Pleomorphic adenoma is one kind of tumor which is the most common benign tumor of major salivary ... more Pleomorphic adenoma is one kind of tumor which is the most common benign tumor of major salivary gland tumors. And can occur in the minor salivary gland tissue that can be found in the nasal cavity. Intranasal pleomorphic adenomas are usually formed by the mucous glands of the nasal septum. We found the pathology result of endoscopic surgery. The purpose of this study was to demonstrate an endoscopic approach to surgery for pleomorphic adenoma of the nasal septum. A 34-year-old woman presented with complaints of nasal congestion and occasional bleeding for at least 3 years. We provide treatment with endoscopic surgery to remove the tumor. In this case we ressected the tumor and also the submucous part of the nasal septal to reach tumor-free margins. Microscopic examination shows epithelial tissue, mesenchymal composed of myxoid stroma and also myoepithelial component with H&E staining. The postoperative follow-up of the patient was good, and there were no complications found in the patient. Benign nasal tumors of the minor salivary glands or benign pleomorphic adenomas of the nose are rare tumors and should be considered in their management. A correct histological diagnosis can be confirmed. Treatment for this disease can be done endoscopically endonasal surgery.

BACKGROUND: Abnormal uterine bleeding (AUB) is one of the clinical symptoms found in malignant co... more BACKGROUND: Abnormal uterine bleeding (AUB) is one of the clinical symptoms found in malignant conditions where the incidence of newly diagnosed cancers reached 100,000 cases in Europe in 2012 with an incidence of around 14.7/100,000 women. The International Endometrial Tumor Analysis (IETA) group, formed in Chicago in 2008, published a consensus stating how to examine and measure the endometrium. CASE REPORT: Mrs. M, 49 years old, came to Gynecology Clinic of USU Hospital on February 22, 2021, at 13.00 WIB with the complaints of prolonged menstruation for 2 months with a volume of >10 times changing pads. Menstrual pain was found. Low back pain was found. A history of bleeding between menstrual cycles was found. On ultrasound examination, anteflexed uterus was found with size of 6.95 × 4.2 × 3.85 cm, thickness of the endometrium: 1.64 cm and concluded as AUB-M (thickening of the endometrium). Then, the patient is diagnosed with AUB-M (thickening of the endometrium) and is planned to undergo a diagnostic curettage. Anatomical pathology results showed a complex hyperplasia endometrium without atypia cells. Acute AUB is defined as bleeding profusely so that prompt treatment is needed to prevent blood loss. Ultrasound examination is performed to assess the size, shape of the uterus, the presence of fibroids, polyps, adenomyosis, and uterine anomalies such as uterine didelphys, sometimes polycystic ovaries are also found. Endometrial biopsy can detect more than 90% of cancers. The pathology of the endometrium can diagnose endometrial cancer or determine the likelihood of cancer. These investigative modalities can assist in the diagnosis of endometrial polyps, adenomyosis, leiomyomas, uterine anomalies, and endometrial thickening associated with hyperplasia and malignancy. Curettage is considered to relieve ongoing menorrhagia. According to the SOGC, administration of nonsteroidal anti-inflammatory drugs can inhibit cyclo-oxygenase and reduce levels of endometrial prostaglandins. CONCLUSION: According to IETA, the vascular pattern in the endometrium is reported to be associated with the presence or absence of a "dominant vessel" or other specific pattern. Endometrial thickness is the maximum measurement in the sagittal plane. The accompanying ultrasound provides the measurement of endometrial thickness in the absence of intracavity fluid; the endometrium should be measured where it appears thickest. If intracavity pathology is present, total thickness of endometrium including the lesion should be recorded. Anatomic pathology by curettage is required in women with abnormal bleeding; histological evaluation of the endometrium may identify infectious or neoplastic lesions such as endometrial hyperplasia or cancer.

The purpose of this review is to seek from the literature regarding the correct procedures for gynecological examinations in children and adolescents, the order of examinations, and how to carry out a good examination so as not to leave a traumatic experience for children.

Ob... more Background: Chronic exposure of volatile substances can cause the impaired olfactory function. Objective: To identify the difference of olfactory function at Gas Station workers in Medan city. Method: Cross-sectional analitic study. A total of 80 persons consisting of 40 gas station operators and 40 non-operators who fulfilled the inclusion criteria. The applied statistical analysis was Mann Whitney, Fisher's Exact, Chi Square, T independent, Spearman with 95% confidence level. Result: Proportion of the impaired olfactory function in the gas station operator group by 77.5% and non-operator group by 0%. Significant correlation was obtained between age, duration of work with the impaired olfactory function (p = 0,047, p = 0,001). The gas station operators will suffer the impaired olfactory function if they works for ≥ 3.89 years. Conclusion: There were differences of olfactory functions on the gas station workers by using Sniffin 'Sticks Test.

Sinonasal tumor is a neoplasm that arises from nasal and paranasal sinus tissue. It is rarely fou... more Sinonasal tumor is a neoplasm that arises from nasal and paranasal sinus tissue. It is rarely found, only <3% of all malignancy in head and neck, and 1% of all malignancy in the body. Although it is rarely found, it can cause serious complication in adult. Unspecific sign and symptoms of sinonasal tumor often make the patients ignore the disease. It leads to the diagnosis of this disease becomes late and the patients come to the doctor in advanced stage. This research was conducted to know about the profile of sinonasal tumor patients in H. Adam Malik Medan Hospital on 2016-2018. The study was a descriptive study with retrospective approach. The data used are secondary data taken from medical records. The data were analyzed using a statistical application programs. Sinonasal tumor patients in RSUP HAM Medan are 158 patients, where 95 patients met the inclusion criteria, consisting of 24 benign tumors (25,3%) and 71 malignant tumors (74,7%). The majority of patients are male (71,6%), and the highest age group in in 51-60 years old (31,6%). The main complaints felt by the patients were nasal symptoms (64,2%), the location of tumors mostly found in nasal cavity (50,5%), the most histopathological type is NKSCC (43,2%), the most occupation is self-employment (26%), and the treatment of the patients are surgery (29,5%).

AIM: The objective of the study was to report the endometriosis case and surgical technique to re... more AIM: The objective of the study was to report the endometriosis case and surgical technique to reduce the diminished ovarian reserve risk. CASE PRESENTATION: A 32-year-old woman was presented with the lower abdominal pain since 1 year ago, intermittent, and got worse during menstruation. The mobile cystic mass was palpable on the abdomen without pain during palpation. Vaginal examination showed mobile and smooth surface cystic mass was palpable, the upper pole was three fingers below the umbilicus, and lower pole was in line with pubic symphysis. Transvaginal ultrasonography (TVUS) showed uterus and right ovary enlargement; and unilocular cyst with homogenous echo pattern. There was a marked increase in CA-125. Based on International Tumor Analysis, the tumor was classified as B1 unilocular cyst and the malignancy risk index was 600. BACKGROUND: Abnormal uterine bleeding (AUB) is one of the clinical symptoms found in malignant co... more BACKGROUND: Abnormal uterine bleeding (AUB) is one of the clinical symptoms found in malignant conditions where the incidence of newly diagnosed cancers reached 100,000 cases in Europe in 2012 with an incidence of around 14.7/100,000 women. The International Endometrial Tumor Analysis (IETA) group, formed in Chicago in 2008, published a consensus stating how to examine and measure the endometrium. CASE REPORT: Mrs. M, 49 years old, came to Gynecology Clinic of USU Hospital on February 22, 2021, at 13.00 WIB with the complaints of prolonged menstruation for 2 months with a volume of >10 times changing pads. Menstrual pain was found. Low back pain was found. A history of bleeding between menstrual cycles was found. On ultrasound examination, anteflexed uterus was found with size of 6.95 × 4.2 × 3.85 cm, thickness of the endometrium: 1.64 cm and concluded as AUB-M (thickening of the endometrium). Then, the patient is diagnosed with AUB-M (thickening of the endometrium) and is planned to undergo a diagnostic curettage. Anatomical pathology results showed a complex hyperplasia endometrium without atypia cells. Acute AUB is defined as bleeding profusely so that prompt treatment is needed to prevent blood loss. Ultrasound examination is performed to assess the size, shape of the uterus, the presence of fibroids, polyps, adenomyosis, and uterine anomalies such as uterine didelphys, sometimes polycystic ovaries are also found. Endometrial biopsy can detect more than 90% of cancers. The pathology of the endometrium can diagnose endometrial cancer or determine the likelihood of cancer. These investigative modalities can assist in the diagnosis of endometrial polyps, adenomyosis, leiomyomas, uterine anomalies, and endometrial thickening associated with hyperplasia and malignancy. Curettage is considered to relieve ongoing menorrhagia. According to the SOGC, administration of nonsteroidal anti-inflammatory drugs can inhibit cyclo-oxygenase and reduce levels of endometrial prostaglandins. CONCLUSION: According to IETA, the vascular pattern in the endometrium is reported to be associated with the presence or absence of a "dominant vessel" or other specific pattern. Endometrial thickness is the maximum measurement in the sagittal plane. The accompanying ultrasound provides the measurement of endometrial thickness in the absence of intracavity fluid; the endometrium should be measured where it appears thickest. If intracavity pathology is present, total thickness of endometrium including the lesion should be recorded. Anatomic pathology by curettage is required in women with abnormal bleeding; histological evaluation of the endometrium may identify infectious or neoplastic lesions such as endometrial hyperplasia or cancer.

Ob... more Background: Chronic exposure of volatile substances can cause the impaired olfactory function. Objective: To identify the difference of olfactory function at Gas Station workers in Medan city. Method: Cross-sectional analitic study. A total of 80 persons consisting of 40 gas station operators and 40 non-operators who fulfilled the inclusion criteria. The applied statistical analysis was Mann Whitney, Fisher's Exact, Chi Square, T independent, Spearman with 95% confidence level. Result: Proportion of the impaired olfactory function in the gas station operator group by 77.5% and non-operator group by 0%. Significant correlation was obtained between age, duration of work with the impaired olfactory function (p = 0,047, p = 0,001). The gas station operators will suffer the impaired olfactory function if they works for ≥ 3.89 years. Conclusion: There were differences of olfactory functions on the gas station workers by using Sniffin 'Sticks Test.

Sinonasal tumor is a neoplasm that arises from nasal and paranasal sinus tissue. It is rarely fou... more Sinonasal tumor is a neoplasm that arises from nasal and paranasal sinus tissue. It is rarely found, only <3% of all malignancy in head and neck, and 1% of all malignancy in the body. Although it is rarely found, it can cause serious complication in adult. Unspecific sign and symptoms of sinonasal tumor often make the patients ignore the disease. It leads to the diagnosis of this disease becomes late and the patients come to the doctor in advanced stage. This research was conducted to know about the profile of sinonasal tumor patients in H. Adam Malik Medan Hospital on 2016-2018. The study was a descriptive study with retrospective approach. The data used are secondary data taken from medical records. The data were analyzed using a statistical application programs. Sinonasal tumor patients in RSUP HAM Medan are 158 patients, where 95 patients met the inclusion criteria, consisting of 24 benign tumors (25,3%) and 71 malignant tumors (74,7%). The majority of patients are male (71,6%), and the highest age group in in 51-60 years old (31,6%). The main complaints felt by the patients were nasal symptoms (64,2%), the location of tumors mostly found in nasal cavity (50,5%), the most histopathological type is NKSCC (43,2%), the most occupation is self-employment (26%), and the treatment of the patients are surgery (29,5%).

CASE PRESENTATION: A 32-year-old woman was presented with the lower abdominal pain since 1 year ago, intermittent, and got worse during menstruation. The mobile cystic mass was palpable on the abdomen without pain during palpation. Vaginal examination showed mobile and smooth surface cystic mass was palpable, the upper pole was three fingers below the umbilicus, and lower pole was in line with pubic symphysis. Transvaginal ultrasonography (TVUS) showed uterus and right ovary enlargement; and unilocular cyst with homogenous echo pattern. There was a marked increase in CA-125. Based on International Tumor Analysis, the tumor was classified as B1 unilocular cyst and the malignancy risk index was 600.
