Muscle fatigue is a temporary and transient muscle incapacity to develop muscle power, and inabil... more Muscle fatigue is a temporary and transient muscle incapacity to develop muscle power, and inability of muscle to perform work or maintain a certain level of physical activity. Several mechanisms are proposed as physiological base of the muscle fatigue: structural and functional changes that occurred in the sarcomera, reduced sarcoplasmic pH value, inhibition of the cross bridges cycle, blocking the process of excitation and contraction coupling, and depletion of energy reserves. Common consecutive process of muscle fatigue is muscle soreness. This phenomenon can lead to a disturbance of the integrity of muscle cells or temporary disruption of muscle function. Acute muscle soreness causes insignificant inconvenience in the athlete because it is short-term and quickly retreat. The delayed onset of muscle soreness (DOMS) often affect sport's performance by limiting the range of motion, decreasing the muscle force and alternating the movement pattern. Recommendations for the prevention of muscle soreness include stretching exercises, gradually increasing of the training intensity and consumption of anti-inflammatory supplements. The scientific and empiric knowledge of the etiology and mechanisms of muscle fatigue and muscle soreness will allow better understanding how to alleviate and attenuate these adverse phenomena in athletes.

One hundred patients, who had committed a first suicidal attempt during the years 2004 and 2005, ... more One hundred patients, who had committed a first suicidal attempt during the years 2004 and 2005, were submitted to a psychiatric interview in the Center for preventing suicide, on the second day of their hospitalization in the Clinic of Urgent Medicine and Toxicology. The levels of depression and anxiety were assessed by the Hamilton scale for depression (HAMD) and Hamilton scales for anxiety (HAMA). The patients were divided in two groups: age 15-25 (n=70) and age older than 25 (n=30). Age-matched healthy volunteers (n=26) were also studied. The results suggest that all patients with a first suicidal attempt showed signs of depression according to HAMD, and a high level of anxiety, whereas the younger patients showed mean scores for minor depression. The older patients showed signs of major depression, within a depressive episode, which required medical treatment. All younger patients were submitted to psychiatric counselling for one month after the suicidal attempt. The healthy patients showed no signs for depression and anxiety. Language: en

Estrogen is an important steroid hormone signal that regulates multiple tissues and functions in ... more Estrogen is an important steroid hormone signal that regulates multiple tissues and functions in the body. The brain is an important target organ for estrogen. In addition to direct effects, estrogen influences brain function through effects on the vasculature and the immune system. Estrogen influences several neurotransmitter systems, including acetylcholine, serotonin, noradrenalin, and glutamate. There are complex mechanisms that underlie estrogen neuroprotective and neurotrophic actions (genomic, non-genomic kinase signalic pathways and membrane estrogen receptors pathways). Estradiol induces gene transcription and rapid membrane signaling mediated by estrogen receptor alpha (ERα), estrogen receptor-beta (ERβ), and a recently characterized G-protein coupled estrogen receptor, each with distinct distributions and ability to influence estradiol-dependent signaling. Vector-mediated expression of estrogen receptors in the hippocampus provides an innovative research approach and suggests that memory depends on the relative expression of ERα and ERβ interacting with estradiol levels. The potential role of estrogen as a neuroprotective factor and a multicellular mode of estrogen action in the regulation of neuronal survival and neurotrophism is discussed, as are potential future directions for the field.

Purpose: Although arterio-venous malformation (AVM) is an embryogenesis disorder, it shows tenden... more Purpose: Although arterio-venous malformation (AVM) is an embryogenesis disorder, it shows tendency for postnatal enlargement, which implies alternate angiogenesis mechanisms, such as reorganization of minor haemorrhages, ischemic lesions, hormonal factors and traumatic aetiology. The paper presents a case of a patient with enlarging AVM shown by neuroradiologic imaging and extending of clinical features. Method: The evolution of the disease is presented by history data, computerized tomography (CT), CT-angiography, neuropsychological and psychiatric findings. Results: 45-year female patient reports frequent headaches, partial seizures and speech difficulties. A head injury in early childhood was reported. She had learning and memorizing difficulties, but completed high school and found a job. Since then she notices speech problems. At the age of 35 she reports frequent numbness and weakness of her right arm and slurred speech and got retired. At the same time arteriography showed AVM. Symptoms aggravated and new angiography ten years later shows enlarged (>6 cm) temporooccipital AVM, supplied by the a. cerebri media and posterior branches and drained into the sinuses. After introducing antiepileptic drugs, no sudden speech interruptions were observed. AVM embolisation was scheduled by a neurosurgeon. Conclusion: The presented case features and untypical evolution and a need for frequent neuroradiological control.

The loss of female sex hormones after menopause contributes to the striking increase in the incid... more The loss of female sex hormones after menopause contributes to the striking increase in the incidence of cardiovascular morbidity and mortality. The present study was designed to evaluate some of the artherosclerotic risk factors: LDL-C, HDL-C and factor VII of coagulation, in women during menopause. The study comprised a number of 77 women divided into three groups. The control group included 25 healthy women in their reproductive period. The perimenopausal group consisted of 27 women, with FSH level under 25mU/ml, and with anamnestic data of irregularity of menstrual cycle. The postmenopausal group encompassed 25 women regarding lack of cycle for more than 12 months. Hormone level was determined with RIA method. Lipid level was determined with standard colorimetric-spectrophotometric method and the concentration of f. VII was determined by method of plasma deficiency. Statistical analysis has shown that there was a significant increase of LDL-C and factor VII in both perimenopausal and postmenopausal examines in comparison with the control group (p<0,001). This study favours the view that decrease in estradiol level and associated increase in LDL-CH and factor VII seen in perimenopausal and postmenopausal women may be responsible for the increased risk of atherosclerotic thromboemembolic complications in women during the most vulnerable period of their life.

T he aim of our study was to provide basic epidemiological data on suicide attempts that resulted... more T he aim of our study was to provide basic epidemiological data on suicide attempts that resulted in admission to the University Clinic of Toxicology and Emergency Medicine in Skopje during a 10-yearperiod (1999-2008). There is a lack of published information regarding suicide attempts in the Republic of Macedonia. Methods: Participants were 1683 patients from the territory of Skopje, who attempted suicides and who were hospitalized in the University Clinic of Toxicology and Emergency Medicine in School of Medicine, Skopje during the period of ten years (1999-2008). The following variables were included in the analysis: age, gender, religion, method of suicide attempt and admission date. The results were analyzed with the Statistic for Windows program, release 7.0. Results: A significantly higher number of suicide attempts were registered in females than in males from the territory of Skopje during the period of 1999-2008. Men who attempted suicide were older than women. Women of Christian religion affiliation attempted suicide more frequently than women of Muslim religion. The greatest number of attempts was during the summer season. The most common method of suicide attempt was intoxication with medications. Conclusions: Our study has shown that attempted suicide rate has had a stable trend over the last decade. It has also shown female predominance of suicide attempts with a greater number of attempts during the summer months. There is a need for intervening strategies to be targeted at younger females.

Introduction: Concern for mental health consequences in general population worldwide has been app... more Introduction: Concern for mental health consequences in general population worldwide has been apparent since the start of the actual pandemic. Aim of the study was to assess the impact of COVID-19 related stressors on mental health in junior medical students during December 2020 COVID-19 related lockdown. Material and methods: The investigation included 275 second-year medical students aged 19-28 years, from the Faculty of Medicine, Ss. Cyril and Methodius University in Skopje during December 2020. Self-administered questionnaire containing standardized instruments for assessment of parameters of psychological distress (BAI, BDI and PSS) were used. COVID-19 related attitudes questionnaire for assessment of status of COVID-19 testing and infection, self-perceived levels of concerns related to academics [COVID-19-AA (academic apprehensions)] and those related to the self and family/friends [COVID-19-GA (general apprehensions)] were included. Results: The prevalence of anxiety, depression and stress was 74.9%, 43.3% and 78.9%, respectively. Almost 29% of students reported high anxiety, 6.2% had severe depression and 21.45% reported high levels of perceived stress. There was a significant positive correlation between COVID-19 GA score and BAI, BDI и PSS scores (p<0.05). Correlations between the COVID-19 AA scores and scores on BAI, BDI and PSS scales were also statistically significant (p=0.0000). Academic apprehension was found a predictive factor for high anxiety, depression and high stress (R 2 = .12, F (1,273) = 38.79, p < .001. β = 1.12, p < .001). Conclusion: General and academic apprehension related to COVID-19 have negative effects on mental wellbeing in second year medical students.
