Materials and Methods: Among patients who underwent RP in our clinic within 10 years, 158 patients with prostate biopsy pathology of ISUP grade group 1 were examined retrospectively. Age, serum prostate-specific antigen (PSA) level, prostate biopsy ISUP grade group, number of cores taken in the prostate biopsy, number of tumor-positive cores, RP pathology ISUP grade group, and pathological stage were evaluated. Results: The mean age (± standard) of the 158 patients whose prostate biopsy pathology was ISUP grade group 1 were 64.07 (±6.6). ISUP group upgrading was detected in 47 patients (29.7%). Objective: This study aimed to investigate pathological lymph node involvement in selected patien... more Objective: This study aimed to investigate pathological lymph node involvement in selected patients and the relationship of prostate-specific antigen (PSA) progression-free survival rates between patients with and without lymph node involvement on preoperative conventional radiologic imaging. Limited data is available about local treatment outcomes in patients with prostate cancer (PCa) having clinical lymph node involvement. Materials and Methods: Using the national PCa database, patients who underwent radical prostatectomy (RP) and pelvic lymph node dissection between 2001 and 2019, with pathologic lymph node involvement, were included in the study. Patients were divided into two groups as those with and without clinical lymph node involvement by preoperative imaging. Results: A total of 213 patients were included in the final analysis, wherein 164 are with and 49 are without lymph node involvement. After the mean follow-up periods of 33.9 months, a significant difference was not found between the two groups in terms of recurrence, adjuvant treatment necessity, and final status. The multivariate analysis for 5-year PSA recurrence-free survival revealed that surgical margin positivity was the only significant factor (p=0.016, hazard ratio: 2.67, confidence interval: 1.19-5.98). Conclusion: Our results revealed that preoperative clinical lymph node status did not affect the 5-year PSA recurrence-free survival in pathologic lymph node involvement of patients treated with RP and pelvic lymph node dissection. Therefore, RP stands as an effective treatment option in selected patients with PCa having clinical lymph node involvement.

Bu calismamizda sekonder hipertansiyon ve yan agrisi ile uroloji poliklinigine basvuran 17 yasinda bir erkek hastayi ve ardindan literaturde reninoma ile ilgili bilgileri sunmayi amacladik. Hastanin yapilan degerlendirmesinde hipokalemik oldugu goruldu ve cekilen batin manyetik rezonans (MR) goruntulemesinde sag bobrekte 4.8x4.8 cm’lik kontrast tutan ekzofitik lezyon saptandi. Hastaya acik nefron koruyucu cerrahi operasyon uygulandi ve patolojik inceleme ile eksize edilen lezyonun jukstaglomeruler hucreli tumor (reninoma) oldugu saptandi. Operasyondan kisa bir sure sonra hastanin hipertansiyon tedavisi amaciyla aldigi tum ilaclar kesildi ve kan basincinin normal seyrettigi ve hipokaleminin ortadan kalktigi goruldu. Bu vaka, sekonder hipertansiyonun sebeplerinden biri olan reninoma tedavisinin onemini ve tanisal yaklasimin tedavinin temeli oldugunu gosteren, klinisyenlere yon verebilecek onemli bir vakadir.

There are lots of studies on the histology of the obstructed segment of UPO patients. Although there are some conflicting results, most of the studies performed to date state that while muscle and neural tissue decrease in UPO, connective tissue and extracellular matrix proteins increase. All these studies have been conducted whether on children or adults. This is the first study that compared the specimens from adult and child population. Our study will help us better understand the progression of ureteropelvic junction obstruction throughout life and create a better understanding on the timing and need of surgery. The extracellular matrix (ECM) is a noncellular component with a crucial role on tissue morphogen... more The extracellular matrix (ECM) is a noncellular component with a crucial role on tissue morphogenesis, differentiation and hemostasis within all tissues and organs. With advancement in the technology and increased data on ECM components, it was realized that many conditions in urinary tract have a close relation with the composition of ECM in the affected tissue. According to some basic research studies, ECM composition may give us important information about the prognosis and progression of disease in addition to the cause and pathophysiology of the diseases such as congenital ureterovesical and ureteropelvic junction obstruction. Afterwards, with better understanding of ECM one can develop new treatment and follow-up models. This chapter will summarize the evidence-based role of ECM in urinary tract conditions.

To evaluate the application of an automated urine analyzer (AUA) for the diagnosis of bladder can... more To evaluate the application of an automated urine analyzer (AUA) for the diagnosis of bladder cancer (BC) Materials and Methods: A retrospective data analysis of 2365 urine specimens from the department of urology has been performed and matched with those patients, who have undergone cystoscopic evaluation or surgical treatment for different urological pathologies during 2018. After matching, clinical records of the patients has been further evaluated in order to select patients with recent or previous BC diagnosis. To assess the diagnostic efficacy of AUA, patients were divided into five groups according to the patient history of BC and healthy controls. Results: A total of 106 patients are included in this study and the majority (69.8%) of the patients are follow-up patients with previous diagnosis and treatment of non-muscle invasive BC (NMIBC). For patients with low-risk NMIBC (n=27), the sensitivity and specificity were calculated as 75% and 100%. For patients with high-risk NMIBC (n=47), who were previously treated with intravesical BCG, the sensitivity and specificity were calculated as 54.5% and 83.3%. All patients in radical cystectomy group (n=7) with muscle invasive BC had positive urine analyses results for atypical cells. And none of the patients in the control group (n=8) had positive AUA results and cystoscopic evaluation also did not show any bladder mass suspicious for BC. Conclusion: The results of this retrospective pilot study showed acceptable sensitivity and specificity rates of the "fluorescence flow cytometry" based AUA and the results of the low-risk group are especially valuable regarding its potential use to decide on performing a follow-up cystoscopy or not. A prospective study is currently on progress to validate the findings of the current study.

Objective: We aimed to share our experience about the super-selective embolization of the vesical artery performed with micro-catheter technique which was used as a palliative approach to control intractable hematuria in patients with bladder cancer. Materials and Methods: Super-selective embolization of the vesical artery with micro-catheter technique was performed in 12 bladder cancer patients whose hematuria could not be controlled with other palliative methods in our clinic. Hemoglobin levels, blood transfusion amounts, complications and urethral catheter removal duration before and after embolization were evaluated. Results: The average age of the patients was 73.3 (65-85, range) years. For the embolization process, n-butyl-2cyaboacrylate (glue) was used as the primary method in 3 patients and polyvinylalcohol (PVA) particle was used in the remaining 9 patients. In two of the patients whose hematuria could not be controlled after PVA embolization glue embolization was performed as the secondary procedure within one week. Super-selective embolization of the vesical artery with micro-catheter technique was performed in all of our cases. In 4 cases, embolization of the vesical artery was performed only to the side of the tumor, which was determined with cystoscopy. Bilateral embolization was performed to the remaining 8 cases. The average hemoglobin value before and after the embolization procedure was 7.9 g/dL and 9.2 g/dL, respectively. The average blood transfusion amounts before and after the procedure were 4 (2-15) and 2.3 (1-4) units erythrocyte/ whole blood, respectively. The average urethral catheter duration after the procedure was 7 (2-16) days in 10 patients, who were treated one single embolization session. Urethral catheters were removed once the hematuria dissolved completely. There were no major complications or mortality related to the treatment after the embolization procedure. Conclusion: Super-selective embolization of the vesical artery performed with micro-catheter technique is a safe and effective alternative in patients with intractable hematuria due to bladder cancer whose hematuria could not be controlled with other palliative methods.

Symptom scores were recorded pre- and 6 months SS for 294 children with OAB unrelated to neurological disorder. Uroflowmetry-electromyography data, total bladder capacity, and a 2-day bladder diary were also recorded, and upper urinary tract deterioration was investigated as indicated. Overactive bladder was treated with standard approaches. No response to treatment was defined as 0-49% reduction in OAB-related symptoms based on SS results. Non-responders underwent additional evaluations as indicated. Two hundred forty-one patients (97%; mean age 9.8 ± 2.8 years; mean follow-up 11 months; range 6-18 months) completed the study. One hundred thirteen (47%) required ultrasonography (USG), and those with abnormal USG had a significantly higher pre- and 6 months SS (p = 0.016). All non-responders (n = 38; 16%) underwent urodynamics evaluation, 34 underwent spinal magnetic resonance imaging (MRI), 34 underwent voiding cystourethrography (VCUG), and 34 underwent dimercaptosuccinic acid scanning (DMSA). Non-responders with terminal detrusor hyperactivity had significantly lower SS after therapy (p = 0.09). Non-responders with abnormal MRI had higher pre- and 6 months SS than those with normal MRI. Thirteen (38%) of the non-responders who required VCUG had vesicoureteral reflux (VUR), and this subgroup had higher pre-treatment SS (p = 0.030). Seven (21%) of the non-responders who required DMSA had scarring, and all 7 had VUR. The subgroup with scarring had higher pre-treatment SS (p = 0.030). Pediatric OAB patients with high 6 months SS have a higher incidence of additional upper urinary tract pathology. Those with low pre-treatment SS require fewer laboratory tests and other assessments. The SS tool can reduce the number of urodynamics evaluations, and other tests required to diagnose renal damage in children with OAB.

Hastanin yapilan degerlendirmesinde hipokalemik oldugu goruldu ve cekilen batin manyetik rezonans (MR) goruntulemesinde sag bobrekte 4.8x4.8 cm’lik kontrast tutan ekzofitik lezyon saptandi. Hastaya acik nefron koruyucu cerrahi operasyon uygulandi ve patolojik inceleme ile eksize edilen lezyonun jukstaglomeruler hucreli tumor (reninoma) oldugu saptandi. Operasyondan kisa bir sure sonra hastanin hipertansiyon tedavisi amaciyla aldigi tum ilaclar kesildi ve kan basincinin normal seyrettigi ve hipokaleminin ortadan kalktigi goruldu. Bu vaka, sekonder hipertansiyonun sebeplerinden biri olan reninoma tedavisinin onemini ve tanisal yaklasimin tedavinin temeli oldugunu gosteren, klinisyenlere yon verebilecek onemli bir vakadir.

There are lots of studies on the histology of the obstructed segment of UPO patients. Although there are some conflicting results, most of the studies performed to date state that while muscle and neural tissue decrease in UPO, connective tissue and extracellular matrix proteins increase. All these studies have been conducted whether on children or adults. This is the first study that compared the specimens from adult and child population. Our study will help us better understand the progression of ureteropelvic junction obstruction throughout life and create a better understanding on the timing and need of surgery. With advancement in the technology and increased data on ECM components, it was realized that many conditions in urinary tract have a close relation with the composition of ECM in the affected tissue. According to some basic research studies, ECM composition may give us important information about the prognosis and progression of disease in addition to the cause and pathophysiology of the diseases such as congenital ureterovesical and ureteropelvic junction obstruction. Afterwards, with better understanding of ECM one can develop new treatment and follow-up models. This chapter will summarize the evidence-based role of ECM in urinary tract conditions.

To evaluate the application of an automated urine analyzer (AUA) for the diagnosis of bladder can... more To evaluate the application of an automated urine analyzer (AUA) for the diagnosis of bladder cancer (BC) Materials and Methods: A retrospective data analysis of 2365 urine specimens from the department of urology has been performed and matched with those patients, who have undergone cystoscopic evaluation or surgical treatment for different urological pathologies during 2018. After matching, clinical records of the patients has been further evaluated in order to select patients with recent or previous BC diagnosis. To assess the diagnostic efficacy of AUA, patients were divided into five groups according to the patient history of BC and healthy controls. Results: A total of 106 patients are included in this study and the majority (69.8%) of the patients are follow-up patients with previous diagnosis and treatment of non-muscle invasive BC (NMIBC). For patients with low-risk NMIBC (n=27), the sensitivity and specificity were calculated as 75% and 100%. For patients with high-risk NMIBC (n=47), who were previously treated with intravesical BCG, the sensitivity and specificity were calculated as 54.5% and 83.3%. All patients in radical cystectomy group (n=7) with muscle invasive BC had positive urine analyses results for atypical cells. And none of the patients in the control group (n=8) had positive AUA results and cystoscopic evaluation also did not show any bladder mass suspicious for BC. Conclusion: The results of this retrospective pilot study showed acceptable sensitivity and specificity rates of the "fluorescence flow cytometry" based AUA and the results of the low-risk group are especially valuable regarding its potential use to decide on performing a follow-up cystoscopy or not. A prospective study is currently on progress to validate the findings of the current study.

Objective: We aimed to share our experience about the super-selective embolization of the vesical artery performed with micro-catheter technique which was used as a palliative approach to control intractable hematuria in patients with bladder cancer. Materials and Methods: Super-selective embolization of the vesical artery with micro-catheter technique was performed in 12 bladder cancer patients whose hematuria could not be controlled with other palliative methods in our clinic. Hemoglobin levels, blood transfusion amounts, complications and urethral catheter removal duration before and after embolization were evaluated. Results: The average age of the patients was 73.3 (65-85, range) years. For the embolization process, n-butyl-2cyaboacrylate (glue) was used as the primary method in 3 patients and polyvinylalcohol (PVA) particle was used in the remaining 9 patients. In two of the patients whose hematuria could not be controlled after PVA embolization glue embolization was performed as the secondary procedure within one week. Super-selective embolization of the vesical artery with micro-catheter technique was performed in all of our cases. In 4 cases, embolization of the vesical artery was performed only to the side of the tumor, which was determined with cystoscopy. Bilateral embolization was performed to the remaining 8 cases. The average hemoglobin value before and after the embolization procedure was 7.9 g/dL and 9.2 g/dL, respectively. The average blood transfusion amounts before and after the procedure were 4 (2-15) and 2.3 (1-4) units erythrocyte/ whole blood, respectively. The average urethral catheter duration after the procedure was 7 (2-16) days in 10 patients, who were treated one single embolization session. Urethral catheters were removed once the hematuria dissolved completely. There were no major complications or mortality related to the treatment after the embolization procedure. Conclusion: Super-selective embolization of the vesical artery performed with micro-catheter technique is a safe and effective alternative in patients with intractable hematuria due to bladder cancer whose hematuria could not be controlled with other palliative methods.

Symptom scores were recorded pre- and 6 months SS for 294 children with OAB unrelated to neurological disorder. Uroflowmetry-electromyography data, total bladder capacity, and a 2-day bladder diary were also recorded, and upper urinary tract deterioration was investigated as indicated. Overactive bladder was treated with standard approaches. No response to treatment was defined as 0-49% reduction in OAB-related symptoms based on SS results. Non-responders underwent additional evaluations as indicated. Two hundred forty-one patients (97%; mean age 9.8 ± 2.8 years; mean follow-up 11 months; range 6-18 months) completed the study. One hundred thirteen (47%) required ultrasonography (USG), and those with abnormal USG had a significantly higher pre- and 6 months SS (p = 0.016). All non-responders (n = 38; 16%) underwent urodynamics evaluation, 34 underwent spinal magnetic resonance imaging (MRI), 34 underwent voiding cystourethrography (VCUG), and 34 underwent dimercaptosuccinic acid scanning (DMSA). Non-responders with terminal detrusor hyperactivity had significantly lower SS after therapy (p = 0.09). Non-responders with abnormal MRI had higher pre- and 6 months SS than those with normal MRI. Thirteen (38%) of the non-responders who required VCUG had vesicoureteral reflux (VUR), and this subgroup had higher pre-treatment SS (p = 0.030). Seven (21%) of the non-responders who required DMSA had scarring, and all 7 had VUR. The subgroup with scarring had higher pre-treatment SS (p = 0.030). Pediatric OAB patients with high 6 months SS have a higher incidence of additional upper urinary tract pathology. Those with low pre-treatment SS require fewer laboratory tests and other assessments. The SS tool can reduce the number of urodynamics evaluations, and other tests required to diagnose renal damage in children with OAB.
