The Importance of Oral Health during Pregnancy: A review

Pregnancy is a transient physiological state which brings about different hormonal changes in a woman’s body. These effects are generalized and there are various oral changes as well. There are a number of especially important alterations in the periodontal conditions within the oral cavity. These changes have important implications as they have been known to cause adverse pregnancy outcomes. Better knowledge about these scenarios among health care professionals and women would go a long way toward avoiding or minimizing these adverse outcomes. Health education is an important tool in creating awareness among pregnant women regarding improvement of their oral health. Awareness among the health professionals and good inter-departmental collaboration would help toward a more efficient treatment of these pregnancy related conditions.

Pregnancy is a dynamic state leading to several physiological transient changes in the body system including the oral cavity. In order to maintain good oral health, the dental treatment should not be avoided. The dental management of pregnant patients involves special consideration as otherwise a hypersensitive vomit reaction in the third trimester of pregnancy can influence the eating behavior of the patient as well as oral hygiene influencing the appearance of dental erosion and tooth decay. Once pregnant women suffer an unpleasant gag reflex in a dental office, they may become phobic, delaying or postponing their dental treatment. The dental management of pregnant patients involves special considerations.

The gestational period must be the focus of attention by dental professionals, with a view to promoting oral health and preventing diseases that affect the oral cavity. Several factors can promote the manifestations of oral alterations during pregnancy, especially hormonal changes and the presence of bacterial plaque due to inefficient oral hygiene. In view of this, developing new ways of clarifying the possibilities of treatment and the meaning of chronic conditions as factors of harm to oral health is fundamental for the promotion of oral health in pregnant women. OBJECTIVE: To discuss the importance of implementing strategies to promote oral health during pregnancy, as well as highlighting the dentist's role in achieving the maintenance of the mother's general health. METHODOLOGY: The present study is a review of the literature with a quantitative approach and descriptive character. The articles were obtained after searching the Database: VHL. Database: LILACS and BBO. With the use of keywords: Oral health, Pregnant women and Health Education. As inclusion criteria: full text available, articles in Portuguese, articles published between the years 2012-2022 and original articles. RESULT: The lack of knowledge about oral health makes many pregnant women continue to practice wrong habits, causing many oral pathologies. Therefore, it is necessary to disseminate correct information about dental treatment during pregnancy, in addition to reinforcing the training of dentists to serve this specific public. CONCLUSION: Based on strategies implemented by dentists in prenatal care, the pregnant woman can act as a multiplier agent of preventive information, thus contributing to the promotion of oral health.

Primary health care providers are in a unique position to treat the pregnant mothers for dental as well as general health for a healthy mother and child. Oral cavity is also subjected to reversible as well as irreversible changes due to hormonal changes during pregnancy. Perinatal mortality rates in Pakistan are more than 10-fold greater than in developed countries. It increased with the severity of periodontal disease. Oral health of women during pregnancy is important. Not only can it compromise the outcomes of pregnancy, it may also affect the overall health of the newborn. Proper dental care during pregnancy is very important, since pregnancy causes various hormonal changes in the body that can actually increase the risk of developing gum diseases. The aim of the study was to assess the oral status of pregnant women who visited Saveetha Dental College. A survey was conducted among pregnant females who had reported to Saveetha Dental College, from the time period of June 2019 to March 2020. A questionnaire consisting of questions on oral health was asked. Patient records were reviewed and 60 pregnant females were selected. Descriptive statistics, cross tabulation and chi-square test were done by SPSS IBM software 20.0. Bleeding gums were reported by 68%, dental caries by 61%, halitosis by 70% while mobility and sensitivity was reported by 35% and 33% respectively. No association was found between the diet of pregnant women and their oral status. Pearson’s Chi Square value: 3.389, p value: 0.06 (>0.05). No association was found between the diet of pregnant women and their oral status. Within the limits of the present study oral status of pregnant women was assessed. Most patients experienced gingival bleeding, dental caries and halitosis while few patients experience mobility and sensitivity. No association was found between the diet of pregnant women and their oral status.

There are lots of myths about the safety of dental care during pregnancy. As a result pregnant women recieve less dental care than when they are pregnant. In our review we tried to emphasise the importance and safety of routine dental care for pregnant women.

A woman undergoes a series of physiological states such as puberty, pregnancy and menarche. A number of anatomical, physiological and hormonal changes occur in pregnancy which not only affects the maternal health but also the health of the child. Oral health is an integral part of overall general health and is often neglected due to lack of knowledge and awareness and an unfavourable attitude. It definitely has its impact upon the systemic health and everyone should be aware about the same. This further needs due consideration in a pregnant women as peculiar oral changes and their impact on the new born infant and pregnancy outcome have been discussed and documented with sufficient evidence. Much has been talked about common oral health issues during pregnancy like gingivitis, periodontitis, pyogenic granuloma etc and their management. Patient's education regarding the oral hygiene and abnormal pregnancy outcome (pre-term birth, low body weight birth) is extremely essential. These conditions may be prevented by appropriate monitoring of dental health in pregnancy and also by providing dental care in such patients. Patients should be referred to the dental health care provider for dental needs during the time of pregnancy. A collaboration of dental and medical health care givers must be made and is an effective channel to provide the best possible treatment and management of these patients. This article discusses the importance of oral health care during pregnancy and how an integration between the medical and dental health provider can go a long way to attend the oral health concerns during this time.

Pregnant women and their providers need more knowledge about the many changes that occur in the oral cavity during pregnancy. In this article we describe the importance of the recognition, prevention, and treatment of oral health problems in pregnant women. We offer educational strategies that integrate interprofessional oral health competencies.

Pregnancy is a dynamic physiological state evidenced by several transient changes and significant hormonal changes like increased production of estrogen and progesteron. Both periodontal disease and caries are highly prevalent among pregnant women. Also association of periodontal disease and preterm low-birth weight has been found. Thus, it is essential to educate pregnant women to promote oral health behaviours. Pregnant women should be emphasised on the relationship of maternal oral health with fetal health and specific preventive oral health care programme should be made an integral part of antenatal care.

Summary This review analyzes the changes that occur during normal pregnancy and describes the main odontogenic infections, suggesting the actual best approach in dental management. Several studies support the hypothesis that periodontal disease is associated with preterm labour and other conditions complicating pregnancy, such as pre-eclampsia and fetal growth restriction. Appropriate dental care and prevention during pregnancy may reduce poor prenatal outcomes and eliminating risk factors. Dental examination before pregnancy is strongly suggested in order to act early on dental and periodontal diseases. Prevention means reducing the presence of bacterial plaque through professional hygiene sessions, education, and motivation to proper oral hygiene at home, education in proper nutrition, a balanced diet, and low intake of sugars. For these reasons, it is essential to have a more intense interdisciplinary collaboration between gynecologist and dentist in order to achieve an optimal women's health, during this particular time in their lives.
