The Importance of Oral Health during Pregnancy: A review

Pregnancy is a transient physiological state which brings about different hormonal changes in a woman’s body. These effects are generalized and there are various oral changes as well. There are a number of especially important alterations in the periodontal conditions within the oral cavity. These changes have important implications as they have been known to cause adverse pregnancy outcomes. Better knowledge about these scenarios among health care professionals and women would go a long way toward avoiding or minimizing these adverse outcomes. Health education is an important tool in creating awareness among pregnant women regarding improvement of their oral health. Awareness among the health professionals and good inter-departmental collaboration would help toward a more efficient treatment of these pregnancy related conditions.

A 2012 national experts' consensus statement concludes that dental care is safe and effective throughout all trimesters of pregnancy, and should not be withheld because of pregnancy. Research to date shows that routine preventive, diagnostic, and restorative dental treatment-including periodontal therapy-during pregnancy does not increase adverse pregnancy outcomes. The pregnant patient should be educated on the importance of oral health care for herself and her child and on expert recommendations for bolstering home oral hygiene care during pregnancy. Oral manifestations may be associated with pregnancy, including gingivitis, pregnancy epulis, and others. Research to date supports the safety of undertaking periodontal treatment during the perinatal period, with no associated adverse pregnancy outcomes. Whether perinatal periodontal treatment can reduce the risks for preterm birth or low birth weight has not been demonstrated thus far in US multicentered randomized controlled trials.

Primary health care providers are in a unique position to treat the pregnant mothers for dental as well as general health for a healthy mother and child. Oral cavity is also subjected to reversible as well as irreversible changes due to hormonal changes during pregnancy. Perinatal mortality rates in Pakistan are more than 10-fold greater than in developed countries. It increased with the severity of periodontal disease. Patients, physicians, and dentists are cautious, often avoiding treatment of oral health issues during pregnancy. This problem is compounded by a lack of clinical guidelines for the prevention and management of common oral conditions in pregnancy. This review analyzes the changes that occur during normal pregnancy and describes the main odontogenic infections, suggesting the actual best approach in dental management. Several studies support the hypothesis that periodontal disease is associated with preterm labour and other conditions complicating pregnancy, such as pre-eclampsia and fetal growth restriction. Appropriate dental care and prevention during pregnancy may reduce poor prenatal outcomes and eliminating risk factors. Dental examination before pregnancy is strongly suggested in order to act early on dental and periodontal diseases. Prevention means reducing the presence of bacterial plaque through professional hygiene sessions, education, and motivation to proper oral hygiene at home, education in proper nutrition, a balanced diet, and low intake of sugars. For these reasons, it is essential to have a more intense interdisciplinary collaboration between gynecologist and dentist in order to achieve an optimal women's health, during this particular time in their lives.

TODAY'S CARE FOR A BETTER TOMORROW: A SYSTEMATIC REVIEW OF DENTAL CARE IN PREGNANCY

Introduction: Pregnancy is a special state during a woman’s life, which is associated with a myriad of complex anatomical, physiologic, metabolic, emotional and physiological changes. There are changes observed in cardiovascular, respiratory, gastrointestinal system as well as changes in oral health and increased susceptibility to oral infections. Majority of pregnant women have been reported to experience oral changes yet they do not seek or are advised not to seek dental care as a part of routine prenatal care. Dentists also needlessly withhold or delay treatment of pregnant patients due to unrealistic fear of injuring patient, fetus or fear of litigations. It is important on the part of stomatologists to understand that pregnancy is not a disease wherein the patients are denied or deferred treatment. Thus, the aim of this article is to emphasize on the significance of oral care during pregnancy and motivate oral health care providers to provide timely and appropriate dental care. Methods: Articles were searched through keywords like dental care during pregnancy, drug safety, oral prenatal care, dentist’s attitude towards treating pregnant patients. Original research article, case reports, randomised control trials were searched through Pubmed and medline search between 2001 and 2015 Results: Generalised lack in awareness about oral manifestations of pregnancy was seen. It was seen that dentist’s usually defer treatment of pregnant patients. Conclusions: Every gestational woman should be encouraged to include dental visits in her routine prenatal care. Dental physicians should team up with other health care providers and keep themselves abreast with the updated guidelines in providing oral care.

Prevalence and Risk for Dental Problems During Pregnancy: A Cross-Sectional Questionnaire Study

Objectives: To assess the prevalence and awareness of dental problems during pregnancy, and to ascertain the risk factors that lead to dental problems in pregnancy. Methods: A cross-sectional questionnaire study was conducted. Patients who were currently pregnant or had been pregnant were selected, and questioned on the frequency and type of dental problems during pregnancy, treatment availed and systemic problems. Recorded data was analysed using SPSS version 17. Results: A total of 64.5% of patients experienced dental problems during pregnancy. Dental caries was the most frequent problem (50.7%), followed by gingivitis (48.4%). Only 14% of patients availed dental treatment. Presence of systemic diseases and low oral hygiene scores were associated with significantly higher incidence of dental problems.
