documentation and preservation of underwater cultural heritage in Bavaria. Due to an ongoing change in techniques
and methods, also the operational areas and duties of the BGfU extended. Underwaterarchaeological work moved
more and more towards observational tasks, as represented by one of the most important underwaterarchaeological
issues in Bavaria, the monitoring of the UNESCO site of Rose Island in Lake Starnberg. Furthermore, monitoring of
potential threads and damaging potentials is also carried out on the so-called “Schindelwrack”, a modern shipwreck
also situated in Lake Starnberg. Outside Bavaria, the BGfU has implemented an own underwaterarchaeological
group in Hesse. In Croatia and Romania, further and generative cooperation have become established with local
archaeological authorities and groups.

Aim: To evaluate the possible relation between cumulative radiation dose and cancer prevalence in patients hospitalized for cardiovascular disease (CD). Methods: An observational single center study was conducted on 16,431 consecutive patients with CD admitted at the Institute between January 1970 and December 2008, with maximum 25 years of follow-up. The cumulative dose from cardiac imaging procedures was estimated from electronic records of all hospital admission adopting effective dose reference levels of AHA statement 2009 (coronary angiography=7 milliSievert (mSv); Percutaneous Coronary Intervention=15 mSv; Sestamibi scan=10 mSv; Thallium scan= 22 mSv). Patients were divided according to their level of exposure express as terziles. The primary outcome was risk of cancer examined using Kaplan-Meier curve and the Log-Rank test. Results: The indexed procedures were found in 12862 pts. Cumulative radiation exposure/patient was 22 (12-42) mSv (median, 25th-75th percentiles). A total of 1234 cancers (including 753 cancer deaths) were diagnosed with a follow-up time of 120±97 months. There was a significant dose-dependent relation between exposure to radiation and subsequent risk of cancer (low dose =7-14;median= >14-36; high=>36 msV) (Panel) (Log Rank <0.001). Lung and colon cancer were prevalent in the population belonging to the middle and highest tertiles (70% and 64%, p<0.001 and p=0.004 respectively). Cancer free survival Conclusion: Patients exposed to high level (>36 mSv) ionizing radiation from cardiac procedures have an higher risk of developing incident cancers in the following twenty-five years.

Introduction : Coronary CT angiography (CTA) provides clinically useful information in selected p... more Introduction : Coronary CT angiography (CTA) provides clinically useful information in selected patients, but high radiation exposure has been a limitation. We evaluated the feasibility and image quality of a new scan mode, prospectively ECG-gated high-pitch spiral imaging, for coronary CTA with an effective dose below 1 mSv. Methods: 60 consecutive patients referred for ruling out coronary stenoses were studied. Selection criteria were absence of previously known coronary disease, body weight ≤ 100 kg, and sinus rhythm Results: Out of 81 patients screened for inclusion, 21 were excluded: 11 because heart rate remained > 60/min even after beta blockade, and 10 for a body weight > 100 kg. In all 60 included patients, imaging was successfully performed. Mean heart rate before and during coronary CTA was 70±11 and 52±4 beats/min. Mean scan length was 117±9 mm, mean duration of data acquisition was 258±20 ms. Mean dose-length product was 62±5 mGy*cm, the resultant mean effective dose was 0.87±0.08 mSv (range: 0.78 – 0.99 mSv). Of 896 coronary artery segments, 847 (95%) had an image quality score of 1, 36 segments (4.6%) a score of 2, 8 segments (1.0%) a score of 3, and 4 segments (0.4%) were “uninterpretable”. Conclusion: In non-obese patients with a low and stable heart rate, coronary CTA using prospectively ECG-triggered high-pitch spiral acquisition can be performed with high image quality and a consistent radiation dose below 1.0 mSv.

The factors influencing genesis of atherosclerosis at specific regions within the coronary arteri... more The factors influencing genesis of atherosclerosis at specific regions within the coronary arterial system are currently uncertain. Local mechanical factors such as shear stress as well as metabolic factors, including inflammatory mediators released from epicardial fat, have been proposed. We analyzed computed tomographic (CT) attenuation of pericoronary adipose tissue in normal versus atherosclerotic coronary segments as defined by intravascular ultrasound (IVUS). We evaluated the data sets of 29 patients who were referred for invasive coronary angiography and in whom IVUS of 1 coronary vessel was performed for clinical reasons. Coronary CT angiography was performed within 24 hours from invasive coronary angiography. Computed tomographic angiography was performed using dual-source CT (Siemens Healthcare; Forchheim, Germany). A contrast-enhanced volume data set was acquired (120 kV, 400 mA/rot, collimation 2 × 64 × 0.6 mm, 60-80 mL intravenous contrast agent). Intravascular ultrasound was performed using a 40-MHz IVUS catheter (Atlantis; Boston Scientific Corporation, Natick, Mass) and motorized pullback at 0.5 mm/s. Sixty corresponding coronary artery segments within the coronary artery system were identified in both dual source computed tomography and IVUS using bifurcation points as fiducial markers. In dual source computed tomography data sets, 8 serial parallel cross sections (2-mm slice thickness) were rendered orthogonal to the center line of the coronary artery for each segment. For each cross section, pericoronary adipose tissue within a radius of 3 mm from the coronary artery and enclosed within the epicardium (excluding coronary veins and myocardium) was manually traced and mean CT attenuation values were obtained. Intravascular ultrasound was used to define coronary segments as follows: presence of predominantly fibrous atherosclerotic plaque (hyperechoic), presence of predominantly lipid-rich atherosclerotic plaque (hypoechoic), and absence of atherosclerotic plaque. In IVUS, 20 coronary segments with fibrous plaque, 20 segments with lipid-rich plaque, and 20 coronary segments without plaque were identified. The mean CT attenuation of pericoronary adipose tissue for segments with any coronary atherosclerotic plaque was -34 ± 14 Hounsfield units (HU), as compared with -56 ± 16 HU for segments without plaque (P = 0.005). The density of pericoronary fat in segments with fibrous versus lipid-rich plaque as defined by IVUS was not significantly different (-35 ± 19 HU vs -36 ± 16 HU, P = 0.8). Mean CT attenuation of pericoronary adipose tissue is significantly lower for normal versus atherosclerotic coronary segments. This supports a hypothesis of different types of pericoronary adipose tissue, the more metabolically active of which might exert local effects on the coronary vessels, thus contributing to atherogenesis.

We sought to determine the influence of CV risk factors on the prevalence of nonobstructive atherosclerosis in patients with normal-appearing coronary arteries in invasive coronary angiography (ICA). In this retrospective analysis, we included 60 consecutive symptomatic patients, having undergone ICA and coronary CTA. Coronary dual source CTA was performed using electrocardiogram-triggered retrospective gated image acquisition at 40%-70% of RR interval (tube voltage 100-120 kV, tube current time product 320-440 mAs, 60 mL contrast, and flow rate 6 mL/s). Out of 60 patients (32 men, mean age 61 ± 11 years) with a normal coronary artery appearance in ICA, 45 (75%) patients showed atherosclerotic plaque in CTA. If image quality in low-dose groups 1 or 2 was non-diagnostic, an additional standard CT examination (as in group 3) was performed. Non-diagnostic image quality was found in 11/56, 4/55, and 2/65 patients (46/896, 4/880 and 3/1,040 coronary segments) in groups 1, 2 and 3, respectively. Median (interquartile ranges) volumes of contrast material, CTDI(vol), DLP and effective dose for low-dose groups 1 and 2 and for standard group 3 were 92.5 (11.3), 75.0 (2.5) and 75.0 (9.0) ml; 8.0 (1.4), 16.8 (4.8) and 48.1 (14.2) mGy; 108.0 (27.3), 246.0 (93.0) and 701.0 (207.8) mGy cm; and 1.5 (0.4), 3.4 (1.3) and 9.8 (2.9) mSv, respectively. A staged coronary CTA protocol with an initial low-dose approach and addition of a standard sequence--should image quality be too low--can lead to a substantial reduction in radiation exposure.

This study evaluated the ability of dual-source computed tomography (DSCT) to detect coronary all... more This study evaluated the ability of dual-source computed tomography (DSCT) to detect coronary allograft vasculopathy (CAV) in heart transplant recipients using intravascular ultrasound (IVUS) as the standard of reference. Thirty patients with heart transplants (81% men, mean age 40 years) underwent DSCT (330-ms gantry rotation, 2 ؋ 64 ؋ 0.6-mm collimation, 60-to 80-ml contrast agent, no additional  blockers) before invasive coronary angiography including IVUS of 1 vessel. Detection of CAV by DSCT was qualitatively defined as the presence of any coronary plaque. Mean heart rate during dual-source computed tomographic scanning was 80 ؎ 14 beats/min. Four hundred fifty-nine segments with a vessel caliber >1.5 mm according to quantitative coronary angiography were evaluated in 30 patients. Of these, 96% were considered to have excellent or good image quality. IVUS detected CAV in 17 of 30 patients (57%) and in 41 of 110 coronary segments (37%). Compared to IVUS, sensitivity, specificity, positive and negative predictive values for the detection of CAV by DSCT were 85%, 84%, 76%, and 91%, respectively. In conclusion, DSCT permits the investigation of transplant recipients concerning the presence of CAV with good image quality and high diagnostic accuracy.

In coronary CT angiography (CTA), both high-grade stenoses and total occlusions of a coronary art... more In coronary CT angiography (CTA), both high-grade stenoses and total occlusions of a coronary artery may appear as a complete interruption of the contrast-enhanced lumen. Parameters to differentiate between occlusions and stenoses have not been systematically assessed. We evaluated 40 consecutive patients with a lesion demonstrating complete interruption of the contrast-enhanced lumen in coronary CTA and in whom invasive coronary angiography was available. Length of the vessel segment without luminal contrast enhancement; luminal enhancement proximal, in and distal to the lesion; degree of coronary remodelling; and the degree of lesion calcification were assessed by a blinded observer una-ware of the invasive angiogram. Mean length of complete occlusions (n=20; range 4-54 mm; mean 16.6±3.5 mm) was significantly longer than for highgrade stenoses (n=20; 2-8 mm; mean 4.6±1.7 mm, p<0.001). A lesion length ≥ 9 mm was 100% specific and 70% sensitive for an occlusion. No significant differences were found for vessel enhancement in or distal to the lesion, remodelling index or degree of calcification. Lesion length is the only parameter that may differentiate complete occlusions and high-grade stenoses in coronary CTA. For lesions ≥ 9 mm, an occlusion is very likely.

Die Kentnis der aufgezeigten Wirkmechanismen können beim Fund archäologisch stärker relevanter Wracks (z.B. römische, byzantinische Wracks) in dieser Lage zu ihrem Schutz genutzt werden. In diesem Fall wird nicht das Gewässer als Ressource in den Mittelpunkt gestellt, sondern es kann die Kenntnis der Prozesse im Gewässer zum Schutz des Kulturgutes eingesetzt werden.
After a sophisticated historical investigation of Geschichtsverein Griesheim (Historical Societ... more After a sophisticated historical investigation of Geschichtsverein Griesheim (Historical Society of Griesheim) the BGfU conducted a first diving survey under the direction of Harry Haarstark on 15 October, 2016. Relics of a Roman bridge had been assumed to be in the Main River at Frankfurt for a long time. The prospection took place on behalf of the Denkmalamt der Stadt Frankfurt am Main (office for monuments of the city Frankfurt/Main) under the supervision of chief officer Dr. Andrea Hampel.
Already in 1884 in the course of the Main canalization wooden piles were hauled with stake shoes and worked rock stones were unearthed by a steam excavator from the riverbed near Schwanheim. By means of old plans, cards and historical reports the historical society located the place at the bank of Schwanheim and limited the area of the survey. The members of the historical society assumed a Roman imperial period bridge in the proximity of the old bridge of Schwanheim, which spanned the Main starting from 1907. The former Neustraße of Schwanheim (today Eifelstraße) is in the line of sight of the Schwanheimer Bahnstraße, which had already been recognized as a Roman road by Dr. Kobelt. In its extension it runs to the Roman castellet „Auf Esch“ near Groß Gerau south of the Main and continues to push along to the Roman Rhine harbour near Gernsheim. In the northern direction it led to the Roman provincial capital Nida – the Roman city which was located between today's Praunheim and Heddernheim. At the new roundabout in Heerstraße this Roman road was recently unearthed as a crossing with the Roman road to the former capital of the Provincia Germania superior Mogontiacum (Mainz).
The diving investigations of the BGfU should supply certainty about the assumed Roman Main bridge. Indeed an approx. 1.50 m high palisade wall towards the shipping channel was found. It includes a cement block made of rubble stone at its inside towards the bank. Within the block at least two approx. 40 cm thick oak stakes are cemented as anchors. Upstream of it the building ends in a no longer completely pointed ice-breaker, while it locks at the downstream side straight in a right angle. The bank side protudes only a few decimeters over the river ground. The upper ends of the wooden stockades are frayed, due to shipping traffic which was sometimes too close to the bank or due to washed stones in floodings. A wooden sample of the palisade could be taken, which proved as a silver fir conifer. Dendrochronological dating will be conducted. The underwater investigations will be continued to give further results for the early historical roads and its network at the Lower Main.
