BACKGROUND: The medical evacuation of premature newborns to institutions providing a higher level of medical care results in the reduction of death risk. The use of pediatric intensive care units in level 2 organizations for the hospitalization of newborns can be a potential solution to the lack of neonatal beds in level 3 institutions. INTRODUCTION: Assessment of the patient’s condition and prediction of outcomes is critically important during pre-transport stabilization and remains the most complex challenges of the activities of transport teams. A significant variety of scales and different requirements for their application indicates that there is no consensus on the choice of a specific scale and predictive tool. Introduction. Standardized assessment of the patient status and the decision on the possibility of transfer is an essential part of the neonatal transport service. However, there are no generally accepted tools for assessing severity and making decisions regarding newborn transfer. Objectives. BACKGROUND: Assessment of the clinical condition, prediction of risks and possible outcomes during the transfer of newborns remains an important part of the work of transport teams. Respiratory disorders remain a significant indication for transfer to medical organizations of a higher level of care. AIM: To study the predictive value of the parameters of respiratory support in newborns requiring medical evacuation for the outcomes of treatment. MATERIALS AND METHODS: The observational, cohort, retrospective study included data from neonatal to patients on ventilators (286 newborns) in the period from August 1, 2017 to December 31, 2018. Цель. Сравнение информационной ценности шкал Quick Sequential (SepsisRelated Organ Failure Assessment (qSOFA), National Early Warning Score (NEWS) и критериев Systemic Inflammatory Response Syndrome (SIRS) в прогнозе неблагоприятных исходов у пациентов приемного отделения в зависимости от наличия инфекции на примере многопрофильного стационара в России. Материалы и методы. Проспективное наблюдательное исследование. Проанализированы данные 270 пациентов, из них 132 (48,8%) с инфекцией и 138 (51,2%) без инфекции. Выполнено сравнение площадей под ROCкривыми (Receiver Operating Characteristic Curve) шкал qSOFA, NEWS и крите риев SIRS в прогнозе комбинированного исхода (летальный исход и/или пребывание в отделении реанимации и интенсивной терапии >1 суток). Результаты. Комбинированный исход наступил у 24 (8,8%) пациентов. В прогнозе данного исхо да в группе пациентов без инфекции площадь под ROCкривой шкалы NEWS (0,908 [95% ДИ 0,847-0,951]) статистически значимо не различалась с площадью под ROCкривой шкалы qSOFA (0,839 [95% ДИ 0,767-0,896]), p = 0,255, и статистически значимо превосходила площадь под ROCкривой критериев SIRS (0,776 [95% ДИ 0,698-0,843]), p = 0,011. Между площадями под ROCкривыми шкалы qSOFA и критериев SIRS статистически значимого различия не выявлено (p = 0,379). В группе пациентов с инфекцией площадь под ROCкривой шкалы NEWS (0,808 [95% ДИ 0,731-0,872]) статистически значимо превосходила площади под ROCкривыми шкал qSOFA (0,715 [95% ДИ 0,630-0,790]), p = 0,004, и критериев SIRS (0,641 [95% ДИ 0,553-0,722]), p = 0,015. Различие между площадями под ROCкривыми шкалы qSOFA и критериев SIRS было статистически не значимым (p = 0,286). Выводы. Шкала NEWS в сравнении со шкалой qSOFA и критериями SIRS имеет наибольшую ин формационную ценность в прогнозе неблагоприятного исхода у пациентов приемного отделения независимо от наличия у них инфекционного очага. В группе пациентов с инфекцией преимущество шкалы NEWS над шкалой qSOFA более очевидно. Прогностическая ценность шкалы qSOFA и кри териев SIRS сравнима в обеих группах пациентов.

Aim. To develop a predictive model to calculate the probability of placental abruption development in patients with initially low obstetric risk by using a multidimensional mathematical analysis of anamnestic, clinical, and genetic data. Materials and methods. This logistic model is based on 212 observations. This study included two groups of healthy women aged 1835 years without a history of spontaneous abortion, preterm labor, stillbirth, preeclampsia, or other complications.
