La detección y tratamiento temprano son cruciales para mejorar el curso de la enfermedad. La estrategia terapéutica habitual es la farmacológica, además, se está investigando los posibles beneficios añadidos al combinarlo con el tratamiento psicológico. El objetivo es focalizar la atención terapéutica en el trastorno bipolar en las fases iniciales, y dar el mejor tratamiento farmacológico y psicológico desde el primer momento. Así podremos prevenir recaídas, que generan alteraciones en factores neurotróficos, y atrofias cerebrales que probablemente podrían ser evitadas. OBJETIVOS: Difundir la eficacia de los tratamientos farmacológicos y psicológicos en el tratamiento del trastorno bipolar de inicio temprano, y dar a conocer la metodología utilizada. MATERIAL Y METODOS:-Presentación de metodología de tratamiento psicológico: psicoeducación y tratamiento familiar.-Descripción del impacto del tratamiento familiar sobre la salud de la familia y del paciente.-Los módulos psicoeducativos se organizan en 12 sesiones para el paciente y 8 para los familiares. The present-day population structure of La Gomera is outstanding in its high aboriginal heritage,... more The present-day population structure of La Gomera is outstanding in its high aboriginal heritage, the greatest in the Canary Islands. This was earlier confirmed by both mitochondrial DNA and autosomal analyses, although genetic drift due to the fifteenth century European colonization could not be excluded as the main factor responsible. The present mtDNA study of aboriginal remains and extant samples from the six municipal districts of the island indeed demonstrates that the pre-Hispanic colonization of La Gomera by North African people involved a strong founder event, shown by the high frequency of the indigenous Canarian U6b1a lineage in the aboriginal samples (65%). This value is even greater than that observed in the extant population (44%), which in turn is the highest of all the seven Canary Islands. Type 1 and type 2 diabetes, complicated with renal disease, have a significantly higher incidence... more Type 1 and type 2 diabetes, complicated with renal disease, have a significantly higher incidence in the Canary Islands than in mainland Spain and other European countries. Present-day Canarian inhabitants consist of a mixed population with North African indigenous and European colonizer ancestors who have rapidly evolved from a rural to an urban life style. The aim of this work was to assess the possible role of genetic and environmental factors on diabetes-related end-stage renal disease incidence in the Canary Islands. Results: For both types of diabetes there is an ethnic susceptibility increased by diabetes family history. Whereas the Y-chromosome does not play a significant role, mitochondrial DNA (mtDNA) haplogroup differences point to a maternal origin for this ethnic predisposition, confirming susceptible and protective effects for haplogroups J and T, respectively. In addition, urban life style seems to be an additional risk factor for type 1 diabetes. Conclusions: The maternal ethnic predisposition to diabetes complicated with kidney disease detected in the Canary Islands signals mtDNA and X-chromosome markers as the best candidates to uncover the genetic predisposition to this disease.

The composition of essential oil isolated from Salvia fruticosa, wild growing in Libya, was analy... more The composition of essential oil isolated from Salvia fruticosa, wild growing in Libya, was analyzed by GC and GC-MS. Forty-five compounds could be identified. The essential oil contained 1,8-cineole (49.34%), camphor (7.53%), ?-pinene (7.38%), myrcene (7.38%), ?-pinene (5.15%), ?-caryophyllene (4.13%) and ?-terpineol (3.25). Antioxidant activity was analyzed using the DPP H free radical scavenging method and low antioxidant activity was found (IC50 = 15.53 mg/ml). The oil was also screened for its antimicrobial activity against eight bacteria (four Gram-negative and four Gram-positive) and eight fungi. The essential oil of S. fruticosa showed minimal inhibitory activity (MIC) at 0.125-1.5 mg/ml and bactericidal (MBC) at 0.5-2.0 mg/ml. In addition, it exhibited fungistatic (MIC) at 0.125-1.0 mg/ml and fungicidal effect (MFC) at 0.125-1.5 mg/ml.

A personal survey of 400 individuals aged over 55 who had been involved in some tourist travel in the last year was undertaken.FindingsA principal component analysis resulted in the extraction of a single factor labeled “cognitive age”. This encouraged us to extend our work and assess whether mtDNA diagnostic polymorphisms, defining geographically structured haplogroups, could be associated with the development of POAG. We sequenced the mtDNA regulatory hypervariable region-I (HVS-I) region and coding regions, comprising haplogroup diagnostic polymorphisms, in 176 POAG patients and 186 matched healthy controls (free of glaucoma by examination) of Saudi Arabia ascendancy. There is room for improvement in the planning and continuity´s care. Objectives: To review the literature on the needs of ostomy patients cared for at home and in primary care. To propose actions to improve comprehensive ostomy patient care during hospital stay and at home. Methods: We performed a bibliometric analysis between October of 2012 and March of 2013 to analyze the ostomy patient's cares once they have been discharged from hospital, which focuses on home care. Literature review was performed using Punto Q from the University of La Laguna in databases such as Dialnet, Scielo, Scopus, Medline and Science Direct.

Se realizó un estudio descriptivo y longitudinal de 53 adultos hipertensos pertenecientes al cons... more Se realizó un estudio descriptivo y longitudinal de 53 adultos hipertensos pertenecientes al consultorio médico No. 55 del Policlínico Universitario "José Martí Pérez" de Santiago de Cuba, desde febrero hasta septiembre de 2012, con vistas a demostrar la efectividad de la auriculopuntura en el tratamiento de estos pacientes. Se halló predominio de la hipertensión arterial ligera (69,6 %) y del sexo femenino (47,1 %). Luego de aplicada la terapia disminuyeron los síntomas más frecuentes: la cefalea, de 50,9 a 5,6 % y el insomnio, de 20,7 a 1,8 %. Los resultados obtenidos evidenciaron la efectividad de este tratamiento no convencional en 92,4 % de los afectados, a la vez que se logró 96,3 % de satisfacción con el proceder empleado. From the technological point of view is usually more important to ensure the ability to react pro... more From the technological point of view is usually more important to ensure the ability to react promptly to changing environmental conditions than to try to forecast them. Evolution Algorithms were proposed initially to drive the adaptation of complex systems to varying or uncertain environments. In the general setting, the adaptive-anticipatory dilemma reduces itself to the placement of the interaction with the environment in the computational schema. Adaptation consists of the estimation of the proper parameters from present data in order to react to a present environment situation. Anticipation consists of the estimation from present data in order to react to a future environment situation. This duality is expressed in the Evolutionary Computation paradigm by the precise location of the consideration of present data in the computation of the individuals fitness function. In this paper we consider several instances of Evolutionary Algorithms applied to precise problem and perform an experiment that test their response as anticipative and adaptive mechanisms. The non stationary problem considered is that of Non Stationary Clustering, more precisely the adaptive Color Quantization of image sequences. The experiment illustrates our ideas and gives some quantitative results that may support the proposition of the Evolutionary Computation paradigm for other tasks that require the interaction with a Non-Stationary environment.

El... more Introducción: la ictericia neonatal es uno de los síndromes más frecuentes de la neonatología. El tratamiento al recién nacido ictérico ha tenido históricamente diferentes guías para sus decisiones. Objetivo: establecer una comparación entre los métodos diagnósticos de la escala visual y la dosificación en sangre de la bilirrubina total en los recién nacidos. Métodos: se realizó una investigación observacional, descriptiva, transversal, en el Hospital Ginecoobstétrico Docente de Guanabacoa de enero a julio de 2008. La muestra estuvo constituida por 86 recién nacidos. Resultados: el mayor número de recién nacidos fueron a término y normopesos. Existió predominio de la zona IV de la escala visual de Kramer. El mayor número de casos tuvo valores de bilirrubina sérica entre 10-12 mg/dL. Conclusiones: existió relación entre ambos métodos de evaluación de la ictericia neonatal.

The intensity inhomogeneity (IIH) is modeled by a multiplicative inhomogeneity field. It is due to the spatial inhomogeneity in the excitatory Radio Frequency (RF) signal and other effects. It has been acknowledged as a greater source of error for automatic segmentation algorithms than additive noise. We propose a new non parametric IIH correction algorithm where the Self Organizing Map (SOM) is used to estimate the IIH field.

Positioning applications became more important in recent years not only for security applications... more Positioning applications became more important in recent years not only for security applications, but also for the mass market. Having a pedestrian navigation system embedded in a mobile phone is a realistic solution since it is equipped with low-cost sensors and the smartphone is located in a nonobstructive way. The location of the smartphone is important, since the position estimation process depends on it. Therefore, we propose to distinguish between pocket or bag, phoning, texting and swinging. We present a standalone inertial pocket navigation system based on an inertial measurement unit. For the computation of the orientation, we have developed an attitude estimator based on an unscented Kalman filter. The update stage has two different updates based on the acceleration and the magnetic field. Therefore, a zero acceleration detector, a magnetic disturbances detector and a static periods detector have been developed. The odometry in our navigation system is computed through an extended Kalman filter. The position is predicted with a movement model which is periodically updated through position corrections computed by the position computer. It comprises a step detector and a step length estimator based on the norm of the acceleration. The performance of our attitude estimator in comparison with the ground truth orientation is shown. The rest of the handheld positions are also tested for orientation. Likewise, we show pocket odometries of different users with the floor plan superimposed.

La leucemia de linfocitos grandes granulares es una entidad poco frecuente, perteneciente al mism... more La leucemia de linfocitos grandes granulares es una entidad poco frecuente, perteneciente al mismo espectro de trastornos que el síndrome de Felty, que puede presentarse en pacientes con artritis reumatoide de larga evolución. Clínicamente se caracteriza por neutropenia persistente e incremento de la susceptibilidad a infecciones bacterianas, asociado a la presencia en sangre periférica y médula ósea de una expansión clonal de linfocitos atípicos con fenotipo de linfocito T citotóxico, o menos frecuentemente de célula NK; y esplenomegalia. Se diagnostica con mayor frecuencia en pacientes con artritis reumatoide seropositiva con importante daño estructural, manifestaciones extraarticulares y valores persistentemente altos de factor reumatoide y VSG, a pesar de poder presentar escasa actividad inflamatoria articular. Presentamos el caso de un varón de 70 años con artritis reumatoide de larga evolución que desarrolló shock séptico secundario a la infección de una prótesis de cadera por Salmonella spp. Presentaba neutropenia persistente, identificándose en sangre periférica y médula ósea una población monoclonal de linfocitos T aberrantes compatibles con leucemia de linfocitos grandes granulares.

Background: Ostomy patients have problems affecting their overall health and problems which affect the family unit that must be addressed in a holistic manner both in hospitals and primary care (home care). There is room for improvement in the planning and continuity´s care. Objectives: To review the literature on the needs of ostomy patients cared for at home and in primary care. To propose actions to improve comprehensive ostomy patient care during hospital stay and at home. Methods: We performed a bibliometric analysis between October of 2012 and March of 2013 to analyze the ostomy patient's cares once they have been discharged from hospital, which focuses on home care. Literature review was performed using Punto Q from the University of La Laguna in databases such as Dialnet, Scielo, Scopus, Medline and Science Direct.

Se realizó un estudio descriptivo y longitudinal de 53 adultos hipertensos pertenecientes al cons... more Se realizó un estudio descriptivo y longitudinal de 53 adultos hipertensos pertenecientes al consultorio médico No. 55 del Policlínico Universitario "José Martí Pérez" de Santiago de Cuba, desde febrero hasta septiembre de 2012, con vistas a demostrar la efectividad de la auriculopuntura en el tratamiento de estos pacientes. Se halló predominio de la hipertensión arterial ligera (69,6 %) y del sexo femenino (47,1 %). Luego de aplicada la terapia disminuyeron los síntomas más frecuentes: la cefalea, de 50,9 a 5,6 % y el insomnio, de 20,7 a 1,8 %. Los resultados obtenidos evidenciaron la efectividad de este tratamiento no convencional en 92,4 % de los afectados, a la vez que se logró 96,3 % de satisfacción con el proceder empleado. From the technological point of view is usually more important to ensure the ability to react pro... more From the technological point of view is usually more important to ensure the ability to react promptly to changing environmental conditions than to try to forecast them. Evolution Algorithms were proposed initially to drive the adaptation of complex systems to varying or uncertain environments. In the general setting, the adaptive-anticipatory dilemma reduces itself to the placement of the interaction with the environment in the computational schema. Adaptation consists of the estimation of the proper parameters from present data in order to react to a present environment situation. Anticipation consists of the estimation from present data in order to react to a future environment situation. This duality is expressed in the Evolutionary Computation paradigm by the precise location of the consideration of present data in the computation of the individuals fitness function. In this paper we consider several instances of Evolutionary Algorithms applied to precise problem and perform an experiment that test their response as anticipative and adaptive mechanisms. The non stationary problem considered is that of Non Stationary Clustering, more precisely the adaptive Color Quantization of image sequences. The experiment illustrates our ideas and gives some quantitative results that may support the proposition of the Evolutionary Computation paradigm for other tasks that require the interaction with a Non-Stationary environment.

El... more Introducción: la ictericia neonatal es uno de los síndromes más frecuentes de la neonatología. El tratamiento al recién nacido ictérico ha tenido históricamente diferentes guías para sus decisiones. Objetivo: establecer una comparación entre los métodos diagnósticos de la escala visual y la dosificación en sangre de la bilirrubina total en los recién nacidos. Métodos: se realizó una investigación observacional, descriptiva, transversal, en el Hospital Ginecoobstétrico Docente de Guanabacoa de enero a julio de 2008. La muestra estuvo constituida por 86 recién nacidos. Resultados: el mayor número de recién nacidos fueron a término y normopesos. Existió predominio de la zona IV de la escala visual de Kramer. El mayor número de casos tuvo valores de bilirrubina sérica entre 10-12 mg/dL. Conclusiones: existió relación entre ambos métodos de evaluación de la ictericia neonatal.

The intensity inhomogeneity (IIH) is modeled by a multiplicative inhomogeneity field. It is due to the spatial inhomogeneity in the excitatory Radio Frequency (RF) signal and other effects. It has been acknowledged as a greater source of error for automatic segmentation algorithms than additive noise. We propose a new non parametric IIH correction algorithm where the Self Organizing Map (SOM) is used to estimate the IIH field.

Positioning applications became more important in recent years not only for security applications... more Positioning applications became more important in recent years not only for security applications, but also for the mass market. Having a pedestrian navigation system embedded in a mobile phone is a realistic solution since it is equipped with low-cost sensors and the smartphone is located in a nonobstructive way. The location of the smartphone is important, since the position estimation process depends on it. Therefore, we propose to distinguish between pocket or bag, phoning, texting and swinging. We present a standalone inertial pocket navigation system based on an inertial measurement unit. For the computation of the orientation, we have developed an attitude estimator based on an unscented Kalman filter. The update stage has two different updates based on the acceleration and the magnetic field. Therefore, a zero acceleration detector, a magnetic disturbances detector and a static periods detector have been developed. The odometry in our navigation system is computed through an extended Kalman filter. The position is predicted with a movement model which is periodically updated through position corrections computed by the position computer. It comprises a step detector and a step length estimator based on the norm of the acceleration. The performance of our attitude estimator in comparison with the ground truth orientation is shown. The rest of the handheld positions are also tested for orientation. Likewise, we show pocket odometries of different users with the floor plan superimposed.

La leucemia de linfocitos grandes granulares es una entidad poco frecuente, perteneciente al mism... more La leucemia de linfocitos grandes granulares es una entidad poco frecuente, perteneciente al mismo espectro de trastornos que el síndrome de Felty, que puede presentarse en pacientes con artritis reumatoide de larga evolución. Clínicamente se caracteriza por neutropenia persistente e incremento de la susceptibilidad a infecciones bacterianas, asociado a la presencia en sangre periférica y médula ósea de una expansión clonal de linfocitos atípicos con fenotipo de linfocito T citotóxico, o menos frecuentemente de célula NK; y esplenomegalia. Se diagnostica con mayor frecuencia en pacientes con artritis reumatoide seropositiva con importante daño estructural, manifestaciones extraarticulares y valores persistentemente altos de factor reumatoide y VSG, a pesar de poder presentar escasa actividad inflamatoria articular. Presentamos el caso de un varón de 70 años con artritis reumatoide de larga evolución que desarrolló shock séptico secundario a la infección de una prótesis de cadera por Salmonella spp. Presentaba neutropenia persistente, identificándose en sangre periférica y médula ósea una población monoclonal de linfocitos T aberrantes compatibles con leucemia de linfocitos grandes granulares.
