However, certain clinical situations do not render the use of implants in which case, the cast partial dentures are still the choice of treatment. During the fabrication of the cast partial denture, transfer of occlusal rest seat to the working cast is done using an impression material. Since the prepared rests are of small dimensions, clinicians generally are not able to isolate defects made in the recording of these preparations. A simple technique that isolates these areas during the recording of occlusal rests is presented. The technique uses colored ink on the prepared area which get transferred to the impression as well as the master cast. The technique is simple and inexpensive and can be particularly useful in academic settings.

Histologically, it has been seen that normal tooth tissue varying from enamel, dentin and pulp may be present within the extra cusp. When present on anterior teeth, it can change occlusal protection that is natural to permanent dentition. We present one such rare case of a talon cusp on maxillary right lateral incisor, which in centric occlusion pushed the mandible posteriorly. The patient had observed temporomandibular joint (TMJ) discomfort but believed it to be due to an endodontically treated tooth. Differentiation was done by occlusal marking of centric and deflected contacts. The patient could not go for orthodontic treatment, so occlusal equilibration was performed to eliminate the posterior slide in centric occlusion. After one-month recall, the patient was no longer feeling any sign of discomfort in the TMJ.

Submandibular fossa is an essential anatomical landmark for prosthodontist, since its functional form can affect the outcome of removable and fixed prosthesis. Palpation of this anatomical landmark should be routine and compulsory, since the depth of the fossa can be subdued due to the presence of the gland. Normally the depth is considered to be in the range of 5 to 6 mm (average 2 mm, we present a unique case of an elderly completely edentulous male patient, who presented with a depth of 11.5 mm and the width of 12.7 mm. Fabrication of the complete denture prosthesis was modified for both maxillary and mandibular arches, including the design of the complete denture prosthesis.

Oral procedures resulting in iatrogenic trauma are common, especially in educational settings. We present a case of an elderly male patient who reported for fabrication of a complete denture prosthesis. Routine clinical examination revealed the presence of large, dilated, tortuous, blue/ purple coloured, bilaterally distributed varicose veins on the undersurface of the tongue. Lesion blanched on the application of pressure. The complete denture fabrication was done after consultation with a physician who instructed to be careful while performing dental procedures. Sharp edges of instruments, materials and procedures were avoided during the fabrication of the prosthesis. A complete denture with balanced occlusion was fabricated and delivered to the patient, who reported being satisfied with the outcome of the denture.

Tooth preparation to receive either restorations or ornaments has been practiced since early ages. Methods of preparing a tooth have evolved from hand cutting to foot driven instruments and then to current day ultra high speed instrumentation and laser cuttings. Basic principles of tooth preparation have also evolved as better understanding of the tooth structure and the material to replace it became more vivid. One of the most efficient clinical guide in tooth preparation is the placement of depth orientation grooves on the surface (known diameter) and then follow the groove at various angles to complete a conservative tooth preparation. However, it has been the experience of the authors that many students fail to understand its clinical importance, and lose the chance to develop the skill of a conservative tooth preparation. Either they underprepared the tooth or they overprepare the tooth. This article is basically a picturesque presentation of various aspects of an orientation groove which has been dissected and each dissected component has been clinically related to the effect that it produces. The article has both academic and clinical significance. Orientation grooves have been evaluated in different physical and mechanical aspects followed by relating them biomechanically to the clinical success. Different aspects of the orientation grooves have come to light like groove anatomy, groove number, groove position, axial inclination and groove dimensions (length, width and depth). The physical, biological and aesthetic principles of the preparation are related to the ideal tooth preparation, which is directly a result of properly placed depth orientation grooves.

from abuse/neglect, which in turn has been associated with anxiety, depression, and helplessness in
the individual. Family caregivers (blood relations), being the most common perpetrators of elder
abuse and neglect (EAN), have also been shown to respond to sensitization if the type of EAN and
the interventions are appropriate. This study was aimed to comparatively analyze the influence
of intervention (psychotherapeutic sensitization of FCG) upon long-term (24 months) treatment
maintenance and satisfaction in elderly neglected patients. One hundred and fifty patients (aged
41–80 years) suffering from elder neglect (EN) (self-confession) and their respective FCGs, fulfilling
the study criteria, participated in this longitudinal 2-year study. The patients were randomly distributed
(simple random, convenient) in two equal groups (75 each), namely Group (GP) A (control)
and GP B (test). A standardized, complete denture treatment was initiated for all the participants.
Both the FCGs and the patients of GP B were sensitized (psychotherapeutic education) for EN,
while there was no such intervention in GP A. The influence of such intervention was measured
for denture maintenance [denture plaque index (DPI) scores] and treatment satisfaction (10-point
visual analog scale). Absolute/relative frequencies and means were major calculations during data
analysis. Differences between the groups for any treatment compliance parameter was done through
the unpaired t-test, while Karl Pearson’s test determined the level of relationship between variables
(p-value < 0.05). Decrease in mean DPI scores (suggesting improvement) was seen among patients
in GP A from 1 month (m = 2.92) to 24 months (m = 2.77). A negligible increase in DPI scores was
observed among patients of GP B from 1 month (m = 1.38) to 24 months (m = 1.44). Differences in
mean values between the two groups were statistically significant at 24-month intervals, while the
relationship between the variables was nonsignificant. FCG sensitization through psychotherapeutic
education shows a long-term positive influence on the treatment compliance (maintenance and
satisfaction). Identifying the existence of EAN among geriatric patients, followed by psychotherapeutic
education of FCGs is recommended for routine medical and dental long-duration treatment
procedures.

With the quantity and quality of the facial vasculature, injuries in the region heal very fast compared to the other parts of the human body. Natural teeth either whole or even a small part have been implicated in the development of dysplasia if they irritate the mucosa continuously. We present a case of a non-healing traumatic ulcer of the mandibular lip, suffered as a result of a fall which was not healing in a young boy aged 19 years. The extraoral examination demonstrated protruded large mandibular lip, protruded maxillary teeth and a large ulcer on the mandibular lip. Intraoral examination revealed a loss of maxillary central incisors with the remaining two lateral incisors directly traumatizing the mandibular lip. Enameloplasty, nightguard and multivitamin therapy were used as a multi-directional treatment plan. The non-healing ulcer healed within a space of a week.

This article focusses on integrating the theoretical attributes of color (hue, value, chroma) to applied clinical sciences, through patient based learning. Cases presented in this article are collection of complex fixed partial denture designs and treatment plannings along with different cases of shade manipulation to overcome the shortcomings of shade guides. 47 female and 40 male students opted enrollment for Clinical Fixed Prosthodontics (SDS 543 and SDS 544) course, for academic year (2020–2021). Students submitted their clinical cases at the end of the course along with their continuous assessment grades. Case selection was based on case history, accurate diagnosis and complexity in prosthesis design. A split pontic type of non rigid connector was incorporated to overcome the problems of pier abutment. A fluted retainer was used for a three unit fixed partial denture in the complex rehabilitation of a patient who had suffered from a gunshot wound. Multiple cases of non conventional shade matching are presented to demonstrate the clinical success of these restorations. A case of wrong perception of tooth shade in a patient is also presented. Students technician communication and the fallout of poor communication is also presented.

Medical/dental education is no exception. Teaching as well as student learning has been compromised with norms like social distancing, personal protective wear and lockdowns. Reduced school times, absence of group discussions and clinical demonstrations have prompted teachers to convey course information in different ways. The fear of contracting coronavirus and then infecting respective families adds to the always stressed medical students and staff. Prior to covid 19, studies have shown students to be less attentive which leads to their poor retention of case based information taught in the clinical sessions. During the pandemic, theory classes could be managed through online portals, but clinical learning in patients cannot be compensated by such means. With a contextual background of such factors, this two part case series is an attempt to fulfill the void in teaching that has occurred during a pandemic. 87 students (47 female, 40 male) who were enrolled for the course (Clinical Fixed Prosthodontics) (SDS 543 and SDS 544) for academic year December 2020-June 2021 were asked to submit their clinical cases on a template along with the grades obtained. Cases of fixed partial denture in Kennedy class 3 partial edentulous situation were selected on the basis of complexity in history, clinical examination, diagnosis and prosthesis design. All cases were analysed under two main heading (psychosocial learning, applied basic sciences). In this the first part of the article, multiple selected cases are presented in a four tier series. Major or minor learning skills that students lacked have been highlighted and discussed. Since many students were not able to see each others cases, the authors predict that publishing the cases will achieve two objectives. Firstly, all students will be able to see each others work and what they were taught and secondly, students are expected to take critical evaluation of authors seriously and use this platform as a means to improve those deficiencies.

It is basically a mechanism for lubricating the skin. the oil produced is a mixture of triglycerides and fatty acid products besides some esters. The secretion makes the surface of the skin impervious to the moisture. Fordyce spots are heterotopic sebecaous glands that can be found on vermilion border of the lips. This article presents a unique case of fordyce spot of vermilion border of the maxillary and mandibular lip along with associated lesions on the surface of the penis and scrotum. The patient was looking for getting a single crown in relation to maxillary right central incisor which was endodontically treated. The patient failed to report for the treatment. diagnosis of fordyce spots was based on history and clinical findings although confirmation of such cases is based on histopathological findings.

They need to function in wet environments like saliva and during non function, they need to be stored in water. This study was aimed to assess the influence of water sorption on flexural strength of two heat and self cure denture resins and to evaluate any difference between storage in artificial saliva and water on its flexural strength. A digitally designed stainless steel die was used to fabricate molds for making samples from heat cure (Trevalon HI, Colto Cure H) and self cure (Colto Cure C,Dentsply RR) denture base acrylic resins. Samples were tested immediately after processing (Group (GP) Acontrol), immersed in water (GP B) and artificial saliva (GP C). After storage of samples at four different time intervals (1 day, 1 week, 2 months, 4months), flexure test was performed on a universal testing machine. Various levels of variables were tested using Kruskal-Wallis H test (p<0.001) while differences between groups was tested using Mann Whitney test (p<0.05). Trevalon demonstrated highest dry strength. All samples demonstrated decline in flexural strength with heat cure showing less than self cure denture base resins. Reduction of flexural strength observed at 2 and 4 month time, showed a statistical difference between groups. Reduction of flexural strength was also significant when self cure resins were stored in artificial saliva. Flexural strength tends to decrease as water sorption within the denture polymer increases. Both saliva and water show pronounced effect on flexural strength.

Research paper thumbnail of Comparative Assessment of the Influence of Aging and Presence of Teeth on the Oral Haptic Perception and Its Relevant Clinical Application in Oral Diagnostic/Therapeutic Procedures

Annals of the Romanian Society for Cell Biology , 2021

Oral haptic perception (OHP) is a potential clinical tool for diagnostic/therapeutic procedures i... more Oral haptic perception (OHP) is a potential clinical tool for diagnostic/therapeutic procedures in medical (endoscopy/intubation) and dental (oral appliances/prosthesis) science. This study was aimed to evaluate the influence of age and tooth loss on OHP scores and the response time (RT) alongwith the impact of denture wearing on the same. The study was conducted on 120 adult subjects, distributed in two groups, Group A [Dentulous (D)] (n=40) and Group B [Edentulous (E)] (n=80). Each group was further divided into subgroups [Group A1 (20-30 yrs), Group A2 (60-70 yrs), Group B1 (No dentures), B2 (maxillary denture), B3 (mandibular denture) and B4 (both dentures)]. Six acrylic specimens (circle, semicircle, ellipse, square, rectangle and triangle) in two different sizes were identified by the patient using oral cavity to quantify an OHP score. Unpaired "t" test was used to compare the means while one way ANOVA (F-test) judged the significance of the different p values. The OHP was found to decrease with age [46.05(20-30 yrs) vs 42.2 (60-70 yrs)] and tooth loss [42.2 (D) vs 24.95 (E) while RT was found to increase as one ages [66.6 (20-30 yrs) vs 277.45 (60-70 yrs) and with loss of tooth [277.45 (D) vs 372.4 (E)]. While the differences for the OHP scores were statistically significant at different levels of p values, the scores of RT were significantly different for only group. OHP scores improved with wearing of both dentures (33.7) as compared to remaining edentulous (24.95). Sensory capabilities of the oral cavity vary with individuals, age and the presence of teeth/dentures. Determination of OHP can predict difficulties to be encountered in diagnostic/therapeutic procedures like endoscopy/intubation/ denture wearing.

This is not because of difficulty to restore but the greater challenge is which prosthesis to select. Conventionally, cantilever fixed partial denture has been the choice in such situations. Advances in implant have, however affected the occurrence of such cases. However, patients where other treatment options are contraindicated, cantilever supported fixed partial denture is an excellent option, provided one has the knowledge and application of the dynamics of occlusion. This article provides an insight of two clinical situationsone where cantilever is given with additional rest and the other where cantilever prosthesis would suffice.

A physician ruled out the existence of any skeletal, cardiovascular or genital signs or symptoms to rule out syndrome. Porcelain fused to metal crowns, were cemented on the endodontically treated teeth.

The molding of the affected complex is carried out in two stages, first is the alveolar molding which is carried out by the oral plate, that is retained extraorally and the second stage involves fabrication of a nasal stent which molds the nasal complex and also reduces the pressure on the nasal cartilage by removing the force, that is exerted as a result of drooping of the tissues. The appliance, after wearing, approximated the cleft segments close together which minimized the extent and the number of surgeries.
