We report a case of a 17-year-old girl with a history of congenital human immunodeficiency virus ... more We report a case of a 17-year-old girl with a history of congenital human immunodeficiency virus (HIV) infection and lipodystrophy secondary to highly active antiretroviral therapy (HAART). She developed severe worsening of preexisting hypertriglyceridemia after treatment with oral contraceptive pills (OCP) for polycystic ovary syndrome. Her hypertriglyceridemia improved upon OCP discontinuation. Although it is known that estrogen combined with progestins have a negative effect on triglycerides and high-density lipoprotein (HDL) cholesterol levels, to our knowledge the association of HAART-related lipodystrophy and severe hypertriglyceridemia after OCP use has not been reported in the literature. We recommend avoiding the use of OCPs in patients with lipodystrophy due to the increased risk of worsening hypertriglyceridemia.

On the basis of strong clinical evidence, the dose of levothyroxine is higher at early ages and progressively decreases into adulthood. On the basis of observational studies and consensus, children with congenital hypothyroidism and a eutopic thyroid gland should discontinue levothyroxine treatment at age 3 years to determine whether their hypothyroidism was transient. On the basis of observational studies, most patients with Hashimoto thyroiditis present with a goiter and without hypothyroidism. On the basis of observational studies, subclinical hypothyroidism is more common among obese children. On the basis of strong clinical evidence, children with central hypothyroidism should have other pituitary hormone deficiencies ruled out.

Background: Forty-three percent of New York City ' s (NYC) school-age children are overweight or ... more Background: Forty-three percent of New York City ' s (NYC) school-age children are overweight or obese, placing them at risk for heart disease and type 2 diabetes mellitus (T2DM). Objective: The objective of this study was to determine if an intensive after-school dance and lifestyle education program would reduce risk factors for heart disease, T2DM, and improve lifestyle choices. Subjects: Subject include 64 fourth-and fi fth-grade students at an elementary school in NYC. Methods: Students received freestyle dance and lifestyle classes for 16 weeks and were evaluated for changes in body composition, endurance, biochemical measurements, and lifestyle choices. Results: Signifi cant improvements in BMI percentiles were found among children in the overweight and obese categories as well as in endurance and biochemical measurements that refl ect heart disease and diabetes risk. Improvement was also reported in lifestyle choices. Conclusion: An intensive after-school dance and lifestyle education program can reduce risk factors for heart disease and T2DM and improve lifestyle choices among elementary school children.

Exercise impairment is a central feature of chronic obstructive pulmonary disease (COPD), and a m... more Exercise impairment is a central feature of chronic obstructive pulmonary disease (COPD), and a minimal clinically important difference (MCID) for 6-min walk distance (6MWD) decline (>30 m) has been associated with increased mortality. The predictors of the MCID are not fully known. We hypothesize that physiological factors and radiographic measures predict the MCID. We assessed 121 COPD subjects during 2 years using clinical variables, computed tomographic (CT) measures of emphysema, and functional measures including diffusion lung capacity for carbon monoxide (DLCO). The association between an MCID for 6MWD and clinical, CT, and physiologic predictors was assessed using logistic analysis. The C-statistic was used to assess the predictive ability of the models. Forty seven (39%) subjects had an MCID. In an imaging-based model, log emphysema and age were the best predictors of MCID (emphysema Odds Ratio [OR] 2.47 95%CI [1.28-4.76]). In a physiologic model, DLCO, age, and male gender were selected the best predictors (DLCO OR 1.19 [1.08-1.31]). The C-statistic for the ability of these models to predict an MCID was 0.71 and 0.75, respectively. In COPD patients the burden of emphysema on CT scan and DLCO predict a clinically meaningful decline in exercise capacity.

The association between primary hypothyroidism and precocious puberty secondary to ovarian hypers... more The association between primary hypothyroidism and precocious puberty secondary to ovarian hyperstimulation has been recognized for over a century. Here, we report the case of a 9-year-old girl with severe primary hypothyroidism, who presented with premature menarche, enlarged pituitary gland, enlarged ovaries with multiple cysts, and elevated prolactin and α -feto protein levels. Pituitary and ovarian radiology findings, and α -feto protein levels normalized a few weeks after hypothyroidism treatment was started. Reviewing the literature we found several reports of increased levels of tumor markers in girls with this association. Thyroid function tests should be always part of the evaluation of patients with precocious puberty especially if the bone age is delayed. Tumor markers and liver function tests may be abnormal in patients with severe hypothyroidism and improve soon after thyroid hormone replacement is started.

Fuller Albright and Donovan McCune published the first reports on individuals with McCune-Albright syndrome (MAS). Since then, the classic triad of precocious puberty, café-aulait spots, and polyostotic bone dysplasia continues to define the syndrome. However, having gathered a better picture of the pathophysiology of MAS, the way this condition is understood has changed. Isolated activating mutations of the alpha subunit of the G protein (GNAS1) have been found in different tissues, including pituitary adenomas, thyroid adenomas, ovarian cysts, monostotic bone dysplasia, and the adrenal glands, to name a few. For this reason, we have added 'and disorders due to activating mutations of GNAS1' to the title of this review. We discuss here the clinical consequences of GNAS1 activating mutations in different body systems and organs, the diagnostic approach to MAS, and the current therapeutic recommendations.

Although rare, the most common presentation in the pediatric population is bilateral hyperplasia of the adrenal glands as opposed to an adenoma. Crohn disease is part of the spectrum of inflammatory bowel disease, which manifests in children as flare-ups of bloody diarrhea and abdominal pain. The association of concurrent Conn syndrome and Crohn disease has been previously presented in two cases in adults. This is the first pediatric case to be reported in the literature.

Regional assessment of lung disease (such as chronic obstructive pulmonary disease) is a critical... more Regional assessment of lung disease (such as chronic obstructive pulmonary disease) is a critical component to accurate patient diagnosis. Software tools than enable such analysis are also important for clinical research studies. In this work, we present an image segmentation and data representation framework that enables quantitative analysis specific to different lung regions on high resolution computed tomography (HRCT) datasets. We present an offline, fully automatic image processing chain that generates airway, vessel, and lung mask segmentations in which the left and right lung are delineated. We describe a novel lung lobe segmentation tool that produces reproducible results with minimal user interaction. A usability study performed across twenty datasets (inspiratory and expiratory exams including a range of disease states) demonstrates the tool's ability to generate results within five to seven minutes on average. We also describe a data representation scheme that involves compact encoding of label maps such that both "regions" (such as lung lobes) and "types" (such as emphysematous parenchyma ) can be simultaneously represented at a given location in the HRCT.

We present an image pipeline for airway phenotype extraction suitable for large-scale genetic and... more We present an image pipeline for airway phenotype extraction suitable for large-scale genetic and epidemiological studies including genome-wide association studies (GWAS) in Chronic Obstructive Pulmonary Disease (COPD). We use scale-space particles to densely sample intraparenchymal airway locations in a large cohort of high-resolution CT scans. The particle methodology is based on a constrained energy minimization problem that results in a set of candidate airway points situated in both physical space and scale. Those points are further clustered using connected components filtering to increase their specificity. Finally, we use the particle locations to perform airway wall detection using an edge detector based on the zero-crossing of the second order derivative. Given the airway wall locations, we compute three phenotypes for airway disease: wall thickening (Pi10,WA%) and luminal remodeling (P%). We validate the airway extraction technique and present results in 2,500 scans for the association of the extracted phenotypes with clinical outcomes that will be deployed as part of the COPDGene study GWAS analysis.

Zinc deficiency alters the immune system and the resistance to infections. To evaluate the effect of two zinc compounds in the prevention of acute respiratory infection and acute diarrhea. Randomized triple-blind community trial with 301 children between 2-5 years of age from six child daycare centers in Medellin, Colombia. Children were distributed in three groups receiving zinc amino acid chelate, zinc sulfate and placebo five days a week for 16 weeks. Daily symptoms of respiratory infection, acute diarrhea and side effects were evaluated. The incidence of respiratory infection was lower with zinc amino acid chelate (1.42 per 1,000 child-days) compared with placebo (3.3 per 1,000 child-days) (RR=0.43, 95% CI: 0.196 to 0.950, p=0.049) and with zinc sulfate (1.57 per 1,000 child-days) (RR=0.90, 95% CI 0.382 to 2.153, p=0.999). Zinc deficiency alters the immune system and the resistance to infections. To evaluate the effect of two zinc compounds in the prevention of acute respiratory infection and acute diarrhea. Randomized triple-blind community trial with 301 children between 2-5 years of age from six child daycare centers in Medellin, Colombia. Children were distributed in three groups receiving zinc amino acid chelate, zinc sulfate and placebo five days a week for 16 weeks. Daily symptoms of respiratory infection, acute diarrhea and side effects were evaluated. The incidence of respiratory infection was lower with zinc amino acid chelate (1.42 per 1,000 child-days) compared with placebo (3.3 per 1,000 child-days) (RR=0.43, 95% CI: 0.196 to 0.950, p=0.049) and with zinc sulfate (1.57 per 1,000 child-days) (RR=0.90, 95% CI 0.382 to 2.153, p=0.999).
