To describe consumer expenditure on opioids prescribed by dentists in Brazil during a 12-month pe... more To describe consumer expenditure on opioids prescribed by dentists in Brazil during a 12-month period. We surveyed individual dispensed prescriptions of opioids in private pharmacies from October 2012 to September 2013. A descriptive analysis of costs included a calculation of the overall and mean maximum price to consumers. Monetary costs were converted from local currency units (Brazilian Real - BRL) to international dollars using purchasing power parity (PPP) exchanging rates. 129,708 prescriptions were retrieved from the database. The overall expenditure on opioids was R$4,316,383.46 BRL or 2,721,315.82(PPP).ThemeancostofanindividualprescriptionwasR2,721,315.82 (PPP). The mean cost of an individual prescription was R2,721,315.82(PPP).ThemeancostofanindividualprescriptionwasR33.27 BRL ($20.98 PPP), ranging from 14.19 to 3,255.60 BRL. Codeine was frequently prescribed (87.2 percent). The expenditure on opioids is a significant cost to Brazilian patients, especially given that the Brazilian Public Health System should dispense these drugs free of charge. Codeine was the most prescribed opioid.

This study explored the association between area-level primary dental care performance and area-l... more This study explored the association between area-level primary dental care performance and area-level demographics, dental treatment need, and health care service indicators. An ecological cross-sectional study was performed in Belo Horizonte, Brazil, in 2010. The 142 primary health care (PHC) units were grouped based on the following variables: access to individual dental treatment, frequency of dental emergencies, and frequency of individual preventive procedures. The independent variables analyzed were demographic variables, dental treatment need, and health care service indicators. The data were obtained from the information systems of the Brazilian Ministry of Health and the city of Belo Horizonte. We explored the associations between membership in a specific PHC cluster type and the independent variables using multinomial logistic regression with a significance level of 5%. Variables such as the high/very high vulnerability of population, rate of completed treatment, and rate of referrals of users to secondary care were independently associated with the clusters ( < 0.05). The performance of primary dental care services was associated with patient demographics, dental treatment need, and referrals. The results of this study have implications for the planning of public policies.

Introduction: Mechanical stress can induce molecular changes in gingival crevicular fluid (GCF) a... more Introduction: Mechanical stress can induce molecular changes in gingival crevicular fluid (GCF) and the peri-odontal ligament (PDL). It is still not clear whether changes in the PDL and GCF are linked. In this study, we aimed to analyze the expression of cytokines in GCF and PDL after mechanical stress. Methods: Twenty-three healthy patients were included. The experimental group consisted of premolars subjected to a force of 0.980 N for 1, 3, 7, 14, 21, or 28 days. The contralateral teeth were the controls. GCF and PDL samples were collected at the same time points for analysis of cytokines using the cytometric bead array. Results: Interleukin (IL)-6 (IL-6) production was significantly elevated in the PDL on day 1 after force application. Significantly strong positive correlations between GCF and PDL in experimental group were seen on days 3 (interferon-gamma), 7 (IL-10), 14 (IL-17A), and 28 (IL-17A, tumor necrosis factor-alpha), and significantly strong negative correlation were seen on days 14 (interferon-gamma) and 21 (IL-2, IL-10). Conclusions: Different patterns of IL-6 expression were seen in the PDL and GCF after mechanical stress. Despite occasional correlations between GCF and PDL, the molecular contributions of the PDL to the GCF changes could not be clearly defined by our model. Objectives: To evaluate adolescents oral health-related quality of life (OHRQoL) in the first 4 m... more Objectives: To evaluate adolescents oral health-related quality of life (OHRQoL) in the first 4 months of fixed orthodontic appliance treatment using parents and caregivers as proxies. Design: Descriptive study. Setting: Participants: A sample of parents and caregivers of 95 adolescents undergoing orthodontic treatment with a fixed appliance. Methods: Participants were required to answer the Brazilian version of the Parental-Caregivers Perceptions Questionnaire (P-CPQ) before adolescent's treatment (T1) and 4 months after bonding of the fixed appliance (T2). Statistical analysis was carried out using the Wilcoxon signed rank test and the Bonferroni correction for the domains of P-CPQ. Results: Among the 95 participants, there were 73 mothers, 18 fathers and 4 were other relations. There was a statistically significant improvement in the overall score as well as in both emotional and social wellbeing subscales (P,0.001). Conclusion: Parents and caregivers report an improvement on their adolescent's OHRQoL in the first 4 months of orthodontic treatment with a fixed appliance.

A questionnaire was administered to all dentists comprising the primary health care service to Belo Horizonte with tenured jobs and 40 work hours per week, totalling a population of 207 professionals. The response rate was 90.34%. A pilot test was conducted with 44 dentists working in primary care for at least two years and who did not participate in the main study. Descriptive statistical analysis involved calculating proportions. No confidence intervals were calculated because this was a census study. In most items (79.0%), professionals&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39; perceptions about the comprehensiveness were overwhelmingly positive. When we stratified the analysis by domain and checked those items about which dentists had a less favourable perception, 22.7% were in the patient welcoming domain, 25.0% were in the bonding domain and 12.5% were in quality of care. Comprehensiveness, as an approach in health care practice, needs to be enhanced, and there is evidence that these dentists are aware of its importance.

Comprehensiveness relates both to the scope of services offered and to a holistic clinical approa... more Comprehensiveness relates both to the scope of services offered and to a holistic clinical approach. The aim of this study was to identify the perception of Brazilian dentists regarding key concepts of comprehensiveness and its importance in primary health care oral health services performed in the public sector. Focus groups were conducted to explore three main concepts, 'patient welcoming', 'bonding' and 'quality of care', as well as their aspects. In total, there were four focus groups comprising seven general dentists, all of whom had at least two years of experience in primary care, from municipalities of varying sizes. Discussions of approximately 60 minutes were conducted, audio-recorded and transcribed. The data were analysed by qualitative thematic analysis, in line with the framework approach. The general dentists reported that they are aware that in the Brazilian Health System, the onset of patient care by primary health care services must happen through 'patient welcoming.' Nevertheless, they suggested that this action is often performed poorly due to the large volume of patients. Although they knew the importance of 'bonding', they realised that there is a lack of education preparing professionals to address interpersonal and social issues. They were aware that 'quality of care' is related to multiple factors. Comprehensiveness, as an approach in dental public health practices, needs to be enhanced, and there is evidence that primary care dentists are aware of the need for such an approach.

Objetivo. Investigar a presença de cárie e sua associação com variáveis socioeconômicas entre esc... more Objetivo. Investigar a presença de cárie e sua associação com variáveis socioeconômicas entre escolares do meio rural de Itaúna, Estado de Minas Gerais, em 2002. Método. Realizou-se um levantamento epidemiológico no qual 476 escolares entre 4 e 15 anos foram submetidos a um exame dentário. Todos os exames foram feitos pelo mesmo examinador, que utilizou a metodologia da Organização Mundial da Saúde. O levantamento das variáveis socioeconômicas (renda familiar mensal, anos de estudo formal do pai e da mãe, ocupação do pai e da mãe, origem da água consumida, sexo e idade da criança) foi feito a partir de entrevistas com os pais dos escolares. Resultados. Mais de três quartos dos pais recebiam até 360 reais (aproximadamente 153 dólares) por mês e relataram ter freqüentado a escola por, no máximo, 4 anos. Foram identificados como livres de cárie em ambas as dentições 17,86% dos escolares. O número médio de dentes permanentes cariados, perdidos e restaurados e de dentes decíduos cariados, com extração indicada e obturados foi de 0,94 (± 1,55) e 4,00 (± 3,46), respectivamente. A análise multivariada mostrou que as crianças com 7 e 8 anos cuja renda familiar mensal era superior a R$ 280 tinham 2,602 (IC95%: 1,004 a 6,745) e 2,854 (IC95%: 1,044 a 7,799) vezes mais chance de apresentar cárie, respectivamente, na dentição decídua e em ambas as dentições. A escolaridade da mãe superior a 3 anos aumentou em 2,813 (IC95%: 1,221 a 6,480) vezes a chance de o escolar com 7 e 8 anos apresentar cárie na dentição permanente. Conclusões. Tendo em vista os resultados -especialmente em relação à dentição decíduaé urgente a implementação de estratégias coletivas em saúde bucal direcionadas a uma faixa etária precoce. Os resultados sugerem que, em populações com privações econômicas, como é o caso do meio rural de Itaúna, um pouco mais de renda e educação podem significar maior acesso a hábitos que originam cárie. Feminization of the odontology course at the State University of Montes Claros, Minas Gerais Stat... more Feminization of the odontology course at the State University of Montes Claros, Minas Gerais State Resumo Este artigo fundamenta a tese de "feminização" da odontologia através da literatura especializada no tema e concentra na revisão bibliográfica a análise deste fenômeno. Para ilustrar, realizou-se um estudo de caso analisando o número de inscritos no processo seletivo para odontologia na Universidade Estadual de Montes Claros -Unimontes, Brasil, apresentando os selecionados e concluintes, por sexo. A análise refere-se aos anos de 1997 a 2006. O curso desde sua implantação tem apresentado maior porcentagem de mulheres, tanto com relação aos ingressantes e concluintes, como no processo seletivo. O referencial teórico analisado vai ao encontro dos dados empíricos desse estudo de caso, donde se conclui que o fenômeno da feminização também ocorre no curso de odontologia da Unimontes.

Inequalities in the distribution of dental caries in Brazil: a bioethical approach Resumo Este ar... more Inequalities in the distribution of dental caries in Brazil: a bioethical approach Resumo Este artigo objetivou analisar a problemática da cárie dentária, enquanto uma questão de saúde pública de relevância epidemiológica, tendo como suporte analítico a visão da Bioética da Proteção. Trata-se de estudo de caso realizado a partir de análise documental dos inquéritos epidemiológicos em saúde bucal, de base nacional, 1986, 2003 e 2010. Os levantamentos apontam desigualdades entre grupos populacionais. Essa desigualdade foi analisada, com relação à redução da doença na população mais jovem, no acesso ao atendimento, e na perda dentária, pior desfecho para a cárie dentária. Os bons resultados apresentados, nos levantamentos, não atinge toda a população, apresentando desigualdades por renda familiar e região brasileira. Constatou-se a partir de 2003 intervenções do Estado em ações protetoras, como adoção de critérios de equidade na destinação dos recursos na saúde e inclusão de populações em vulneração, contribuindo para redução da cárie. No entanto, considera-se necessário avançar mais nos modelos assistenciais em saúde bucal usando a Epidemiologia como uma ferramenta estruturante, já que as diferenças regionais ainda permaneceram marcantes, em interface com uma bioética protetora e socialmente responsável.

The integrality of the attention in diabetics with periodontal disease Resumo O objetivo deste es... more The integrality of the attention in diabetics with periodontal disease Resumo O objetivo deste estudo foi avaliar como está organizado o atendimento aos indivíduos com diabetes melito, usuários do SUS, a partir dos dados das condições periodontais apresentadas por este grupo, em Belo Horizonte (MG). Para tanto, uma amostra representativa de trezentos indivíduos foi selecionada para avaliação clínica e entrevista. Foram realizadas entrevistas também com os gerentes de unidades de saúde. Dos avaliados, 55% apresentaram gengivite, 35,3%, periodontite e 9,7% eram saudáveis. Em relação à integralidade da atenção ao diabético no SUS, pôde-se observar que, apesar da maioria estar sob acompanhamento médico, somente 27,3% estavam sob tratamento odontológico na rede básica, 3,6% recebiam atendimento especializado em odontologia e apenas 3,4% eram atendidos por outros profissionais da saúde. O atendimento interdisciplinar e a atenção em todos os níveis do sistema são fatores essenciais para a integralidade das ações em saúde. Palavras-chave Integralidade, Doença periodontal, Diabetes melito, Saúde bucal, Atenção integral à saúde Abstract The aim of this study was to evaluate the periodontal conditions of individuals with diabetes and to analyze how SUS has contributed to the health attention of these people in Belo Horizonte, Minas Gerais State. For that, a sample of 300 individuals was selected and interviews with diabetics and health unit managers were conducted. From those, 55% presented gingivitis, 35.3% periodontitis and 9.7% were healthy. As for the integral attention for diabetics in SUS, it was observed that despite the fact that most of them were under medical supervision, only 27.3% were under dental treatment in basic health care units, 3.6% had specialized dental care and only 3.4% were seen by other health workers. Interdisciplinary care and attention in all levels of the system are essential factors for the integrality of health actions.

Method: A cross-sectional study was carried out in 13 municipalities in the Jequitinhonha Valley, Southeast region, by a single examiner (intra-examiner Kappa=0.85). The Significant Caries Index (SiC) and the DMFT were used to measure the dental caries experience. Data were analyzed using SPSS, version 18.0. The chi-square test was used to detect differences in the sample composition, the Mann-Whitney test measured the difference between the average and the percentage of DMFT components and the Kruskal-Wallis test evaluated the variation in the prevalence of dental caries according to age and in SiC and nonSiC groups, with a significance level of 5%. Objective: To evaluate self-medication in a public emergency dental service in Belo Horizonte, Br... more Objective: To evaluate self-medication in a public emergency dental service in Belo Horizonte, Brazil, 2010 and factors associated to the maintenance of drug therapy initiated as self-medication after dental assistance. Method: A survey was performed including 174 patients who have been assisted by emergency dental service. Only a trained dentist applied a structured pre-tested questionnaire (kappa>0.80) to patients during 60 days in 2010. Anatomical Therapeutic Chemical (ATC) Classification System was used to classify the drugs used. Dependent variable was “maintenance of self-medication” and independent variables were gender, age, duration of pain in days, drug classification and clinical diagnosis. Clinical diagnosis was defined by only one dentist. Bivariate analysis was carried out in SPSS 19.0 software for Windows. The study was approved by the Human Research Ethics Committee. A questionnaire was administered to all dentists comprising the primary health care service to Belo Horizonte with tenured jobs and 40 work hours per week, totalling a population of 207 professionals. The response rate was 90.34%. A pilot test was conducted with 44 dentists working in primary care for at least two years and who did not participate in the main study. Descriptive statistical analysis involved calculating proportions. No confidence intervals were calculated because this was a census study. In most items (79.0%), professionals&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39; perceptions about the comprehensiveness were overwhelmingly positive. When we stratified the analysis by domain and checked those items about which dentists had a less favourable perception, 22.7% were in the patient welcoming domain, 25.0% were in the bonding domain and 12.5% were in quality of care. Comprehensiveness, as an approach in health care practice, needs to be enhanced, and there is evidence that these dentists are aware of its importance.

The aim of this study was to identify the perception of Brazilian dentists regarding key concepts of comprehensiveness and its importance in primary health care oral health services performed in the public sector. Focus groups were conducted to explore three main concepts, 'patient welcoming', 'bonding' and 'quality of care', as well as their aspects. In total, there were four focus groups comprising seven general dentists, all of whom had at least two years of experience in primary care, from municipalities of varying sizes. Discussions of approximately 60 minutes were conducted, audio-recorded and transcribed. The data were analysed by qualitative thematic analysis, in line with the framework approach. The general dentists reported that they are aware that in the Brazilian Health System, the onset of patient care by primary health care services must happen through 'patient welcoming.' Nevertheless, they suggested that this action is often performed poorly due to the large volume of patients. Although they knew the importance of 'bonding', they realised that there is a lack of education preparing professionals to address interpersonal and social issues. They were aware that 'quality of care' is related to multiple factors. Comprehensiveness, as an approach in dental public health practices, needs to be enhanced, and there is evidence that primary care dentists are aware of the need for such an approach.

Investigar a presença de cárie e sua associação com variáveis socioeconômicas entre escolares do meio rural de Itaúna, Estado de Minas Gerais, em 2002. Método. Realizou-se um levantamento epidemiológico no qual 476 escolares entre 4 e 15 anos foram submetidos a um exame dentário. Todos os exames foram feitos pelo mesmo examinador, que utilizou a metodologia da Organização Mundial da Saúde. O levantamento das variáveis socioeconômicas (renda familiar mensal, anos de estudo formal do pai e da mãe, ocupação do pai e da mãe, origem da água consumida, sexo e idade da criança) foi feito a partir de entrevistas com os pais dos escolares. Resultados. Mais de três quartos dos pais recebiam até 360 reais (aproximadamente 153 dólares) por mês e relataram ter freqüentado a escola por, no máximo, 4 anos. Foram identificados como livres de cárie em ambas as dentições 17,86% dos escolares. O número médio de dentes permanentes cariados, perdidos e restaurados e de dentes decíduos cariados, com extração indicada e obturados foi de 0,94 (± 1,55) e 4,00 (± 3,46), respectivamente. A análise multivariada mostrou que as crianças com 7 e 8 anos cuja renda familiar mensal era superior a R$ 280 tinham 2,602 (IC95%: 1,004 a 6,745) e 2,854 (IC95%: 1,044 a 7,799) vezes mais chance de apresentar cárie, respectivamente, na dentição decídua e em ambas as dentições. A escolaridade da mãe superior a 3 anos aumentou em 2,813 (IC95%: 1,221 a 6,480) vezes a chance de o escolar com 7 e 8 anos apresentar cárie na dentição permanente. Conclusões. Tendo em vista os resultados -especialmente em relação à dentição decíduaé urgente a implementação de estratégias coletivas em saúde bucal direcionadas a uma faixa etária precoce. Os resultados sugerem que, em populações com privações econômicas, como é o caso do meio rural de Itaúna, um pouco mais de renda e educação podem significar maior acesso a hábitos que originam cárie. Feminization of the odontology course at the State University of Montes Claros, Minas Gerais Stat... more Feminization of the odontology course at the State University of Montes Claros, Minas Gerais State Resumo Este artigo fundamenta a tese de "feminização" da odontologia através da literatura especializada no tema e concentra na revisão bibliográfica a análise deste fenômeno. Para ilustrar, realizou-se um estudo de caso analisando o número de inscritos no processo seletivo para odontologia na Universidade Estadual de Montes Claros -Unimontes, Brasil, apresentando os selecionados e concluintes, por sexo. A análise refere-se aos anos de 1997 a 2006. O curso desde sua implantação tem apresentado maior porcentagem de mulheres, tanto com relação aos ingressantes e concluintes, como no processo seletivo. O referencial teórico analisado vai ao encontro dos dados empíricos desse estudo de caso, donde se conclui que o fenômeno da feminização também ocorre no curso de odontologia da Unimontes.

Inequalities in the distribution of dental caries in Brazil: a bioethical approach Resumo Este ar... more Inequalities in the distribution of dental caries in Brazil: a bioethical approach Resumo Este artigo objetivou analisar a problemática da cárie dentária, enquanto uma questão de saúde pública de relevância epidemiológica, tendo como suporte analítico a visão da Bioética da Proteção. Trata-se de estudo de caso realizado a partir de análise documental dos inquéritos epidemiológicos em saúde bucal, de base nacional, 1986, 2003 e 2010. Os levantamentos apontam desigualdades entre grupos populacionais. Essa desigualdade foi analisada, com relação à redução da doença na população mais jovem, no acesso ao atendimento, e na perda dentária, pior desfecho para a cárie dentária. Os bons resultados apresentados, nos levantamentos, não atinge toda a população, apresentando desigualdades por renda familiar e região brasileira. Constatou-se a partir de 2003 intervenções do Estado em ações protetoras, como adoção de critérios de equidade na destinação dos recursos na saúde e inclusão de populações em vulneração, contribuindo para redução da cárie. No entanto, considera-se necessário avançar mais nos modelos assistenciais em saúde bucal usando a Epidemiologia como uma ferramenta estruturante, já que as diferenças regionais ainda permaneceram marcantes, em interface com uma bioética protetora e socialmente responsável.

The integrality of the attention in diabetics with periodontal disease Resumo O objetivo deste es... more The integrality of the attention in diabetics with periodontal disease Resumo O objetivo deste estudo foi avaliar como está organizado o atendimento aos indivíduos com diabetes melito, usuários do SUS, a partir dos dados das condições periodontais apresentadas por este grupo, em Belo Horizonte (MG). Para tanto, uma amostra representativa de trezentos indivíduos foi selecionada para avaliação clínica e entrevista. Foram realizadas entrevistas também com os gerentes de unidades de saúde. Dos avaliados, 55% apresentaram gengivite, 35,3%, periodontite e 9,7% eram saudáveis. Em relação à integralidade da atenção ao diabético no SUS, pôde-se observar que, apesar da maioria estar sob acompanhamento médico, somente 27,3% estavam sob tratamento odontológico na rede básica, 3,6% recebiam atendimento especializado em odontologia e apenas 3,4% eram atendidos por outros profissionais da saúde. O atendimento interdisciplinar e a atenção em todos os níveis do sistema são fatores essenciais para a integralidade das ações em saúde. Palavras-chave Integralidade, Doença periodontal, Diabetes melito, Saúde bucal, Atenção integral à saúde Abstract The aim of this study was to evaluate the periodontal conditions of individuals with diabetes and to analyze how SUS has contributed to the health attention of these people in Belo Horizonte, Minas Gerais State. For that, a sample of 300 individuals was selected and interviews with diabetics and health unit managers were conducted. From those, 55% presented gingivitis, 35.3% periodontitis and 9.7% were healthy. As for the integral attention for diabetics in SUS, it was observed that despite the fact that most of them were under medical supervision, only 27.3% were under dental treatment in basic health care units, 3.6% had specialized dental care and only 3.4% were seen by other health workers. Interdisciplinary care and attention in all levels of the system are essential factors for the integrality of health actions.

Method: A cross-sectional study was carried out in 13 municipalities in the Jequitinhonha Valley, Southeast region, by a single examiner (intra-examiner Kappa=0.85). The Significant Caries Index (SiC) and the DMFT were used to measure the dental caries experience. Data were analyzed using SPSS, version 18.0. The chi-square test was used to detect differences in the sample composition, the Mann-Whitney test measured the difference between the average and the percentage of DMFT components and the Kruskal-Wallis test evaluated the variation in the prevalence of dental caries according to age and in SiC and nonSiC groups, with a significance level of 5%. Objective: To evaluate self-medication in a public emergency dental service in Belo Horizonte, Br... more Objective: To evaluate self-medication in a public emergency dental service in Belo Horizonte, Brazil, 2010 and factors associated to the maintenance of drug therapy initiated as self-medication after dental assistance. Method: A survey was performed including 174 patients who have been assisted by emergency dental service. Only a trained dentist applied a structured pre-tested questionnaire (kappa>0.80) to patients during 60 days in 2010. Anatomical Therapeutic Chemical (ATC) Classification System was used to classify the drugs used. Dependent variable was “maintenance of self-medication” and independent variables were gender, age, duration of pain in days, drug classification and clinical diagnosis. Clinical diagnosis was defined by only one dentist. Bivariate analysis was carried out in SPSS 19.0 software for Windows. The study was approved by the Human Research Ethics Committee.
