The PSA is a probable prostate cancer tumor marker, although it cannot distinguish prostate cancer from BPH solely. To assess the usefulness of free, total, and free/total PSA ratios in differentiating BPH and prostate cancer has been studied under this research work. The work was carried out at RMCH during April 2021 to March 2022, 350 purposively selected urology outpatients and inpatients at RMCH were studied cross-sectional study. Trans-rectal ultrasound (TRUS) and serum PSA levels were analyzed along with Digital rectal examination (DRE). A digital guided prostatic biopsy using a monopty biopsy gun confirmed the diagnosis. Data analysis was done in SPSS 26 software program. Prostatic cancer was more prevalent in men aged 70-80, whereas BPH was more common in men aged 60-70 (p = 0.00032). Histopathology revealed 95 prostate cancer patients, 82 with PSA >4 ng/ml and 85 patients had f/t PSA ratio>0.16. BPH was found in 255 individuals. 139 BPH patients had total PSA >4 ng/ml and 245 BPH patients had f/t PSA ratio ≤0.16. Total PSA had sensitivity and specificity of 86.32% and 45.49%, respectively, lower than f/t ratio. The (Positive predictive value) PPV and accuracy was significantly higher in f/t ratio than total PSA. Along with total PSA, free and free/total PSA ratio may help detect prostate cancer early and distinguish BPH from prostate cancer.

In patients with NS, other proteins like thyroid hormones and hormone binding protein are also lost in urine in addition to albuminuria. So, this study was done to find out the thyroid function status of nephrotic syndrome children and to compare the thyroid function of healthy control children. Method: This cross sectional comparative study was carried out in the Department of Paediatrics, Rajshahi Medical College and Nuclear Medicine Centre, Rajshahi. Total 30 patients of both sex and age between 2-12 years diagnosed as nephrotic syndrome were included in this study and another 30 non-nephrotic syndrome apparently healthy children were taken as control. History, clinical examinations and laboratory investigations were recorded in preformed questionnaire. Thyroid function statuses also were evaluated in both patient and healthy control. After data collection, statistical analysis was done by computerized software. Results: In this study, 63.3% (n=19) nephrotic syndrome children had secondary hypothyroidism, overt hypothyroidism was 27.3% (n=7) and subclinical hypothyroidism was 6.7 % (n=2). Conclusion: In patients of childhood nephrotic syndrome different forms of thyroid dysfunctions are commonly associated due to loss of thyroid hormones and hormone binding's protein.

Introduction: Menstruation-related poor knowledge and understanding among adolescent girls may le... more Introduction: Menstruation-related poor knowledge and understanding among adolescent girls may lead to unsafe personal health care practice that ultimately increases the risk of genitourinary tract infections, drop-out from school, poor academic performance, and overall poor quality of life. Being a developing country, it is an unrecognized problematic area in Bangladesh. Therefore, it is crucial to assess knowledge and practice on menstrual hygiene among urban adolescent girls in Bangladesh. Methods: This was a community-based cross-sectional study conducted in the Department of Community Medicine of Rajshahi Medical College among 495 purposively selected urban adolescent girls. Data were collected using a semi-structured interviewer-administered questionnaire. Results: The majority (57.2%) of the respondents in the study were more than 15 years old, and most (93.3%) were Muslim. Most of the girls (82.5%) have experienced menarche within the age range of 12-15 years, and the majority of respondents, 259 (52.32%), were informed about menstruation before menarche. Their source of information about menarche was mothers in most cases. Out of 495 adolescent girls who participated in this study, 70.9% had good knowledge of menstruation. About 82% of girls used commercially made sanitary pads, and 35.2% changed pads or clothes more than three times a day during menstruation. Conclusion: The majority (70.9%) of adolescent girls had good knowledge of menstruation, and 73.3% had a good practice on menstrual hygiene. Therefore, there is a need to design and implement universal awareness creation and advocacy programs to improve their knowledge of safe and healthy menstruation management and promote good hygienic practices by creating an appropriate supportive home and social environment for all the adolescent girls.

Like other countries, coronary heart disease prevalence is also rising among the adults in Bangladesh. Besides conventional risk factors for coronary heart disease, genetic predisposition and some novel issues like high salt intake, arsenicosis, hypovitaminosis D and air pollution may play important role in the aetiopathogenesis of coronary heart disease in this population. Formulation of appropriate policy and more emphasis on preventive strategy may help combat coronary heart disease in Bangladesh. The main aim of this study was to find out the pattern of coronary heart disease among the patients admitted in Cardiology department of Rajshahi Medical college Hospital and to relate their socio-demographic and economic characteristics with the disease. It was a cross sectional type of descriptive study during February 2019 to July 2019. A total of 352 patients of Cardiology department considered as sample who admitted for their heart problem during the study period selected by purposive sampling. Majorities (85.8%) of the respondents were >40 years among them 74.4% were male. Most (52.0%) of the respondents were from nuclear family and smokers (65.62%). They had history of diabetes mellitus (22.4%) and lead sedentary life (11.4%). Regarding coronary heart disease 74.7% had ischemic heart disease and right coronary artery disease (66.7%). About 42.1% were hypertensive patients and 94.0% took medical treatment. Type of heart disease was found significantly associated with age group of the respondents (p<0.01), smoking habit (p<0.05) and history of abuse substances currently (p<0.05). High coronary heart disease prevalence along with an upward trend was observed in Bangladeshi adults and older age. Proper strategies are required for primary prevention of coronary heart disease so that a further increase can be alleviated and the morbidity and mortality associated with it can be reduced.

It has been suggested that a birthweight limit of 2.5 kg should not be regarded as valid for all ... more It has been suggested that a birthweight limit of 2.5 kg should not be regarded as valid for all populations as the cutoff point of low-weight births because of demographic, genetic and environmental differences. Countries often choose alternative cutoff values for low birthweight for clinical purposes. Bangladesh also needs to choose a convenient cutoff value for low birthweight. A total of 770 live singleton full-term normal newborns were included in this study by stratified sampling; birthweight was measured using the Detecto-type baby weight machine. Newborns were followed up to the end of their first week of life. For data collection a pretested structured questionnaire and an Apgar Score estimating checklist were used. Chi-square test was applied to assess the association of different birthweight strata and neonatal health outcomes. Multiple logistic regression analyses were carried out to identify the independent effects of different levels of birthweight on early neonatal health. The neonates having birthweight Յ2 kg had a significantly higher risk of early neonatal mortality and morbidity than the higher level birthweight group. Birth asphyxia was the commonest cause of early neonatal mortality and morbidity. Borderline birthweight (>2 to <2.5 kg) neonates experienced the same mortality and morbidity rates as the normal birthweight neonates during their early neonatal life. Birthweight Յ2 kg may be one of the criteria for admission to a neonatal intensive care unit whereas more than 2 kg may not require admission unless otherwise necessary.

COVID-19 displays variable clinical presentations. The severe COVID-19 represents a fulminant pathological condition and most of the patients run a downhill course if extensive medical measures are not adopted. The major challenges about COVID-19 are related to develop strategies to manage huge populations of mild and moderate cases of COVID-19 with two realistic purposes: (1) early negativity of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus and (2) arrest of progression of moderate COVID-19 patients from developing severe complications. Although several medications have been repurposed for these purposes, none of these have passed the test of time in global perspective. Thus, there remains a pressing need to develop new and novel innovative management strategies for these patients as new variants of SARS-CoV-2 have been destroying the normal public health delivery system of different countries from time to time. The study presented here has checked the safety and efficacy of a herbal medication, leaves of Euphorbia neriifolia Linn (E. neriifolia), in mild and moderate COVID-19 patients. Sixty patients (30 mild COVID-19 and 30 moderate COVID-19) were enrolled in the study. Fifteen mild COVID-19 patients received standard of care (SOC) management, and the remaining 15 patients received SOC plus E. neriifolia. The moderate COVID-19 patients similarly received either SOC (N = 15) or SOC plus E. neriifolia (N = 15). Although there were marked diversity regarding biochemical parameters of these patients at entry, the moderate COVID-19 patients receiving E. neriifolia showed decrease in C-reactive protein and D-dimer and increase in oxygen saturation 7 days after trial commencement. However, these improvements were not detected in moderate COVID-19 patients receiving SOC. Hospital staying was significantly lower in both mild and moderate COVID-19 patients receiving SOC plus E. neriifolia than those receiving only SOC. Taken together, it may be proposed that usage of E. neriifolia may have beneficial effects regarding management for COVID-19 patients, especially for those in developing and resource-constrained countries, although a conclusive statement may not be given due to small sample size. This herbal medication is also pertinent in the context of emergence of OMICRON variant of COVID-19 as the overload of SARS-CoV-2infecetd patients may be addressed considerably by this medication without hospitalization, if proper communication between patients and physicians can be ensured.

Smoking is associated with recurrent tuberculosis and its related mortality. Also, it could affect clinical manifestations, bacteriological conversion and outcome of treatment. This study aimed to evaluate the pattern of tobacco smoking, history of previous quit attempts and attitude towards quitting in tuberculosis patients. Materials and Methods: It was a cross-sectional study done amongst tuberculosis patients presented to DOTS corner of Rajshahi Medical College Hospital. 315 patients entered the study as "First Come First Serve". Selfreported questionnaires were designed according to the standard questionnaires of smoking pattern. Results: Sixty one percent (61%) patients (n=192) were smoker before the diagnosis of tuberculosis. 51.1% were current smokers at the time of interview and were continuing smoking after the diagnosis of tuberculosis. Only 10.2% discontinued smoking after diagnosis. 68.2% smokes less than 10 cigarettes per day. 53.1% was between 31-50 age group. 21-30 is the commonest age group (62%) to start smoking is and 66.1% smoked less than 10 years before diagnosis. To reduce stress was the most common cause to continue smoking (48.9%). 71.9% smokers showed keen interest to stop smoking. Conclusion: Considering the prevalence of smoking in tuberculosis patients, evaluation of tobacco smoking status in such patients and motivating them to quit smoking could be considered as important steps in their treatment process.

A simple random sampling technique selected the 296 senior staff nurses who met the inclusion criteria. The study period was January 2018 to July 2018. The instruments consisted of the demographic questionnaire and awareness level of nurses regarding the management of stroke questionnaire. The researcher developed all questionnaires from the literature reviews. This study result indicated that the mean age of the subjects was 30.1, with an SD of 8.9. Their age range was 21-56 years. The PSA is a probable prostate cancer tumor marker, although it cannot distinguish prostate cancer from BPH solely. To assess the usefulness of free, total, and free/total PSA ratios in differentiating BPH and prostate cancer has been studied under this research work. The work was carried out at RMCH during April 2021 to March 2022, 350 purposively selected urology outpatients and inpatients at RMCH were studied cross-sectional study. Trans-rectal ultrasound (TRUS) and serum PSA levels were analyzed along with Digital rectal examination (DRE). A digital guided prostatic biopsy using a monopty biopsy gun confirmed the diagnosis. Data analysis was done in SPSS 26 software program. Prostatic cancer was more prevalent in men aged 70-80, whereas BPH was more common in men aged 60-70 (p = 0.00032). Histopathology revealed 95 prostate cancer patients, 82 with PSA >4 ng/ml and 85 patients had f/t PSA ratio>0.16. BPH was found in 255 individuals. 139 BPH patients had total PSA >4 ng/ml and 245 BPH patients had f/t PSA ratio ≤0.16. Total PSA had sensitivity and specificity of 86.32% and 45.49%, respectively, lower than f/t ratio. The (Positive predictive value) PPV and accuracy was significantly higher in f/t ratio than total PSA. Along with total PSA, free and free/total PSA ratio may help detect prostate cancer early and distinguish BPH from prostate cancer.

So, this study was done to find out the thyroid function status of nephrotic syndrome children and to compare the thyroid function of healthy control children. Method: This cross sectional comparative study was carried out in the Department of Paediatrics, Rajshahi Medical College and Nuclear Medicine Centre, Rajshahi. Total 30 patients of both sex and age between 2-12 years diagnosed as nephrotic syndrome were included in this study and another 30 non-nephrotic syndrome apparently healthy children were taken as control. History, clinical examinations and laboratory investigations were recorded in preformed questionnaire. Thyroid function statuses also were evaluated in both patient and healthy control. After data collection, statistical analysis was done by computerized software. Results: In this study, 63.3% (n=19) nephrotic syndrome children had secondary hypothyroidism, overt hypothyroidism was 27.3% (n=7) and subclinical hypothyroidism was 6.7 % (n=2). Conclusion: In patients of childhood nephrotic syndrome different forms of thyroid dysfunctions are commonly associated due to loss of thyroid hormones and hormone binding's protein.

Being a developing country, it is an unrecognized problematic area in Bangladesh. Therefore, it is crucial to assess knowledge and practice on menstrual hygiene among urban adolescent girls in Bangladesh. Methods: This was a community-based cross-sectional study conducted in the Department of Community Medicine of Rajshahi Medical College among 495 purposively selected urban adolescent girls. Data were collected using a semi-structured interviewer-administered questionnaire. Results: The majority (57.2%) of the respondents in the study were more than 15 years old, and most (93.3%) were Muslim. Most of the girls (82.5%) have experienced menarche within the age range of 12-15 years, and the majority of respondents, 259 (52.32%), were informed about menstruation before menarche. Their source of information about menarche was mothers in most cases. Out of 495 adolescent girls who participated in this study, 70.9% had good knowledge of menstruation. About 82% of girls used commercially made sanitary pads, and 35.2% changed pads or clothes more than three times a day during menstruation. Conclusion: The majority (70.9%) of adolescent girls had good knowledge of menstruation, and 73.3% had a good practice on menstrual hygiene. Therefore, there is a need to design and implement universal awareness creation and advocacy programs to improve their knowledge of safe and healthy menstruation management and promote good hygienic practices by creating an appropriate supportive home and social environment for all the adolescent girls.

Besides conventional risk factors for coronary heart disease, genetic predisposition and some novel issues like high salt intake, arsenicosis, hypovitaminosis D and air pollution may play important role in the aetiopathogenesis of coronary heart disease in this population. Formulation of appropriate policy and more emphasis on preventive strategy may help combat coronary heart disease in Bangladesh. The main aim of this study was to find out the pattern of coronary heart disease among the patients admitted in Cardiology department of Rajshahi Medical college Hospital and to relate their socio-demographic and economic characteristics with the disease. It was a cross sectional type of descriptive study during February 2019 to July 2019. A total of 352 patients of Cardiology department considered as sample who admitted for their heart problem during the study period selected by purposive sampling. Majorities (85.8%) of the respondents were >40 years among them 74.4% were male. Most (52.0%) of the respondents were from nuclear family and smokers (65.62%). They had history of diabetes mellitus (22.4%) and lead sedentary life (11.4%). Regarding coronary heart disease 74.7% had ischemic heart disease and right coronary artery disease (66.7%). About 42.1% were hypertensive patients and 94.0% took medical treatment. Type of heart disease was found significantly associated with age group of the respondents (p<0.01), smoking habit (p<0.05) and history of abuse substances currently (p<0.05). High coronary heart disease prevalence along with an upward trend was observed in Bangladeshi adults and older age. Proper strategies are required for primary prevention of coronary heart disease so that a further increase can be alleviated and the morbidity and mortality associated with it can be reduced.

It has been suggested that a birthweight limit of 2.5 kg should not be regarded as valid for all ... more It has been suggested that a birthweight limit of 2.5 kg should not be regarded as valid for all populations as the cutoff point of low-weight births because of demographic, genetic and environmental differences. Countries often choose alternative cutoff values for low birthweight for clinical purposes. Bangladesh also needs to choose a convenient cutoff value for low birthweight. A total of 770 live singleton full-term normal newborns were included in this study by stratified sampling; birthweight was measured using the Detecto-type baby weight machine. Newborns were followed up to the end of their first week of life. For data collection a pretested structured questionnaire and an Apgar Score estimating checklist were used. Chi-square test was applied to assess the association of different birthweight strata and neonatal health outcomes. Multiple logistic regression analyses were carried out to identify the independent effects of different levels of birthweight on early neonatal health. The neonates having birthweight Յ2 kg had a significantly higher risk of early neonatal mortality and morbidity than the higher level birthweight group. Birth asphyxia was the commonest cause of early neonatal mortality and morbidity. Borderline birthweight (>2 to <2.5 kg) neonates experienced the same mortality and morbidity rates as the normal birthweight neonates during their early neonatal life. Birthweight Յ2 kg may be one of the criteria for admission to a neonatal intensive care unit whereas more than 2 kg may not require admission unless otherwise necessary.

COVID-19 displays variable clinical presentations. The severe COVID-19 represents a fulminant pathological condition and most of the patients run a downhill course if extensive medical measures are not adopted. The major challenges about COVID-19 are related to develop strategies to manage huge populations of mild and moderate cases of COVID-19 with two realistic purposes: (1) early negativity of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus and (2) arrest of progression of moderate COVID-19 patients from developing severe complications. Although several medications have been repurposed for these purposes, none of these have passed the test of time in global perspective. Thus, there remains a pressing need to develop new and novel innovative management strategies for these patients as new variants of SARS-CoV-2 have been destroying the normal public health delivery system of different countries from time to time. The study presented here has checked the safety and efficacy of a herbal medication, leaves of Euphorbia neriifolia Linn (E. neriifolia), in mild and moderate COVID-19 patients. Sixty patients (30 mild COVID-19 and 30 moderate COVID-19) were enrolled in the study. Fifteen mild COVID-19 patients received standard of care (SOC) management, and the remaining 15 patients received SOC plus E. neriifolia. The moderate COVID-19 patients similarly received either SOC (N = 15) or SOC plus E. neriifolia (N = 15). Although there were marked diversity regarding biochemical parameters of these patients at entry, the moderate COVID-19 patients receiving E. neriifolia showed decrease in C-reactive protein and D-dimer and increase in oxygen saturation 7 days after trial commencement. However, these improvements were not detected in moderate COVID-19 patients receiving SOC. Hospital staying was significantly lower in both mild and moderate COVID-19 patients receiving SOC plus E. neriifolia than those receiving only SOC. Taken together, it may be proposed that usage of E. neriifolia may have beneficial effects regarding management for COVID-19 patients, especially for those in developing and resource-constrained countries, although a conclusive statement may not be given due to small sample size. This herbal medication is also pertinent in the context of emergence of OMICRON variant of COVID-19 as the overload of SARS-CoV-2infecetd patients may be addressed considerably by this medication without hospitalization, if proper communication between patients and physicians can be ensured.

Smoking is associated with recurrent tuberculosis and its related mortality. Also, it could affect clinical manifestations, bacteriological conversion and outcome of treatment. This study aimed to evaluate the pattern of tobacco smoking, history of previous quit attempts and attitude towards quitting in tuberculosis patients. Materials and Methods: It was a cross-sectional study done amongst tuberculosis patients presented to DOTS corner of Rajshahi Medical College Hospital. 315 patients entered the study as "First Come First Serve". Selfreported questionnaires were designed according to the standard questionnaires of smoking pattern. Results: Sixty one percent (61%) patients (n=192) were smoker before the diagnosis of tuberculosis. 51.1% were current smokers at the time of interview and were continuing smoking after the diagnosis of tuberculosis. Only 10.2% discontinued smoking after diagnosis. 68.2% smokes less than 10 cigarettes per day. 53.1% was between 31-50 age group. 21-30 is the commonest age group (62%) to start smoking is and 66.1% smoked less than 10 years before diagnosis. To reduce stress was the most common cause to continue smoking (48.9%). 71.9% smokers showed keen interest to stop smoking. Conclusion: Considering the prevalence of smoking in tuberculosis patients, evaluation of tobacco smoking status in such patients and motivating them to quit smoking could be considered as important steps in their treatment process.

Bangladesh is also in the grip of the menace. The only way to get rid of the menace is prevention of these diseases which demands awareness against the diseases. This study was undertaken to assess the level of knowledge of adult people of Puthia Upazila regarding prevention of the seclected non-communicable diseases (NCDs). Methods: The study was carried out in the Department of Community Medicine Rajshahi Medical College, Rajshahi over a period of 2 months from April 2018 to May 2018. All adult people (18 years onwards) residing in the rural area of Puthia Upazila were the respondents (study population) in this study.
