MOUTH DISSOLVING TABLETS: AS A POTENTIAL DRUG DELIVERY SYSTEM - A REVIEW Review Article

In the present scenario, there is an ever increasing demand for more patient compliant dosage forms. One of an important innovation in this direction is the development of mouth dissolving tablets that dissolve or disintegrate instantly upon contact with recipient’s tongue or buccal mucosa. They have proved to be ideal for geriatric and pediatric population, people suffering from dysphagia, clinical conditions where water intake is not available and for drugs undergoing high first pass metabolism. Now day’s formulation research is breaking barriers of conventional methods. Recently, MDTs have take over an important position in the market by overcoming previously administration problems and contributing to extension of patient life, which have difficulty in swallowing tablets and capsules. Upon introduction into the mouth, these tablets dissolve/ disintegrate in the mouth without additional water for easy administration of pharmaceutical ingredients. These dosage forms are also used to attain instant a higher concentration of drug in body for immediate actions. These are novel dosage forms which dissolve in mouth cavity within a few seconds. This article attempts at discussing ideal properties, advantages, limitation, choice of drug candidates, need of formulation,approaches for preparation of MDTs, Patented technologies on MDTs and Evaluation tests of MDTs.

Mouth Dissolving Tablet: A Better Approach to Drug Delivery

Researchers throughout the world are focusing intensively on the methods for the development of new drug delivery systems to enhance patient's compliance. The oral route however still remained as the best administration route of therapeutic agents for its ease of ingestion, pain avoidance and versatility. Hence, fast dissolving tablets become an emerging trend in the pharmaceutical industry. Fast dissolving tablets are ideal for all types of people, including for people who have swallowing difficulties, paediatric, geriatric, and bedridden patients. It is also for active patients who are busy, travelling and may not have access to water. Fast dissolving tablets are also known as orodispersible tablets, mouth-dissolving tablets, orally disintegrating tablets, melt-in mouth tablets, rapimelts, porous tablets, quick dissolving etc. This type of tablets disintegrates quickly once introduced into the mouth in the absence of additional water for easy administration of active pharmaceutical ingredients. In this review article, drug candidates suitable for fast dissolving drug delivery and the available marketed products have been listed.

Recent Advances in Manufacturing Technologies and Future Prospects of Mouth Dissolving Tablets (FDTs)

Due to the increasing demand of novel drug delivery, the fast disintegrating drug delivery system has become one of the mile stone in the novel drug delivery system. The introduction of fast dissolving drug delivery system has encountered the delivery of conventional dosage form. Recent advances in the formulation development and processing technologies meet the efforts to achieve more sophisticated drug delivery system. Mouth dissolving tablets need to be formulated for pediatric, geriatric, psychotic patients, bedridden and for those who are busy in travelling and may not have to assess to water. The drugs delivered in MDT’S may be absorbed in the pregastric sites of highly permeable buccal and mucosal tissues of the oral cavity and they may be suitable of low molecular weight and highly permeable drugs.

Mouth Dissolving Tablets-A Review

Oral delivery is currently the gold standard in the pharmaceutical industry where it is regarded to be safe, economical, easy to form and the most important is the patient compliance. Other various dosage forms are also available in the market which are used to prevent, care and treat the diseases. But the tablets are most popular among all the dosage forms existing because of its ease of administration, rigidness and patient compliance. But many patients express difficulty in swallowing tablets so to overcome this problem recent advances in drug delivery has expressed dosage forms which disintegrates rapidly when placed upon tongue usually within a matter of second's i.e in 60 secs. These are novel dosage forms which dissolve in saliva within a few seconds, when put on tongue. MDTs can be administered anywhere and anytime, without the need of water and are thus quite suitable for children, elderly and mentally disabled patients.

Mouth Dissolving Tablets (MDTs) have received ever-increasing demand during the last decade and the field has become a rapidly growing area of research in the pharmaceutical industry. Upon introduction into the mouth, these tablets dissolve or disintegrate in the mouth in the absence of additional water for easy administration of active pharmaceutical ingredients. The popularity and usefulness of the formulation resulted in development of several MDT technologies. This review describes various formulations and technologies developed to achieve fast dissolution/dispersion of tablets in the oral cavity. In particular, this review describes in detail MDT technologies based on lyophilization, moulding, sublimation and compaction, as well as approaches to enhance the MDT properties, such as spray drying, moisture treatment, sintering, and use of sugar-based disintegrants.

The demand for MDT (Mouth Disintegrating Tablet) has been increasing from the last decade particularly in geriatric, pediatric and patient with some sort of disabilities in swallowing. MDTs are those tablets which when placed in mouth get dissolved rapidly in saliva without the need of liquid and can be swallowed. European pharmacopoeia adopted the term Orodispersible tablet for MDTs. Mouth disintegrating tablets are also known as Fast melting tablets, Orodispersible tablets, fast dissolving/dispersing tablets or melt in mouth tablets. This article reviews the potential benefits offered by MDTs as an oral drug delivery system for various kinds of patients suffering from different diseases and disabilities. Desired characteristics and challenges for developing fast disintegrating drug delivery systems, quality control tests, various techniques used in the preparation of fast disintegrating drug delivery systems like lyophilization technologies, tablet molding method, sublimation techniques, spray drying techniques, mass extrusion technology, direct compression method and uses of super-disintegrates. It also reviews the patented technologies for fast dissolving tablets, advantages and disadvantages of different technologies for preparing fast disintegrating dosage form, future prospective for MDTs.
