Introduction: The properties of colostrum were recognized and investigated more thoroughly in the... more Introduction: The properties of colostrum were recognized and investigated more thoroughly in the first half of the eighties, when the immune factors and growth factors it contains were pointed out. Numerous studies show that the administration of colostrum benefits the subjects with type 2 diabetes mellitus as it gradually regulates appetite, improves utilization of nutrients, especially glucose, and leads to a significant decrease in body fat. Materials and methods: The following study is aimed at verifying a possible reduction in the use of insulin in 27 subjects with type 2 diabetes, treated with goat colostrum in the form gastro-resistant tablets of 300 mg (4/die). Results: In subjects with type 2 diabetes treated with insulin, the administration of colostrum has obtained a significant reduction of insulin dosage and normalization of blood glucose levels. Conclusions: The effects of colostrum are presumably linked to increased levels of IGF-1 that improves the utilization of glucose, stimulates glycogen and protein synthesis.

Objective: The concomitance of psoriasis and high IgE levels in the same patient, possibly associ... more Objective: The concomitance of psoriasis and high IgE levels in the same patient, possibly associated to allergies, should not come as a surprise anymore, because both diseases are characterised by an immunological disorder, involving cytokines and other inflammatory mediators, with massive activation of the cell-mediated immunity. Materials and methods: During a period of 18 months, using a radio-immunologic method, we assessed the level of IgE in patients with severe psoriasis who were not responding to the common therapy. Results: Patients with severe psoriasis had high levels of IgE. Conclusions: IgEs have an important role in pathogenesis of psoriasis and atopic dermatitis, but the mechanism is not yet clear.

Background Egg allergy is the second most prevalent form of food allergy in childhood. In spite o... more Background Egg allergy is the second most prevalent form of food allergy in childhood. In spite of the evidence accumulated, inoculating egg allergy children with attenuated vaccines grown on chick embryo cell cultures, such as the measles, mumps, and rubella (MMR) vaccine, is regarded (erroneously) as potentially dangerous or even anaphylactogenic, by many. An issue perceived as particularly con icting also by Health Professionals. Case presentation A 15-year-old boy, with a history of severe egg allergy in early infancy, who was still sensitized to egg allergens, including baked egg, had never received MMR vaccination, in fear of possible anaphylaxis, in spite of the fact that this vaccination is mandatory in the rst year of life, in Italy. Because of that, he was not allowed to attend school, longer, and was referred to us in order to assess the potential risk of MMR vaccination. Upon thorough allergological workup, sensitization to MMR vaccine components was excluded by an in vivo approach, consisting in skin prick tests, intradermal tests, and subcutaneous injection test, corroborated by vaccine-speci c B-lymphocyte proliferation assay, ex vivo. Tcell proliferation in response to MMR vaccine was also excluded. Eventually, the boy was inoculated with MMR vaccine and was readmitted to school. Conclusions The diagnostic strategy adopted appears feasible and easy-to-perform and may be adopted in controversial cases (as the one reported), characterized by previous severe allergic reactions to egg. The B-lymphocyte proliferation assay we developed may represent a useful and reliable tool not only in research but also in clinical practice.

An association between autoimmune disease and malignant melanoma (MM) has often been reported in ... more An association between autoimmune disease and malignant melanoma (MM) has often been reported in the literature as a positive prognostic factor for MM. Consequently, we evaluated the influence of different autoimmune diseases on the prognosis of MM. To evaluate the prognosis of patients with MM who also had an autoimmune disorder, whether tumour-associated, paraneoplastic or drug-induced. Autoimmune diseases were classified and analysed as tumour-associated, paraneoplastic or drug-induced. Patients were enrolled according to their clinicopathological features and matched with control groups. Kaplan-Meier analysis was used to estimate disease-free survival (DFS) and overall survival (OS), and log-rank test was used to evaluate differences between the survival curves. In total, 49 patients with MM and tumour-associated autoimmune disease were included in our analysis. No case of paraneoplastic autoimmune disease was detected. Chronic Urticaria is a difficult to define condition from the nosographic standpoint, with comple... more Chronic Urticaria is a difficult to define condition from the nosographic standpoint, with complex pharmacological management, that heavily impacts the life of the patient. Some forms show not to be responsive to anti H1 anti-histaminic and require other treatments. One of these can be the treatment with Cyclosporine A (CsA). This study, open and sequential, reports the results of short-term treatment over a sample of adults (21 patients) of both sexes, all suffering from chronic urticaria with IgE levels higher than 200 mU/ml treated with 4 mg/kg/die of CsA. The results obtained show a reduction in the levels of total IgE and a significant improvement in symptoms; there were no adverse effects. Cyclosporine is an excellent treatment for chronic urticaria because it reduces the activity of T lymphocytes and reduction of the histamine release from the mast cells and basophils.

Background: Oxaliplatin is a platinum derivative commonly used in patients with advanced colorect... more Background: Oxaliplatin is a platinum derivative commonly used in patients with advanced colorectal cancer. Hypersensitivity reactions to platinum-derived drugs represent a rare acute toxicity. It has been previously described a particular form of drug hypersensitivity that selectively affects the cardiac tissue, called allergic angina or Kounis syndrome. The mast cell degranulation is the main pathophysiological cause of this syndrome, and is mainly triggered by antibiotics and hymenoptera venom in sensitized individuals. Here we further identified oxalin-specific B cells that proliferate in response to oxaliplatin stimulation. Case Description: We describe herein a case of a 52-year-old woman admitted to the Emergency room due to chest pain upon oxaliplatin administration (4 th cycle). By mean of electrocardiography, troponin level evaluation, transthoracic echocardiography and coronary angiography, we diagnosed the patient with Kounis syndrome. Moreover, using a flow cytometry-based proliferation assay in vitro, we confirmed the presence of oxaliplatin-specific proliferating B cells. The patient was hospitalized in coronary care unit (CCU), and experienced remission of cardiac symptoms. Conclusions: In an Emergency Medicine setting, the right diagnosis and the adequate treatment of this syndrome still represent a challenge to avoid post-allergic myocardial effects including ischemia, conduction defects, arrhythmias, and dysfunction of the cardiac muscle cells.

Background This case is the first report describing rapid, successful treatment of severe atopic ... more Background This case is the first report describing rapid, successful treatment of severe atopic dermatitis (AD) and comorbid type-2 inflammatory diseases in the same patient, with dupilumab treatment, with no side-effects. Case presentation We report on effects of dupilumab in a patient with severe AD, a long-standing history of a mild, perennial allergic rhino-conjunctivitis, moderate asthma and chronic rhinosinusitis with nasal polyps (CRSwNP). Conclusions Patients suffering from AD, asthma, allergic rhinitis and CRSwNP may be eligible for dupilumab single treatment that is possibly advantageous also from the pharmaco-economic standpoint.

Quantification of this capacity is of major importance in diagnostic and research tools. While phenotypic characterization of CTLs is frequent and easily performed, their function is indeed more difficult to assess. CTLs are responsible for the lysis of cells expressing foreign or modified antigen peptides on their MHC class I molecules. Here we describe the detailed protocol used for the in vitro specific lysis of target cells.

mAb therapy for experimental metastatic melanoma relies on activating receptors for the Fc portio... more mAb therapy for experimental metastatic melanoma relies on activating receptors for the Fc portion of IgG (FcgR). Opposing results on the respective contribution of mouse FcgRI, FcgRIII, and FcgRIV have been reported using the gp75-expressing B16 melanoma and the protective anti-gp75 mAb TA99. We analyzed the contribution of FcgRs to this therapy model using bioluminescent measurement of lung metastases loads, novel mouse strains, and anti-FcgR blocking mAbs. We found that the TA99 mAb-mediated effects in a combination therapy using cyclophosphamide relied on activating FcgRs. The combination therapy, however , was not more efficient than mAb therapy alone. We demonstrate that FcgRI and, unexpectedly, FcgRIII contributed to TA99 mAb therapeutic effects, whereas FcgRIV did not. Therefore, FcgRIII and FcgRI are, together, responsible for anti-gp75 mAb therapy of B16 lung metastases. Opposing results on the respective contribution of mouse FcgRI, FcgRIII, and FcgRIV have been reported using the gp75-expressing B16 melanoma and the protective anti-gp75 mAb TA99. We analyzed the contribution of FcgRs to this therapy model using bioluminescent measurement of lung metastases loads, novel mouse strains, and anti-FcgR blocking mAbs. We found that the TA99 mAb-mediated effects in a combination therapy using cyclophosphamide relied on activating FcgRs. The combination therapy, however , was not more efficient than mAb therapy alone. We demonstrate that FcgRI and, unexpectedly, FcgRIII contributed to TA99 mAb therapeutic effects, whereas FcgRIV did not. Therefore, FcgRIII and FcgRI are, together, responsible for anti-gp75 mAb therapy of B16 lung metastases.
