In the second surgical time, under general anesthesia, surgery was performed for enucleation of the lesion and exodontia of the dental element 18 by means of Caldwell-luc access. After ostectomy of the anterior wall of the maxillary sinus, where purulent secretion was detected in the right maxillary sinus and the presence of the cystic capsule. The lesion was removed by curettage and the extracted dental element 18. The surgical specimen was sent to the laboratory for histopathological analysis, confirming with the diagnosis of a dentigerous cyst. Conclusion: The dentigerous cyst originates through the separation of the follicle that surrounds an included tooth and is the most common of the odontogenic cyst. It is found in patients with a wide age range and the characteristics of the dentigerous cyst are: Radiographically: well delimited unilocular radiolucent lesion associated with the crown of an erupted tooth. Clinically: usually asymptomatic. The diagnosis is confirmed by anatomopathological examination. Purpose The aim of this study was to compare the biomechanical characteristics of 1 plate with 2 ... more Purpose The aim of this study was to compare the biomechanical characteristics of 1 plate with 2 positional screws and a 3-dimensional (3-D) ladder plate for fixation of the sagittal split ramus osteotomy (SSRO) in large mandibular advancement. Methods This was a cross-sectional cadaveric study. The sample consisted of 30 hemimandibles of human cadavers provided by the Anatomic Donor Program at the University of Alabama at Birmingham, School of Medicine (Birmingham, AL). The biomechanical evaluation consisted of the load resistance value, which was measured during the peak load, and allowed for 1-, 3-, and 5-mm displacements at the osteotomy site. The load resistance was recorded in newtons (N). Results The means of the strengths of the 1-, 3-, and 5-mm displacements were determined by using 1-way analysis of variance (ANOVA), followed by the post hoc Tukey's test. Statistically significant differences were observed only for the 5-mm displacement. There was a difference in the 3-mm group, although it was not significant. Conclusions The biomechanical evaluation and load resistance value at the osteotomy site of 1 plate with 2 positional screws is higher than those of the 3-D ladder plate for fixation of the sagittal split ramus osteotomy (SSRO) in large mandibular advancement.

Aims and Objectives: Many situations in the dental office can provoke medical emergencies. Lack o... more Aims and Objectives: Many situations in the dental office can provoke medical emergencies. Lack of training and inability to overcome the medical emergencies can lead to serious consequences and legal actions. The aim of the study is to investigate and assess the knowledge, attitude, and perceived confidence of dental students and interns in the management of medical emergency. Materials and Methods: A self-administered structured questionnaire was distributed to 153 of the undergraduate dental students and interns in Qassim province. Questionnaire consisted of nineteen questions pertaining to knowledge and awareness regarding syncope, cardiopulmonary resuscitation (CPR), intravenous drugs, measuring vital signs, and handling situation of aspiration of a foreign body, bleeding, and choking. Data were analyzed by Statistical Package for Social Sciences (SPSS) version 21.0. Results: Fifty-seven percent was the response rate received from the questionnaire. Eighty-nine percent and 30% of the participants inquired about the medical history and vital signs before dental treatment, respectively. Only 37% of participants were confident to handle any medical emergency in the dental office. Seventy percent knew the correct location of chest compression and 67% were familiar about the right compression ventilation ratio showing significant difference between academic years and interns (P = 0.003). Females were significantly more aware about the management of bleeding after extraction than the males (65%, and 47%, respectively; P = 0.035). Thirty-five percent and 53% chose the correct management to relieve choking in responsive and unresponsive adult or child, respectively. A total of 28% of the participants reported syncope as the most common emergency situation. Conclusion: Participants were lacking confidence in handling medical emergencies even though the majority of them inquired the medical history. Most of them have a good knowledge regarding CPR, but regarding airway obstruction, the knowledge was not at an acceptable level. Annual basic life support and emergency courses should be mandatory in dental teaching curriculum.

The purpose of this retrospective study was to evaluate the correlation of maxillomandibular adva... more The purpose of this retrospective study was to evaluate the correlation of maxillomandibular advancement (MMA) and airway volume changes in patients with obstructive sleep apnea (OSA), and to determine the surgical skeletal movements necessary to achieve an increase in total airway volume (TAV) of !70%. Thirty patients with OSA treated by MMA were evaluated. Pre-and postoperative cone beam computed tomography images were used to determine the horizontal distance and angular changes in surgical parameters and linear, area, and volumetric airway parameters. Postoperatively, the horizontal distance of surgical parameters (A-point, UI, B-point, pogonion, and menton) and craniofacial angulation (SNA and SNB) increased significantly, similar to total surface area, TAV, and minimum cross-sectional area of the airway (p < 0.0001). The total airway length decreased significantly (p < 0.0001). The mean increase in TAV was 67.2%. There were positive correlations between linear surgical changes and the percentage change in TAV. All surgical parameters were predictive of a change in TAV !70%. The optimal surgical change was 6 mm for A-point, 7.9 mm for UI, 7.6 mm for B-point, 11.2 mm for pogonion, and 10 mm for menton. In conclusion, maxillary advancement of less than 10 mm was adequate in this study to obtain an increase in the TAV of at least 70%.

Objectives: The aim of this study was to find a possible link between the Operative time of orthognathic surgery and the postoperative length of hospital stay (LHS). Methods: This is a retrospective cohort study of patients' records who underwent orthognathic surgery between January 2009 and December 2013 at Al-Nahda Hospital-Oman. The following variables were recorded for each patient: age, gender, indication for surgery, type of procedure, operative time, LHS, and need for blood transfusion. Data analysis were carried out using IBM SPSS (Version 22). Findings: 215 patients were therefore eligible for inclusion. We found that there was a statistically significant correlation between the operative time of orthognathic surgery and length of hospital stay (p > 0.001). Other factors that have a statistically significant influence in LHS include female (p = 0.021), the indication for the surgery (p = 0.033), type of procedure (LeFort I, bimax and trimax) (p = 0.002) and if the patient needed blood transfusion postoperatively (p = 0.005). The age of the patient did not influence the LHS (p = 0.358). Conclusion: LHS after orthognathic surgery is influenced by the operative time, gender of the patient, indication for the surgery, procedure type, and if blood transfusion is needed.

We intend to measure the baseline vitamin D level in patients with oral squamous cell carcinoma (OSCC) and to compare same with non-cancer controls to determine any association. Materials and methods: Patients with OSCC presenting to our clinics were included in this study. Their baseline serum vitamin D levels were measured prior to cancer treatment after obtaining their consents. These patients were then matched with at least 2 cancer-free subjects to serve as controls and whose serum vitamin D levels were also measured. The serum vitamin D levels obtained for the two groups were then categorized into normal (>35 ng/ml), mild deficiency (25-35 ng/ml), moderate deficiency (12.5-25 ng/ml), and severe deficiency (<12.5 ng/ml). The data were analyzed statistically and the two groups compared. Results: A total of 51 patients with OSCC (Male 22 [43%] and female 29 [57%]) and 113 cancer-free controls (Male 36 [31.86%] and female 77 [68.14%]) were included in the study. The commonest site for OSCC was the tongue, accounting for 45% of the cancer cases. Mean age for cancer patients was 59.33 years ±12.54 and 49.24 years ±15.79 for the control. Among the OSCC patients, 74.51% had moderate to severe vitamin D deficiencies, whereas only 20.35% had a moderate deficiency in the control group with no severe deficiency. Conclusion: Logistic regression analysis shows a positive association between vitamin D deficiency and OSCC risk especially in levels below 25 ng/ml. This further corroborates the assertion that vitamin D deficiency may be a useful indicator of OSCC. It may, therefore, be necessary to routinely prescribe vitamin D supplements to subjects with moderate to severe deficiencies in order to decrease the chances of OSCC development.

In the present study we have synthesized a novel organic-inorganic hybrid thin sheet membrane using chitosan/polyvinyl alcohol and montmorillonite clay followed by non-solvent induced phase inversion technique. This hybrid clay-polymeric nanofiltration membrane possesses excellent overall performance, such as enhanced hydrophilic nature, and holds good rejection rate. Analytical techniques such as Fourier transform infrared spectroscopy (FT-IR), X-ray diffractometer (XRD) and Scanning electron microscopy (SEM) have been employed to characterize the membrane material. Membrane characterizations such as pure water flux, membrane hydraulic resistance, water content, percentage of porosity and pore size were also evaluated. This case reports a 23-year-old male reported to the Department of Oral and Maxillofacial surgery at National Guard Health Affairs, King Abdulaziz Medical City, Riyadh, Saudi Arabia by his mother with chief complaint of asymptomatic, progressive and bilateral enlargement of the mandible. Sixty hemodialysis patients (30 males, 30 females) with a mean age of 44.4±9.5 years comprised the study group. Periodontal parameters such as plaque index (PI), bleeding on probing (BOP), probing pocket depth (PPD) and clinical attachment level (CAL) were measured in these patients and 60 age and sex-matched control subjects. The data was tabulated and analyzed. The mean duration of dialysis was 5.50±3.02 years. The plaque index (PI) bleeding on probing (BOP), Periodontal probing depth (PPD) and clinical attachment level (CAL) were significantly higher in patients undergoing hemodialysis than in control subjects. The mean clinical attachment level was significantly higher among the dialysis patients (2.78±0.83 mm) than the control subjects (1.97±0.53 mm). Diffuse large B-cell lymphoma (DLBCL) is an aggressive B-cell lymphoma histologically characterized by diffuse proliferation of large neoplastic B-lymphoid cells with a nuclear size equal to or exceeding normal histiocytic nuclei. A case of DLBCL of the mandible in an 18 years old male patient is presented. This report discusses this rare malignancy, including clinical presentation, histopathologic features, immunologic profile, treatment and prognosis. Though lymphoma of mandible is rare, it must be considered in differential diagnosis of swellings arising in the region. Questionnaire consisted of nineteen questions pertaining to knowledge and awareness regarding syncope, cardiopulmonary resuscitation (CPR), intravenous drugs, measuring vital signs, and handling situation of aspiration of a foreign body, bleeding, and choking. Data were analyzed by Statistical Package for Social Sciences (SPSS) version 21.0. Results: Fifty-seven percent was the response rate received from the questionnaire. Eighty-nine percent and 30% of the participants inquired about the medical history and vital signs before dental treatment, respectively. Only 37% of participants were confident to handle any medical emergency in the dental office. Seventy percent knew the correct location of chest compression and 67% were familiar about the right compression ventilation ratio showing significant difference between academic years and interns (P = 0.003). Females were significantly more aware about the management of bleeding after extraction than the males (65%, and 47%, respectively; P = 0.035). Thirty-five percent and 53% chose the correct management to relieve choking in responsive and unresponsive adult or child, respectively. A total of 28% of the participants reported syncope as the most common emergency situation. Conclusion: Participants were lacking confidence in handling medical emergencies even though the majority of them inquired the medical history. Most of them have a good knowledge regarding CPR, but regarding airway obstruction, the knowledge was not at an acceptable level. Annual basic life support and emergency courses should be mandatory in dental teaching curriculum.

Objectives: The aim of this study was to find a possible link between the Operative time of orthognathic surgery and the postoperative length of hospital stay (LHS). Methods: This is a retrospective cohort study of patients' records who underwent orthognathic surgery between January 2009 and December 2013 at Al-Nahda Hospital-Oman. The following variables were recorded for each patient: age, gender, indication for surgery, type of procedure, operative time, LHS, and need for blood transfusion. Data analysis were carried out using IBM SPSS (Version 22). Findings: 215 patients were therefore eligible for inclusion. We found that there was a statistically significant correlation between the operative time of orthognathic surgery and length of hospital stay (p > 0.001). Other factors that have a statistically significant influence in LHS include female (p = 0.021), the indication for the surgery (p = 0.033), type of procedure (LeFort I, bimax and trimax) (p = 0.002) and if the patient needed blood transfusion postoperatively (p = 0.005). The age of the patient did not influence the LHS (p = 0.358). Conclusion: LHS after orthognathic surgery is influenced by the operative time, gender of the patient, indication for the surgery, procedure type, and if blood transfusion is needed.

We intend to measure the baseline vitamin D level in patients with oral squamous cell carcinoma (OSCC) and to compare same with non-cancer controls to determine any association. Materials and methods: Patients with OSCC presenting to our clinics were included in this study. Their baseline serum vitamin D levels were measured prior to cancer treatment after obtaining their consents. These patients were then matched with at least 2 cancer-free subjects to serve as controls and whose serum vitamin D levels were also measured. The serum vitamin D levels obtained for the two groups were then categorized into normal (>35 ng/ml), mild deficiency (25-35 ng/ml), moderate deficiency (12.5-25 ng/ml), and severe deficiency (<12.5 ng/ml). The data were analyzed statistically and the two groups compared. Results: A total of 51 patients with OSCC (Male 22 [43%] and female 29 [57%]) and 113 cancer-free controls (Male 36 [31.86%] and female 77 [68.14%]) were included in the study. The commonest site for OSCC was the tongue, accounting for 45% of the cancer cases. Mean age for cancer patients was 59.33 years ±12.54 and 49.24 years ±15.79 for the control. Among the OSCC patients, 74.51% had moderate to severe vitamin D deficiencies, whereas only 20.35% had a moderate deficiency in the control group with no severe deficiency. Conclusion: Logistic regression analysis shows a positive association between vitamin D deficiency and OSCC risk especially in levels below 25 ng/ml. This further corroborates the assertion that vitamin D deficiency may be a useful indicator of OSCC. It may, therefore, be necessary to routinely prescribe vitamin D supplements to subjects with moderate to severe deficiencies in order to decrease the chances of OSCC development.

In the present study we have synthesized a novel organic-inorganic hybrid thin sheet membrane using chitosan/polyvinyl alcohol and montmorillonite clay followed by non-solvent induced phase inversion technique. This hybrid clay-polymeric nanofiltration membrane possesses excellent overall performance, such as enhanced hydrophilic nature, and holds good rejection rate. Analytical techniques such as Fourier transform infrared spectroscopy (FT-IR), X-ray diffractometer (XRD) and Scanning electron microscopy (SEM) have been employed to characterize the membrane material. Membrane characterizations such as pure water flux, membrane hydraulic resistance, water content, percentage of porosity and pore size were also evaluated. Sixty hemodialysis patients (30 males, 30 females) with a mean age of 44.4±9.5 years comprised the study group. Periodontal parameters such as plaque index (PI), bleeding on probing (BOP), probing pocket depth (PPD) and clinical attachment level (CAL) were measured in these patients and 60 age and sex-matched control subjects. The data was tabulated and analyzed. The mean duration of dialysis was 5.50±3.02 years. The plaque index (PI) bleeding on probing (BOP), Periodontal probing depth (PPD) and clinical attachment level (CAL) were significantly higher in patients undergoing hemodialysis than in control subjects. Diffuse large B-cell lymphoma (DLBCL) is an aggressive B-cell lymphoma histologically characterized by diffuse proliferation of large neoplastic B-lymphoid cells with a nuclear size equal to or exceeding normal histiocytic nuclei. A case of DLBCL of the mandible in an 18 years old male patient is presented. This report discusses this rare malignancy, including clinical presentation, histopathologic features, immunologic profile, treatment and prognosis. Though lymphoma of mandible is rare, it must be considered in differential diagnosis of swellings arising in the region.
