Unmittelbarer Zwang (UZ) ist ein Rechtsbegriff, der die hoheitliche Einwirkung auf Personen oder Sachen mittels körperlicher Gewalt, Hilfsmittel der körperlichen Gewalt oder Waffen durch zuständige und befugte Amtsträger umfasst und wesentlicher Bestandteil und Ausdruck der Staatsgewalt ist. Es gilt prinzipiell und grundlegend das Verhältnismäßigkeitsprinzip. Das heißt, dass auch die Polizei Gewalt generell nur ausnahmsweise anwenden darf und nur dann, wenn sie eine polizeiliche Maßnahme auf andere Weise nicht durchsetzen kann.

Przymus bezpośredni może polegać na (art. 18 ust. 3 ustawy): * przytrzymaniu, * przymusowym podaniu leków, * unieruchomieniu, * izolacji. W stosunku do osób jedynie uniemożliwiających funkcjonowanie szpitala psychiatrycznego lub jednostki organizacyjnej pomocy społecznej stosuje się wyłącznie przytrzymanie lub przymusowe podanie leku. Wymieniona wyżej ustawa (art. 34) przewiduje możliwość zastosowania przymusu bezpośredniego wobec osoby przyjętej do szpitala psychiatrycznego bez jej zgody, gdy jest to konieczne do dokonania niezbędnych czynności leczniczych (mających na celu usunięcie przyczyn przyjęcia bez zgody), a także w celu zapobieżenia samowolnemu opuszczeniu przez tę osobę szpitala psychiatrycznego.
