A Rare Case of Bilateral Malignant Mixed Mullerian Tumor of Ovary

Malignant mixed mesodermal tumor (MMMT) of ovary accounts for less than 1% of all ovarian tumors. These aggressive tumors with malignant epithelial and sacromatous elements occur mainly in postmenopausal, low parity women These tumors have aggressive clinical course and very poor prognosis. We herein present a case of malignant mixed mesodermal tumor In a 57 year old postmenopausal woman who presented with complaints of pain and mass per abdomen with loss of appetite and progressive anemia for the last 15 days.There was no history of vaginal bleeding and no relevant past medical history. A large palpable mass in the pelvis was observed during examination. A contrast enhanced CT scan of abdomen revealed a large,complex abdominopelvic mass with multiple omental deposits along with gross ascites.Ascitic fluid cytology reported metastatic ovarian adenocarcinoma.The patient was planned for NeoAdjuvant chemotherapy to be followed by interval cytoreduction surgery and further adjuvant chemotherapy. At approximately 5 months postoperatively, the patient remains alive and recurrence free.

Malignant mixed Mulleriantumour (MMMT) is also known as Carcinosarcoma. It is a malignant neoplasm arising in the uterus, the ovaries, the fallopian tubes or any body part that contains epithelial and mesodermal tissue. It is very rare and aggressive type of malignancy, accounts for 2-5% of all tumours derived from body of uterus.It is considered as a metaplastic carcinoma. MMMTs of uterus present in postmenopausal women (median age 65) with bleeding per vagina and pain in abdomen. They pose a diagnostic dilemma. This tumour is usually seen associated with chronic estrogen stimulation, radiation therapy and tamoxifen therapy. Nulliparity, diabetes and obesity are the predisposing factors. The outcome correlates with the stage of disease and depth of myometrial invasion. Five year disease free survival by advancing stage is poor. Combination chemotherapy with isofosfamide and cisplatin may improve progression free survival. We present a case of 60 year old, who presented to our OPD with complaint of postmenopausal bleeding and on imaging it was considered to be endometrial carcinoma. The biopsy report was inconclusive. She later underwent Hysterectomy with bilateral salpingo-oopherectomy with lymphadenectomy. Histopathlogy of specimen showed Malignant mixed Mulleriantumour. It is important that the clinician should have high index of suspicion for postmenopausal patients presenting with per vaginal bleeding and should consider possibility of Malignant mixed Mulleriantumour.

Background : Malignant mixed Mullerian tumors (MMMTs) are metaplastic carcinomas including both carcinomatous and sarcomatous elements. Case: A 65-year-old postmenopausal female presented to outpatient department with chief complaints of blood mixed vaginal discharge for the last 10 days. Speculum examination revealed a large, necrotic, foul smelling polyp of approximately 4 x 4 centimeters size seen coming out through the external os. Vaginal examination revealed that the cervix was dilated and taken up and the same polypoid mass was felt coming out through the os. Polypectomy along with endometrial biopsy was done and tissue was sent for histopathological examination which revealed features of malignant mixed mullerian tumor in the polyp and the endometrial biopsy showed necrotic tissue. Malignant mixed Müllerian tumor, also known as malignant mixed mesodermal tumor, MMMT and carcinosarcoma, is a malignant neoplasm found in the uterus, the ovaries, the fallopian tubes and other parts of the body that contains both carcinomatous (epithelial tissue) and sarcomatous (connective tissue) components. A 50-year female P1L1A1 with complaints of pervaginum bleeding and pain in abdomen was diagnosed to have carcinosarcoma endometrium for which hysterectomy with bilateral salphingo-opherectomy was done.

Objective: To present a case of malignant mixed müllerian tumor of the ovary, a rare and aggressive ovarian malignant tumor with poor prognosis. Case Report: A 52-year-old woman consulted our outpatient department with complaints of abdominal distention and a firm palpable mass over her lower abdomen. Physical examination and computerized tomography revealed cystic mass lesions on the bilateral adnexal areas. Ovarian cancer was suspected, so the patient underwent exploratory laparotomy. Optimal debulking surgery was performed, and final pathology revealed malignant mixed müllerian tumor of the ovary. Chemotherapy using ifosfamide and cisplatin were administered postoperatively, and adjuvant was also administered. After six cycles of chemotherapy, the patient is well with no signs of recurrence. Conclusion: Ovarian malignant mixed müllerian tumor usually yields poor outcomes; hence, aggressive treatment with optimal debulking surgery followed by combination chemotherapy using ifosfamide and cisplatin may improve patient outcomes. 49(1):87-90]

Primary mixed mullerian tumor of the vagina—A case report with review of the literature

Malignant mixed mullerian tumor (MMMT) is a rare entity. The commonest site of this tumor in the female genital tract is the uterus followed by cervix. Primary MMMT of vagina is extremely rare. We are reporting this rare entity, with a brief review of the literature, in a 48-year-old perimenopausal female who presented with a history of passage of urine per vagina. On pelvic examination, a polypoidal mass arising from the anterior wall of the vagina was identified. Histopathological examination revealed the biphasic nature of the tumor. Immunohistochemistry confirmed the diagnosis of MMMT of vagina. To conclude, although rare, clinicians, oncologists, and pathologists should identify this malignant tumor for appropriate treatment and management.

Common presentation of these patients is post-menopausal bleeding with uterine enlargement. Here we present the case of 55 year old women with post-menopausal bleeding. Total abdominal hysterectomy with good vaginal cuff with bilateral salpingo-oophrectomy with bilateral pelvic lymphadenectomy was done and a diagnosis of malignant mixed mullerian tumour with Genital Tuberculosis was made.

A malignant mixed Mullerian tumour (MMMT) of the uterine corpus is an extremely rare and aggressive malignancy. There are very few studies regarding the outcome of MMMT patients in India. Hence, we conducted the present study to analyse the outcome of MMMTs at our institute. To study the clinical profile, prognostic features, and treatment outcome of MMMT with multimodal therapy. A five-year retrospective study of the MMMT cases diagnosed and treated at our centre was conducted. Twenty patients with pathological proven diagnosis of MMMT treated at our institute from January 2007 to May 2012 were analysed. These patients underwent comprehensive surgical staging followed by adjuvant therapy in the form of chemotherapy alone or chemoradiotherapy. These patients were analysed for event-free survival (EFS), and their outcomes were correlated with histology, therapy, myometrial invasion, and the stage of disease. Bilateral ovarian malignant mixed Mullerian tumour: A rare entity

Malignant mixed Mü llerian tumor (MMMT) is a rare neoplasm of the female genital tract. We report a case of bilateral ovarian MMMT in a 10-year-old female dog. Ovaries were only moderately enlarged with a papillary surface and firm nodules. Multiple metastases were observed in the abdominal cavity and pulmonary parenchyma. Histologically, both ovaries had intermingled carcinomatous and sarcomatous components with cartilage and bone. Metastatic lesions were not mixed. The peritoneal metastases were carcinomatous; pulmonary metastases were sarcomatous. Carcinomatous elements of the MMMT were immunohistochemically positive for cytokeratin (CK) AE1/AE3, CK7, and vimentin and estrogen receptors. Conversely, the sarcomatous cells were positive for vimentin, but negative for CKs. Chondrocytes also expressed S-100 protein. On the basis of similarities to human ovarian MMMT, the diagnosis was heterologous malignant mixed Mü llerian tumor of the ovary.
