle ear and mastoid exploration for cholesteatoma. This may probably be the oldest person to be managed surgically for cholesteatoma due to congenital ear anomalies and first such reported case.

There are historic descriptions of its association with systemic illnesses like typhoid fever or as a result of spread of infection following upper respiratory tract catarrh. The etiology of laryngeal abscess in recent times has changed to those with underlying malignancy, trauma due to airway instrumentation, nasogastric intubation, and external beam radiotherapy. Here we report a case of acute laryngeal abscess following catarrh, identical to the historic descriptions, probably the first such report in the modern era, highlighting the importance of its early diagnosis and high index of suspicion, which led to a very good recovery in our patient.

Materials and Methods: Computed tomography of Paranasal sinuses of 50 patients was studied for clinical suspicion of various sinonasal pathologies. Results: The most commonly encountered anatomical variations in this study were Deviated Nasal Septum in 78% (39 patients), followed by Concha Bullosa in 36% (18 patients), Agger Nasi cell in 18% (nine patients), Pneumatised septum in 12% (six patients), Paradoxical Middle Turbinate and Septated Maxillary Sinus in 10% (five patients each) and Pneumatised Uncinate Process 6% (three patients). In quite a few patients we witnessed more than one variation. Conclusion: The anatomical variations in the nose and ostiomeatal complex are not uncommon, with the most frequent ones involving the nasal septum and the middle turbinate.
