Levatores prostate was visualized on a cadaver dissection previously. PATIENTS AND METHODS Eighty outpatients with lower urinary tract complaints or were suspected clinically to have prostate cancer were submitted to TRUS-guided prostate biopsy. The ages of outpatients were ranging from 45 to 81. Patients were randomized in 2 groups: Group-I, with 40 patients submitted to local anesthesia by periprostatic injection of 1 cc 1% lidocaine before biopsy; and group-II, with 40 controls the biopsy was performed without local anesthesia. The anatomical region for anesthesia was determined via dissection. The name of this anatomical region is levatores prostatae and it has got high nerve density. Objective: Higher morbidity rates, extensive scar tissue formation and longer hospitalization per... more Objective: Higher morbidity rates, extensive scar tissue formation and longer hospitalization periods of the standard open surgical methods for ureteropelvic obstruction has led to acceleration of minimal invasive techniques. Success rate and clinical outcomes of laparoscopic pyeloplasty has become comparable with open surgery. Purpose: The aim was to evaluate the clinical results and complications of transperitoneal laparoscopic pyeloplasty in ureteropelvic obstruction. Material and Methods: Thirty-three patients with ureteropelvic obstruction were enrolled into this study. Eighteen patients underwent Anderson-Hynes dismembered Pyeloplasty and 15 underwent Y-V plasty. Patients were followed-up at postoperative 3., 6. months and then yearly. Partial or total relief of symptoms and improvement in diuretic renogram were accepted as success. Results: Median age was 34.5±15.5(13-74) years, number of males were 13 (39.4%) and females were 20 (60.6%). Median Body Mass Index (BMI) was 25.3±15.4(18.5-33.4). Eight were asymptomatic, and 23 had intermittent pain, preoperatively. Fourteen cases had left ureteropelvic obstruction (42.4%) and 19 had in the right kidney. Intraoperatively 19 cases had aberrant vessel. Mean surgery time was 127.9±38.9 (68-245) minutes, median anastomosis time was 20.8±7.3 (8-39) minutes. Median blood loss was 57.1±28.3 (20-150) ml, median postoperative drainage time was 2.6±1.1 (2-7) days. Only one had prolonged ileus and peritoneal irritation findings. Median narcotic and nonnarcotic requirements were 21.5±4.8 (15-30) and 132.6±37.2 (75-200) mg/day, respectively. Median follow-up period was 35.1±13.6 (11-59) months. Conclusions: Laparoscopic pyeloplasty, is minimally invasive and reliable technique replaces open pyeloplasty in many institutions. Shorter hospitalization, lower postoperative morbidity rates, better cosmetic results and higher success rates can be easily achieved..

Spinal cord injuries (SCIs) may cause predictable abnormalities in genital sexual arousal and cha... more Spinal cord injuries (SCIs) may cause predictable abnormalities in genital sexual arousal and changes in the ability to achieve orgasm. In this review, sexual dysfunction and reproductive abnormalities and their treatment possibilities in male and female patients with SCI were reviewed. Literature from 1982-2011 examining the neuroanatomy of sexual functions, sexual dysfunction and reproductive abnormalities and their treatment alternatives both in male and female patients with SCI was reviewed with PubMed. In both genders, the sympathetic, parasympathetic and motor-sensory innervations of genitals are localized at the T11-L2, and S2-4 and S2-3 spinal cord levels, respectively. Patients with lesions above or at level of T11-L2 have reflex erections but no psychogenic erections. Patients with lesions in between levels of L2 and S2 spinal segments have reflex and psychogenic erections. Patients with lesions at level of S2-3 or below can have psychogenic erections but no reflex erections. Patients with lesions above the T10 level have autonomic, somatic, vibration and electroejaculations but no nocturnal ejaculation. Patients with lesions at the level of the thoracolumbar (T11-L2) region can have electroejaculation but no autonomic, somatic, vibration and nocturnal ejaculations. Patients with lesions below the L2 levels can produce emissions and electroejaculation but no other type of ejaculation. Reproductive dysfunction in men with SCI is due to a combination of ED, ejaculatory failure, and abnormal semen parameters. Women with no lesions at the T11-L2 levels may have psychogenic genital vasocongestion, while reflex lubrication and orgasm is more prevalent in women who have preserved the sacral reflex arc (S2-S5). A better knowledge of neuroanatomy and neurophysiology in SCI patients provides a more predictable understanding of sexual and reproductive dysfunctions.

Üriner sistem taşına bağlı herhangi bir obstrüksiyonun kontrastsız çok kesitli bilgisayarlı tomografi (KÇKBT) ile belirlenmesi tedavi zamanlaması hakkında karar vermek açısından çok önemlidir. Çalişmamızda deneysel hidronefroz oluşturulan ratlarda renal parankim dansitesinde meydana gelen değişiklikler KÇKBT ile değerlendirildi. Bu çalışmada sol üreterlerinde total veya parsiyel obstrüksiyon oluşturulmuş Spraque-Dawley ratların böbrek parankim dansiteleri deneyin 1.,3. ve 30. günlerinde KÇKBT ile değerlendirilerek, sham ve kontrol grubu ratların böbrek parankim dansiteleri ile karşılaştırıldı. Parsiyel üreteral obstrüksiyon oluşturulmuş ratların böbrek parankim dansiteleri ile kontrol grubunun böbrek parankim dansiteleri arasında anlamlı bir fark olmadığı saptandı. Bununla birlikte total üreteral obstrüksiyon oluşturulmuş ratların böbrek parankim dansitelerinin karşı taraf böbrekleri ve kontrol grubu ile karşılaştırıldığında anlamlı olarak azaldığı saptandı. Böbrek üst, orta ve alt segmentlerinin ortalama dansite değerleri benzerdi. Elde edilen bu bulguların insanlarda üreter obstrüksiyonun sonuçlarının değerlendirilmesinde yararlı olabileceğini düşünmekteyiz. KÇKBT değerlendirilmesinde renal parankim dansitesinin azalması renal parankimal hasarın bir göstergesi olabilir.

Only 26 cases of this rare testicular tumor have been reported in the literature, 6 of which developed metastases. We report a case of granulosa cell tumor of adult type in a 47-years-old man. Within three-year follow-up period without any adjuvant therapy, no metastatic progression was observed. A adult-type testicular granulosa cell tumor is reported with reference to literature-based discussion.

The aim of this study is to show and compare the actions of Simvastatin (Simv) and Erythropoietin (Epo) in renal expression of nuclear factor kappa B (NFκB), transforming growth factor-β (TGF-β), basic fibroblast growth factor (bFGF), platelet-derived growth factor B (PDGF-B), fibronectin and development of interstitial fibrosis in rats with unilateral ureteral obstruction (UUO). A total of 48 Sprague-Dawley rats were allocated to 4 groups of sham, Epo, Simv and control. Unilateral ureteral ligation was performed on all rats except the Sham group. For interstitial fibrosis Masson&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s trichrome stain and for the expression of TGF-β, PDGF-B, bFGF, NFκB and fibronectin, immunohistochemical methods were used. In the Epo and Simv groups, expression of TGF-β and fibronectin and staining with Masson&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s trichrome were less compared to the control group. In addition, fibronectin expression in the Epo group was less than the Simv group. Unlike the Simv group, NFκB and bFGF expression in the Epo group were less when compared to the control group. Consequently, it was seen that both Epo and Simv prevented fibrosis in ON. Epo was superior in this effect by suppressing the expressions of NFκB and bFGF more effectively than Simv. Based on this finding, Epo might be a better agent than Simv in the prevention of fibrosis in ON.

We investigated Peyronie's disease prospectively in cases treated for penile fracture (PF) within the last 20 years. Medical records of 63 cases treated for PFs were reviewed. Subjects were required to self-assess their current penile morphologies and sexual functions. Penile nodules and Peyronie's plaques were also evaluated with physical examination, ultrasonography and magnetic resonance imaging (MRI), and penile curvatures with auto-photography, and sexual function with international erectile function index (IIEF). Of the 63 cases (mean age 37 years), 46 who had mean follow-up of 63 months were re-evaluated. The mean IIEF-5 score was 23.2±3.1. Painful erections (n ¼ 5), penile nodules (n ¼ 5) and also penile curvatures o201 (n ¼ 2) were investigated. No Peyronie's plaque was palpated in any of the cases. Ultrasound and MRI showed fibrotic nodules of 5 mm in diameter, which extended into the subtunical area in the rupture site in 54% of the cases, although any thickening and Peyronie's plaque were not found in the tunica albuginea and intracavernosal septum of the cases examined. In PF patients treated surgically, the erectile function and penile morphology were preserved. In our cases PFs did not induce the development of Peyronie's disease.

Patients with lesions at level of S2-3 or below can have psychogenic erections but no reflex erections. Patients with lesions above the T10 level have autonomic, somatic, vibration and electroejaculations but no nocturnal ejaculation. Patients with lesions at the level of the thoracolumbar (T11-L2) region can have electroejaculation but no autonomic, somatic, vibration and nocturnal ejaculations. Patients with lesions below the L2 levels can produce emissions and electroejaculation but no other type of ejaculation. Reproductive dysfunction in men with SCI is due to a combination of ED, ejaculatory failure, and abnormal semen parameters. Women with no lesions at the T11-L2 levels may have psychogenic genital vasocongestion, while reflex lubrication and orgasm is more prevalent in women who have preserved the sacral reflex arc (S2-S5). A better knowledge of neuroanatomy and neurophysiology in SCI patients provides a more predictable understanding of sexual and reproductive dysfunctions.

Üriner sistem taşına bağlı herhangi bir obstrüksiyonun kontrastsız çok kesitli bilgisayarlı tomografi (KÇKBT) ile belirlenmesi tedavi zamanlaması hakkında karar vermek açısından çok önemlidir. Çalişmamızda deneysel hidronefroz oluşturulan ratlarda renal parankim dansitesinde meydana gelen değişiklikler KÇKBT ile değerlendirildi. Bu çalışmada sol üreterlerinde total veya parsiyel obstrüksiyon oluşturulmuş Spraque-Dawley ratların böbrek parankim dansiteleri deneyin 1.,3. ve 30. günlerinde KÇKBT ile değerlendirilerek, sham ve kontrol grubu ratların böbrek parankim dansiteleri ile karşılaştırıldı. Parsiyel üreteral obstrüksiyon oluşturulmuş ratların böbrek parankim dansiteleri ile kontrol grubunun böbrek parankim dansiteleri arasında anlamlı bir fark olmadığı saptandı. Bununla birlikte total üreteral obstrüksiyon oluşturulmuş ratların böbrek parankim dansitelerinin karşı taraf böbrekleri ve kontrol grubu ile karşılaştırıldığında anlamlı olarak azaldığı saptandı. Böbrek üst, orta ve alt segmentlerinin ortalama dansite değerleri benzerdi. Elde edilen bu bulguların insanlarda üreter obstrüksiyonun sonuçlarının değerlendirilmesinde yararlı olabileceğini düşünmekteyiz. KÇKBT değerlendirilmesinde renal parankim dansitesinin azalması renal parankimal hasarın bir göstergesi olabilir.

The aim of this study is to show and compare the actions of Simvastatin (Simv) and Erythropoietin (Epo) in renal expression of nuclear factor kappa B (NFκB), transforming growth factor-β (TGF-β), basic fibroblast growth factor (bFGF), platelet-derived growth factor B (PDGF-B), fibronectin and development of interstitial fibrosis in rats with unilateral ureteral obstruction (UUO). A total of 48 Sprague-Dawley rats were allocated to 4 groups of sham, Epo, Simv and control. Unilateral ureteral ligation was performed on all rats except the Sham group. For interstitial fibrosis Masson&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s trichrome stain and for the expression of TGF-β, PDGF-B, bFGF, NFκB and fibronectin, immunohistochemical methods were used. In the Epo and Simv groups, expression of TGF-β and fibronectin and staining with Masson&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s trichrome were less compared to the control group. In addition, fibronectin expression in the Epo group was less than the Simv group. Unlike the Simv group, NFκB and bFGF expression in the Epo group were less when compared to the control group. Consequently, it was seen that both Epo and Simv prevented fibrosis in ON. Epo was superior in this effect by suppressing the expressions of NFκB and bFGF more effectively than Simv. Based on this finding, Epo might be a better agent than Simv in the prevention of fibrosis in ON.

We investigated Peyronie's disease prospectively in cases treated for penile fracture (PF) within the last 20 years. Medical records of 63 cases treated for PFs were reviewed. Subjects were required to self-assess their current penile morphologies and sexual functions. Penile nodules and Peyronie's plaques were also evaluated with physical examination, ultrasonography and magnetic resonance imaging (MRI), and penile curvatures with auto-photography, and sexual function with international erectile function index (IIEF). Of the 63 cases (mean age 37 years), 46 who had mean follow-up of 63 months were re-evaluated. The mean IIEF-5 score was 23.2±3.1. Painful erections (n ¼ 5), penile nodules (n ¼ 5) and also penile curvatures o201 (n ¼ 2) were investigated. No Peyronie's plaque was palpated in any of the cases. Ultrasound and MRI showed fibrotic nodules of 5 mm in diameter, which extended into the subtunical area in the rupture site in 54% of the cases, although any thickening and Peyronie's plaque were not found in the tunica albuginea and intracavernosal septum of the cases examined. In PF patients treated surgically, the erectile function and penile morphology were preserved. In our cases PFs did not induce the development of Peyronie's disease.

When imaging findings suggest that an intratesticular mass is likely to be an epidermoid cyst, conservative management (enucleation with testicular preservation) can be performed rather than orchidectomy. This case report records an epidermoid cyst in a 12-year-old boy in whom the US findings allowed testis-sparing surgery instead of orchidectomy.
