Combined Talus Fracture with Medial Malleolar Fracture: A Case report with Review of Literature

Introduction: Combined talar body fracture with a medial malleolar fracture is rare in literature. Case Presentation: A 30-year-old female presented to our hospital with complaints of pain and swelling over the right ankle joint with difficulty in bearing weight following a motor vehicle accident. Investigation showed talar body fracture of type 2 Sneppen (classification) with an ipsilateral medial malleolar fracture. Fractured talus was openly reduced with the help of reduction clamp and appropriate sized Herbert screw applied to hold the reduction. The medial malleolar fracture was fixed with an appropriate-sized cannulated cancellous screw after confirming satisfactory reduction. The patient was followed up for wound complications, AVN changes, early osteoarthritic changes, and functional outcome. Fracture of The Sustentaculum Tali: Features and Surgical Management for Fracture Reduction

Sustentaculum tali is a horizontal shelf-like projection on the medial part of the calcaneum. The medial facet articulates to the talus and supports the medial column of the foot. The spring ligament originates at the plantar aspect of sustentaculum tali, and flexor hallucis longus passes below it and indirectly supports the longitudinal arch of the foot. Sustentaculum tali is a strong cortical bone, so its isolated fracture is unusual. Therefore, the fracture of the sustentaculum tali is easily misdiagnosed. The patients complain of pain below the medial malleolus and also on passive movement of the flexor hallucis longus tendon. The patient has a history of falls from height with the foot supinated or associated talus fracture. Sustentaculum tali fractures are sometimes considered to be extra-articular fractures. A case of unilateral ball-and-socket ankle joint associated with short limb and hypoplastic sustentaculum tali is reported. We think that hypoplastic sustentaculum tali was the cardinal feature in the anatomical and kinematic alterations of the foot in this patient. We review the embryonic development of hindfoot to ascertain the possible origin of this unusual finding.

Anatomical reconstruction of displaced sustentaculum tali fractures via a direct medial approach. Displaced fractures of the sustentaculum tali with incongruity or depression of the medial facet of the subtalar joint, entrapment of the flexor hallucis longus or flexor digitorum longus tendons, fracture line extending into the posterior facet of the subtalar joint. Infected or grossly contaminated soft tissue, severely restricted vascular supply to the foot, high perioperative risk. Introduction: Lauge-Hansen classification for ankle fractures is helpful in directing the management of the fracture and has got prognostic significance. However, trimalleolar fractures with double medial malleolar fracture are not yet described. Case Report: Two cases of trimalleolar fracture-dislocation of the ankle with a double fracture of the medial malleolus are reported. Both of them had a Weber B fracture of the lateral malleolus, accompanied by a posterior dislocation of the ankle and a fracture of the posterior malleolus. The medial malleolus though presented an interesting variation, a large vertical or oblique fragment was combined with a small horizontal fragment of its tip. Conclusion: We propose a separate subtype in the Lauge-Hansen classification of supination with combined external rotation and adduction when this pattern of the medial malleolus is accounted. Awareness of this fracture pattern will help better pre-operative planning.

Introduction The presence of a posterior malleolar (PM) fragment has a negative prognostic impact in ankle fractures. The best treatment is still subject to debate. The aim of this study was to assess the medium-to-long-term clinical and functional outcome of ankle fractures with a PM fragment in a larger patient population. Materials and methods One hundred patients (69 women, 31 men, average age 60 years) with ankle fractures including the PM were evaluated clinically and radiographically. Patients with Bartoníček–Rammelt type 3 and 4 fracture displayed a significant female preponderance. Fixation of the PM was performed in 63% and tailored to the individual fracture pattern. Results Internal fixation of the PM fragment was negatively correlated with the need for syndesmotic screw placement at the time of surgery (p = 0.010).
