Colchicine Toxicity: A Patient With Pneumopericardium

Colchicine is a highly active alkoloid with antiinflammatory properties. It is effective in gout, BehetÕs disease, familial Mediterranean fever, cirrhosisi and may be effective in scleroderma, sarcoidosis and skin disorders. Colchicine overdose is a rare but serious problem. Gastrointestinal distress is the earliest and most common manifestation and myelosuppression, cardiovascular collapse and respiratory failure are common life-threatenning side effects (1, 2). We report a patient with colchicine toxicity who developed pneumopericardium during his clinical course. Pneumopericardium has not been reported before and should be considered in the case of colchicine overdose.

Multiorgan dysfunction due to colchicine

Colchicine is used in the treatment of many diseases. It should be used very carefully as the treatment dose and toxic dose are very close to each other and may lead to multiorgan failure. Although colchicine toxicity is usually seen in suicidal use, it may also occur when used in therapeutic doses. In this report, we present a case of a 70-year-old patient who used colchicine due to pericardial effusion over two months and died from multiorgan failure, to draw attention to colchicine toxicity due to using it in therapeutic doses.

Acute colchicine intoxication complicated with complete AV block

Colchicine, is an old and well-known drug, used for treatment of rheumatic diseases. Nausea, vomiting, abdominal pain, and diarrhea are the clinical symptoms of colchicine poisoning. Cardiotoxicity can lead to mortality. We report a case of colchicine intoxication complicated with complete heart block. A 9-year-old patient ingesting colchicine 0.4-0.5 mg/kg was transferred because of elevation of liver enzymes, and deterioration of kidney functions and cytopenia. History of colchicine ingestion had been unknown at time of admission. After initial fluid and electrolyte treatment electrolyte imbalance ameliorated but kidney and liver functions worsened. In the third day of admission (7th day of ingestion), she confessed taking colchicine pills. Her state of consciousness became comatose and endotracheal intubation required. ACUTE PANCREATITIS RELATED TO THERAPEUTIC DOSING WITH COLCHICINE: A CASE REPORT

Background:- Colchicine is used in the treatment and prophylaxis of gout. It possesses a narrow therapeutic window, frequently resulting in dose-limiting gastrointestinal side-effects such as diarrhoea and emesis. As colchicine is a cellular anti-mitotic agent, the most serious effects include myelosuppression, myoneuropathy and multiple organ failure. This occurs with intentional overdose or with therapeutic dosing in patients with reduced clearance of colchicine due to pre-existing renal or hepatic impairment. Acute pancreatitis has rarely been reported, and only in association with severe colchicine overdose accompanied by multi-organ failure. Case presentation:- We report a case of acute pancreatitis without other organ toxicity related to recent commencement of colchicine for acute gout, occurring in an elderly male with pre-existing renal impairment. Conclusion:- 1) Colchicine should be used with care in elderly patients or patients with impaired renal function. 2) Aside from myelosuppression, myoneuropathy and multiple organ failure, colchicine may now be associated with acute pancreatitis even with therapeutic dosing; this has not previously being reported.
