We attempted to perform a systematic review of original articles published between 1990 to 2020 based on certain selection criteria to estimate the case fatality risk of this disease in children. We addressed the pattern of complications of this disease with its impact on mortality We calculated pooled prevalence with a random effects model (restricted maximum likelihood method) that assumed varying effect sizes between studies using R statistical software. We reported the case fatality rate and its 95% confidence intervals (CIs) in the pooled analysis. Thirty-seven articles with a total of 3329 children were identified for inclusion. Methods: The undergraduate medical students pursuing pharmacology in 2017-18 in our institute was divided into two groups after stratified randomization based on the last semester grades. After a session of didactic lecture on ‘Drugs affecting Calcium Metabolism’ and a pre-test, one group was taught using traditional tutorial methods and another group using the concept map method. Finally, a post-test was taken and feedback received from the intervention group. Scrotal Calcinosis (SC) is a sporadic approachable disease distinguished by appearance of multipl... more Scrotal Calcinosis (SC) is a sporadic approachable disease distinguished by appearance of multiple pulmonary nodules within the standard scrotal corium. A 34 year old Bengali (Indian) man presented with six month chronicle of scrotal growth that swiftly increased in size and number. Examination of skin divulged pain-free multiple growths (1 cm to 2 cm in size) below the scrotal corium. Histological enquiry showed no epithelial interlining. Serum and Urinary volume of calcium and phosphates were within usual limitations. Pathogenesis of scrotal calcinosis is not distinctly familiar. In our manifestation no confirmation of cystic formation was found around indurate medium. This proposed that SC might be ascendant in genesis. Therapy of disease is postoperative but foremost methodology is yet to be decided.

She had diffi culty in eating and smiling. Vesicular non pruritic rashes were visible in various stages of healing over face and body except palm and soles. She complained of bilateral headaches but no vomiting, or weakness in her limbs or alteration of her taste sensation. She was alert and cooperative with normal speech and memory. B/L lower motor neuron type of facial nerve palsy was found. Routine blood work up, serology for HIV and Lyme disease, chest x ray, CSF study and MRI brain were normal. She was prescribed 60 mg of prednisolone and 4000 mg of acyclovir every day for 14days. She recovered completely from the neurodefi cit within 14 days.

NSAIDs are responsible for the delayed diagnosis (due to suppression of clinical features), disease progression and adverse effects. Objective: To determine the factors leading to self-administration of NSAIDs, their usage pattern and possible adverse effects. To compare NSAID’s use by prescription and over the counter acquisition. Materials and Methods: The present study investigates the determinants and effects of self-administration of NSAIDs by patients attending a tertiary care hospital during August- October 2019. The patients were interviewed regarding basic socio demographic variables, their rationale behind taking NSAIDs along with adverse effects experienced by them with a validated questionnaire. Their awareness pertaining to the drugs was also assessed. Results: 55.2% of the population were aware about adverse drug reaction due to NSAIDs but only 17% suffered any consequence of such activity. 23% of the patients were not aware about the nature of the disease whereas 44.2% visited physician less than 1 month. Awareness regarding adverse drug reaction is more in elderly and middle aged in compare to youth. The intention to administer the drugs was relief of musculoskeletal pain (62.9%) followed by relief of headache (15.2%) followed by others. The people at the medicine shops are advisors to majority of the people administering drugs without prescription (42.9%) followed by family members (32.4%), colleagues as well as friends. Conclusion: We found large proportion of patients taking self-prescribed NSAIDs for symptomatic relief and advice by overenthusiastic peers. The first case is an urban middle class middle-aged housewife presented with significant cosmetic problems including hypopigmentation and telangiectasia over her face, along with patchy hypopigmentation over elbow. In the second case, an urban middle-aged man was prescribed an inappropriate regimen of nimesulide tablets and presented with maculopaupular rash in the extremities and erythema multiforme all over his body. Lack of legal supervision over 'quack' prescribing may lead to therapeutic misadventures which oftentimes remain undetected by pharmacovigilance programmes.

The first part of the topic deals with the definition of the recession or downturn. The harsh impact of the downturn on the Indian economy is discussed in the said portion. The second part deals with the objectives of the study. In this section, the main objective is to focus on the positive impact of the downturn on the Indian economy. The third part deals with the research methodology. The fourth part deals with the discussion of the following three topics: a) How can the Indian economy benefit from the downturn? b) How the delisting of the MNCs can be a blessing in disguise? c) How some of the Indian companies emerged as the winners in spite of the crippling economic conditions? The companies discussed above not only raced ahead in good times, but also marched ahead of their rivals in orderto gain the ground in the downturn. They made it possible because of the distinctiveness of their business models, their strategic decision making capability, and by simply following the fundamentals of the business. The final segment deals with the recommendations for Corporate India, by which the meltdown can be converted into streams of opportunity, not only for a recession, but also for future uncertainties. Hence, a downturn can be turned into a blessing in disguise.

Periodical review of antibiograms is of utmost importance for optimum patient benefit. To identify the spectrum of organisms responsible for urinary tract infection and evaluate the pattern of antibiotic sensitivity of the organisms. Urine samples were collected from all consecutive patients getting admitted in the medicine indoor irrespective of symptomatology. The clean-catch technique of midstream urine was used for patients able to void spontaneously while specimens of catheterized patients were obtained prior to catheter change. All samples were sent for routine examination as well as culture sensitivity tests. Descriptive statistical methods were used with the help of SPSS-15th version. Anti‑inflammatory effect of tea has also been depicted in different papers. Therefore we had set forward this study to examine the potential anti‑inflammatory and analgesic activities of green tea in different experimental animal models. Aims: Evaluation of anti‑inflammatory effects of green tea on rat. Evaluation of analgesic effects of green tea on mice. Materials and Methods: Green tea decoction (10% and 20%) was prepared by soaking 20 g of green tea in 100 ml boiled water separately, soaked for 2 mins and thereafter filtered. Acute anti‑inflammatory activity of tea decoction was evaluated using carrageenan and dextran whereas central and peripheral analgesic activities were evaluated by tail immersion test and acetic acid‑induced writhing test, respectively. Study Design: This is an experimental study. Results: Green tea decoction (10% and 20%) has shown significant anti‑inflammatory effects (65% and 70%) and (50% and 71%), respectively, on carrageenan and dextran‑induced acute inflammatory models which can be comparable with the standard drug indomethacin (93% and 98.3%, respectively). In central analgesic model Green tea decoction (10% and 20%) has shown no analgesic action at different hours as the reaction time was less than 10 seconds at all time interval. But at peripheral analgesic model green tea decoction (10% and 20%) has shown 20% and 35.74% inhibition, respectively, as compared to control group. Aspirin shows around 39.81% of inhibition compared to control. Conclusion: Taken together, our data indicate that green tea (20%) has a potential anti‑inflammatory and peripheral analgesic action and this corroborates with the current trend of tea being promoted as ‘health drink’.

Background: Graves' disease is a unique conglomeration of cardiovascular, neurological, ophthalmo... more Background: Graves' disease is a unique conglomeration of cardiovascular, neurological, ophthalmological, and other systemic manifestations. In this study we have tried to explore the psychiatric dimensions of this disease. Aims: This study attempted to explore clinical features, types, and treatment outcome of psychiatric disturbances in Graves disease. Study Design: This is a purposive study following the criteria of DSM IV. Materials and Methods: A total of 36 adult patients of newly diagnosed Graves disease and 30 age-and sex-matched controls were included. Data enumerated were age, sex, date of admission, analysis of psychiatric signs, and symptoms by one independent observer, diagnostic categorization, effect of treatment, and outcome. Follow-up evaluation was done after 1 year. Statistical Analysis: Statistical analysis was done by the standard error of difference, the chi-square test, and paired Student's T-test. Results: Among 36 patients 32 were female and 4 were male. Fifteen patients (41.67%) were diagnosed with generalized anxiety disorders (GAD), 6 (16.67%) with mood disorder, 6 (16.67%) with obsessive compulsive disorder (OCD), and 2 each with personality disorder and schizophreniform disorder. The common symptoms were insomnia, irritability, and anxiety. The Frequency of GAD was statistically more significant in the Graves disease group in comparison to control. Fourteen patients agreed to take both antithyroid and antipsychotropic medications (group 1). The rest were treated with only antithyroid drug (group 2). There was significant improvement in both groups and no difference between the groups. Conclusion: The prevalence of certain psychiatric manifestations in Graves' disease was significantly higher than in the control group. There was no significant difference between therapy with antithyroid drugs and combination of antithyroid with psychotropic medications.

Malignancies of various descriptions have been reported in this disease, the commonest being solid tumors of head/neck (squamous cell carcinoma). We report the case of a female patient with DC, who presented to us with severe wasting and primary amenorrhea and died of carcinoma stomach in our hospital 3 weeks later.

This study was undertaken to evaluate the demographic and etiological pattern of a rural population in Eastern India. Objectives. 1. To study the spectrum of etiology of prolonged fever. 2. To evaluate the demographic characteristics of the patients. Methods: All patients with undiagnosed fever for more than three weeks attending the Medicine outpatient Department were included in the study. History and clinical examination were recorded in a proforma. All patients under went routine as well as relevant special investigations including RDT for Malaria, IgM ELISA for Leptospira and Chikungunya and Brucella. Blood, urine and other body fluid culture including BACTEC and CB NAAT (as required) were also done. Endoscopies, imaging studies and guided biopsies were performed where needed. Results were recorded and analysed using appropriate descriptive statistical methods using SPSS Software (version 20). Results: A total number of 110 patients were included in the study. Of them 52(47.2%) were male and 58 (52.7%) were female. The age of the patients varied from 15 years to 78 years mean age being 33.68±18.77. Conclusion: The patients were mainly from rural background, most common districts being Murshidabad (40%) followed by24 Parganas S (20%), Midnapur (14.5%) and Nadia (10.9%). Socioeconomic status of the patients varied from lower (47.2%) lower middle class (39%) to middle class(13.8%). 65.3% of the patients were literate of whom 10.9 % of the people were matriculate while 1.8% were graduates. The commonest etiology of prolonged fever were infections 76(69%) out of which Tuberculosis 44(40%), took the lions' share. Tuberculosis was followed by chronic UTI (8.2%) enteric fever(5.5%) and malaria(4.5%). This was followed by various malignancies 14(12.7%) and inflammatory disorders 12(10.9%) It was notable that while prevalence of infections were comparable in males and females the prevalence of cancers (15.3%vs10.3%) and inflammatory disorders (7.6%vs 13.8%) varied. No diagnosis could be reached in 3.6% cases.

Objectives: Package inserts (PIs) are used by physicians and other health-care providers as ready... more Objectives: Package inserts (PIs) are used by physicians and other health-care providers as ready source of approved prescribing information. In India, they are subject to statutory regulations that specify the information to be provided under various headings. Uniformity of PIs with optimal level of information is desirable, the absence of which may lead to medication errors. This observational study aimed to evaluate the information adequacy and accuracy of PIs available in the Indian market. Materials and Methods: PIs of drugs marketed in India, and approved by United States Food and Drug Administration, were collected from various retail pharmacies through purposive sampling. The adequacy and accuracy of the information in each PI were evaluated with the help of a 25-item checklist prepared as per stipulations mentioned in statutory guidelines. Each required item of information was scored 1 if present and appropriate or 0 if absent or deemed incomplete or inaccurate. A total information adequacy score (IAS), with maximum value 25, was thereby calculated. Results: From the total 135 PIs analyzed, the median IAS was 17 (interquartile range 15–19). Deficiencies were observed under important headings. For example, “references” were mentioned in only 6.67% and “date of last updating” in only 19.26% of PIs. Other notable shortcomings were in “disposal” (not mentioned in 92.59%), “effects on ability to drive and use machines” (76.30%), “pharmaceutical incompatibilities” (66.67%), “shelf life” (62.96%), “excipients” (60.00%), and “overdose” (17.78%) information. Information on “generic name,” “composition,” and “indications” were however provided by all (100%) PIs. Conclusions: The information provided by PIs in India being inadequate, may not be able to serve as a reliable source of information.

She had diffi culty in eating and smiling. Vesicular non pruritic rashes were visible in various stages of healing over face and body except palm and soles. She complained of bilateral headaches but no vomiting, or weakness in her limbs or alteration of her taste sensation. She was alert and cooperative with normal speech and memory. B/L lower motor neuron type of facial nerve palsy was found. Routine blood work up, serology for HIV and Lyme disease, chest x ray, CSF study and MRI brain were normal. She was prescribed 60 mg of prednisolone and 4000 mg of acyclovir every day for 14days. She recovered completely from the neurodefi cit within 14 days.

NSAIDs are responsible for the delayed diagnosis (due to suppression of clinical features), disease progression and adverse effects. Objective: To determine the factors leading to self-administration of NSAIDs, their usage pattern and possible adverse effects. To compare NSAID’s use by prescription and over the counter acquisition. Materials and Methods: The present study investigates the determinants and effects of self-administration of NSAIDs by patients attending a tertiary care hospital during August- October 2019. The patients were interviewed regarding basic socio demographic variables, their rationale behind taking NSAIDs along with adverse effects experienced by them with a validated questionnaire. Their awareness pertaining to the drugs was also assessed. Results: 55.2% of the population were aware about adverse drug reaction due to NSAIDs but only 17% suffered any consequence of such activity. 23% of the patients were not aware about the nature of the disease whereas 44.2% visited physician less than 1 month. Awareness regarding adverse drug reaction is more in elderly and middle aged in compare to youth. The intention to administer the drugs was relief of musculoskeletal pain (62.9%) followed by relief of headache (15.2%) followed by others. The people at the medicine shops are advisors to majority of the people administering drugs without prescription (42.9%) followed by family members (32.4%), colleagues as well as friends. Conclusion: We found large proportion of patients taking self-prescribed NSAIDs for symptomatic relief and advice by overenthusiastic peers. The first case is an urban middle class middle-aged housewife presented with significant cosmetic problems including hypopigmentation and telangiectasia over her face, along with patchy hypopigmentation over elbow. In the second case, an urban middle-aged man was prescribed an inappropriate regimen of nimesulide tablets and presented with maculopaupular rash in the extremities and erythema multiforme all over his body. Lack of legal supervision over 'quack' prescribing may lead to therapeutic misadventures which oftentimes remain undetected by pharmacovigilance programmes.

Urine samples were collected from all consecutive patients getting admitted in the medicine indoor irrespective of symptomatology. The clean-catch technique of midstream urine was used for patients able to void spontaneously while specimens of catheterized patients were obtained prior to catheter change. All samples were sent for routine examination as well as culture sensitivity tests. Descriptive statistical methods were used with the help of SPSS-15th version. Anti‑inflammatory effect of tea has also been depicted in different papers. Therefore we had set forward this study to examine the potential anti‑inflammatory and analgesic activities of green tea in different experimental animal models. Aims: Evaluation of anti‑inflammatory effects of green tea on rat. Evaluation of analgesic effects of green tea on mice. Materials and Methods: Green tea decoction (10% and 20%) was prepared by soaking 20 g of green tea in 100 ml boiled water separately, soaked for 2 mins and thereafter filtered. Acute anti‑inflammatory activity of tea decoction was evaluated using carrageenan and dextran whereas central and peripheral analgesic activities were evaluated by tail immersion test and acetic acid‑induced writhing test, respectively. Study Design: This is an experimental study. Results: Green tea decoction (10% and 20%) has shown significant anti‑inflammatory effects (65% and 70%) and (50% and 71%), respectively, on carrageenan and dextran‑induced acute inflammatory models which can be comparable with the standard drug indomethacin (93% and 98.3%, respectively). In central analgesic model Green tea decoction (10% and 20%) has shown no analgesic action at different hours as the reaction time was less than 10 seconds at all time interval. But at peripheral analgesic model green tea decoction (10% and 20%) has shown 20% and 35.74% inhibition, respectively, as compared to control group. Aspirin shows around 39.81% of inhibition compared to control. Conclusion: Taken together, our data indicate that green tea (20%) has a potential anti‑inflammatory and peripheral analgesic action and this corroborates with the current trend of tea being promoted as ‘health drink’.

In this study we have tried to explore the psychiatric dimensions of this disease. Aims: This study attempted to explore clinical features, types, and treatment outcome of psychiatric disturbances in Graves disease. Study Design: This is a purposive study following the criteria of DSM IV. Materials and Methods: A total of 36 adult patients of newly diagnosed Graves disease and 30 age-and sex-matched controls were included. Data enumerated were age, sex, date of admission, analysis of psychiatric signs, and symptoms by one independent observer, diagnostic categorization, effect of treatment, and outcome. Follow-up evaluation was done after 1 year. Statistical Analysis: Statistical analysis was done by the standard error of difference, the chi-square test, and paired Student's T-test. Results: Among 36 patients 32 were female and 4 were male. Fifteen patients (41.67%) were diagnosed with generalized anxiety disorders (GAD), 6 (16.67%) with mood disorder, 6 (16.67%) with obsessive compulsive disorder (OCD), and 2 each with personality disorder and schizophreniform disorder. The common symptoms were insomnia, irritability, and anxiety. The Frequency of GAD was statistically more significant in the Graves disease group in comparison to control. Fourteen patients agreed to take both antithyroid and antipsychotropic medications (group 1). The rest were treated with only antithyroid drug (group 2). There was significant improvement in both groups and no difference between the groups. Conclusion: The prevalence of certain psychiatric manifestations in Graves' disease was significantly higher than in the control group. There was no significant difference between therapy with antithyroid drugs and combination of antithyroid with psychotropic medications.

This study was undertaken to evaluate the demographic and etiological pattern of a rural population in Eastern India. Objectives. 1. To study the spectrum of etiology of prolonged fever. 2. To evaluate the demographic characteristics of the patients. Methods: All patients with undiagnosed fever for more than three weeks attending the Medicine outpatient Department were included in the study. History and clinical examination were recorded in a proforma. All patients under went routine as well as relevant special investigations including RDT for Malaria, IgM ELISA for Leptospira and Chikungunya and Brucella. Blood, urine and other body fluid culture including BACTEC and CB NAAT (as required) were also done. Endoscopies, imaging studies and guided biopsies were performed where needed. Results were recorded and analysed using appropriate descriptive statistical methods using SPSS Software (version 20). Results: A total number of 110 patients were included in the study. Of them 52(47.2%) were male and 58 (52.7%) were female. The age of the patients varied from 15 years to 78 years mean age being 33.68±18.77. Conclusion: The patients were mainly from rural background, most common districts being Murshidabad (40%) followed by24 Parganas S (20%), Midnapur (14.5%) and Nadia (10.9%). Socioeconomic status of the patients varied from lower (47.2%) lower middle class (39%) to middle class(13.8%). 65.3% of the patients were literate of whom 10.9 % of the people were matriculate while 1.8% were graduates. The commonest etiology of prolonged fever were infections 76(69%) out of which Tuberculosis 44(40%), took the lions' share. Tuberculosis was followed by chronic UTI (8.2%) enteric fever(5.5%) and malaria(4.5%). This was followed by various malignancies 14(12.7%) and inflammatory disorders 12(10.9%) It was notable that while prevalence of infections were comparable in males and females the prevalence of cancers (15.3%vs10.3%) and inflammatory disorders (7.6%vs 13.8%) varied. No diagnosis could be reached in 3.6% cases.

Objectives: Package inserts (PIs) are used by physicians and other health-care providers as ready... more Objectives: Package inserts (PIs) are used by physicians and other health-care providers as ready source of approved prescribing information. In India, they are subject to statutory regulations that specify the information to be provided under various headings. Uniformity of PIs with optimal level of information is desirable, the absence of which may lead to medication errors. This observational study aimed to evaluate the information adequacy and accuracy of PIs available in the Indian market. Materials and Methods: PIs of drugs marketed in India, and approved by United States Food and Drug Administration, were collected from various retail pharmacies through purposive sampling. The adequacy and accuracy of the information in each PI were evaluated with the help of a 25-item checklist prepared as per stipulations mentioned in statutory guidelines. Each required item of information was scored 1 if present and appropriate or 0 if absent or deemed incomplete or inaccurate. A total information adequacy score (IAS), with maximum value 25, was thereby calculated. Results: From the total 135 PIs analyzed, the median IAS was 17 (interquartile range 15–19). Deficiencies were observed under important headings. For example, “references” were mentioned in only 6.67% and “date of last updating” in only 19.26% of PIs. Other notable shortcomings were in “disposal” (not mentioned in 92.59%), “effects on ability to drive and use machines” (76.30%), “pharmaceutical incompatibilities” (66.67%), “shelf life” (62.96%), “excipients” (60.00%), and “overdose” (17.78%) information. Information on “generic name,” “composition,” and “indications” were however provided by all (100%) PIs. Conclusions: The information provided by PIs in India being inadequate, may not be able to serve as a reliable source of information.
