The current study aimed 1) to describe changes in patient-reported outcomes and physical activity... more The current study aimed 1) to describe changes in patient-reported outcomes and physical activity measured with an accelerometer preoperatively, 6 months and 2 years postoperatively in older patients undergoing total knee arthroplasty (TKA) for arthritis, and 2) to examine the predictors of the changes in physical activity (PA). This study included 58 patients (mean age 72.6 years, 84.5% women) who completed the Oxford Knee Score (OKS) and the 8-item Short Form Health Survey. Physical activity measured mean steps per day, duration of light physical activity and moderate-to-vigorous physical activity (MVPA) per week. All PA indicators and patient-reported outcomes improved 6 months postoperatively. After 6 months, knee-related pain and function gradually improved, and MVPA increased. The OKS was a sole predictor of improvement in PA during the 2-year study period, suggesting the importance of disease-specific quality of life.

This study aimed to examine the following: (1) predictors of sleep quality and (2) fluctuations in sleep quality in patients with SLE. Patients with SLE were recruited from three rheumatology centers in Japan. We collected demographic and clinical data and data on sleep quality as measured by the Pittsburgh Sleep Quality Index (PSQI), the Medical Outcome Study Short Form-12, and the Lupus Patient Reported Outcome Tool (LupusPRO). Fluctuations in sleep quality were examined by administering the PSQI a second time after a 2-week interval. We used multiple linear regression analysis to predict sleep quality. Of 205 patients who completed the survey, 62.9% showed poor sleep quality. The largest fluctuation in sleep quality was for Bwaking in the middle of the night or early morning.B LupusPRO pain/vitality^was a major predictor of poor sleep. The other significant predictors were mostly LupusPRO subscales and clinical variables and SF-12 subscales were mostly non-predictive. The majority of the participants had poor sleep quality. A lupus-specific QoL scale is important for understanding poor sleep quality in SLE patients. Symptom management appeared to play a key role in improving sleep quality.

Methods: Participants were 205 lupus patients recruited from three rheumatology centers in Japan. Demographic data were collected and quality of life was assessed using the LupusPRO and the Short Form Health Survey-12. Disease activity was evaluated by physicians using the Systemic Lupus Erythematosus Activity Index. Some participants completed questionnaires 10-14 days after the first survey. Internal consistency reliability, test-retest reliability, content validity and convergent validity were examined, and confirmatory factor analysis was performed. Results: Participants' mean age was 47.8 AE 13.6 years. Older participants scored lower on physical quality of life and higher on coping than younger participants. The LupusPRO showed satisfactory test-retest reliability (n ¼ 111). Test-retest reliability was lower for the mental and social aspects of quality of life, indicating fluctuations in quality of life during the two-week interval. Internal consistency reliability was good and convergent validity with the corresponding domains of the Short Form Health Survey-12 was satisfactory. Confirmatory factor analysis showed a good model fit. Conclusion: The Japanese LupusPRO is a reliable and valid measure to evaluate treatment interventions for systemic lupus erythematosus. However, only cross-sectional surveys on these health outcomes have been conducted, and the association between day-today fluctuations remains unknown. Objectives: We aimed to characterize daily fluctuations in sleep quality, physical activity, pain, and fatigue in patients with SLE. Method: Exploratory study with a cross-sectional design. Two rheumatology centers (a university hospital and a prefectural hospital) in Japan between September 2017 and May 2019. The sample size was set to 20. Demographic and clinical data were collected. Sleep and physical activity were measured with monitoring devices; pain and fatigue levels were recorded daily during the 4-week period. The Pittsburgh Sleep Quality Index, Short Form Health Survey-12, the Japanese version of the Lupus Patient Outcome, and SLE Disease Activity Index 2000 were collected at the start and end of the study. Descriptive statistics and coefficients of variation (CV) were tabulated to examine daily fluctuations. Pearson correlation coefficients were obtained for monitored variables. Results: The mean age was 43.7 ± 8.5 years, and the mean SLE duration was 16.0 ± 7.2 years. The mean moderate-to-vigorous physical activity (MVPA) duration was 7.8 ± 5.8 min/day, and the mean total sleep duration was 391.8 ± 65.3 min, with a mean sleep efficiency of 88.6% ± 6.1%. Daily fluctuations were high for leaving the bed frequency, MVPA duration, pain, and waking after sleep onset.

The aims of this study were to: (1) describe the physical activity (PA) and quality of life (QOL)... more The aims of this study were to: (1) describe the physical activity (PA) and quality of life (QOL) in living donor liver transplant (LDLT) recipients pre-operatively and at 3 months and 6 months post-operatively; (2) compare PA and QOL at 6 months post-operatively with a healthy control group; and (3) explore pre-operative factors that predict PA changes. Methods: Patients over 20 years of age who were undergoing LDLT were recruited. PA was measured based on the number of steps/day and time spent performing moderate-to-vigorous PA (MVPA) during 1 week using an accelerometer. QOL was assessed based on a physical (PCS) and mental (MCS) component summary of the eight-item Short-Form Health Survey. The LDLT and healthy control groups were matched for age (±3 years) and sex. Pre-operative factors predicting a change in PA were calculated using a generalized linear mixed model. Results: Twenty-four patients completed the study. By 6-months post-LDLT, the MCS and PCS were comparable to those in the control group. The number of steps (3887 steps/day) and MVPA (29.3 min/week) showed significant improvement by 6 months post-operatively, but remained much lower compared with those in the control group. The multi-variate analysis showed that younger age (p<0.01, p=0.04) and higher skeletal muscle mass (SMM; p<0.01, p=0.03) were 2 predictors of improvement in number of steps and MVPA. Conclusion: This study suggests the need for pre-operative interventions by healthcare professionals that focus on outcomes such as improving low SMM to facilitate post-operative PA recovery.

Background: The nursing shortage and high turnover rates have become a global issue. Implementati... more Background: The nursing shortage and high turnover rates have become a global issue. Implementation of preceptorship is an effective approach to retain new nurses in healthcare facilities. By better understanding procedures and effectiveness of the program, retention of new nurses can be enhanced. Objective: We conducted a systematic literature review to evaluate the effectiveness of preceptorship program on increasing retention of new nurses, its strategies, and the outcomes. Methods: We conducted a literature review analyzing 13 observational or experimental studies that investigated the effectiveness of the preceptorship program as an intervention. Results: The levels of evidence of these studies were between Level II and III. According to the results, the preceptorship program improved nursing competence, job satisfaction, and quality of care, and it reduced turnover rates of new nurses as well as manpower costs. Discussion: The intervention is recommended to continue for at least 3 months. Strategies such as methods and process for selecting appropriate instructors, development of multi-dimensional training programs, and creation of evaluation systems require long-term planning and monitoring. In order to observe the outcome of implementation, follow-up discussions should continue for more than 3 months and reporting channels should be created as well. The performance indicators were generated from questionnaire with more subjective than objective questions. In order to determine the efficiency of the intervention, we suggest incorporating more objective assessments into future indicators for improvement of nursing competency evaluation. Conclusion: The results of the systematic review suggest that the preceptorship program is a beneficial solution for retaining new nurses.

Extensive gastrointestinal surgery surveillance data in Japan were analyzed to examine the differ... more Extensive gastrointestinal surgery surveillance data in Japan were analyzed to examine the differences in the risk factors for surgical site infection (SSI) between laparotomy and laparoscopic abdominal procedures. Surgical procedures investigated in the study were gastrectomy, cholecystectomy, colectomy, rectal resection, and appendectomy. A total of 32,629 patients were included in the study. The study participants were divided into two groups according to the year of surgery, 2003-2009 (first study period) and 2010-2015 (second study period), due to the increase in the number of laparoscopic surgeries in the second study period. The incidence of SSI was stratified by three SSI classifications (superficial incisional, deep incisional, and organ/space SSI). Multiple logistic regression analysis was performed to predict the risk factors for SSI. The percentage of laparoscopic surgeries performed has increased linearly since 2010. Patients in the second study period were significantly older and had a higher prevalence of SSI risk factors compared with those in the first study period. In addition, the predictive factors changed substantially in most surgical procedures between the two study periods. Wound class � 3 was a ubiquitous risk factor for superficial incisional SSI (SI-SSI) and organ/space SSI (OS-SSI) in both open (laparotomy) and laparoscopic procedures in the first study period. Meanwhile, in the second study period, operative duration was a ubiquitous risk factor in both procedures.

A postal survey was conducted to study the use of public hospitals by foreigners, and the type of... more A postal survey was conducted to study the use of public hospitals by foreigners, and the type of services or programs they offer and the types of problems nurses experience in caring for foreign patients. Questionnaires were sent to the directors of nursing in all public hospitals (n=157) with 300 beds or more in prefectures with over 10,000 registered foreigners in 2000. The director of nursing was asked to forward questionnaires to assistant chief nurses in the medical and surgical wards. The response rates were 61.1% for directors of nursing and 61.8% for assistant chief nurses. Nearly 100% of the hospitals were reported to have had foreign patients both at outpatient and at inpatient services. The majority of the hospitals offered services or programs related to foreign patients, such as interpreter services. A survey of assistant chief nurses revealed that almost all the respondents reported to have had an experience of caring for foreign patients, and 90% of them had problems...

After the oral administration of 50% glycerol (12 mL/kg), serum, CSF and inner ear fluids from sc... more After the oral administration of 50% glycerol (12 mL/kg), serum, CSF and inner ear fluids from scala tympani perilymph, scala vestibuli perilymph, and scala media endolymph were collected from normal guinea pigs under sodium pentobarbital anesthesia (25–35 mg/kg). The sodium and potassium concentrations were determined by microflame photometry. Increases in sodium concentrations were found in CSF, scala tympani perilymph, scala vestibuli perilymph, and cochlear endolymph. No significant change was observed in the serum. These sodium increases were considered to be due to the dehydration caused by the osmotic action of glycerol. Potassium concentration was increased only in scala tympani perilymph. Oral administration of glycerol was found to be more gradual and effective in dehydration compared to intravenous injection.

Conclusion: The results of the systematic review suggest that the preceptorship program is a beneficial solution for retaining new nurses.

This study aimed to examine the following: (1) predictors of sleep quality and (2) fluctuations in sleep quality in patients with SLE. Patients with SLE were recruited from three rheumatology centers in Japan. We collected demographic and clinical data and data on sleep quality as measured by the Pittsburgh Sleep Quality Index (PSQI), the Medical Outcome Study Short Form-12, and the Lupus Patient Reported Outcome Tool (LupusPRO). Fluctuations in sleep quality were examined by administering the PSQI a second time after a 2-week interval. We used multiple linear regression analysis to predict sleep quality. Of 205 patients who completed the survey, 62.9% showed poor sleep quality. The largest fluctuation in sleep quality was for Bwaking in the middle of the night or early morning.B LupusPRO pain/vitality^was a major predictor of poor sleep. The other significant predictors were mostly LupusPRO subscales and clinical variables and SF-12 subscales were mostly non-predictive. The majority of the participants had poor sleep quality. A lupus-specific QoL scale is important for understanding poor sleep quality in SLE patients. Symptom management appeared to play a key role in improving sleep quality.

Extensive gastrointestinal surgery surveillance data in Japan were analyzed to examine the differ... more Extensive gastrointestinal surgery surveillance data in Japan were analyzed to examine the differences in the risk factors for surgical site infection (SSI) between laparotomy and laparoscopic abdominal procedures. Surgical procedures investigated in the study were gastrectomy, cholecystectomy, colectomy, rectal resection, and appendectomy. A total of 32,629 patients were included in the study. The study participants were divided into two groups according to the year of surgery, 2003-2009 (first study period) and 2010-2015 (second study period), due to the increase in the number of laparoscopic surgeries in the second study period. The incidence of SSI was stratified by three SSI classifications (superficial incisional, deep incisional, and organ/space SSI). Multiple logistic regression analysis was performed to predict the risk factors for SSI. The percentage of laparoscopic surgeries performed has increased linearly since 2010. Patients in the second study period were significantly older and had a higher prevalence of SSI risk factors compared with those in the first study period. In addition, the predictive factors changed substantially in most surgical procedures between the two study periods. Wound class � 3 was a ubiquitous risk factor for superficial incisional SSI (SI-SSI) and organ/space SSI (OS-SSI) in both open (laparotomy) and laparoscopic procedures in the first study period. Meanwhile, in the second study period, operative duration was a ubiquitous risk factor in both procedures.

A postal survey was conducted to study the use of public hospitals by foreigners, and the type of... more A postal survey was conducted to study the use of public hospitals by foreigners, and the type of services or programs they offer and the types of problems nurses experience in caring for foreign patients. Questionnaires were sent to the directors of nursing in all public hospitals (n=157) with 300 beds or more in prefectures with over 10,000 registered foreigners in 2000. The director of nursing was asked to forward questionnaires to assistant chief nurses in the medical and surgical wards. The response rates were 61.1% for directors of nursing and 61.8% for assistant chief nurses. Nearly 100% of the hospitals were reported to have had foreign patients both at outpatient and at inpatient services. The majority of the hospitals offered services or programs related to foreign patients, such as interpreter services. A survey of assistant chief nurses revealed that almost all the respondents reported to have had an experience of caring for foreign patients, and 90% of them had problems...

After the oral administration of 50% glycerol (12 mL/kg), serum, CSF and inner ear fluids from sc... more After the oral administration of 50% glycerol (12 mL/kg), serum, CSF and inner ear fluids from scala tympani perilymph, scala vestibuli perilymph, and scala media endolymph were collected from normal guinea pigs under sodium pentobarbital anesthesia (25–35 mg/kg). The sodium and potassium concentrations were determined by microflame photometry. Increases in sodium concentrations were found in CSF, scala tympani perilymph, scala vestibuli perilymph, and cochlear endolymph. No significant change was observed in the serum. These sodium increases were considered to be due to the dehydration caused by the osmotic action of glycerol. Potassium concentration was increased only in scala tympani perilymph. Oral administration of glycerol was found to be more gradual and effective in dehydration compared to intravenous injection.
