Serological investigation of Mycoplasma genitalium in infertile women

BACKGROUND: The role of Mycoplasma genitalium in the pathogenesis of pelvic inflammatory disease has not been characterized. METHODS: Sera from 308 infertile women were investigated for antibodies to M. genitalium by immunoblotting. Women with tubal factor infertility (TFI) made up 132 of the patients, 67 of the women had an infertile male partner and 109 were infertile for unknown reasons. RESULTS: Of the TFI patients 29 (22.0%) were seropositive to the major adhesin, MgPa, of M. genitalium versus 11 (6.3%) in the group of women with normal tubes. No cross-reactions between MgPa and P1 of the related Mycoplasma pneumoniae were found. Besides, MgPa positive sera were confirmed by immunoblotting using a cloned fragment of the C-terminal part of MgPa specific to M. genitalium. Chlamydia trachomatis is known to be able to cause infertility as a result of salpingitis. Therefore, the sera were tested against C. trachomatis using a commercial ELISA test. Seventy-five (56.8%) of the TFI patients were seropositive to C. trachomatis. Eight (27.6%) TFI patients seropositive to MgPa were negative to C. trachomatis. CONCLUSIONS: This study indicates that M. genitalium may be an independent risk factor in the development of an inflammatory process leading to scarring of the uterine tubes in women and thereby causing infertility.

Association of Mycoplasma genitalium with infertility in North Indian women

Data regarding the association of Mycoplasma genitalium with infertility is scarce. This study was planned to look for the presence and association of M. genitalium in women with infertility. A prospective observational study was conducted on 100 cases of infertile women. The control group included 100 healthy fertile women. Samples of first void urine (FVU), endocervical swabs (ECS), and endometrial biopsies were subjected to polymerase chain reaction targeting MgPa gene to look for the presence of M. genitalium DNA. All endometrial biopsy samples were subjected to histopathological examination. A detailed clinical history of patients was taken, and all relevant investigations were recorded. M. genitalium was found in 16% of women with infertility from either of the samples that is, FVU and/or ECS and/or endometrium biopsy, and none from controls. Detection of genital mycoplasmal infections among infertile females by multiplex PCR

Women reproductive system is a suitable environment for growth of various pathogen and nonpathogenic microorganisms. Mycoplasmataceae is a family of bacteria which cause oligosymptomatic genital infections. The complications caused by these bacteria may lead to infertility in women. The aim of this study was detection of genital Mycoplasma hominis, Ureaplasmaurealyticum and Mycoplasma genitalium among infertile females who referred to the infertility clinics. A total of 104 infertile women (in reproductive age) who referred to infertility clinics in the city of Sanandaj, Kurdistan, Iran, from February to May 2013 were selected for this study. Isolates were identified serologically as M. hominis or Ureaplasma urealyticum (U. urealyticum) by growth inhibition test (disc method). Result: Two genital mycoplasmas were detected out of 263 specimens by culture method. The positive specimens were identified as M. hominis. Conclusion: The result demonstrates lower values and further investigations for rapid detection of genital mycoplasmas in infertile female patients using PCR could be important and necessary for the detection of mycoplasmas infections.

Prevalence and Clinical Significance of Mycoplasma genitalium in Gynecologic Patients

Objective: Mycoplasma genitalium has been recognized as a cause of male urethritis, and there is now evidence suggesting it causes cervicitis and pelvic inflammatory disease (PID) in women. Methods: Prevalence, risk factors and co-infections with other sexually transmitted pathogens were collected in a cross-sectional study looking at 400 women at the gynecologic clinics of a university medical center in the United States. Bacterial vaginosis and trichomoniasis were diagnosed using Amsel's criteria, gram stain and trichomonas culture respectively. Cervicitis and PID were clinically diagnosed. After testing for Chlamydia trachomatis and Neisseria gonorrhoeae, the residual cervical swab transport medium (Gen-Probe/Hologic ®) was stored at-70°C. Stored samples were later analyzed for M. genitalium by a research use only transcription-mediated amplification assay using procedures similar to those established for APTIMA Combo2 assay for C. trachomatis and N. gonorrhoeae (Gen-Probe/Hologic ®). Results: The overall prevalence of infection with C. trachomatis, N. gonorrhoeae, T. vaginalis and M. genitalium was found to be 7.8%, 1.8%, 10.43% and 8.9%, respectively. Prevalence of M. genitalium was comparable to that of C. trachomatis and greater than the prevalence of N. gonorrhoeae. Univariate analysis of M. genitalium status showed that participants with lower condom use had an increased probability of M. genitalium (p=0.037). Conclusion: Prevalence of M. genitalium was comparable to C. trachomatis in our study, but more research is needed to clarify pathogenicity.

Isolation and Molecular Identification of Mycoplasma Hominis in Infertile Female and Male Reproductive System

Background: Infection of urogenital system with Mycoplasma potentially affect reproductive system and increases infants mortalities. Therefore, detection of these organisms is an important issue that should be considered and appropriate diagnostic methods should be used to identify these microorganisms. In the female reproductive system, infection can affect different parts of the cervix, endometrium, and fallopian tube. The extent of this infection in different diseases and its pathogenesis might be related to anatomic site of involvement. Some infections can lead to infertility in both males and females. Genital infection with Mycoplasmas have devastating effects on reproductive organs and cause fertility disorders and mortality in infants. In recent years, many studies have been conducted to isolate these pathogens; however, the isolates have not been identified so far. Objectives: The aim of this study was to determine the molecular identity of Mycoplasma hominis isolated from infertile female and male reproductive system in the Infertility Center of Kerman. Materials and Methods: This descriptive study was performed purposefully on 100 infertile females and 100 infertile males who were referred to the Infertility Center of Kerman during a six-month period. The collected samples of semen and vaginal swabs were examined for the presence of M. hominis by PCR. The samples with positive results in PCR were selected for molecular identification. Alignment of samples sequence was performed using MEGA 5 software through Neighbor-joining method. Results: Among 100 samples from infertile males, the presence of genus Mycoplasma was confirmed in 45 cases of which 15 cases were infected with M. hominis. Among 100 samples from infertile female, the presence of genus Mycoplasma was confirmed in 43 cases of which 18 case were infected with M. hominis. The positive samples were sequenced and the phylogenetic tree was plotted. Conclusions: The results showed that 37.5% of infertile males and females were infected with M. hominis. Analysis of the nucleotide sequences of the study isolates indicates a particular variety among these isolates. In comparing the isolates in the study, a very little genotypic similarity was found among some of them.

Mycoplasma genitalium antibody among infertile women in Kano, Northwestern Nigeria

Background: Infertility is an emerging health problem in many countries of the world including Nigeria. A serological study of the role of Mycoplasma genitalium in pelvic inflammatory disease and ectopic pregnancy

Objectives: Establishing a connection between the emerging urogenital tract pathogen Mycoplasma genitalium and upper genital tract infection in women would be of major importance. The aim of this study was to evaluate the association between M genitalium antibodies and pelvic inflammatory disease (PID) and ectopic pregnancy (EP) using a lipid-associated membrane protein-enzyme immunoassay (LAMP-EIA) method. Methods: The LAMP-EIA was used to analyse sera obtained from patients with clinical PID and EP collected in Sweden between February 1984 and April 1986. Sera from healthy pregnant women (Ctrl) collected during approximately the same period were used as controls. Evidence of chlamydial infection was investigated using a commercial anti-Chlamydia trachomatis EIA assay. Results: The LAMP-EIA was specific as determined by a lack of cross-reactivity with other Mycoplasma species. The LAMP-EIA showed that 17% (33/193) of the PID patients were M genitalium positive as compared to 18% (15/82) of the EP patients and 15% (36/246) of the Ctrl women. No significant association could be demonstrated between M genitalium antibodies and PID or EP in crude or adjusted logistic regression. Antibodies against C trachomatis were demonstrated in 54% of the PID and 57% of the EP patients, and also in 37% of the Ctrl women, showing a statistically significant association.
