Background: Diltiazem belongs to Ca++ channel blockers group of drugs used for multiple disorders... more Background: Diltiazem belongs to Ca++ channel blockers group of drugs used for multiple disorders like hypertension, cardiac arrhythmias, angina pectoris and peripheral vascular disease. It causes relaxation of the vascular smooth muscles by a well known mechanism that is blockage of L-type of the calcium channels inhibiting the entry of Ca++ into the muscle cells. Current study explored its effects on platelet aggregation and their molecular basis. Methodology: This experimental study was conducted at Dr. Panjwani Center for Molecular Medicine and Drug Research, ICCBS, University of Karachi. Blood samples from 20 normal volunteers were taken under aseptic measures after informed consent. Blood was centrifuged at 1000RPM to receive PRP (platelet rich plasma). PRP was mixed with the PAF and Arachidonic Acid with the help of Eppendorf pipette and placed in aggregometer to observe the aggregation at different concentrations. Then different concentrations of Diltiazem were added to these solutions to observe the inhibition of platelet aggregation at different concentration. IC50 of Diltiazem was calculated by dose response curves. Results: We observed that Diltiazem successfully reduced the PAF and AA induced platelet aggregation at IC50 =15uM (Inhibitory Concentration). That is an additional aspect of this agent. Conclusion: Diltiazem has dual role in the treatment and prevention of ischemic heart disease and ischemic stroke by reducing the blood pressure and inhibiting the platelet aggregation.

A new triterpenoidal saponin of hederagenin named symphytoxide A has been isolated from the ethan... more A new triterpenoidal saponin of hederagenin named symphytoxide A has been isolated from the ethanolic extract of the roots of Symphytum officinale and characterized on the basis of chemical investigations and spectroscopic studies as 3-O-[beta-D-glucopyranosyl-(1-->2)-beta-D-glucopyranosyl-(1-->4)- alpha-L-arabinopyranosyl] hederagenin. The structure of this new saponin was established on the basis of 1D and 2D NMR experiments including heteroCOSY, COSY-45 degrees as well as HMBC measurements and other spectroscopic techniques. The saponin exhibited hypotensive activity in anesthetized rats.

Different formulations were prepared by varying concentrations of HPMCK4M and Eudragit L100 (EL100) by solvent casting method. Oleic acid and Propylene glycol were employed as penetration enhancers and Polyethylene glycol 400 was used as plasticizer. Formulated patches were subjected to various evaluating parameters including pre-formulation studies FTIR, SEM and UV analysis and post formulation studies such as organoleptic evaluation, thickness, folding endurance, weight uniformity, flatness, pH determination, percentage moisture loss, drug content and permeation studies. FTIR revealed no interaction between drug and polymers. X-ray diffractometry (XRD) studies showed was that both drugs were of crystalline nature which was further confirmed by SEM. Case Report Findings: A sixty seven-years-old male admitted in emergency department of our hospital, having clinical history of swelling and pain in both the lower limb and feet, on physical examination, provisionally diagnosed as a case of Cellulitis and deep vein thrombosis(DVT). Blood specimen was obtained for general hematological investigations. Full blood count (FBC) was performed on sysmex XP-100 hematological analyzer which showed invalid findings especially red cells indices which were not corresponding to the hemoglobin (Hb) concentration of the patient. Blood sample was repeated, to confirm invalid red cells indices which showed values as in the 1st blood specimen. Blood smears revealed aggregation of red cells.
