Objectives: Anesthesiology and Algology healthcare workers work under difficult conditions a majo... more Objectives: Anesthesiology and Algology healthcare workers work under difficult conditions a majority of the time, and their physical and mental status must be determined in order to improve working conditions. In this study, the main goal was to evaluate the burnout level of Anesthesiology and Algology healthcare workers in the Middle Anatolian region of Turkey. Methods: A questionnaire was sent to Anesthesiology and Algology healthcare workers of hospitals with an Algology clinic in the Middle Anatolian region and were returned by e-mail in March and April 2008. In the questionnaire, descriptive features and the Turkish validated Maslach burnout inventory (MBI) were evaluated. The MBI has 3 aspects: emotional exhaustion (EE), depersonalization (D) and personal accomplishment (PA). All questionnaires were evaluated by SPSS 11.5 program. Results: 113 people were enrolled, of whom 18.8% (21) were specialist doctors and 41.1% (46) were residents. Among the doctors, the mean daily and weekly working periods were 10.3+/-2.3 and 61.3+/-19 hours, respectively. The period spent by doctors in Algology was 100% in 9.6%, 75% in 9.7% and 25% in 61.5%. Working conditions were evaluated, and 20.9% of healthcare workers were unsatisfied with the physical conditions, 19.3% with the working period and 52.5% with wages. MBIs in doctors were calculated as EE 14.7+/-5, D 5.7+/-3.5, and PA 21.6+/-4.2. Conclusion: This study revealed similar high burnout scores among healthcare workers, which reflect a serious burnout in the Anesthesiology and Algology group in Turkey. Burnout may be affected by dissatisfaction with working conditions. Improvement in physical conditions and reorganization to achieve psychological support might be helpful towards improving the health of healthcare workers.

Mitochondrial transfer can be summarized as the process of transferring isolated mitochondria to the damaged tissue. In this way, it is aimed to improve the mitochondrial dysfunction in areas with impaired mitochondrial functions such as heart damaged tissue. Although there are many studies on this subject, especially cardiomyocytes, the protective effects of the application in processes such as myocardial ischemia and reperfusion injury continues to be investigated. Although there are different procedures for transferring the isolated mitochondrin to the damaged tissue, many studies have reported positive results regarding the application. In this review, it is aimed to look at the subject from a wide window, while examining the studies done in this field in the literature.

Aim: Foreign body aspirations are a serious respiratory illness that can threaten life, particula... more Aim: Foreign body aspirations are a serious respiratory illness that can threaten life, particularly in childhood, in the first three years of life. We aimed to compare the qualitative changes and their results of the asphyxiated foreign bodies of the first five-year-old children we encountered in the tropical region with literature. Material and Method: In Nyala Sudan Turkey Education and Research Hospital, 11 patients in the range of 0-5 years between the dates of 04/11/2018 and 05/30/2018 with history of foreign body aspiration was accepted after a physical examination and radiological imaging with a preliminary diagnosis. All patients underwent rigid bronchoscopy under general anesthesia, foreign bodies detected were removed by using alligator forceps with no significant complication. Results: The youngest patient was 9 months old while the oldest was 5 years old and the average age was 2.43. Foreign bodies were removed by rigid bronchoscopy in 5 (45.45%) patients from the right main bronchus, 2 (18.18%) in the left main bronchus and 4 (36,36%) in the trachea. Nine (81,81%) of the extracted foreign bodies were organic (bread, peanut, papaya, grasshopper, mango) and 2 (18,18%) were inorganic (stone, battery). Conclusion: In advanced countries, it is easy to reach parents and raise awareness about the subject, but this is a difficult situation in regions where socioeconomic level is low, communication and economic problems are experienced. Foreign body aspirations may differ in the subtropic belt due to many challenging conditions, but there is no difference in the complications that can be encountered at the end.

Tracheal injury is a rare pathology, but 30% of all cases can be fatal and 50% of those patients ... more Tracheal injury is a rare pathology, but 30% of all cases can be fatal and 50% of those patients will deteriorate in a couple of hours and may lead to death. The course of treatment must be scheduled by clinical status and severity of pathology; conservative approaches can be followed for minor injuries, but major surgeries must be performed for advanced cases. A 28-year-old male, who suffered from caught with a lasso around the neck in a traffic accident, was brought to our emergency service from a distant medical facility. It has been observed that tracheal integrity was lost, subcutaneous and mediastinal emphysema were evident, and bilateral pneumothorax was significant, especially at right with thorax computed tomography. His clinical condition was bad. Tube thoracostomy placement was successfully achieved from the right side, and neck exploration was performed for tracheostomy in operating theater; a total tracheal rupture was observed between the level of 2 amd 3 cricoid cartilage, and the procedure was terminated after placing a tracheostomy cannula to the distal segment of trachea to wait for considering reconstruction following clinical progression and hemodynamic stabilization of the patient. Following the 7 th day after the first operation, the patient was operated. The trachea was repaired by suturing using 3-0 polypropylene sutures. The patient was discharged after 12 h of intensive care and 5 days of follow-up in surgery service. Blunt tracheal injuries may not present any symptoms or signs but may also manifest dramatically. Mortality and morbidity rates may be drastically higher. For this reason, physical examination, radiological findings, and bronchoscopy should be performed early in diagnosis and the most appropriate treatment option that can prevent mortality should be decided quickly.

Background: The scar tissue formed by episiotomy during vaginal delivery, and the related pain, i... more Background: The scar tissue formed by episiotomy during vaginal delivery, and the related pain, is very frequent. The change in the normal anatomy can cause cosmetic and physiologic problems. It can affect and cause deterioration in sexual functions. Therefore, making the right diagnosis and applying the right surgical procedures are very important. Aims: Our aim was to examine the effect of the perineoplasty operation on the sexual dysfunctions that present due to vaginal delivery. Study Design: Self-controlled study. Methods: Forty patients, who attended our clinic between April 2012 and May 2013, and who were between the ages of 20 and 50 years, were included in the study. The patients had complaints of scar tissue in the perineum and various sexual dysfunctions after vaginal delivery, and they were suitable for perineoplasty. The Female Sexual Function Index (FSFI) questionnaire was applied to the patients before and 6 months after the operation, and the results were compared. Results: After the perineoplasty operation, there was a statistically significant improvement in the patients in the domains of sexual desire, arousal, lubrication, orgasm, and sexual satisfaction (p<0.005). However, there was no significant improvement in the feeling of pain during sexual intercourse (p=0.184). The mean±SD total FSFI score increased significantly after the operation (p<0.005). Conclusion: The sexual dysfunctions that develop due to perineal damage during vaginal delivery can benefit significantly from the perineoplasty operation if the indications are correct. However, vaginal perineoplasty did not provide an improvement in dyspareunia.

Penetrating thoracic injuries are risk factors for high mortality and morbidity, so they need to be diagnosed correctly and timely in order to perform an effective intervention. In this study, patients with penetrating thoracic trauma were treated through surgical intervention accompanying minimal morbidity–mortality compared to literature, even though they were admitted relatively late. Methods: In this retrospective study, 29 gunshot and stab wounds penetrating thoracic trauma patients were admitted to the emergency department of Nyala Sudan Turkey Education and Research Hospital between April and September 2018. Results: Of the 29 patients, 7 (24.13%) were gunshot injuries and 22 (75.86%) were injured with a sharp cutting tool. Objective: In smokers, COVID-19 may have a more severe course due to impaired mucociliary activit... more Objective: In smokers, COVID-19 may have a more severe course due to impaired mucociliary activity, increased permeability, and inflammation of the airway epithelium. However, data on the impact of smoking in patients with COVID-19 pneumonia are conflicting. The study aimed to evaluate the effects of smoking on laboratory parameters, intensive care unit (ICU) length of stay, length of hospital stay, and mortality in COVID-19 patients hospitalized in the intensive careunit. Methods: Medical records of 576 patients who were followed up in the ICU for COVID-19 between January and September 2021 were analyzed retrospectively. Demographic data, comorbidities, laboratory parameters (hemoglobin, white blood cell (WBC), lymphocyte, neutrophil, thrombocyte, AST, ALT, CRP, D-dimer, ferritin, BNP, albumin), ICU and hospital stay of the patients were recorded. The patients were divided into 2 groups according to whether they smoked or not. Results: The data of 576 patients were evaluated in the study. The mean age of the patients was 69±14.8 years, and 53.8% were male. The smoking rate was 30%, and the smoking rate in men was 76.9%. The intensive care unit admission age of smokers was lower than non-smokers (p=0.01). The presence of coronary artery disease (CAD), chronic obstructive pulmonary disease (COPD), and malignancy was significantly higher in smokers. Among the laboratory parameters, white blood cell count (WBC) and neutrophil were higher in smokers (p=0.01). There was no relationship between ICU length of stay, length of hospital stay, and mortality in smokers (p=0.769, p=0.699, p=0.852, respectively). Conclusion: We did not find any significant association between smoking and COVID-19 mortality. We recommend clinicians to monitor WBC and neutrophil count closely as markers of possible progression to critical illness in patients hospitalized in the ICU due to smoking COVID-19.

Bir çok konjenital anomalinin bulunduğu bu sendrom mental retardasyon, kraniyofasiyal deformiteler ve parmak anomalileri ile karakterizedir. Bu sendroma sahip çoçuklarda diş problemleri nedeniyle genel anestezi veya sedasyon altında girişim gerekebilir. Ondört yaşında RTS tanısı olan ve dental tedavi amacıyla Gazi Üniversitesi Tıp Fakültesi Diş Hekimliği Fakültesine başvuran erkek çocukta yapılan ilk değerlendirmede mikrosefali, retrognati, geniş el başparmağı ve kooperasyon kısıtlılığı saptandı. Mallampati skoru II olarak değerlendirildi. İntravenöz kanulasyon ve standart monitörizasyonu takiben, nazal maske ile spontan solunumu koruyacak şekilde O2/N2O (%50/50) uygulanmaya başlandı ve gerekli sedasyon düzeyine ulaşmak amacıyla ketamin ve midazolam verildi. Her ne kadar olgumuzda derin sedasyon uygulaması herhangi bir komplikasyon gelişmeksizin uygulanmış olsa da, zor havayolu olasılığı göz önüne alınarak algoritmalarda yer alan tüm önerilerin uyulması gerektiğini düşünmekteyiz.

There are many reports and reviews about foreign objects in various body parts, but the thoracic ... more There are many reports and reviews about foreign objects in various body parts, but the thoracic wall foreign objects are rarely seen, and there is no treatment consensus. In this study, we aimed to investigate the treatment of three patients who were admitted to the emergency department due to a thoracic gunshot wound. In their follow-up, they survived with a bullet in their thoracic wall, and there were no further thoracic complications associated. Details of the hospital records of the three patients referred to the Nyala Sudan-Turkey Training and Research Hospital, with foreign bodies-bullets or shrapnel-in the thoracic wall were reviewed, retrospectively. The treatment of thoracic wall foreign bodies is still controversial and as a result, small, noninfectious, noncomplicated thoracic wall foreign bodies can be followed, after consulting with the patients.

Among the laboratory parameters, white blood cell count (WBC) and neutrophil were higher in smokers (p=0.01). There was no relationship between ICU length of stay, length of hospital stay, and mortality in smokers (p=0.769, p=0.699, p=0.852, respectively). Conclusion: We did not find any significant association between smoking and COVID-19 mortality. We recommend clinicians to monitor WBC and neutrophil count closely as markers of possible progression to critical illness in patients hospitalized in the ICU due to smoking COVID-19.

We aimed to compare the qualitative changes and their results of the asphyxiated foreign bodies of the first five-year-old children we encountered in the tropical region with literature. Material and Method: In Nyala Sudan Turkey Education and Research Hospital, 11 patients in the range of 0-5 years between the dates of 04/11/2018 and 05/30/2018 with history of foreign body aspiration was accepted after a physical examination and radiological imaging with a preliminary diagnosis. All patients underwent rigid bronchoscopy under general anesthesia, foreign bodies detected were removed by using alligator forceps with no significant complication. Results: The youngest patient was 9 months old while the oldest was 5 years old and the average age was 2.43. Foreign bodies were removed by rigid bronchoscopy in 5 (45.45%) patients from the right main bronchus, 2 (18.18%) in the left main bronchus and 4 (36,36%) in the trachea. Nine (81,81%) of the extracted foreign bodies were organic (bread, peanut, papaya, grasshopper, mango) and 2 (18,18%) were inorganic (stone, battery). Conclusion: In advanced countries, it is easy to reach parents and raise awareness about the subject, but this is a difficult situation in regions where socioeconomic level is low, communication and economic problems are experienced. Foreign body aspirations may differ in the subtropic belt due to many challenging conditions, but there is no difference in the complications that can be encountered at the end.

Bir çok konjenital anomalinin bulunduğu bu sendrom mental retardasyon, kraniyofasiyal deformiteler ve parmak anomalileri ile karakterizedir. Bu sendroma sahip çoçuklarda diş problemleri nedeniyle genel anestezi veya sedasyon altında girişim gerekebilir. Ondört yaşında RTS tanısı olan ve dental tedavi amacıyla Gazi Üniversitesi Tıp Fakültesi Diş Hekimliği Fakültesine başvuran erkek çocukta yapılan ilk değerlendirmede mikrosefali, retrognati, geniş el başparmağı ve kooperasyon kısıtlılığı saptandı. Mallampati skoru II olarak değerlendirildi. İntravenöz kanulasyon ve standart monitörizasyonu takiben, nazal maske ile spontan solunumu koruyacak şekilde O2/N2O (%50/50) uygulanmaya başlandı ve gerekli sedasyon düzeyine ulaşmak amacıyla ketamin ve midazolam verildi. Her ne kadar olgumuzda derin sedasyon uygulaması herhangi bir komplikasyon gelişmeksizin uygulanmış olsa da, zor havayolu olasılığı göz önüne alınarak algoritmalarda yer alan tüm önerilerin uyulması gerektiğini düşünmekteyiz.

There are many reports and reviews about foreign objects in various body parts, but the thoracic ... more There are many reports and reviews about foreign objects in various body parts, but the thoracic wall foreign objects are rarely seen, and there is no treatment consensus. In this study, we aimed to investigate the treatment of three patients who were admitted to the emergency department due to a thoracic gunshot wound. In their follow-up, they survived with a bullet in their thoracic wall, and there were no further thoracic complications associated. Details of the hospital records of the three patients referred to the Nyala Sudan-Turkey Training and Research Hospital, with foreign bodies-bullets or shrapnel-in the thoracic wall were reviewed, retrospectively. The treatment of thoracic wall foreign bodies is still controversial and as a result, small, noninfectious, noncomplicated thoracic wall foreign bodies can be followed, after consulting with the patients.
