Salah satu faktor pencetusnya adalah systemic lupus erythematosus (SLE). Minoksidil topikal 2% adalah terapi pilihan yang bekerja dengan meningkatkan aliran darah dan oksigen ke folikel rambut. Antioksidan oral yang mengandung keratin (cynatine HNS) juga memperbaiki kesehatan rambut sehingga dapat diberikan sebagai terapi tambahan pada kasus ET. Kasus: Perempuan berusia 19 tahun dengan keluhan rambut rontok sejak 1 tahun yang lalu. Pasien mengeluhkan demam, ruam merah di wajah dan nyeri sendi 5 bulan lalu dengan hasil tes ANA >1/80, dsDNA positif lemah, Scl-70 positif lemah dan Ro-60 positif lemah. Perjalanan penyakit sifilis pada pasien dengan HIV lebih cepat dan berisiko tinggi menjadi neurosifilis. Koinfeksi sifilis dan human immunodeficiency virus (HIV) dikaitkan dengan penurunan CD4 dan peningkatan HIV-RNA. Perbaikan keduanya dikaitkan dengan terapi sifilis yang efektif, salah satunya dengan terapi doxycycline. Doxycycline memiliki efek antiinflamasi yang berhubungan dengan penurunan viral load serta peningkatan CD4 pada pasien positif HIV. It is estimated that CA occurs in >80% of reproductive age in many countries, with the most common types being low risk types HPV 6 and 11, but sometimes could be combined with high-risk types are HPV 16, 18. Gen E6 is an oncoprotein that cooperates with E7 to provide a suitable environment for viral DNA replication and inhibits cellular apoptotic processes. The E6 gene has characteristic properties as a p53 tumor suppressor protein. Purpose: To analyse the high-risk types HPV 16 and 18 in CA patients. Methods: This study was a cross-sectional, observational analytic study that assessed data collection concurrently over time. Specimens were patients diagnosed with condyloma acuminatum, biopsies were performed, and immunohistochemical (IHC) staining was also performed to examine E6 of high risk types HPV 16 and 18. The sampling technique used sequential sampling, and the inclusion criteria included patients who came to our hospital for treatment. Result: Twenty patients with CA were include in this study consist of 11 male and 9 female, meanwhile 9 subjects in homosexual group and 11 subjects in heterosexual group. Based on IHC examination 33% cases were detected of high-risk types HPV 16 and 18 especially among homosexual group. According to t-test obtained significant result of highrisk types and low-risk types of HPV from IHC staining (p=0.038). Conclusion: Condyloma acuminatum is a sexually transmitted infection that commonly caused by low-risk types HPV 6 and 11, but this study showed high risk types HPV16 and 18 also found in MSM (Male Sex with Male) patients with CA, so that it is necessary to be aware of the possibility of transformation to malignancy.

Background: Bacterial vaginosis (BV) is the most common vaginal infection in women of reproductiv... more Background: Bacterial vaginosis (BV) is the most common vaginal infection in women of reproductive age and increases the risk of acquiring human immunodeficiency virus (HIV) and other sexually transmitted diseases (STDs). Data regarding the association between sexual activity and BV acquisition is contradictory. Thus, tracking the prevalence and associated risk factors of BV among female sex workers (FSWs) is necessary to improve our understanding and control of STDs and the HIV epidemic. Methods: The subjects of this study were FSWs in Surakarta. This study adopted a cross-sectional design, and diagnosis was established using Amsel criteria. Interviews were conducted using questionnaires to collect data of risk factors, including demographic factors (age, education level, daily income, marital status, location of sex), sociologic factors (age of first intercourse; age of first FSW encounter; duration of work as an FSW; number of clients/day; condom, contraceptive, and vaginal douching use; violence experienced as an FSW; smoking habits; alcohol consumption; and drug use), and knowledge factors (knowledge of STDs and HIV). Statistical analysis was performed by using bivariate analysis. Results: We interviewed 175 FSWs with an average age of 40.66 years. BV was detected in 28 subjects (16%). Risk factors associated with BV include alcohol (p = 0.022) and drug use (p = 0.022). Conclusion: Other than routine screening for STDs and HIV, FSWs should be encouraged to participate in prevention programs aiming to regulate alcohol and drug use to decrease the incidence of BV infection.

It commonly occurs in the head and neck area exposed to ultraviolet light (UV). Data in Dr. Moewardi hospital 2013-2015 showed that basalioma was the most common type of skin cancer. This study aimed to describe the basalioma skin cancer in Dr. Moewardi hospital, Surakarta. Subjects and Method: This cross-sectional study was conducted at Dr. Moewardi Hospital, Surakarta, Central Java, from 2016 to 2019. A sample of 70 patients diagnosed with basalioma skin cancer was selected for this study. The study variables were gender, age, occupation, tumor location, tumor size, and clinical tumor type. The data were collected from the medical record. The data were described in percent. Results: As many as 64.3% of basalioma patients were female, 30% were farmers, 90.02% aged ≥50 years, 72% had tumors located in the midface region, 70% had a tumor in the H-zone, 72.8% had tumor size <2 cm, and 58.5% had a nodular tumor. Conclusion: The frequency distribution of basalioma in Dr. Moewardi Hospital has been described by gender, age, tumor location, tumor size, and histopathological type.

Fractional resurfacing lasers such as CO 2 laser and erbium-doped yttrium aluminum garnet (Er: YAG) have been widely used for acne scar treatment and give satisfying results have complications such as pain, hyperpigmentation, infection, prolonged erythema, and scarring. Fractional erbium: yttrium scandium gallium garnet (Er: YSGG) laser solves previous laser techniques' disadvantages. It has an ideal wavelength of 2790 nm, which balances the epidermis layer's vaporization and thermal damage in the dermis's superficial layer. Platelet Rich Plasma (PRP) is an autologous concentration of human platelets in a small plasma volume that secreted various growth factors that can speed up the scar healing process. Case report: A-24 years old male student came with a chief complaint of acne scars on his face four years before the examination. The patient's condition is classified as moderate-severe acne scarring based on Goodman and Baron's criteria (qualitative global acne scarring system). Acne scars treatment plan was single fractional erbium: YSGG laser for the left side of the face and combined fractional Er: YSGG laser with topical autologous PRP on the right side. This procedure was performed in 2 sessions with an interval of 4 weeks, the final evaluation at 12 weeks after the first procedure. Clinical improvement of acne scars was assessed using qualitative and quantitative scales based on Goodman and Baron's, patient and observer scar assessment scale (POSAS), quartile grading scale, and patient satisfaction level. Conclusion: In the final evaluation, there was an improvement on the entire grading scale on the right side of the face (Er: YSGG laser + PRP) and the face's left side (Er: YSGG laser only). Post-treatment erythema was found on both sides of the patient's face but disappeared within three days on the face's right side and five days on the face's left side.

Cutibacterium acnes is considered one of the key contributing factors even though many Cutibacterium acnes colonies are on healthy skin. Cutibacterium acnes induces lipogenesis and production of sebum. Through its production of porphyrins, it may act as a catalytic agent in squalene oxidation that is comedogenic, thus aggravating comedogenesis. We review the dual role of Cutibacterium acnes to provide an understanding of the pathophysiology of acne vulgaris.

This 4-weeks, double-blind, randomized, controlled clinical study was conducted to compare the ef... more This 4-weeks, double-blind, randomized, controlled clinical study was conducted to compare the effects of topical caffeine 0.25% with 0.5% on barrier function and elasticity in the skin. A total of 41 healthy subjects consisted of male and female aged 20-60 y.o. The subjects were grouped into two. Each group received a topical caffeine with different concentrations of 0.25% and 0.5%. Hydroxyethylcellulose (HEC) gel as the placebo has 97.5% water content. Physical stability test was conducted at the preparation. The treatment was performed on the forehead and periorbital by applying a topical caffeine 0.25% on one forehead and periorbital and 0.5% on the others. Application of gels was performed 2 times daily for 4 weeks. Measurement of Transepidermal Water Loss (TEWL) using Tewameter® TM300 and elasticity using Elastometer® EM25 were performed before and after treatment. Clinical evaluations for efficacy were made at baseline and after two and 4 weeks of gel use. The measurements results were analyzed using one-way Annova. Overall, the results of the study showed that topical caffeine applications can improve skin barrier function and increase the elasticity the skin especially as anti wrinkle.

The skin changes and manifestations related to COVID-19 infection remain not clearly understood. Cutaneous manifestations related to COVID-19 had been reported. Our patient manifested atypical cutaneous exanthem on her legs, with no other abnormalities found. We used oral azithromycin 500 mg, dexamethasone 0.5 mg, vitamin C 100 mg, and paracetamol 500 mg, which are available at Badak Baru Primary Health Care. The exanthem has improved after 10 days of treatment.

Background: Peutz-Jeghers syndrome (PJS) is an autosomal dominant inherited disorder, characteriz... more Background: Peutz-Jeghers syndrome (PJS) is an autosomal dominant inherited disorder, characterized by intestinal hamartoma polyps in association with distinct patterns of skin and mucosal macular melanin deposition. Patients with PJS have a 15-fold increased risk of developing bowel cancer compared to the general population. Case: A 47-year-old woman complained of black patches appearing on the lower and upper lips. Several months ago, the patient said that black patches appeared on the gums and inner cheeks. Gastrointestinal complaints such as nausea or vomiting were previously denied. Dermatological examination in the oris et ginggiva et buccalis region showed multiple well-defined hyperpigmented macules. Histopathological examination found an increase of melanocytes in the stratum basale with rows of melanocytes. Patient had an endoscopy in the gastrointestinal tract and histopathological examination of the colonic mucosa, no abnormalities were found and no signs of malignancy were found on histopathological examination. Conclusion: Result of histopathological examination showed an increase in the number of melanocytes in the stratum basale with lined melanocytes. Based on histopathological examination in this patient more leads to the diagnosis of PJS

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are acute severe cutaneous ad... more Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are acute severe cutaneous adverse reactions commonly caused by medication. Precise evaluation of disease severity in initial setting must be obtained to start appropriate treatment. Neutrophil and lymphocyte ratio (NLR) plays a role in displaying inflammatory reaction while eosinophils count (EC) influences immunological dysregulation including the proliferation of cytotoxic cells in early onset of SJS/TEN. Objective: To evaluate whether NLR and EC serve as prognostic markers of disease severity in patients with SJS/TEN using SCORTEN. Methods: A single center study with retrospective study included SJS/TEN patients at Dr. Moewardi General Hospital Surakarta in January 1st 2018-December 31st 2020. The required laboratory data was assessed at the beginning of the patient's admission through medical records. The significance analysis were performed using one-way ANOVA and Spearman while the receiver-operator curve were used to evaluate the prognostic value of variables for severity in SJS/TEN patients. Results: The total sample in this study was 24 patients with majority female (58%) and range from 25 to >50 years (54%). The results demonstrated of significant difference and positively correlated between NLR and EC with severity of SJS/TEN (p<0.01; r>0.05). The specificity and sensitivity of 51%;61% and 70%;60%, respectively. Conclusion: NLR and EC can be used as prognosticators of severity in SJS/TEN while further research on other inflammatory markers with increased number of samples and study centers are needed to provide more actual data.

It is estimated that CA occurs in >80% of reproductive age in many countries, with the most common types being low risk types HPV 6 and 11, but sometimes could be combined with high-risk types are HPV 16, 18. Gen E6 is an oncoprotein that cooperates with E7 to provide a suitable environment for viral DNA replication and inhibits cellular apoptotic processes. The E6 gene has characteristic properties as a p53 tumor suppressor protein. Purpose: To analyse the high-risk types HPV 16 and 18 in CA patients. Methods: This study was a cross-sectional, observational analytic study that assessed data collection concurrently over time. Specimens were patients diagnosed with condyloma acuminatum, biopsies were performed, and immunohistochemical (IHC) staining was also performed to examine E6 of high risk types HPV 16 and 18. The sampling technique used sequential sampling, and the inclusion criteria included patients who came to our hospital for treatment. Result: Twenty patients with CA were include in this study consist of 11 male and 9 female, meanwhile 9 subjects in homosexual group and 11 subjects in heterosexual group. Based on IHC examination 33% cases were detected of high-risk types HPV 16 and 18 especially among homosexual group. According to t-test obtained significant result of highrisk types and low-risk types of HPV from IHC staining (p=0.038). Conclusion: Condyloma acuminatum is a sexually transmitted infection that commonly caused by low-risk types HPV 6 and 11, but this study showed high risk types HPV16 and 18 also found in MSM (Male Sex with Male) patients with CA, so that it is necessary to be aware of the possibility of transformation to malignancy.

Background: Bacterial vaginosis (BV) is the most common vaginal infection in women of reproductiv... more Background: Bacterial vaginosis (BV) is the most common vaginal infection in women of reproductive age and increases the risk of acquiring human immunodeficiency virus (HIV) and other sexually transmitted diseases (STDs). Data regarding the association between sexual activity and BV acquisition is contradictory. Thus, tracking the prevalence and associated risk factors of BV among female sex workers (FSWs) is necessary to improve our understanding and control of STDs and the HIV epidemic. Methods: The subjects of this study were FSWs in Surakarta. This study adopted a cross-sectional design, and diagnosis was established using Amsel criteria. Interviews were conducted using questionnaires to collect data of risk factors, including demographic factors (age, education level, daily income, marital status, location of sex), sociologic factors (age of first intercourse; age of first FSW encounter; duration of work as an FSW; number of clients/day; condom, contraceptive, and vaginal douching use; violence experienced as an FSW; smoking habits; alcohol consumption; and drug use), and knowledge factors (knowledge of STDs and HIV). Statistical analysis was performed by using bivariate analysis. Results: We interviewed 175 FSWs with an average age of 40.66 years. BV was detected in 28 subjects (16%). Risk factors associated with BV include alcohol (p = 0.022) and drug use (p = 0.022). Conclusion: Other than routine screening for STDs and HIV, FSWs should be encouraged to participate in prevention programs aiming to regulate alcohol and drug use to decrease the incidence of BV infection.

It commonly occurs in the head and neck area exposed to ultraviolet light (UV). Data in Dr. Moewardi hospital 2013-2015 showed that basalioma was the most common type of skin cancer. This study aimed to describe the basalioma skin cancer in Dr. Moewardi hospital, Surakarta. Subjects and Method: This cross-sectional study was conducted at Dr. Moewardi Hospital, Surakarta, Central Java, from 2016 to 2019. A sample of 70 patients diagnosed with basalioma skin cancer was selected for this study. The study variables were gender, age, occupation, tumor location, tumor size, and clinical tumor type. The data were collected from the medical record. The data were described in percent. Results: As many as 64.3% of basalioma patients were female, 30% were farmers, 90.02% aged ≥50 years, 72% had tumors located in the midface region, 70% had a tumor in the H-zone, 72.8% had tumor size &lt;2 cm, and 58.5% had a nodular tumor. Conclusion: The frequency distribution of basalioma in Dr. Moewardi Hospital has been described by gender, age, tumor location, tumor size, and histopathological type.

Fractional resurfacing lasers such as CO 2 laser and erbium-doped yttrium aluminum garnet (Er: YAG) have been widely used for acne scar treatment and give satisfying results have complications such as pain, hyperpigmentation, infection, prolonged erythema, and scarring. Fractional erbium: yttrium scandium gallium garnet (Er: YSGG) laser solves previous laser techniques' disadvantages. It has an ideal wavelength of 2790 nm, which balances the epidermis layer's vaporization and thermal damage in the dermis's superficial layer. Platelet Rich Plasma (PRP) is an autologous concentration of human platelets in a small plasma volume that secreted various growth factors that can speed up the scar healing process. Case report: A-24 years old male student came with a chief complaint of acne scars on his face four years before the examination. The patient's condition is classified as moderate-severe acne scarring based on Goodman and Baron's criteria (qualitative global acne scarring system). Acne scars treatment plan was single fractional erbium: YSGG laser for the left side of the face and combined fractional Er: YSGG laser with topical autologous PRP on the right side. This procedure was performed in 2 sessions with an interval of 4 weeks, the final evaluation at 12 weeks after the first procedure. Clinical improvement of acne scars was assessed using qualitative and quantitative scales based on Goodman and Baron's, patient and observer scar assessment scale (POSAS), quartile grading scale, and patient satisfaction level. Conclusion: In the final evaluation, there was an improvement on the entire grading scale on the right side of the face (Er: YSGG laser + PRP) and the face's left side (Er: YSGG laser only). Post-treatment erythema was found on both sides of the patient's face but disappeared within three days on the face's right side and five days on the face's left side.

Cutibacterium acnes is considered one of the key contributing factors even though many Cutibacterium acnes colonies are on healthy skin. Cutibacterium acnes induces lipogenesis and production of sebum. Through its production of porphyrins, it may act as a catalytic agent in squalene oxidation that is comedogenic, thus aggravating comedogenesis. We review the dual role of Cutibacterium acnes to provide an understanding of the pathophysiology of acne vulgaris.

This 4-weeks, double-blind, randomized, controlled clinical study was conducted to compare the ef... more This 4-weeks, double-blind, randomized, controlled clinical study was conducted to compare the effects of topical caffeine 0.25% with 0.5% on barrier function and elasticity in the skin. A total of 41 healthy subjects consisted of male and female aged 20-60 y.o. The subjects were grouped into two. Each group received a topical caffeine with different concentrations of 0.25% and 0.5%. Hydroxyethylcellulose (HEC) gel as the placebo has 97.5% water content. Physical stability test was conducted at the preparation. The treatment was performed on the forehead and periorbital by applying a topical caffeine 0.25% on one forehead and periorbital and 0.5% on the others. Application of gels was performed 2 times daily for 4 weeks. Measurement of Transepidermal Water Loss (TEWL) using Tewameter® TM300 and elasticity using Elastometer® EM25 were performed before and after treatment. Clinical evaluations for efficacy were made at baseline and after two and 4 weeks of gel use. The measurements results were analyzed using one-way Annova. Overall, the results of the study showed that topical caffeine applications can improve skin barrier function and increase the elasticity the skin especially as anti wrinkle.

Our patient manifested atypical cutaneous exanthem on her legs, with no other abnormalities found. We used oral azithromycin 500 mg, dexamethasone 0.5 mg, vitamin C 100 mg, and paracetamol 500 mg, which are available at Badak Baru Primary Health Care. The exanthem has improved after 10 days of treatment.

Patients with PJS have a 15-fold increased risk of developing bowel cancer compared to the general population. Case: A 47-year-old woman complained of black patches appearing on the lower and upper lips. Several months ago, the patient said that black patches appeared on the gums and inner cheeks. Gastrointestinal complaints such as nausea or vomiting were previously denied. Dermatological examination in the oris et ginggiva et buccalis region showed multiple well-defined hyperpigmented macules. Histopathological examination found an increase of melanocytes in the stratum basale with rows of melanocytes. Patient had an endoscopy in the gastrointestinal tract and histopathological examination of the colonic mucosa, no abnormalities were found and no signs of malignancy were found on histopathological examination. Conclusion: Result of histopathological examination showed an increase in the number of melanocytes in the stratum basale with lined melanocytes. Based on histopathological examination in this patient more leads to the diagnosis of PJS

Precise evaluation of disease severity in initial setting must be obtained to start appropriate treatment. Neutrophil and lymphocyte ratio (NLR) plays a role in displaying inflammatory reaction while eosinophils count (EC) influences immunological dysregulation including the proliferation of cytotoxic cells in early onset of SJS/TEN. Objective: To evaluate whether NLR and EC serve as prognostic markers of disease severity in patients with SJS/TEN using SCORTEN. Methods: A single center study with retrospective study included SJS/TEN patients at Dr. Moewardi General Hospital Surakarta in January 1st 2018-December 31st 2020. The required laboratory data was assessed at the beginning of the patient's admission through medical records. The significance analysis were performed using one-way ANOVA and Spearman while the receiver-operator curve were used to evaluate the prognostic value of variables for severity in SJS/TEN patients. Results: The total sample in this study was 24 patients with majority female (58%) and range from 25 to >50 years (54%). The results demonstrated of significant difference and positively correlated between NLR and EC with severity of SJS/TEN (p<0.01; r>0.05). The specificity and sensitivity of 51%;61% and 70%;60%, respectively. Conclusion: NLR and EC can be used as prognosticators of severity in SJS/TEN while further research on other inflammatory markers with increased number of samples and study centers are needed to provide more actual data.
