Endemska (balkanska) nefropatija (EN) je kronicna tubulointersticijska nefropatija udružena s kar... more Endemska (balkanska) nefropatija (EN) je kronicna tubulointersticijska nefropatija udružena s karcinomima gornjeg dijela urotela uzrokovana aristolohicnom kiselinom te se danas smatra okolisnim oblikom u svijetu prisutne nefropatije aristolohicne kiseline. Bolest je zabilježena u ruralnim podrucjima uz pritoke velikih rijeka u Bosni i Hercegovini, Bugarskoj, Hrvatskoj, Rumunjskoj i Srbiji. Prevalencija EN se krece između 0, 4-12, 3% uz nedavne izvjestaje koji isticu smanjivanje prevalencije i incidencije bolesti u pojedinim žaristima. S tehnoloskim napretkom u obradi žita tijekom posljednjih desetljeca izloženost toksinu je eliminirana ili svedena na minimum te je za ocekivati da ce incidencija bolesnika s EN u nadomjesnom lijecenju terminalnog zatajenja bubrežne funkcije biti u opadanju. Recentno usvojenim i publiciranim konsenzusom po prvi puta su prihvaceni uniformirani kriteriji u svim zemljama s EN pomocu kojih je moguce kriticki revidirati bolesnike. Sljedeca kljucna tocka u ispitivanju promjene prevalencije, incidencije te karakteristika bolesnika s EN je stvaranje međunarodnog registra bolesnika s EN, te je Referalni centar za glomerulopatije i tubulointersticijske bolesti u suradnji s Hrvatskim drustvom za nefrologiju, dijalizu i transplantaciju i strucnjacima koji vode registar Europskog drustva za nefrologiju, dijalizu i transplantaciju (ERA EDTA) pokrenulo inicijativu. U sklopu toga dobiven je ERA-EDTA projekt Endemic (Balkan) Nephropathy int he ERA-EDTA Registry (EN-REG) u koji su ukljuceni autori ovog sažetka…

Tubular proteinuria and enzymuria are hallmarks of endemic nephropathy (EN). The role of I/D angi... more Tubular proteinuria and enzymuria are hallmarks of endemic nephropathy (EN). The role of I/D angiotensin convertase (ACE) gene polymorphism has not yet been elucidated in this peculiar chronic tubulointerstitial nephritis, and our aim was to investigate the role of this polymorphism in EN focusing on the urinary N-acetyl-β-D-glucosaminidase (NAG) excretion, a biomarker of proximal tubular damage. ACE genotype and allele frequencies were determined in 229 farmers (147 women and 82 men) from an endemic Croatian village. The farmers were stratified according to the WHO criteria into the following subgroups: those 'at risk' for EN (n = 37), 'suspected of having EN' (n = 57), and 'others' (n = 135). There were 74 (32.3%) subjects homozygous for the D allele, 99 (43.2%) heterozygous (ID genotype) and 56 (24.4%) homozygous for the I allele. No differences in allele frequency were found between the established WHO subgroups (p > 0.05). In the whole group, DD subjects had significantly higher values of diastolic blood pressure (p = 0.003) and urinary NAG than subjects with ID and II genotype (5.5 ± 1.2 vs. 4.0 ± 3.0 vs. 3.8 ± 4.2, respectively; p = 0.023). The highest values of serum creatinine (p = 0.02), proteinuria (p = 0.03) and urinary NAG (6.0 ± 3.7 vs. 3.7 ± 2.1 vs. 3.0 ± 1.6, respectively; p = 0.008) were observed in those suspected of having EN group with the DD genotype. ACE gene polymorphism is not a risk factor for EN. However, it might influence the clinical course of EN, and increased excretion of NAG might be a prognostic marker of this chronic tubulointerstitial nephritis.

Objective: Smartphone-based applications have been considered as a great opportunity to increase ... more Objective: Smartphone-based applications have been considered as a great opportunity to increase adherence and improve control of hypertension. However, several important items should be elucidated before applying this armature in real life. One of first tasks is to identify target population which would most likely use these applications. This was the aim of our study which was conducted in a group of individuals (aged 18 years or older) participating in the 2017 World Hypertension Day in Croatia. Design and method: This study was organized by the Croatian Society of Hypertension on 2017 World Hypertension day at 26 sites in 5 cities in Croatia (hospital open points, central squares and pharmacies). Along with BP measurement, a short questionnaire on hypertension awareness/salt intake/smartphone use was completed at the time of the interview by the physicians, trained nurses, pharmacist and medical students. Results: A total of 2175 subjects, 873 (40.1%) men, 1211 (59.9%) women were examined. Smartphones were used by 36.4% examined subjects (no gender difference), average age was 53.9 (15.6), and in the group of subjects who are using smartphone men were older (56.8(15.2) vs. 52.0(15.6); p < 0.001). We analyzed utilization of smartphone in several age categories and significant difference was found among age groups: under 35 years, 35–55 years, 55–70 years and above 70 years (87.5%, 70.4%, 33.6% and 13.3%, respectively). In the whole group 32.1% of individuals answered positive on the question of whether they would use mobile application for blood pressure control (no gender difference). Average age of those who answered positive was 55.7 (15.2) years (m vs. 58.1(15.2) vs. 54.2(15.1), p = 0.013). Significant difference in positive answer was obtained among age groups: under 35 years, 35–55 years, 55–75 years and above 70 years (58.7%, 57.7%, 32.4%, 13.3%, respectively, p < 0.001). Conclusions: In this large cohort we found that one-third of subjects having smartphone would use mobile applications for hypertension. Interestingly, all subjects older than 55 years and 58 % of subjects younger than 55 years who own a smartphone would like to use mobile applications for BP control.

Accurate assessment of glomerular fi ltration rate (GFR) is important in ascertaining health stat... more Accurate assessment of glomerular fi ltration rate (GFR) is important in ascertaining health status, especially in older individuals and in populations at risk of chronic kidney disease. Recently we observed that smokers with renal artery stenosis and renovascular hypertension had lower GFR when estimated by cystatin-C, than with creatinine-based estimates. We sought to resolve this issue. Design and method: Data from the Diabetes Control and Complications Trial (DCCT) were used to estimate GFR in individuals with diabetes. Iohexol clearance was used as the standard, and was compared to GFR estimates using creatinine and cystatin-C. Creatinine and cystatin-C estimates were calculated with the 24-hour urine creatinine clearance and the CKD-EPI cystatin-C. formula, and compared to iohexol based estimates using the Bland-Altman method. Results: There were 441 patients with iohexol, creatinine and cystatin-C values. The average age was 27.6 ± 6.6 years. The mean iohexol GFR for the population was 127.5 ± 19.2 ml/min. The cystatin-C and creatinine-based estimates were close to the iohexol estimates in non-smokers (differing by-7.7 ± 24.7 and-2.7 ± 31.1 ml/min respectively, p = ns). However in smokers, the cystatin-C estimates were considerably lower than iohexol-based GFR estimates (difference =-21.4 ± 25.1 ml/min p < 0.001), while creatinine based estimates were not (difference =-4.6 ± 21.6 ml/min, p = 0.58). Conclusions: Cystatin-C based estimates of GFR are accurate in non-smoking diabetics and yet substantially and signifi cantly underestimate GFR in diabetics that smoke cigarettes.

A wide heterogeneity was evident, with 53 different nationalities, subdivided in Eastern Europe (38.2%), Northern Africa (17.6%), Center and Southern Africa (12.9%), Latin America (12.8%), Indian subcontinent (9.6%), Far East (5.5%), Middle East (3.4%) macro-areas. Gender distribution and body mass index were comparable in the Italian and in the Migrant groups. Despite a 10-year age difference (50 ± 12 vs. 41 ± 15 years; p < 0.001), the overall prevalence of hypertension was similar in the two groups (44.7% in Italians vs. 43.4% in Migrants), as defi ned by BP>140/90 mmHg and/ or current antyhypertensive treatment. When stratifi ed by age, Migrants presented signifi cantly higher BP values, the prevalence of hypertension being at least 10% higher than in Italian residents in any decade group. A similar trend was observed for awareness, active treatment and satisfactory BP control rates. Also the rate of proteinuria and glycosuria was higher in the spot urine sample analysis. Conclusions: In Migrants, hypertension prevalence, treatment rate and control rate are signifi cantly higher than in the Italian resident population. In the future years this will inevitably increase the burden of cardiovascular disease on society and health system. These data underscore the urgent need of prevention and intervention in this special population, trying to take into proper account all the involved social, cultural, economic and health-related factors.

In the whole group 32.1% of individuals answered positive on the question of whether they would use mobile application for blood pressure control (no gender difference). Average age of those who answered positive was 55.7 (15.2) years (m vs. 58.1(15.2) vs. 54.2(15.1), p = 0.013). Significant difference in positive answer was obtained among age groups: under 35 years, 35–55 years, 55–75 years and above 70 years (58.7%, 57.7%, 32.4%, 13.3%, respectively, p < 0.001). Conclusions: In this large cohort we found that one-third of subjects having smartphone would use mobile applications for hypertension. Interestingly, all subjects older than 55 years and 58 % of subjects younger than 55 years who own a smartphone would like to use mobile applications for BP control.

Accurate assessment of glomerular fi ltration rate (GFR) is important in ascertaining health stat... more Accurate assessment of glomerular fi ltration rate (GFR) is important in ascertaining health status, especially in older individuals and in populations at risk of chronic kidney disease. Recently we observed that smokers with renal artery stenosis and renovascular hypertension had lower GFR when estimated by cystatin-C, than with creatinine-based estimates. We sought to resolve this issue. Design and method: Data from the Diabetes Control and Complications Trial (DCCT) were used to estimate GFR in individuals with diabetes. Iohexol clearance was used as the standard, and was compared to GFR estimates using creatinine and cystatin-C. Creatinine and cystatin-C estimates were calculated with the 24-hour urine creatinine clearance and the CKD-EPI cystatin-C. formula, and compared to iohexol based estimates using the Bland-Altman method. Results: There were 441 patients with iohexol, creatinine and cystatin-C values. The average age was 27.6 ± 6.6 years. The mean iohexol GFR for the population was 127.5 ± 19.2 ml/min. The cystatin-C and creatinine-based estimates were close to the iohexol estimates in non-smokers (differing by-7.7 ± 24.7 and-2.7 ± 31.1 ml/min respectively, p = ns). However in smokers, the cystatin-C estimates were considerably lower than iohexol-based GFR estimates (difference =-21.4 ± 25.1 ml/min p < 0.001), while creatinine based estimates were not (difference =-4.6 ± 21.6 ml/min, p = 0.58). Conclusions: Cystatin-C based estimates of GFR are accurate in non-smoking diabetics and yet substantially and signifi cantly underestimate GFR in diabetics that smoke cigarettes.

A wide heterogeneity was evident, with 53 different nationalities, subdivided in Eastern Europe (38.2%), Northern Africa (17.6%), Center and Southern Africa (12.9%), Latin America (12.8%), Indian subcontinent (9.6%), Far East (5.5%), Middle East (3.4%) macro-areas. Gender distribution and body mass index were comparable in the Italian and in the Migrant groups. Despite a 10-year age difference (50 ± 12 vs. 41 ± 15 years; p < 0.001), the overall prevalence of hypertension was similar in the two groups (44.7% in Italians vs. 43.4% in Migrants), as defi ned by BP>140/90 mmHg and/ or current antyhypertensive treatment. When stratifi ed by age, Migrants presented signifi cantly higher BP values, the prevalence of hypertension being at least 10% higher than in Italian residents in any decade group. A similar trend was observed for awareness, active treatment and satisfactory BP control rates. Also the rate of proteinuria and glycosuria was higher in the spot urine sample analysis. Conclusions: In Migrants, hypertension prevalence, treatment rate and control rate are signifi cantly higher than in the Italian resident population. In the future years this will inevitably increase the burden of cardiovascular disease on society and health system. These data underscore the urgent need of prevention and intervention in this special population, trying to take into proper account all the involved social, cultural, economic and health-related factors.
