Latar belakang: Bentuk dokumentasi pertama kali dalam rekam medis dimulai dengan pencatatan data demografi pasien baru yang pertama kali datang ke rumah sakit. Isian data demografi mencakup identitas diri yang lengkap serta merupakan penentu upaya pemberian layanan kesehatan. Saat survei ditemukan kekosongan data identitas pasien yang dapat mempengaruhi isi dan kualitas rekam medis, secara keseluruhan bisa mengaburkan identifikasi pasien yang sesuai identitasnya. The Complete Systematic Land Registration Program (PTSL) has succeeded in creating a digital database based on land parcels that can be utilized by various parties. However, there is still a need to further study the need for cadastral data for various purposes. This research specifically focuses on analyzing the PTSL database to meet the needs of the various parties involved. A qualitative descriptive method was used in this study, through interviews with various agencies that required data based on land parcels. The results of the study show that the PTSL database is currently still used sectorally by the Land Office and has not yet involved other agencies. Therefore, it is necessary to design an appropriate structure and system for the PTSL program results database, so that it can support the use of the database by various parties for multi-stakeholder needs. Dalam memberikan kepastian hukum dan perlindungan hukum hak milik atas tanah, berdasarkan Peraturan Pemerintah No. 24 Tahun 1997 tentang Pendaftaran Tanah, Pemerintah wajib mendaftar seluruh bidang tanah di wilayah Indonesia baik melalui pendekatan sistematis maupun sporadik. Metode yang digunakan dalam melakukan penelitian merupakan metode penelitian secara yuridis empiris dan proses pembuatan penelitian yang lakukan bersifat penelitian deskriptif analitis. Optimalisasi Faktor–Faktor Yang Mempengaruhi Keberhasilan Pendaftaran Tanah Sistematis Lengkap

The government continuously strives to carry out land registration in all Indonesian regions to ensure legal certainty. By the government initiatives, the completion of land registration throughout Indonesia is planned to be completed in 2024 through the Complete Systematic Land Registration (PTSL) program. In its implementation, the number and speed of achievement of registered land parcels among Land Off ices are different. Based on this fact, it is very important to conduct a research on what factors that affect the achievement/realization of the Complete Systematic Land Registration (PTSL) signif icantly. The method that was used in this research was a quantitative approach, and the data was processed by using multiple regression tests to get the signif icant simultaneously influential factors. The factors that had a signif icant effect simultaneously are: the number of human resources involved in PTSL, the number of juridical data collectors that are involved, the number of computers used in PTSL, the number of available theodolite, the number of available GPS, and less than 3%-slope of the area. Optimization of these factors can be done by optimizing the human resources involved in the PTSL team through participatory mapping, computer support tools, the use of measuring instruments in various slopes, and strengthening PTSL perceptions and innovations.

Pendaftaran Tanah Sistematik Lengkap adalah proses pendaftaran tanah untuk pertama kali, dilakukan serentak bagi semua obyek Pendaftaran Tanah di seluruh Indonesia dalam satu wilayah desa atau kelurahan. Permasalahannya adalah 1) Apakah kegiatan Pendaftaran Tanah Sistematik Lengkap di Kota Cirebon Tahun Anggaran 2018 memenuhi target, 2) Apakah hambatan yang dihadapi Kantor Pertanahan Kota Cirebon dalam rangka kegiatan Pendaftaran Tanah Sistematik Lengkap di Kota Cirebon Tahun Anggaran 2018, 3) Bagaimanakah penyelesaian yang ditempuh Kantor Pertanahan Kota Cirebon mengatasi hambatan dalam rangka kegiatan Pendaftaran Tanah Sistematik Lengkap Tahun Anggaran 2018.
