Research paper thumbnail of Uncovering multi-level mental healthcare barriers for migrants: a qualitative analysis across China, Germany, Netherlands, Romania, and South Africa

BMC public health, Jun 14, 2024

Background Forced displacement is a significant issue globally, and it affected 112 million peopl... more Background Forced displacement is a significant issue globally, and it affected 112 million people in 2022. Many of these people have found refuge in low-and middle-income countries. Migrants and refugees face complex and specialized health challenges, particularly in the area of mental health. This study aims to provide an in-depth qualitative assessment of the multi-level barriers that migrants face in accessing mental health services in Germany, Macao (Special Administrative Region of China), the Netherlands, Romania, and South Africa. The ultimate objective is to inform tailored health policy and management practices for this vulnerable population. Methods Adhering to a qualitative research paradigm, the study centers on stakeholders' perspectives spanning microsystems, mesosystems, and macrosystems of healthcare. Utilizing a purposive sampling methodology, key informants from the aforementioned geographical locations were engaged in semi-structured interviews. Data underwent thematic content analysis guided by a deductive-inductive approach. Results The study unveiled three pivotal thematic barriers: language and communication obstacles, cultural impediments, and systemic constraints. The unavailability of professional interpreters universally exacerbated language barriers across all countries. Cultural barriers, stigmatization, and discrimination, specifically within the mental health sector, were found to limit access to healthcare further. Systemic barriers encompassed bureaucratic intricacies and a conspicuous lack of resources, including a failure to recognize the urgency of mental healthcare needs for migrants. Conclusions This research elucidates the multifaceted, systemic challenges hindering equitable mental healthcare provision for migrants. It posits that sweeping policy reforms are imperative, advocating for the implementation of strategies, such as increasing the availability of language services, enhancing healthcare providers' capacity, and legal framework and policy change to be more inclusive. The findings substantially contribute to scholarly discourse

Since CIs in Germany are mainly organized on a regional basis, there are no nationwide data available. The aim of this explorative cross-sectional study is to gain a better understanding of the work of CIs in Germany. Methods In cooperation with the Federal Chamber of Psychotherapists and the Federal Association of Non-statutory Welfare, a nationwide online survey was conducted between June and August 2022. Interpreters’ working conditions and qualification were assessed. Interpreters were recruited primarily through interpreter pools and training institutions in Germany. Results Across all 16 federal states, n = 1199 people clicked on the survey, of which n = 873 responses were usable for analysis. Most of the participants are female (74%) and born abroad (77%). The average work experience as interpreters is MED = 6 years (IQR = 3-10). 44% reported interpr...

However, changing social patterns, norms, and values suggest changes in how social determinants may predict subjective wellbeing. The current analysis tests a serial mediation hypothesis in which social participation and social resources mediate the effect of general trust on subjective wellbeing.Data from 8725 participants were pooled from the German part of the European Social Survey (ESS) Wave 10. This study proposes exploring work-life balance (WLB) as a predictor of Generation Z’s mental health outcomes. Methods A cross-sectional online study was conducted to collect data on mental health outcomes and WLB from a sample of Generation Z (N = 134). Three regression models were calculated to examine the predictive effect of WLB on stress, anxiety, and depressive symptoms. Results The results show a strong statistically significant association between all three mental health outcomes. The regression model suggests work-life balance as a predictor of stress (β = .- 404 p = .001, R2 = .16) and anxiety (β = .-256 p = .001, R2 = .06). Conclusion WLB must be considered a significant factor in mental health problems among Generation Z.

high prevalence of mental disorders, there is an increasing
focus on the psychotherapeutic care of these people. Interpreters
are generally needed to enable communication between
therapist and patient. Material and Methods: Based on
guided interviews, opportunities, challenges and recommendations
for interpreter-aided therapy were examined from the
psychotherapists’ (n = 5) and the interpreters’ (n = 3) point of
view and analyzed following the rules of qualitative content
analysis according to Mayring. Results: The interview partners
named the necessity of legally regulated financing of interpreter
expenses and the establishment of permanent interpreter
pools and specialized therapy centers as external conditions
for successful interpreter-supported psychotherapies.
They emphasized the importance of a clear separation of
roles, preliminary talks and follow-up discussions, the good
match between patient and interpreter with regard to their
cultural background, as well as full and accurate first-person
translations. Within the triad, they underlined the importance
of a harmonious relationship between therapist and interpreter.
Conclusions: Successful interpreter-aided psychotherapy
requires the structural framework and the skills and burdens
of the psychotherapists and interpreters to be taken into consideration.
Further empirical studies including the patients’
perspective are needed.

expenses and the establishment of permanent interpreter
pools and specialized therapy centers as external conditions
for successful interpreter-supported psychotherapies.
They emphasized the importance of a clear separation of
roles, preliminary talks and follow-up discussions, the good
match between patient and interpreter with regard to their
cultural background, as well as full and accurate first-person
translations. Within the triad, they underlined the importance
of a harmonious relationship between therapist and interpreter.
Conclusions: Successful interpreter-aided psychotherapy
requires the structural framework and the skills and burdens
of the psychotherapists and interpreters to be taken into consideration.
Further empirical studies including the patients’
perspective are needed.

Methods: Using interpreter-supported guided interviews,
10 refugee patients who experienced interpreter-supported
psychotherapy were asked about factors influencing
the development of a trusting therapeutic alliance in
the triad. The analysis of the interviews followed the rules
of content-structuring qualitative content analysis. Results:
A total of 11 factors were identified which could be
assigned to the interpreter, therapist, or patient. In the
analysis, the central role of the interpreter in establishing
a therapeutic alliance in the triad became particularly
clear. Conclusions: Consideration of the factors that, from
the patients’ perspective, require the establishment and maintenance of a trusting alliance within the triad, as well
as the recommendations for action derived from this for
psychotherapists and interpreters can lead to an improvement
in the therapeutic treatment of refugees.
