Altering the macronutrient composition of the diet influences hunger and satiety. Studies have compared high- and low-protein diets, but there are few data on carbohydrate content and ketosis on motivation to eat and ad libitum intake. We aimed to compare the hunger, appetite, and weight-loss responses to a high-protein, low-carbohydrate [(LC) ketogenic] and those to a high-protein, medium-carbohydrate [(MC) nonketogenic] diet in obese men feeding ad libitum. Seventeen obese men were studied in a residential trial; food was provided daily. Subjects were offered 2 high-protein (30% of energy) ad libitum diets, each for a 4-wk period-an LC (4% carbohydrate) ketogenic diet and an MC (35% carbohydrate) diet-randomized in a crossover design. Body weight was measured daily, and ketosis was monitored by analysis of plasma and urine samples. Hunger was assessed by using a computerized visual analogue system. Niacin (Vitamin B 3) is significantly important for the metabolism of animals and human beings due to its incorporation into the coenzymes NAD and NADP. For dairy cows, the microbial synthesis of niacin in the rumen is an important source besides its availability from feed and endogenous formation. This vitamin is involved in various energy-yielding pathways and for synthesis of amino acid and fatty acid; for which it is important for milk production. Supplementation of niacin has beneficial effects on growth of cattle. The production performance of dairy cows fed with niacin at 6g/day may not be satisfactory, but supplementation of 12g of niacin per head per day can increase milk production by about 1lb. An optimistic return on asset is possible if the supplementation is limited to high producing early lactation cows. Supplementing the dairy animals with a dose of 6-12g of niacin will not only protect them from various metabolic diseases but will also help them defend from severe heat stress; ultimately leading to augmentation of their health, production potential and economy.

Nutritional ketosis is effective to contrast seizure disorders and various acute and chronic neurological disorders. From a physiological perspective, glucose is the primary metabolic fuel for cells. However, many neurodegenerative disorders have been recently associated with impaired glucose transport and metabolism and with mitochondrial dysfunction causing energy deficits, such as in Alzheimer’s disease, Parkinson’s disease, general seizure disorders, and traumatic brain injury. Therapeutic ketosis can be considered as a metabolic therapy by providing alternative energy substrates, which may have potent cellular protective properties independent of their bioenergetic function. Therapeutic ketosis leads to metabolic adaptations that may improve brain metabolism, restore mitochondrial ATP production, decrease reactive oxygen species production, reduce inflammation, and increase neurotrophic factors’ function. Prior to the advent of exogenous insulin for the treatment of diabetes mellitus (type II) in the 1920's, the general guidelines for therapy were represented only by dietary modifications. At the time, diet recommendations aimed to control blood glucose (which in most cases was only glycosuria) and were dramatically different from current low-fat, high-carbohydrate dietary recommendations for patients with diabetes. The aim of this review article is to analyze the current literature in matter of therapeutic ketosis and its successful clinical applications in diabetes type I and II, thus paving the road towards a wider and safer use of this metabolic approach in these patients.

The need for effective clinical interventions in chronic neurological diseases such as TBI and other variants of chronic neurological conditions have been called for in the literature. The cellular and neurochemical mechanisms addressed in recent literature have focused around three common themes that traverse all of these condition classes: immune and autoimmune mechanisms, inflammatory pathways and oxidative phosphorylation or other energy production damage. Limits to the effectiveness of pharmaceutical and surgical approaches are apparent, and complicated by the physiological interconnectedness of such pathways. A growing call for non-drug, non-surgical options has evolved due to the dangers of poly-pharmacy, the lifestyle illnesses, and emerging evidence pointing to functional measures and methods. This study aimed to evaluate clinical and biochemical changes in dairy cows during periparturient period. It was carried out on 38 multiparous Holstein/Fersian crossbreed dairy cows 3–11 years old. Using ultrasonography for measurement backfat thickness (BFT) for detection of the energy deficit. BFT on dairy cattle is used to monitor changes in subcutaneous fat thickness which represent changes in energy stores. On the basis of clinical examination and assessment of BHBA concentration in blood, cows were classified to four groups, control healthy cows (C = 16), subclinical ketosis (SCK = 10), cows with retained placenta (RP = 6), and clinical mastitis (CM= 6). BFT showed a significant decrease (p < 0.05) after parturition in all diseased groups compared with healthy. The plasma level of NEFA and BHBA showed a significant increase (p < 0.05) at 2 weeks a.p (antepartum) and 2 weeks p.p (postpartum) in RP and SCK than other groups, respectively. The plasma level of glucose showed a significant decrease (p < 0.05) at 2 weeks p.p in SCK group. The plasma level of insulin showed a significant decrease (p < 0.05) at 4 weeks a.p in RP group and in all groups after parturition. It could be concluded that production diseases induced clinical, biochemical, and subcutaneous fat changes in dairy cows.

In Ramadan, fasting individuals consume two meals per day, the first of which is before down to prepare for fasting (Sahur), and second meal is at sunset (Iftar). Fasting is a ritual and religious obligation with numerous beneficial health effects, especially in patients with diet-dependent diseases. Fasting could significantly reduce peptic and duodenal ulcers in combination with H2 blockers and improve the lipid profile, especially through the regulation of total cholesterol, high-density lipoprotein, and lo-density lipoprotein, which may be involved in the incidence of heart failure. Furthermore, fasting could enhance the systolic and diastolic blood pressure more significantly compared to the use of drugs such as perindopril/indapamide. Evidence suggests that ischemic stroke and cancer could also be improved by intermitted fasting via multiple pathways and factors such as BDNF, bFGF, GRP78, Hsp70, tyrosine kinase receptor B (TrkB), fibroblast growth factor receptor 1 (FGFR1), and ketosis. The present study aimed to systematically review the published literature regarding the effects of Ramadan fasting on the mentioned diseases.
