Based on a standardisation of the format to report and the representation of clinical information on WWW, the multilingual multimedia case has been developed and till now 350 cases have been included. User satisfaction and user acceptance of ELCANO are under evaluation.

Frequent oblique muscle overaction in infantile esotropia and the development of such overaction ... more Frequent oblique muscle overaction in infantile esotropia and the development of such overaction after horizontal surgical alignment in patients with normal oblique function prior to surgery, led us to study this dysfunction in two hundred and eighty-one cases of infantile esotropia and ninety-eight of late onset esotropia. Inferior oblique overaction proved more frequent than superior oblique overaction in both groups. However, the incidence of inferior oblique overaction was not significantly different in infantile esotropia versus esotropia of late onset, while superior oblique overaction proved more frequent in infantile esotropia. Post-surgical development of oblique muscle overaction in infantile esotropia patients with normal preoperative oblique function reached 19.3%. In esotropia of late onset, the development of oblique muscle overaction after horizontal surgery was minimal and only observed in two cases.

This paper reviews the development of the field of medical informatics in Latin America. It also ... more This paper reviews the development of the field of medical informatics in Latin America. It also describes the results of a computer-based data management system, named AGUSTINA, which is comprised of maternal and infant data on 7570 deliveries that occurred between June, 1990 and December, 1996 in a hospital in the surroundings of Buenos Aires, Argentina. These data were fundamental for the instrumentation of preventive community-oriented activities in the area. Finally, this paper describes recommendations for future actions in the area of medical informatics in Latin America. Considering the importance of standarizing vision testing in order to permit early detection of o... more Considering the importance of standarizing vision testing in order to permit early detection of ocular disorders, we decided: 1) to prove that the control of the visual system of children 0 to 6 years of age can be carried out by pediatricians according to previously established patterns; 2) to determine the time employed for the tests and the level of cooperation of the children according to age, in order to select the simplest test to be used; 3) to determine the most frequent pathologies and derivate them to the pediatric ophthalmologist for confirmation of diagnosis. Material & methods. Visual examination was carried out in 387 children 0 to 6 years of age in the Neonatology Service and in the Children Health Section of the Hospital Nacional "Prof. A. Posadas". Healthy new born infants with rooming-in internation up to 4 days were included. The tests were selected according to age. Results. The age of the 387 children was from 2 days up to 5 years 11 months and 29 days, and the time requested for the tests between 3 min 4 sec and 11 min 8 sec. The age of cooperation in visual acuity tests, stereopsis and color vision were evaluated and proved to be greater in children older than 3 years. The 2 and 1/2 year old were excluded from visual acuity tests. The results were normal in: Ocular semiology: 382/387 (98.7%), red reflex: 387 (100%). Fundus could be examined in 301/387 (77.7%), all were normal Ocular fixation test was carried out after 4 months: 345/346 (99.7%). The alternate covered eye test and the Hirschberg test were taken from 6 months onward: 304/327 (93%). Visual acuity test was used in children older than 2 1/2 years: 121/123 (98.4%), stereopsis after 3 years: 124/125 (99.2%), and color vision test after 3 years: 132/132 (100%). Ocular pathologies were found in 5.2%. Conclusion. It is possible to carry out vision testing, performing the examinations 4 times in the first 6 years. The necessary studies of vision could be carried out by a health team or teachers. Standards for the care of vision by health care teams of teachers are proposed.
