Background: Acute coronary syndrome (ACS) resulting from culprit lesion in left main coronary art... more Background: Acute coronary syndrome (ACS) resulting from culprit lesion in left main coronary artery (LMCA) can cause rapid hemodynamic deterioration. It is important to identify these patients early to facilitate timely revascularization. ST segment elevation in aVR greater than or equal to V1 (aVR-V1≥ 0) has been suggested as a sensitive predictor of LMCA disease. As a result of balanced forces, we hypothesized that ST deviation in V6 greater than or equal to ST deviation in V1 (V6-V1≥ 0) might be a good determinant of LMCA disease.Methods: We compared admission 12-lead ECGs of ACS resulting from culprit LMCA lesion (n = 75, group I) with ACS resulting from culprit left anterior descending lesion (n = 81, group II). Group I was selected over a period of 10 years. We compared V6-V1≥ 0 to aVR-V1≥ 0 in both groups. We also looked at ratios of ST deviations in V6,V1 (V6/V1≥ 1) and aVR,V1 (aVR/V1≥ 1) in patients where ST segment in V1 was not isoelectric (group I = 54 and group II = 55).Results: ST deviation in V6 was significantly greater in group I as compared to group II (P < 0.001). The reliabilities of V6-V1≥ 0, V6/V1≥ 1, aVR-V1≥ 0, and aVR/V1≥ 1 in predicting LMCA disease were determined.Conclusion: This is the largest series of ECG analysis on ACS resulting from culprit LMCA lesion. V6-V1≥ 0 and V6/V1≥ 1 were more sensitive in predicting LMCA as culprit vessel in comparison to previously reported greater ST segment elevation in aVR than V1.

There is an urgent need for a robust, clinically useful predictive model for survival in a hetero... more There is an urgent need for a robust, clinically useful predictive model for survival in a heterogeneous group of patients with metastatic cancer referred to radiation oncology. From May 2012 to August 2013, 143 consecutive patients with stage IV cancer were prospectively evaluated by a single radiation oncologist. We retrospectively analyzed the effect of 29 patient, laboratory and tumor-related prognostic factors on overall survival using univariate analysis. Variables that were statistically significant on univariate analysis were entered into a multivariable Cox regression to identify independent predictors of overall survival. The median overall survival was 5.5 months. Four prognostic factors significantly predicted survival on multivariable analysis including ECOG performance status (0-1 vs. 2 vs. 3-4), number of active tumors (1 to 5 vs. ≥6), albumin levels (≥3.4 vs. 2.4 to 3.3 vs. <2.4 and primary tumor site (Breast, Kidney or Prostate vs. Other). We undertook a study to determine the incidence of hepatitis C in patients with persistent elevat... more We undertook a study to determine the incidence of hepatitis C in patients with persistent elevations of aminotransferases, either alanine aminotransferase (ALT) or aspartate aminotransferase (AST). We conducted a retrospective review of medical records at the Family Practice Center from 1992 to 1993. Patients in whom aminotransferase levels were elevated 1.5 times normal for at least 6 months were eligible for the study. Sixty patients met our eligibility criteria. Patients were tested for hepatitis B and C and other possible causes of elevated aminotransferase levels. Of these 60 patients, 35 (58 percent) tested positive for hepatitis C virus by first-generation enzyme-linked immunosorbent assay (ELISA-I); 30 had positive results confirmed by second-generation recombinant immunoblot assay (RIBA-II). The other 5 patients were lost to follow-up, and their status was not confirmed. Of the 30 patients with a positive RIBA-II, 14 had liver biopsies showing chronic persistent hepatitis or chronic active hepatitis, both consistent with hepatitis C infection. Of the 60 patients, 2 (3 percent) had hepatitis B. None had active hepatitis B coexisting with hepatitis C. Our data show a much greater incidence of hepatitis C (50 percent) in our patients with chronic elevations of aminotransferase levels compared with data reported from previous studies.

The purpose of this study was to examine the relationship of performance on the Comprehensive Ost... more The purpose of this study was to examine the relationship of performance on the Comprehensive Osteopathic Medical Licensing Examination (COMLEX-USA) Level 1 with academic performance at colleges accredited by the American Osteopathic Association (AOA). Eighteen (95%) of 19 AOA-accredited colleges and 2146 students (91% of those taking the June 1999 examination) met criteria and participated. Students were classified by school representatives on the basis of academic performance in the first 2 years of the curriculum. The relationships of Level 1 performance with assigned classifications and grade point averages (GPAs) were studied. Of students classified in the highest 20% academically, the Level 1 pass rate was 100%, with a mean score of 599. Of students classified in the lowest 5%, the pass rate was 63.5%, with a mean of 416.3. For 16 schools that provided GPAs, the within-school correlations between Level 1 scores and GPAs ranged from r = 0.76 to r = 0.85, with a mean correlation...

Our objective was to provide a descriptive analysis of prescriptions written for controlled opioids from a database of emergency department (ED) visits prior to the enactment of the I-STOP law, which requires New York prescribers to consult the Prescription Monitoring Program (PMP) prior to prescribing Schedule II, III, and IV controlled substances for prescriptions of greater than five days duration.
