The study was taken to evaluate the students’ perceptions about the need of speaking skills and the urgency to participate in language enhancement activities keeping in view the generally unsatisfactory position of the Saudi students in speaking English language. The sample population for this research was taken from three colleges, i.e. the first grade students from Engineering, Business and Computer Science of this university. Image of white women occur frequently in postcolonial writings. This paper attempts to carry out ... more Image of white women occur frequently in postcolonial writings. This paper attempts to carry out a comprehensive analysis of the white women's portrayals in the selected Pakistani postcolonial fiction to determine the comparative discrepancy between the assumptions and reality about the role of white women in the colonies. The white women being the part of civilizing mission of the white man, are seen with a particular light by the indigenous people because in comparison to the white man, white women's role has been that of a benevolent mother. This problematizes the situation and hence calls for the investigation into the portrayals and the roles of the white women as projected by the indigenous writers. The study delimits to Forster, Sidhwa, and Hamid and analyses the selected chunks of the text under the lens of theoretical frame work proposed by Jayawardena within the postcolonial context.

Introduction: Subcutaneous targeted stimulation (TS) involves placing a neurostimulating implant ... more Introduction: Subcutaneous targeted stimulation (TS) involves placing a neurostimulating implant subcutaneously at the epicentre of the painful area (target) rather than stimulating the peripheral nerves supplying it. It was first reported in 2003 and the first permanent implant case series followed in 2006 1 . It is effective in treating chronic painful conditions of non-dermatomal distribution resulting in reduction of pain for varying duration. We are presenting an evaluation of our experience with permanent TS in the treatment of various chronic pain conditions.

During this study, one new coumarin; 7-O-b-D-glucopyranoside-2H-1-benzopyran-2-one (1) and three ... more During this study, one new coumarin; 7-O-b-D-glucopyranoside-2H-1-benzopyran-2-one (1) and three quinoline alkaloids; 3-hydroxy, 2, 2, 6-trimethyl -3, 4, 5, 6-tetrahydro-2H-pyrano[3,2-c] quinoline 5-one (2), ribalinine (3) and methyl isoplatydesmine (4) were isolated from the aerial parts of Skimmia laureola and their structures established by spectroscopic studies. Compounds 2-4 were found to be linear mixed type inhibitors of acetylcholinesterase (K i ¼ 110.0, 30.0 and 30.0 mM, respectively). Compounds 2 and 3 were also found to be linear mixed type inhibitors of butyrylcholinesterase, while compound 4 was a noncompetitive inhibitor of the enzyme (K i ¼ 90.0, 70.0 and 19.0 mM, respectively). The inhibition of acetyl-and butyryl-cholinesterase enzymes persists as the most promising therapeutic strategy for activating the impaired cholinergic functions in Alzheimer's disease and related dementias.

ABSTRACT Three new pregnane-type steroidal alkaloids salignenamide-A 1 [(20S, 2′Z) - 20 - (N,N - ... more ABSTRACT Three new pregnane-type steroidal alkaloids salignenamide-A 1 [(20S, 2′Z) - 20 - (N,N - dimethylamino) - 3β - (3′, 4′ - dimethyl-2-pentenamido)-pregnane], salignenamide-B 2 [(20S,2′Z)-20- (N,N-dimethylamino)-3β-(3′-methyl-2′-butenamido)-pregna-4, 14-diene] and 2,4-diacetoxyepipachysamine-D 3 [(20S)-20-(N,N-dimethylamino)-3β-benzamido-2β, 4β-diacetoxypregnane] have been isolated from the ethanolic extracts of the whole plant of Sarcococca saligna. The structures of these steroidal alkaloids were established on the basis of detailed spectroscopic studies.

Approximately 18% of the patients are dissatisfied with the result of total knee replacement. How... more Approximately 18% of the patients are dissatisfied with the result of total knee replacement. However, the relation between dissatisfaction and prosthetic alignment has not been investigated before. We retrospectively analysed prospectively gathered data of all patients who had a primary TKR, preoperative and one-year postoperative Oxford Knee Scores (OKS) and postoperative computed tomography (CT). The CT protocol measures hip-knee-ankle (HKA) angle, and coronal, sagittal and axial component alignment. Satisfaction was defined using a five-item Likert scale. We dichotomised dissatisfaction by combining '(very) dissatisfied' and 'neutral/not sure'. Associations with dissatisfaction and change in OKS were calculated using multivariable logistic and linear regression models. 230 TKRs were implanted in 105 men and 106 women. At one year, 12% were (very) dissatisfied and 10% neutral. Coronal alignment of the femoral component was 0.5 degrees more accurate in patients who were satisfied at one year. The other alignment measurements were not different between satisfied and dissatisfied patients. All radiographic measurements had a P-value>0.10 on univariate analyses. At one year, dissatisfaction was associated with the three-months OKS. Change in OKS was associated with three-months OKS, preoperative physical SF-12, preoperative pain and cruciate retaining design. Neither mechanical axis, nor component alignment, is associated with dissatisfaction at one year following TKR. Patients get the best outcome when pain reduction and function improvement are optimal during the first three months and when the indication to embark on surgery is based on physical limitations rather than on a high pain score. 2.

A 20-year male, right side rear seat passenger of a medium sized car wearing a seat belt was invo... more A 20-year male, right side rear seat passenger of a medium sized car wearing a seat belt was involved in a road traffic accident at approximately 30 miles/ h. The car was damaged from the front and right front door. He was trapped in the car for about 45 min. On arrival in the hospital he was haemodynamically stable and noted to have seat belt marks over the chest and abdomen along with extensive swelling and bruising on right side of his abdomen, the skin being intact. Secondary survey did not reveal any other injuries.

Purpose: Staphylococcus aureus is a flexible pathogen causing variety of infections in humans and... more Purpose: Staphylococcus aureus is a flexible pathogen causing variety of infections in humans and other animals. The current investigation was aimed to study the prevalence rate and sensitivity profile of MRSA strains isolated from clinical samples to various antimicrobial agents. Methods: Isolates from three different clinical specimens; pus, body fluids and blood were obtained from patients, belonging to various parts of District Peshawar of Khyber Pakhtunkhwa. Each sample was processed by conventional method using blood, MacConkey and CLED agar. Staphylococcus aureus was identified and confirmed by Gram staining, catalase test and coagulase test. Antibiotic susceptibility pattern of MRSA to different antibiotics was confirmed by standard Kirby-Bauer disc diffusion method. Results: A total of 855 clinical samples were studied, out of which 45 (5.26%) were found positive for MRSA, among which 32 (71%) were from pus samples, 8 (18%) were from fluids samples and 5 (11%) were from blood samples. The frequency of MRSA strains was more in males (69%) than in females (31%). All MRSA isolates showed complete resistance to oxacillin, ampicillin and penicillin and complete sensitivity to linezolid, teicoplanin, and vancomycin. These MRSA strains also showed various degrees of resistance to other antimicrobials such as ceftriaxone (77.77%), cefoxitin (64.44%), erythromycin (82.22%), clindamycin (71.11%), co-amoxiclav (75.55%), fusidic acid (66.66%), and gentamycin (73.3%). Conclusion: Our results show lower MRSA prevalence in Peshawar than the previous reports in Peshawar and other areas of Pakistan. However; since MRSA is multidrug resistant, proper culture sensitivity test should be carried out for proper choice of antibiotic/s to treat MRSA infections.

Obesity is considered an independent risk factor for adverse outcome after arthroplasty surgery. ... more Obesity is considered an independent risk factor for adverse outcome after arthroplasty surgery. Data on 191 consecutive total hip arthroplasties were prospectively collected. Body mass index (BMI) was calculated for each patient and grouped into nonobese (BMI b30 kg/m 2 ), obese (BMI 30-34.9 kg/m 2 ), and morbidly obese (BMI ≥35 kg/m 2 ). Primary outcomes included functional improvement (Oxford hip score, 6-minute walk test and Short Form-12 Health Survey general health questionnaire) and postoperative complications. Subgroup analysis of surgeons' overall perception of operative technical difficulty was also performed. This study shows that total hip arthroplasties in obese patients were perceived, by the surgeon, to be significantly more difficult. However, this did not translate to an increased risk of complications, operation time, or blood loss, nor suboptimal implant placement. In addition, our results suggest that obese patients gain similar benefit from hip arthroplasty as do nonobese patients, but morbidly obese patients have significantly worse 6-minute walk test scores at 6 weeks.

Obesity is considered an independent risk factor for adverse outcome after arthroplasty surgery. ... more Obesity is considered an independent risk factor for adverse outcome after arthroplasty surgery. Data on 191 consecutive total hip arthroplasties were prospectively collected. Body mass index (BMI) was calculated for each patient and grouped into nonobese (BMI b30 kg/m 2 ), obese (BMI 30-34.9 kg/m 2 ), and morbidly obese (BMI ≥35 kg/m 2 ). Primary outcomes included functional improvement (Oxford hip score, 6-minute walk test and Short Form-12 Health Survey general health questionnaire) and postoperative complications. Subgroup analysis of surgeons' overall perception of operative technical difficulty was also performed. This study shows that total hip arthroplasties in obese patients were perceived, by the surgeon, to be significantly more difficult. However, this did not translate to an increased risk of complications, operation time, or blood loss, nor suboptimal implant placement. In addition, our results suggest that obese patients gain similar benefit from hip arthroplasty as do nonobese patients, but morbidly obese patients have significantly worse 6-minute walk test scores at 6 weeks.
