Schistosomiasis, caused by Schistosoma trematode worms, represents a significant global health ch... more Schistosomiasis, caused by Schistosoma trematode worms, represents a significant global health challenge. This review offers a thorough examination of the disease's epidemiology, transmission dynamics, diagnostic modalities, and treatment options. Diagnostic techniques encompass direct parasitological methods, immunological assays, DNA/RNA detection, and biomarker utilization, each with distinct advantages and limitations. There is an urgent need for improved diagnostic tools with enhanced sensitivity and specificity. Praziquantel remains the cornerstone of treatment, exhibiting efficacy against all Schistosoma species, while the potential of artemisin derivatives in combination therapy is also explored. In this review, we focus on the importance of praziquantel administration as the central aspect of schistosomiasis treatment, highlighting ongoing efforts to optimize its utilization for improved patient outcomes.

Latar belakang: Derajat keparahan Covid-19 dikaitkan dengan koagulasi dan kerusakan endotel yang ... more Latar belakang: Derajat keparahan Covid-19 dikaitkan dengan koagulasi dan kerusakan endotel yang dipicu oleh reaksi inflamasi akibat dari pelepasan sitokin proinflamasi yang berlebihan oleh karena infeksi SARS CoV-2. Metode: Penelitian deskriptif analitik yang dilakukan dengan pendekatan kohor retrospektif dilakukan di Rumah Sakit Umum Pemerintah Wahidin Sudirohusodo Makassar dengan mengumpulkan data rekam medis pasien Covid-19 pada bulan Juni 2020 -Juni 2021 dengan menggunakan total sampling yang memenuhi kriteria inklusi dan eksklusi. Hasil: Pada penelitian ini didapatkan rerata trombosit lebih rendah (265.442) pada kasus berat, sedangkan rerata fibrinogen (405,54) dan D-dimer (9,84) lebih tinggi pada derajat keparahan lebih berat, serta Prothrombin time (PT) lebih panjang (10,92) pada derajat keparahan lebih berat. Kesimpulan: Profil koagulasi penderita Covid-19 didatakan rerata rombosit lebih rendah pada derajat keparahan lebih berat, sedangkan fibrinogen dan D-dimer lebih tinggi serta Prothrombin time (PT) lebih panjang pada derajat keparahan lebih berat.

The coagulopathy mechanism involves the process of coagulation and endothelial damage triggered by an inflammatory response of the SARS-CoV-2 infection due to excessive release of proinflammatory cytokines. This study aimed to determine the association of coagulation factors as potential predictors of COVID-19 patient outcomes. METHODS This retrospective study was performed on 595 patients at Wahidin Sudirohusodo Hospital, Makassar, from June 2020 to June 2021. Participants were recruited using total sampling and assessed for COVID-19 severity using the World Health Organization classification and coagulation factors (D-dimer, fibrinogen, thrombocyte, and prothrombin time [PT]). Pneumoconiosis had threatened cement workers. Serologic abnormalities had found in pneumoconiosis. Surfactant Protein A (SP-A) levels increased in silica-exposed workers. Surfactant Protein A (SP-A) may be a helpful biomarker for the early diagnosis of pneumoconiosis, but it has not yet been studied in Indonesia. Methods: The design of this study was observational with cross-sectional. A sampling of cementexposed workers was done by consecutive sampling. The subjects were 88, approach population of 67 cement exposed workers from September 2017-March 2018 and 17 healthy people as control. The serum level of SPA was measured by the ELISA method. Cement exposed workers is a worker in the production area and workers in the quarry area. Results: The total number of research subjects met the criteria was 67, and the control subjects were 21. The mean serum SPA level in the study subject group or the exposed group was 6.02 ng/ml, and the mean SPA level in the control group was 4.50 ng/ml. The difference in SPA levels between the exposed and control groups was different but not significant, with value of P=0.084. Conclusion: SPA levels in the exposed and control groups were different but not statistically significant.

Thyroid crisis is an emergency due to impaired thyroid function caused by various conditions, par... more Thyroid crisis is an emergency due to impaired thyroid function caused by various conditions, particularly infections such as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that result in the dysfunction of various vital organs. We report a case of a 31-year-old Indonesian female with a 2-year history of hyperthyroidism with elevated thyroid-stimulating hormone (TSH) receptor antibodies. (TRAb) who developed thyroid crisis possibly in association with SARS-CoV-2 pneumonia, sepsis, and disseminated intravascular coagulation (DIC). Prior to admission, she was treated for her hyperthyroidism with propylthiouracil and had been in stable remission for a year. She was admitted to the Emergency Room with complaints of watery stools, icteric sclerae, jaundice, coughing, and shortness of breath. The physical examination showed a World Health Organization (WHO) performance score of 4, delirium, blood pressure within normal limits, tachycardia, tachypnea, axillary temperature of 36.7°C, icteric sclerae, jaundice, and exophthalmos. There was a 3 cm palpable nodule on the right side of the neck. Auscultation of the lungs revealed bilateral pulmonary rales. Abdominal examination noted a palpable liver and enlarged spleen. Laboratory tests showed thrombocytopenia, electrolyte imbalance, hypoalbuminemia and elevated transaminases. The thyroid function tests showed a suppressed TSH level with an elevated free thyroxine (FT4) level. The SARS-CoV-2 polymerase chain reaction (PCR) swab test was positive. Initial patient management was with supportive therapy that included favipiravir and anti-hyperthyroidism medication; however, despite these interventions, her condition continued to deteriorate and she died after a few hours. This case demonstrates no difference in therapy between patients with thyroid crises and COVID-19 or other infections. Proper and timely treatment is important for reducing mortality rates.

Background:Incidence of pulmonary TB in type-2 diabetes mellitus patients is still high. Gender, ... more Background:Incidence of pulmonary TB in type-2 diabetes mellitus patients is still high. Gender, age, nutritional status, duration of diabetes, blood glucose control, smoking habit, and HbA1c, are among risk factors that influence its occurrence. This study aims to analyze the risk factors that contribute to the incidence of pulmonary TB in type-2 DM patients in Makassar. Methods:A survey analysis study at Wahidin Sudirohusodo Hospital. Hasanuddin University Hospital. and educational network hospital.on December 2019 until the desired sample reached. Sample collection using consecutive sampling. Data analysis using SPSS verse 22. Statistical analysis using pearson's correlation. chi square test. and multiple logistic regression (backward method). Significant result if p value <0.05. Results:Study included 225 type-2 DM patients. From study shows distribution of pulmonary TB in type-2 DM patients is 13.3%. Incidence of pulmonary TB was high in patient with underweight (OR=6.7 and p<0.05), duration of DM>5 years (OR=3.8 and p<0.05), and uncontrolled blood glucose (OR=2.7 and p<0.05). There was no significant correlation between gender (female 19 from 135, male 11 from 90 with p>0.05), age (age >60yo 15 from 92, age ≤60yo 15 from 133 with p>0.05), smoking habit (yes 10 from 84, no 20 from 141 with p>0.05) and HbA1c (A1c <7 was 0 from 7 and A1c ≥7 was 30 from 165 with p>0.05) with pulmonary TB incidence. Discussions:Incidence pulmonary TB in type-2 DM patients according to BMI was significant (p<0.05) because condition of malnutrition can lowering the immune status by decreased the limphocytes production and immune proliferation also decreased of IFNgamma and IL-2 level, and increased of TGF-beta. Incidence according to duration of DM was significant (p<0.05). Incidence according to blood glucose control was significant (p<0.05), DM and uncontrolled blood glucose can lead to an immune compromise condition, some of risk factors that also can contribute to infection disease. Conclusion:There was significant correlation between underweight, duration of DM >5 years, and uncontrolled blood glucose with pulmonary TB incidence.

Phosphatonin levels are regulated by the hormone phosphatonin, which the most commonly associated with CKD is fibroblast growth factor-23 (FGF-23), mainly synthesized by bone cells. The increase in FGF-23 in CKD subjects is a physiological response to stabilize phosphate levels. This study is a retrospective study between 2017 to 2019 from the medical records of NSCLC patients who receive first-line chemotherapy and thise who recieve EGFR-TKI. There was no significant correlation between gender (female 19 from 135, male 11 from 90 with p>0.05), age (age >60yo 15 from 92, age ≤60yo 15 from 133 with p>0.05), smoking habit (yes 10 from 84, no 20 from 141 with p>0.05) and HbA1c (A1c <7 was 0 from 7 and A1c ≥7 was 30 from 165 with p>0.05) with pulmonary TB incidence. Discussions:Incidence pulmonary TB in type-2 DM patients according to BMI was significant (p<0.05) because condition of malnutrition can lowering the immune status by decreased the limphocytes production and immune proliferation also decreased of IFNgamma and IL-2 level, and increased of TGF-beta. Incidence according to duration of DM was significant (p<0.05). Incidence according to blood glucose control was significant (p<0.05), DM and uncontrolled blood glucose can lead to an immune compromise condition, some of risk factors that also can contribute to infection disease. Conclusion:There was significant correlation between underweight, duration of DM >5 years, and uncontrolled blood glucose with pulmonary TB incidence.

Lung cancer is the type of non-small cell carcinoma (NSCLC) consists of non-squamous and squamous. Non-small cell lung cancer of non squamous types consist of adenocarcinoma and large cell carcinoma. Currently, lung cancer therapy is highly developed of chemotherapeutic agents to targeted therapy especially EGFR-TKI. This study aims to assess the survival rate of NSCLC patients who receive first line chemotherapy and those who recieve EGFR-TKI therapy at Wahidin Sudirohusodo hospital. Methods. This study is a retrospective study between 2017 to 2019 from the medical records of NSCLC patients who receive first-line chemotherapy and thise who recieve EGFR-TKI. Patients with platinum-based chemotherapy and EGFR-TKI with gefitinib therapy 1x250 mg/day or erlotinib 1x150mg/day and or afatinib 1x40 mg/day.
