In this study we aimed to determine whether thyroid USG is useful in the absence of any complaint... more In this study we aimed to determine whether thyroid USG is useful in the absence of any complaints and prominent self/family history or risk of thyroid pathology. Method: Thyroid USG results of 1198 asymptomatic adults, who had no self or family history of any thyroid pathology, were retrospectively included. In all cases USG was done by the same radiologist. Thyroid nodules were classified based on their sonographic characteristics suspicious for malignancy according to American Thyroid Association (ATA)-2015 guideline. Results: Based on ATA-2015 criteria 5% (n=61) detected nodule with high risk and 27% (n=325) detected nodule with intermediate risk for malignancy. Three percent (n=33) had nodules with low risk for malignancy based on USG findings. Conclusions: USG alone may not be a decisive screening tool for thyroid carcinoma. Personal/family history and risks may guide in which cases thyroid USG would be more reliable.

In this study, we investigated the relationship of mean platelet volume (MPV) and neutrophil/lymphocyte ratio (NLR) with inflammation and proteinuria in patients with CKD Stage 3-4. Healthy individuals who applied to nephrology clinic for checkup purposes acted as controls. Fifty-three patients and 30 healthy controls were included in the study. Patients with diabetes mellitus, active infection, malignancy, and coronary artery disease were excluded from the study. Granulocytic sarcoma develops during the courses of acute myleoid leukemia, myelodisplastic syndrome and myeloproliferative diseases and frequently it is observed on the bones, soft tissue, periost, lymph nodes and skin. Here we report a 60 year old female patient who is at first diagnosed as diffuse large B cell lymphoma according to the biopsy result of the 6x10 cm nodular lesion on the anterolateral side of her left calf and at last diagnosed as granulocytic sarcoma upon reevaluation of the biopsy material. Granulocytic sarcomas are frequently confused with lymphomas or the sarcomas. Here the difficulties of the diagnosis of granulocytic sarcoma is discussed and reviewed in association with the literature.

ABSTRACT Objective: To evaluate the association between cognitive functions and metabolic status ... more ABSTRACT Objective: To evaluate the association between cognitive functions and metabolic status in cases with gestational diabetes mellitus (GDM). Methods: In this study 44 patients with GDM and 45 normal pregnant were included. Depression was evaluated with Beck&#39;s depression inventory (BDI). Cognitive functions were evaluated with Montreal cognitive assessment (MOCA), paced auditory serial addition test (PASAT), spatial recall test (SRT), symbol digit modalities (SDMT), and word list generation (WLG). Results: The mean gestational age, educational level and account of previous birth of the subjects in the study were not statistically different between the groups. The mean scores of BDI of the 2 groups were not statistically different. MOCA score and SDMT was significantly decreased in GDM in comparison to NP (p=0.005, p=0.04 respectively). Also, SDMT score was inversely correlated with number of pregnancies, postprandial blood glucose and HbA1c.The scores of PASAT, SRT, SPART and WLG were statistically similar between the groups. Conclusions: Our findings may be a clue for early onset of impairment in cognitive functions in cases with new onset diabetes during pregnancy. © Georg Thieme Verlag KG Stuttgart · New York.

Increased bowel length and delayed transit time may cause functional disturbance of the bowel. The objective of the current study is to evaluate the frequency of irritable bowel syndrome (IBS) in cases with acromegaly. Twenty-three active cases with acromegaly newly diagnosed between 2010-2011 were included in the study. The control group consisted of ninety gender and age-matched healthy controls (HC). All cases were questioned for presence of IBS using Rome III criteria. Abdominal ultrasonography and colonoscopy results of acromegalic patients were obtained. Background: Nonalcoholic fatty liver disease (NAFLD) shares similar risk factors with coronary ar... more Background: Nonalcoholic fatty liver disease (NAFLD) shares similar risk factors with coronary artery (CAD). NAFLD is getting more common and present in up to one-third of the Turkish population while Turkish people change their traditional lifestyle nutrutinal behaviors. We aimed to determine if NAFLD has any role in the development of CAD. The other question was the need of screening or surveillance of NAFLD patients for CAD in Turkey. Method: We evaluated the risk factors for CAD and demografic characteristics of NAFLD patients. 34 patients diagnosed with NAFLD by ultrasonography and alcohol cosumption history were enrolled. Demographic and anthropometric parameters, oral glucose tolerance test with insulin levels, transaminase levels, C-reactive protein (CRP) and all components of metabolic syndrome criteria were measured in all patients. The patients were evaluated for CAD by a cardiologist according to guidelines. Results: Elevated transaminase level was found in 37% of NAFLD patients. This increase was significantly associated with body mass index, waist/hip ratio, fasting glucose, hyperinsulinemia, low high density lipoprotein (HDL), male sex, high CRP levels. CAD was found in 32.6% of patients with elevated transaminase and only 10.2% of patients with normal transaminase. Conclusion: These findings strongly suggest that NAFLD is likely to be associated with increased CAD risk. Screening and surveillance stratagies for CAD may be suggested to patients with NAFLD especially when they have elevated transaminases (steatohepatitis) in Turkey.

Pelvis x-ray revealed alterations of bone density in the right iliac. Abdomino-pelvic ultra-sound showed iliac, lumbo aortic and retrocrural lymphadenopathies. Renal, pelvic, thyroid and breast ultrasound were normal. Laboratory revealed increased LDH (1 372 U/L) and anemia (Hb 11.8 g/dL). Further evaluation showed thrombocytopenia (18 000) and rising values of LDH that reached 4 404U/L. Protein electrophoresis, beta 2 microglobulin and anti-HIV 1 and 2 were negative. Myelogram revealed hypercellular bone marrow with invasion by atypical cells. A CT revealed multiple iliac, retroperitoneal and supraclavicular adenopathies; lytic lesion in the left scapula; lytic mass in the right iliac bone about 12x13 cm, destroying the iliac bone, sacrum and right sacroiliac joint and invading the right gluteus muscle. Histology of CT-guided biopsy of the lesion revealed a ALCL, with CD30, CD4, ALK and EMA positive markers. The final diagnosis was systemic anaplastic lymphoma, stage IV-B, with bone marrow and muscle skeletal involvement. She was medicated with cyclophosphamide, doxorubicin, vincristine and prednisone (CHOP chemotherapy)and died of sepsis 2 weeks later. Discussion & conclusion: ALK-positive anaplastic large-cell lymphomas have a more favorable prognosis, but staging is also a very important prognostic factor. It is therefore extremely important an early diagnosis. Unfortunately,in this particular case, the patient had a late diagnosis and did succumb to overwhelming infection before the result of chemotherapy could be evaluated.

Thyrotropin alfa (Thyrogen) has many advantages over THW. Radiation dose to laboratory staff while drawing blood for tests on the day 5 is one of the disadvantages of preferring Thyrogen. Our aim was to compare day 3 and day 5 blood test results after Thyrogen injections. In our study, Thyrogen was preferred in 32 differentiated thyroid cancer patients with a mean age of 50.5 ± 12.3 years. Thyrogen was injected on day 1 and day 2 intramuscularly in all patients before I-131 was given on day 3. A cohort of pregnant women were divided into three groups: women with gestational diabetes mellitus, ones with normal results both after the 50 gr and 100 gr OGTT (CG-1) and ones having a positive result after the 50 gr OGTT screening but negative results for gestational diabetes mellitus (GDM) after the 100 gr OGTT (CG-2). The newborn length in CG-1 was greater than in GDM and CG-2 (p= 0.002 and p= 0.02). Fasting blood glucose and insulin resistance (IR) were negatively correlated with length of the newborns (r=-0.3, p=0.03 and r=-0.3, p=0.01). In this study, we investigated the relationship of mean platelet volume (MPV) and neutrophil/lymphocyte ratio (NLR) with inflammation and proteinuria in patients with CKD Stage 3-4. Healthy individuals who applied to nephrology clinic for checkup purposes acted as controls. Fifty-three patients and 30 healthy controls were included in the study. Patients with diabetes mellitus, active infection, malignancy, and coronary artery disease were excluded from the study. Granulocytic sarcoma develops during the courses of acute myleoid leukemia, myelodisplastic syndrome and myeloproliferative diseases and frequently it is observed on the bones, soft tissue, periost, lymph nodes and skin. Here we report a 60 year old female patient who is at first diagnosed as diffuse large B cell lymphoma according to the biopsy result of the 6x10 cm nodular lesion on the anterolateral side of her left calf and at last diagnosed as granulocytic sarcoma upon reevaluation of the biopsy material. Granulocytic sarcomas are frequently confused with lymphomas or the sarcomas. Here the difficulties of the diagnosis of granulocytic sarcoma is discussed and reviewed in association with the literature.

Methods: In this study 44 patients with GDM and 45 normal pregnant were included. Depression was evaluated with Beck&#39;s depression inventory (BDI). Cognitive functions were evaluated with Montreal cognitive assessment (MOCA), paced auditory serial addition test (PASAT), spatial recall test (SRT), symbol digit modalities (SDMT), and word list generation (WLG). Results: The mean gestational age, educational level and account of previous birth of the subjects in the study were not statistically different between the groups. The mean scores of BDI of the 2 groups were not statistically different. MOCA score and SDMT was significantly decreased in GDM in comparison to NP (p=0.005, p=0.04 respectively). Also, SDMT score was inversely correlated with number of pregnancies, postprandial blood glucose and HbA1c.The scores of PASAT, SRT, SPART and WLG were statistically similar between the groups. Conclusions: Our findings may be a clue for early onset of impairment in cognitive functions in cases with new onset diabetes during pregnancy. © Georg Thieme Verlag KG Stuttgart · New York.

The objective of the current study is to evaluate the frequency of irritable bowel syndrome (IBS) in cases with acromegaly. Twenty-three active cases with acromegaly newly diagnosed between 2010-2011 were included in the study. The control group consisted of ninety gender and age-matched healthy controls (HC). All cases were questioned for presence of IBS using Rome III criteria. Abdominal ultrasonography and colonoscopy results of acromegalic patients were obtained. Background: Nonalcoholic fatty liver disease (NAFLD) shares similar risk factors with coronary ar... more Background: Nonalcoholic fatty liver disease (NAFLD) shares similar risk factors with coronary artery (CAD). NAFLD is getting more common and present in up to one-third of the Turkish population while Turkish people change their traditional lifestyle nutrutinal behaviors. We aimed to determine if NAFLD has any role in the development of CAD. The other question was the need of screening or surveillance of NAFLD patients for CAD in Turkey. Method: We evaluated the risk factors for CAD and demografic characteristics of NAFLD patients. 34 patients diagnosed with NAFLD by ultrasonography and alcohol cosumption history were enrolled. Demographic and anthropometric parameters, oral glucose tolerance test with insulin levels, transaminase levels, C-reactive protein (CRP) and all components of metabolic syndrome criteria were measured in all patients. The patients were evaluated for CAD by a cardiologist according to guidelines. Results: Elevated transaminase level was found in 37% of NAFLD patients. This increase was significantly associated with body mass index, waist/hip ratio, fasting glucose, hyperinsulinemia, low high density lipoprotein (HDL), male sex, high CRP levels. CAD was found in 32.6% of patients with elevated transaminase and only 10.2% of patients with normal transaminase. Conclusion: These findings strongly suggest that NAFLD is likely to be associated with increased CAD risk. Screening and surveillance stratagies for CAD may be suggested to patients with NAFLD especially when they have elevated transaminases (steatohepatitis) in Turkey.

Renal, pelvic, thyroid and breast ultrasound were normal. Laboratory revealed increased LDH (1 372 U/L) and anemia (Hb 11.8 g/dL). Further evaluation showed thrombocytopenia (18 000) and rising values of LDH that reached 4 404U/L. Protein electrophoresis, beta 2 microglobulin and anti-HIV 1 and 2 were negative. Myelogram revealed hypercellular bone marrow with invasion by atypical cells. A CT revealed multiple iliac, retroperitoneal and supraclavicular adenopathies; lytic lesion in the left scapula; lytic mass in the right iliac bone about 12x13 cm, destroying the iliac bone, sacrum and right sacroiliac joint and invading the right gluteus muscle. Histology of CT-guided biopsy of the lesion revealed a ALCL, with CD30, CD4, ALK and EMA positive markers. The final diagnosis was systemic anaplastic lymphoma, stage IV-B, with bone marrow and muscle skeletal involvement. She was medicated with cyclophosphamide, doxorubicin, vincristine and prednisone (CHOP chemotherapy)and died of sepsis 2 weeks later. Discussion & conclusion: ALK-positive anaplastic large-cell lymphomas have a more favorable prognosis, but staging is also a very important prognostic factor. It is therefore extremely important an early diagnosis. Unfortunately,in this particular case, the patient had a late diagnosis and did succumb to overwhelming infection before the result of chemotherapy could be evaluated.

Thyrotropin alfa (Thyrogen) has many advantages over THW. Radiation dose to laboratory staff while drawing blood for tests on the day 5 is one of the disadvantages of preferring Thyrogen. Our aim was to compare day 3 and day 5 blood test results after Thyrogen injections. In our study, Thyrogen was preferred in 32 differentiated thyroid cancer patients with a mean age of 50.5 ± 12.3 years. A cohort of pregnant women were divided into three groups: women with gestational diabetes mellitus, ones with normal results both after the 50 gr and 100 gr OGTT (CG-1) and ones having a positive result after the 50 gr OGTT screening but negative results for gestational diabetes mellitus (GDM) after the 100 gr OGTT (CG-2). The newborn length in CG-1 was greater than in GDM and CG-2 (p= 0.002 and p= 0.02). Fasting blood glucose and insulin resistance (IR) were negatively correlated with length of the newborns (r=-0.3, p=0.03 and r=-0.3, p=0.01).
